  • fNIRS(functional Near Infrared Spectroscopy)는 비침습형 뇌기능 분석 시스템으로 뇌활성화 시 옥시 헤모글로빈(oxy-hemoglobin)과 디옥시헤모글로빈(deoxy-hemoglobin) 변화량을 측정할 수 있는 장치이다. 본 논문에서는 뇌기능 분석 장치인 fNIRS를 이용하여 피험자가 컴퓨터 게임 중에 어떤 뇌활성화 패턴을 보이는지를 실험하였다. 컴퓨터 게임 주의 및 집중 시 뇌의 전두엽(Frontal Lobe) 영역이 활성화 및 변화되는 것을 실험결과로 확인하였다. 그리고 게임 중 다른 사람이 피험자에게 개입을 하였을 때 전두엽의 활성화 영역이 다른 패턴을 보이는 것을 실험결과로 확인하였다.

Functional Brain Mapping Using $H_2^{15}O$ Positron Emission Tomography ( II ): Mapping of Human Working Memory ($H_2^{15}O$ 양전자단층촬영술을 이용한 뇌기능 지도 작성(II): 작업 기억의 지도 작성)

  • Purpose: To localize and compare the neural basis of verbal and visual human working memory, we performed functional activation study using $H_2^{15}O$ PET. Materials and Methods: Repeated $H_2^{15}O$ PET scans with one control and three different activation tasks were performed on six right-handed normal volunteers. Each activation task was composed of 13 match-ing trials. On each trial, four targets, a fixation dot and a probe were presented sequentially and subject's task was to press a response button to indicate whether or not the probe was one of the previous targets. Short meaningful Korean words, simple drawings and monochromic pictures of human faces were used as matching objects for verbal or visual memory. All the images were spatially normalized and the differences between control and activation states were statistically analyzed using SPM96. Results: Statistical analysis of verbal memory activation with short words showed activation in the left Broca's area, promoter cortex, cerebellum and right cingulate gyrus. In verbal memory with simple drawings, activation was shown in the larger regions including where activated with short words and left superior temporal cortex, basal ganglia, thalamus, prefrontal cortex, anterior portion of right superior temporal gyrus and right infero-lateral frontal cortex. On the other hand, the visual memory task activated predominantly right-sided structures, especially inferior frontal cortex, supplementary motor cortex and superior parietal cortex. Conclusion: The results are consistent with the hypothesis of the laterality and dissociation of the verbal and visual working memory from the invasive electrophysiological studies and emphasize the pivotal role of frontal cortex and cingulate gyrus in working memory system.

The Serial Change of Cerebral Hemodynamics by Vascular Territory after Extracranial-Intracranial Bypass Surgery in Patients with Atherosclerosis of Cerebral Arteries (죽상 동맥 경화성 뇌혈관 폐색 환자에서의 두개외강-내강 우회로술 후의 혈관 영역별 연속 혈류역학 변화)

  • Purpose: To assess the effect of extracranial-intracranial (EC-IC) bypass surgery on hemodynamic improvement, we evaluated serial regional cerebral hemodynamic change of the middle cerebral artery (MCA) in symptomatic patients with atherosclerotic occlusion of the internal carotid artery (ICA) or MCA using $^{99m}Tc$-ECD acetazolamide stress brain perfusion SPECT (Acetazolamide SPECT). Materials and Methods: The patients who had suffered a recent stroke with atherosclerotic ICA or MCA occlusion underwent EC-IC bypass surgery and Acetazolamide SPECT at 1 week before and three to six months after surgery. For image analysis, attenuation corrected images were spatially normalized to SPECT templates with SPM2. Anatomical automated labeling was applied to calculate mean counts of each Volume-Of-Interest (VOI). Seven VOIs of bilateral frontal, parietal, temporal regions of the MCA territory and the ipsilateral cerebellum were defined. Using mean counts of 7 VOIs, cerebral perfusion index and perfusion reserve index were calculated. Results: Seventeen patients (M:F =12:5, mean age $53{\pm}2yr$) were finally included in the analysis. The cerebral blood flow of the parietal region increased at 1 week (p = 0.003) and decreased to the preoperative level at 3-6 months (p = 0.003). The cerebrovascular reserve of the frontal and parietal regions increased significantly at 1 week after surgery (p<0.01) and improved further at 3-6 months. Conclusion: Cerebrovascular reserve of the MCA territory was significantly improved at early postoperative period after EC-IC bypass and kept improved state during long-term follow-up, although cerebral blood flow did not significantly improved. Therefore, cerebrovascular reserve may be a good indicator of postoperative hemodynamic improvement resulted from bypass effect.

