A Study on the Interregional Actual State and Influential Factors after the Application of Denture Insurance (틀니 보험적용에 따른 지역간 차이와 이용 영향 요인 연구)

  • The purpose of this study is to analyze the factors influencing the regional differences and the denture use behavior after the elderly denture health insurance benefits for the elderly over 65 years old in Seoul and Gangwon provinces. The data was used in the "2015 Community Health Survey". The results of the study are as follows; 1) There is a significant difference in denture use among elderly in Seoul and elderly in Gangwon area. 2) There is significant differences in denture use among the elderly in the Gangwon area. According to the age, household income, basic living conditions, occupation, education level and subjective oral health, There is significant differences in all of 2 areas. 3) Some 16.6% of the elderly in Seoul and some 22.3% of the elderly in Gangwon province answered 'yes'. The reasons for not having the required dental care were the most economical reasons in all of 2 areas. Therefore, the elderly denture health insurance benefit project considering various aspects is necessary.

A Study in Terms of the Impact of Oral Health Care·Management item on the Self-Perceived Oral Condition of the Elderly (구강보건진료·관리 항목이 노인의 자가인식구강상태에 미치는 영향력 연구)

  • This study targeted 501 the elderly ages 65 or over in three places including the Senior Welfare Center and the college for seniors located in Daegu City and Gyeongsangbuk-do. The study results on the preference for oral health care management items across different ages showed that the age group of 65~69 fluoride application; the age group of 65~74 scaling and education of brushing; the age group of 70~74 denture cleaning and maintenance; the age group of 75 denture production. In the relationship between the self-perceived oral condition and the items of oral health care management, respondents who currently have a poor oral condition or have greater chewing trouble, or have higher dry mouth, preference the manufacturing of dentures in most cases. Respondents who have a lower number of teeth or wear dentures preference dentures cleansing and management in most cases. Respondents who have shaking teeth or have frequent tooth pain preference tooth extraction. Since the preference for oral health care management items may differ by their age or oral condition, oral health management customized to each individual should be applied.

A Study on the Recognition about National Health Insurance Coverage of Denture, Implant of Elderly People (일부 노인층의 틀니, 임플란트 건강보험에 대한 인식도 연구)

  • The purpose of this study was to investigate the recognition on the national health insurance of denture, implant among the elderly. This survey was performed on 238 of the elderly aged over 60 years in Daejeon. The research was performed using a self-reported questionnaire and interview method from June to July, 2014. The collected data was analyzed using chi-square test, multiple response frequencies by PASW Statistics ver. 18.0. Recognition of national health insurance denture coverage was 76.9%. Channel of information awareness is higher in the media (61.8%). Awareness of application time (36.4%), medical expense by insurance (43.2%) is generally low. And awareness of denture follow up management is significantly low (18.6%). Time of denture and implant coverage needs were over 60 and 65 years old respectively. The respondents want the national health insurance to help medical expenses over 50%. Period of implant and denture re-production required unlimitedness 32.0% and 47.8%, participation to oral hygiene (dentures) management by dental hygienist was 94.1%. In conclusion, denture and implant coverage was higher awareness, but details were not recognized. Therefore, we should provide more detailed information. To increase the efficiency of national health insurance should be considered to lower the coverage age.

Recognition on national health insurance coverage and the actual condition of denture among the elderly at senior welfare centers in Seoul (ORIGINAL ARTICLE - 노인틀니 보험화에 대한 인식과 틀니 사용실태 조사 - 서울특별시 일부 노인복지관이용 노인을 대상으로 -)

  • The aim of this study was to investigate the recognition on the national health insurance and the actual condition of denture among the elderly in Seoul city. This survey was performed on 710 of the elderly aged over 60 years and visited the senior welfare centers. The recognition of the elderly on the national health insurance of denture was 61.7%, but there was a low recognition on the details. The elderly had started using dentures from 66.24 years old. The average used period of the past denture was 7.09 years. In conclusions, the provider is required to promote the insurance coverage of denture to increase the coefficient of utilization of the elderly. Also, further studies for the extending coverage of the details are needed.

