Healthy Japan 21 objectives and strategies (일본건강증진 사업의 목표 및 추진전략: Healthy Japan 21)

  • Healthy Japan 21의 목적은 삶의 질을 향상시킴은 물론 노동가능 인구집단의 유병률을 줄이고 건강수명을 연장시키기 위하여, 21세기 모든 일본인들이 더욱 건강하고 행복한 삶을 향유할 수 있도록 국가사업을 활성화하는 것이다. 구체적 목적은 조기사망을 예방하고 건강생활을 향상하는 것으로, Healthy Japan 21의 전략적 기획과정에서 건강수명의 연장을 실현하기 위하여 2010년까지 달성되어야 하는 구체적인 목적들이 또한 제시되었다. 조기사망을 감소시키기 위하여 사고, 암, 자살, 심장병 감소의 중요성을 인식하고 9개의 주요 목표로 영양, 신체활동, 휴식과 이완, 금연, 절주, 구강보건, 당뇨병 예방, 심혈관계질환 예방, 암 예방을 설정하였다. 흡연, 알콜, 식사 그리고 운동과 같은 생활양식은 스트레스, 비만, 고혈압과 같은 위험요인 및 질병관리와 관련이 있으며, 위험요인은 암, 심장병, 구강질환의 발생과 밀접한 관련을 갖는다. 따라서 질병의 발생을 예방하기 위해서는 건강증진과 일차예방이 강조되어야 한다. 일차예방에 중점을 두기 위해서 우리는 전통적인 질병관리의 중점사항인 정기적인 건강검진을 통해 질병 조기발견을 노력을 게을리 하지 말아야 할 것이다. 아울러 의료비 감소, 병상에 있는 사람들의 감소, 사회세의 감소도 또한 달성되어야 하는 사업의 결과로 설정되어 있다. 가장 최근의 Healthy Japan 2000(1998-99)의 평가에 따르면 목표들의 15%가 달성되었거나 초과 달성된 것으로 나타났다. 이 중 아동과 청소년의 사망률의 경우, 1-14세 아동의 사망률은 1987년부터 26%가 감소되어 2000년도 목표인 인구 100,000명단 28명의 사망을 초과 달성한 것으로 평가되었다. Healthy Japan 21의 두 가지 주요 전략은 일반 인구집단을 위한 전략과 고위험 집단을 대상으로 하는 전략으로 구성된다. 개인의 건강한 생활양식으로의 변화를 포함한 우리의 건강증진 노력은 사람 중심으로 개인의 선택을 기반으로 하고 있다. 이러한 노력을 지원하기 위하여, 각 개인이 정보를 갖은 상태에서 올바른 선택을 할 수 있도록 적당량의 올바른 정보를 제공하는 것이 필수적이다. 이와 같은 일본의 건강증진계획은 2000년 3월에 Healthy Japan 21이 설립되었으며, 2000-2002년 사이 모든 현이 자신의 사업계획을 설정하였으며, 2001-2005년에는 약 반수 정도의 지방자치단체들이 자신들의 사업계획을 확정하였다. 건강증진을 이루는 중요한 수단은 파트너 쉽에 있다. 정부조직 뿐 아니라 건강보험회사, 보건의료서비스 제공자, 교육단체, 대중매체, 사기업, 봉사단체 등을 포함한 건강분야의 조직들은 자신들의 전문적 기술들을 한데 모아 서로 협력하여야 한다. 또 하나의 중요한 수단은 건강 지지적인 환경이다. 개인의 건강증진 노력을 체계화함으로써 지지적인 환경을 조성할 수 있다. Healthy Japan 21에 대한 평가는 2005년에 중간평가가, 2010에 최종평가가 있을 예정이다. 평가결과들은 이후에 진행될 사업의 향상을 위한 기준으로 활용될 예정이다.

Effects of Walking and Behavior Modification Program on Body Composition, Physical Fitness and Metabolic Syndrome Related Factors in Obese Girls (걷기운동과 행동수정 프로그램이 비만여중생의 체조성, 체력 및 대사증후군 관련인자에 미치는 영향)