  • Tract-Based Spatial Statics (TBSS) after obtaining the image by examining a diffusion tensor image that can determine the presence or absence of damage to the cerebral white matter and gray matter for middle-aged men aged 30 to 50 with the starting age of drinking as a variable. As a result of measuring and analyzing the FA (fractional anisotropy) value of the brain gray matter region to the hippocampal region nerve fibers, the lower the alcohol start age in all regions, the lower the anisotropy measurement value, but the FA value was statistically significant. The study results indicated by the FA results measured in this study are that the earlier the drinking start age, the more severe the morphological changes in all neurological and anatomical brain regions in the hippocampal region of the brain gray matter and seriously affect the nerve fiber tissue. It can be said to harm and damage nerve fibers and affect functional morphological variations associated with alcohol.

Quantification of Cerebrovascular Reserve Using Tc-99m HMPAO Brain SPECT and Lassen's Algorithm (Tc-99m HMPAO 뇌 SPECT와 Lassen 알고리즘을 이용한 뇌혈관 예비능의 정량화)

  • Purpose: For quantitative estimation of cerebrovascular reserve (CVR), we estimated the cerebral blood flow (CBF) using Lassen's nonlinearity correction algorithm and Tc-99m HMPAO brain SPECT images acquired with consecutive acquisition protocol. Using the values of CBF in basal and acetaBolamide (ACZ) stress states, CBF increase was calculated. Materials and Methods: In 9 normal subjects (age; $72{\pm}4$ years), brain SPECT was performed at basal and ACZ stress states consecutively after injection of 555 MBq and 1,110 MBq of Tc-99m HMPAO, respectively. Cerebellum was automatically extracted as reference region on basal SPECT image using threshold method. Assuming basal CBF of cerebellum as 55 ml/100g/min, CBF was calculated lot every pixel at basal states using Lassen's algorithm. Cerebellar blood flow at stress was estimated comparing counts of cerebellum at rest and ACZ stress and Lassen's algorithm. CBF of every pixel at ACZ stress state was calculated using Lassen's algorithm and ACZ cerebellar count. CVR was calculated by subtracting basal CBF from ACZ stress CBF for every pixel. The percent CVR was calculated by dividing CVR by basal CBF. The CBF and percentage CVR parametric images were generated. Results: The CBF and percentage CVR parametric images were obtained successfully in all the subjects. Global mean CBF were $49.6{\pm}5.5ml/100g/min\;and\;64.4{\pm}10.2ml/100g/min$ at basal and ACZ stress states, respectively. The increase of CBF at ACZ stress state was $14.7{\pm}9.6ml/100g/min$. The global mean percent CVR was 30.7% and was higher than the 13.8% calculated using count images. Conclusion: The blood flow at basal and ACZ stress states and cerebrovascular reserve were estimated using basal/ACZ Tc-99m-HMPAO SPECT images and Lassen's algorithm. Using these values, parametric images for blood flow and cerebrovascular reserve were generated.

  • 현존하는 정보처리 시스템 중에서 가장 뛰어난 성능을 지니고 있는 것은 인간의 두뇌라고 할 수 있다. 두뇌의 정보처리 메커니즘을 보다 정확하게 구현할 수 있는 시스템은 입력에 대한 정확한 인지 능력, 상황 판단 능력, 학습 및 추론 능력, 출력의 결정 능력 등의 성능 구현은 물론이며, 감정과 비교될 수 있는 시스템의 상태를 평가하여 판단 및 결정에 적용함으로써 매우 뛰어난 지능형 시스템이 쥘 수 있다. 이러한 뇌 정보처리 시스템의 구현에 앞서 본 논문에서는 생물학적인 대뇌 피질의 구조를 살피고 정보의 처리 영역을 고찰하고 정보의 흐름을 소개하였으며 이를 바탕으로 뇌 정보처리 메커니즘을 공학적인 측면에서 해석해 보았다. 특히, 뇌 영역의 기능 및 구조적인 특징, 정보의 처리과정 등을 공학적으로 해석하였으며 이는 뇌의 기능을 모방한 공학적인 모델을 구현하는데 있어서 기초가 될 것이다.

Brain Atrophy and White Matter Lesions on Magnetic Resonance Imaging in Alzheimer's Disease (알쯔하이머 치매 환자의 뇌자기공명영상(腦磁氣共鳴影像)에 나타난 뇌위축(腦萎縮)과 뇌백질병변(腦白質病變)에 대한 연구)

  • Objects : To investigate the relationship between the age of onset with the atrophy and the white matter hyperintensities observed in the brain MRI of Alzheimer patients. Methods : The authors measured volumetrically cortical and ventricular brain atrophy and rated semiquantitatively white matter signal hyperintensities in nine presenile and 18 senile Alzheimer patients, who were matched for dementia severity, according to NINCDS-ADRDA criteria and in age-matched 10 presenile and 11 senile control subjects. Results : Presenile Alzheimer patients showed significantly greater cortical and ventricular atrophy indices(p<0.05) but no difference in white matter hyperintensity scores compared to the age-matched control group. On the contrary, senile Alzheimer patients showed significantly greater white matter hyperintensity scores(p<0.05) but no difference in cortical and ventricular atrophy indices compared to the age-matched control group. Conclusion : An earlier onset was related to marked brain atrophy with less white matter lesions and a later onset is related to marked white matter lesions with less brain atrophy in Alzheimer's disease. Our results suggested the passible difference in the pathophysiology between the presenile and the senile-onset Alzheimer's disease.