  • The purpose of this study was to provide obese middle school girls with walking and behavior modification program(WBMP) by investigating effects of the exercise on their body composition(BC), physical fitness(PF) and metabolic syndrome(MS). Analysis result of the difference between before and after the exercise(paired and independent t-test) was acquired by measuring BC, PF and MS for 12 weeks(60min/day, three a week). 24 Volunteers were 14 years over ${\geq}223.64kg/m^2$ and 15 years over ${\geq}24.00kg/m^2$. In the body composition, waist circumference(WC), body fat mass(BFM), %fat, arm muscle circumference(AMC), total body water(TBW), physical score(PS) were significant difference between the exercise group(EG) and the control group(CG), also including all factors in PF. The components of metabolic syndrome were significant difference between EG and CG, but HDL-C, systolic & diastolic blood pressure were no difference. The metabolic syndrome factors by separate of waist girth were no difference between EG and CG, but BMI, diastolic blood pressure were significant difference. The prevalence of metabolic syndrome was 4 of 24(16.7%) before intervention, but it dropped from 2 to 1 after intervention in WBMP. The components of metabolic syndrome was distributed a big WC>low HDL-C>high TG>hypertension. The changes of AMC, We, PS, BW, 50m run and BFM effect the factors on BMI(35%), WHR(waist-hip ratio, 69%), HDL-C(42%), fasting blood glucose(65%), HOMA-IR index(34%) and systolic blood pressure(39%) respectively. There were significantly increased in breakfast frequency, walking steps, and meal eating period, but decreased in snacks and eating out in exercise group.

Natural Origin Polymers: Applications as Wound Care Materials (자연 고분자 : 상처 치료 재료로 활용)

  • Wound care is a health industry concern affecting millions worldwide. Recent increase in metabolic disorders such as diabetes comes with elevated risk of wound-based complications. Treatment and management of wounds are difficult practices due to complexity of the wound healing process. Conventional wound dressings and treatment applications only provide limited benefits which are mainly aimed to keep wound protected from external factors. To improve wound care, recent developments make biopolymers to be of high interest and importance to researchers and medical practitioners. Biopolymers are polymers or natural origin produced by living organisms. They are credited to be highly biocompatible and biodegradable. Currently, studies reported biopolymers to exhibit various health beneficial properties such as antimicrobial, anti-inflammatory, hemostatic, cell proliferative and angiogenic activities which are crucial for effective wound management. Several biopolymers, namely chitosan, cellulose, collagen, hyaluronic acid and alginic acid have been already investigated and applied as wound dressing agents. Different derivatives of biopolymers have also been developed by cross-linking with other molecules, grafting with other polymers, and loading with bioactive agents or drugs which showed promising results towards wound healing without any undesired outcome such as scarring and physiological abnormalities. In this review, current applications of common biopolymers in wound treatment industry are highlighted to be a guide for further applications and studies.

Recent Studies on Natural Products that Improve Browning (Browning 촉진에 관여하는 최근 천연물의 동향)

  • The prevalence of obesity is increasing worldwide, and since obesity is associated with dietary factors and sedentary lifestyles, it is a disease that is readily developing in the modern population. Because obesity is accompanied by serious complications such as diabetes and cardiovascular disease, prevention and treatment are important. Currently, drugs such as liraglutide and phentermine are used to treat obesity by suppressing appetite and inducing gastrointestinal motility delay. However, various side effects may occur, including thyroid cancer, cardiovascular problems, and central nervous system disorders. Therefore, to explore an obesity treatment method with relatively few side effects, a method known as "fat browning" was introduced to change white adipose tissue into brown adipose tissue to increase energy consumption. Ongoing studies are attempting to find effective natural substances to safely induce browning. Many natural substances have been identified. The induction of browning by treatment with natural substances generally involves three mechanisms: positive control of browning-inducing factors, inhibition of differentiation into white adipose tissue, and the activation of mechanisms related to browning. In this study, we describe plant extracts with known browning-inducing effects, such as strawberry, black raspberry, cinnamomum cassia, and Ecklonia stolonifera extracts. We also summarize the underlying mechanisms of action identified thus far, including the signaling pathway mediated by these extracts to induce browning. Furthermore, the effects of brown adipose tissue generated through browning on heart disease as an endocrine organ disruptor are discussed.

Association between Hypertriglyceridemic-Waist Phenotype and Metabolic Abnormalities in Hypertensive Adults (고혈압자의 고중성지방혈증-허리 표현형과 대사이상 사이의 연관성)

  • The hypertriglyceridemic-waist (HTGW) phenotype can predict the risk of coronary artery disease. This study evaluated the relationship between the HTGW phenotype and metabolic abnormalities in hypertensive adults. The test was conducted on adult hypertension patients aged 20 or older who underwent health checkups from January 2018 to December 2021 at general hospitals in Gyeonggi Province. The HTGW phenotype was triglyceride ≥150 mg/dL and waist circumference ≥90 cm for men and ≥85 cm for women. The prevalence of the HTGW phenotype in the study subjects was 17.9%. After adjusting for age, gender, and BMI, the odds ratio of the HTGW group was 5.09 (95% confidence interval [CI]: 3.545~7.309) for low HDL-cholesterol, 1.68 (95% CI: 1.176~2.411) for high LDL-cholesterol, was 2.92 (95% CI: 2.009~4.235) for high total cholesterol, 3.39 (95% CI: 2.124~5.412) for diabetes and 1.85 (95% CI: 1.286~2.674) for hyperuricemia compared to the normal triglyceride levels with the normal waist (NTNW) group. The area under curve values of the HTGW phenotype for diagnosing metabolic syndrome were 0.849 for all subjects, 0.858 for men, and 0.890 for women. In conclusion, the HTGW phenotype is closely related to metabolic abnormalities and is a useful indicator for monitoring adult hypertensive patients with metabolic syndrome.

Nutritional Status and Health Risks of Low Income Elderly Women in Gwangju Area (광주지역 저소득층 여자노인의 영양상태와 건강위험요인에 관한 연구)

  • This study was performed to identify association between nutritional status and health risks of the elderly. This was a cross-sectional study involving low income elderly women in Gwangju, Korea (${\geq}$65y, n = 92). Socio-demographics, life style characteristics, health conditions, dietary intakes based on 24h-recall method, anthropometric measures, and clinical biochemistry parameters were examined. Anthropometric and clinical parameters included wt, ht, waist, hip, body protein, body fat, abdominal fat, total cholesterol, HDL-cholesterol, triglyceride, total protein, albumin, hemoglobin, hematocrit, fasting blood glucose, ferritin, IL-2, IL-6, TNF-${\alpha}$, CRP, TAS, TBARS, systolic blood pressure, and diastolic blood pressure. The subjects were divided into three groups based on age (65-74y, 75-84y, 85y${\leq}$) and were divided into two groups according to the sum of the Nutrition Screening Initiative (NSI) checklist score (adequate nutritional status, NSI score ${\leq}$3; at risk of malnutrition, NSI score >3). Mean and frequency of variables were estimated. Analysis of Variance, Tukey test, Chi-square test, and Multiple linear regression analyses were performed. Mean BMI and body fat were 25.1 $kg/m^2$ and 40.0%, respectively. However, for over 80% of subjects, the intakes of energy, fiber, thiamin, riboflavin, niacin, folate, Ca, K, and Zn were less than the Korean DRI (EAR or AI). The subjects who had lower NSI score tended to have better health status, eat meals frequently, have less depression, and exercise regularly. The subjects who had higher NSI score tended to have tooth problems, to eat alone most of time, and to be physically unable to cook or feed. Serum IL-6 and TNF-${\alpha}$ were significantly related with nutritional status which suggested higher tendency of inflammatory response. Serum IL-2, TAS, and glucose were significantly correlated with body fat (%) or abdominal fat (%). These results suggest that improving the nutritional status, increasing regular exercise, maintaining normal weight are beneficial to health care of low income elderly women.

The Clinical Characteristics of Endobronchial Anthracofibrotic Pigmentation (기관지 탄분 섬유화증의 임상양상)

  • Objectives : Endobronchial anthracofibrotic pigmentation, which presents as dark black or brown pigmentation mucosal changes of multiple bronchi combined with bronchial fibrosis and obstruction, is not a rare finding when performing diagnostic bronchoscopy for Koreans. This study was performed to define the clinical characteristics and to determine the association of these finding with the Korean life style and such other diseases as coal workers, pneumoconiosis or tuberculosis in the patients with anthracofibrotic pigmentation. Methods : This retrospective analysis was conducted on 70 (5.2%) patients with endobronchial anthracofibrotic pigmentation, among a total of 1340 patients who underwent bronchoscopy. The distinctive clinical features, the personal life style, the past medical history, the histology and microbiology, the radiologic finding and the natures of the bronchoscopic lesions were analyzed. Results : This mean age of the patients with anthracofibrotic pigmentation was $60.6{\pm}9.2$ year old and the male to female ratio was 1:1.7. The common respiratory symptoms of these patients were coughing and sputum (81%, 57/70), and this was followed in order by dyspnea and hemoptysisir. The symptoms were not related with smoking and an occupational history such as being a coal worker and so on. Pneumonia was most common finding on the radiologic studies. On bronchoscopy, the right middle lobe bronchus was most commonly involved. The most common associated disease was tuberculosis, and 40 cases (57.1%) were diagnosed by AFB staining, TB PCR, bronchoscopic guided tissue biopsy and a past history of tuberculosis. Other diseases related with anthracotic pigmentation were hypertension, diabetes, COPD, lung cancer, pneumoconiosis and asthma. Conclusion : These results suggest that endobronchial anthracofibrotic pigmentation was mostly related with pulmonary tuberculosis rather than with coal- related disease. Endobronchial anthracofibrotic pigmentation was more prevalent in older age females in Korea.