Evaluation of Hemodynamic Failure with Acetazolamide Challenged $^{123}I-IMP$ Brain SEPCT and PET (PET과 Acetazolamide 부하 $^{123}I-IMP$ 뇌혈류 SPECT를 이용한 혈역학적 부전의 평가)

  • Purpose : Cerebral blood flow (CBF) reactivity to acetazolamide (ACZ) is useful to select patients with hemodynamic failure. However, it is still a matter of speculation that varying degrees of regional CBF increases after ACZ administration represent the severity or stage of regional hemodynamic failure as assessed by positron emission tomography (PET). We studied to elucidate whether ACZ challenge $^{123}I-IMP$ brain single photon emission tomography (SPECT) can accurately grade the seventy of regional hemodynamic failure. Materials and Methods: Eighteen patients (M: 16, F: 2, average age: 61 years) with unilateral occlusive disease of the internal carotid artery or the trunk of the middle cerebral artery (MCA). Patients undewent $^{123}I-IMP$ brain SPECT study with acetazolamide challenge and PET study was carried out within 2 weeks before and after SPECT study. Five healthy volunteers with a mean age of 48 years (range: 28-73 yr, M: 3, F: 2) underwent PET studies to determine normal values. In SPECT study, an asymmetry index (Al)-the percentage of radioactivity of region of interest (ROI) in the occlusive cerebrovascular lesion to the contralateral homologous ROI-was used for numerical evaluation of relative $^{123}I-IMP$ distribution. In PET study, regional CBF, oxygen extraction fraction (OEF), cerebral metabolic rate of oxygen ($CMRO_2$) and cerebral blood volume (CBV) values were measured with $^{15}O-labeled$ gas inhalation method and the values were used for comparison with Al (Al during acetazolamide challenge-Al of basal study) on the SPECT study. ROls were classified by severity into three groups (normal, stage I and stage II). Results: Mean values of Al in areas with normal, stage I and stage II hemodynamic failure were $6.25{\pm}7.77%\;(n=107),\;-10.38{\pm}10.41%\:(n=117)\;and\;13.30{\pm}10.51%\;(n=140)$, respectively. Al significantly differed with each groups (p<0.05). Correlation between Al and CBF, OEF and CBV/CBF in hemisphere with occlusive cerebrovascular lesion was 0.20 (p<0.01), -0.28 (p<0.01) and -0.28 (p<0.01), respectively. Conclusion: We concluded that $^{123}I-IMP$ brain SPECT with acetazolamide challenge could determine the severity ad stage of regional hemodynamic failure as assessed by PET.

Efficacy Assessment of Endovascular Stenting in Patients with Unilateral Middle Cerebral Artery Stenosis Using Statistical Probabilistic Anatomical Mapping Analysis of Basal/Acetazolamide Brain Perfusion SPECT (기저/아세타졸아미드 국소뇌혈류 SPECT의 확률 뇌지도 분석을 이용한 일측 중대뇌동맥 협착환자에서 시행한 스텐트 삽입술의 효용성 평가)

  • Purpose: The aim of this study was to evaluate the hemodynamic changes after endovascular stenting in patients with unilateral middle cerebral artery (MCA) stenosis using statistical probabilistic anatomical mapping (SPAM) analysis of basal/acetazolamide (ACZ) Tc-99m ECD brain perfusion SPECT. Materials and Methods: Eight patients (3 men and 5 women, 64.8$\pm$10.5 years) who underwent endovascular stenting for unilateral MCA stenosis were enrolled. Basal/ACZ Tc-99m ECD brain perfusion SPECT studies were performed by one-day protocol before and after stenting. Using SPAM analysis, we compared basal cerebral perfusion (BCP) counts and cerebrovascular reserve (CVR) index of the MCA territory before stenting with those after stenting. Results: After stenting, no patient had any complication nor additional stroke. In SPAM analysis, 7 out of the 8 patients had improved BCP counts of the MCA territory and 7 out of the 8 patients had improved CVR index of the MCA territory after stenting. Before stenting, the mean BCP counts and CVR index in the affected MCA territory were 47.1$\pm$2.2 ml/min/100 g and -2.1$\pm$2.9%, respectively. After stenting, the mean BCP counts and CVR index in the affected MCA territory were improved significantly (48.3$\pm$2.9 ml/min/100 g, p =0.025 and 0.1$\pm$1.3%, p =0.036). Conclusion: This study revealed that SPAM analysis of basal/ACZ brain perfusion SPEG would be helpful to evaluate hemodynamic efficacy of endovascular stenting in unilateral MCA stenosis.