Health Conditions and Health Behaviors of Merchants at Traditional Markets (전통시장 상인의 건강상태와 건강행태)

  • This cross-sectional descriptive study was conducted to determine the health conditions and health behaviors of merchants at traditional markets and compare them with members of the general population[ED highlight - do you mean "members of the general population"?]. The data were collected from 307 merchants at N traditional market in C City from 1-20 June 2015 at D Health Center of C City after physical measurement and blood collection and then analyzed using SPSS WIN 21.0. The male, female, and overall smoking rates of merchants at the traditional market were 34.2%, 3.5% and 11.1%, the annual drinking rates were 68.4%, 38.5%, and 45.9%, and the rates of subjects under stress and the rates of experiencing depression were 23.7%, 28.5%, and 27.4% and 5.3%, 6.1%, and 5.9%, respectively, which were all relatively lower than members of the general population[ED highlight - please ensure that my changes here are in keeping with your intended meaning.]. The male, female and overall rates of medium-level physical activities and walking were 32.9%, 19.9%, and 23.1% and 85.5%, 79.2%, and 80.8%, respectively, to have relatively good health behaviors, and the mental health is relatively fine than members of the general population[ED highlight - please clarify this, I cannot infer your intended meaning.]. Relationship of sodium consumption with obesity in Korean adults based on Korea National Health and Nutrition Examination Survey 2010~2014 (2010년~2014년 국민건강영양조사 자료를 이용한 성인의 나트륨 섭취와 비만과의 관련성)

  • Purpose: Excess sodium intake may contribute to the etiology of hypertension and cardiovascular disease risk. World Health Organization (WHO) recommends a daily sodium intake of less than 2 g. The aim of this study was to estimate the association of sodium intake with obesity in Korean adults. Methods: This study used Dietary intake and Health data on 22,321 subjects aged 30 years and over from the Korea National Health and Nutrition Examination Survey (KNHANES) 2010~2014. Information on dietary intake was obtained by the one day 24-hour recall method in KNHANES, and sodium intake was classified into five groups (< 2,000 mg, 2,000~4,000 mg, 4,000~6,000 mg, 6,000~8,000 mg, ${\geq}8,000mg$). Obesity was defined as having a body mass index (BMI) higher than $25kg/m^2$. Intake of sodium and obesity status were analyzed by logistic regression with SPSS Statistics 23. Results: Men tended to have a higher sodium intake than women (p < 0.001). After adjusting for age, sex, year, daily energy intake, education level, smoking status, drinking status, physical activity, and chronic diseases and comparing the highest sodium intake group (${\geq}8,000mg$) with the lowest intake group (< 2,000 mg), the OR of obesity was 1.351 (95% CI: 1.032~1.767) in men. The OR of obesity in the sodium intake group (4,000~6,000 mg) was 1.232 (95% CI: 1.063~1.427) in women. Conclusion: Our findings suggest an independent relationship between sodium intake and as increased risk of obesity in Korean adults, implying the necessity for future research on low-sodium diet intervention in relation to obesity.

Comparison of the health and nutritional status of Korean elderly considering the household income level, using the 2018 Korea National Health and Nutrition Examination Survey (가구소득수준에 따른 남녀 노인의 건강 및 영양섭취 실태 비교: 2018년 국민건강영양조사 자료를 이용하여)

  • Purpose: This study examined the dietary behavior, health status and nutrient intake by considering the level of household income of elderly people using data obtained from the Korea National Health and Nutrition Survey (KNHANES VII) 2018. Methods: The study subjects were 1,355 elderly people over 65 years old (558 men, 797 women). Based on their household income, participants were classified into three groups: low-income, middle-income, and high-income. The variables consisted of general characteristics, dietary behavior, health status, health related behavior, and dietary intakes. Dietary data were estimated by the 24-hour dietary recall. Results: In men, the low-income group encompassed older, less educated, less employed, and living with family of first generation. However, in women, there were no differences in employment by the level of income, and women living alone had lower income than subjects living with family. Elderly men in the high-income group had a significantly higher level of nutrient intake (energy, protein, fat, phosphorous, riboflavin, niacin and vitamin C). Men in the low-income group consumed a significantly lower intake of fruits, seaweeds and eggs, including total food. Women in the low-income group had significantly less intake of protein, fat, calcium, phosphorous, iron, vitamin A, riboflavin, and niacin whereas women in the high-income group had significantly higher intake of sugar & sweet, eggs, and beverages, including total food. Conclusion: These results suggest that the level of household income is an important factor that influences food and nutrient intake in the Korean elderly. The socioeconomic status needs to be considered differently among elderly men and women when implementing food assistant programs and designing nutrition education programs.