Knowledge of the dental health insurance standard in the dental hygienists: based on the elderly denture (치과위생사의 치과건강보험 산정기준에 대한 지식조사: 노인틀니 중심으로)

  • Objectives: The purpose of the study is to investigate the knowledge of the dental health insurance standard of elderly denture in the dental hygienists. Methods: A self-reported questionnaire was completed by 183 dental hygienists in Busan and Gyeongnam. The questionnaire consisted of general profiles of the subjects(11 items) and 10 items of the dental health insurance standard of the elderly denture including subject of spplication, application method, calculation of range, and temporary denture. Each question was measure by Likert 5 points scale. Cronbach's alpha was 0.936 in the study. The data were analyzed using SPSS 20.0 program. Results: The knowledge of the dental health insurance standard on the elderly denture in the dental hygienists was high in cognition of the renewed standard change(p<0.001), individual career(p<0.05), and recognition of the current standard(p<0.05). Conclusions: It is continuously necessary to inform the dental hygienists of the knowledge of dental health insurance standard. The web-based program for the dental health insurance education should be provided continuously for the dental hygienists.

Factors Related to Subjective Oral Health Level of the Elderly (노인의 주관적 구강건강수준 관련 요인)

  • This study was conducted to investigate the effects of oral care behavior, oral health care self-efficacy, and social support on the subjective oral health level of the elderly. The data survey was conducted from September 17, 2019 to November 22, 2019 for the elderly living in Daejeon Metropolitan City and parts of Chungcheongnam-do, and the collected data were analyzed by 𝑥2-test, Pearson correlation analysis, and logistic regression analysis. As a result of the survey, subjective oral health level increased by 3.242 times when dentures were not used and 2.339 times when the number of brushings per day was 3 or more times. In addition, as the oral health care self-efficacy and social support increased 1.755 times and 1.192 times, respectively, the subjective oral health level also increased. Based on the above results, in order to improve the subjective oral health level of the elderly, opportunities for oral health education such as denture care methods and toothbrushing lessons should be expanded. In addition, there is a need to prepare policies to reinforce oral health care self-efficacy and social support.

Association between dentures use and prevalence of depression in elderly (노인 틀니와 우울증 유병률과의 관계)

  • Objectives: This study aimed to assess the association between the use of dentures by the elderly and the prevalence of depression by using the original data from the 2017 Community Health Survey as basic data for the promotion of oral health and oral health policy development in the elderly. Methods: The study subjects were 67,835 elderly over 65 years of age. The raw data were collected from 2017 Community Health Survey in Korea Center for Disease Control and Prevention. Results: Among the demographic and sociological characteristics, the higher the age, the lower the educational level, the worse the subjective health level, and the higher the denture use. The higher the age, the lower the education level, the lower the income level, the worse the subjective health level, the more stress they have, the more sleep time, and the higher the prevalence of depression. The association between the use of dentures and the prevalence of depression was 1.071 times higher than that of not using dentures, and it was found that the prevalence of depression was affected. Conclusions: As a result, it was confirmed that the use of dentures influenced the prevalence of depression. Institutional support will be needed to establish a national-level prevention policy that can maintain and promote healthy oral conditions and prevent the need dentures in the elderly.

Dental Utilization Associated Factors among Elderly (노인의 치과의료서비스 이용에 영향을 미치는 요인)

  • The purpose of this study was to examine the relationship of predisposing, enabling, need factors and oral health care factors to the dental utilization in 4,521 senior citizens based on the 5th National Health and Nutrition Examination Survey data. As for the relationship of predisposing, enabling and need factors to the necessity of denture, higher academic credential and higher income level that were respectively one of predisposing and need factors led to a higher necessity of denture, and the relationship of these factors was statistically significant. As to influential factors for their dental utilization in Model 1, there were significant differences according to gender, marital status and whether to subscribe to private health insurance or not. In Model 2, the need factors of Model 1 were adjusted. As a result, the respondents who didn't receive any unsatisfactory dental treatment made 1.35-fold more dental utilization, and the respondents who complained about mastication difficulty made 1.34-fold more dental utilization. There were significant differences according to gender, age, marital status, academic credential, whether to subscribe to private health insurance, unsatisfactory dental treatment experience and mastication difficulty. Age, unsatisfactory dental treatment experience and whether to complain about mastication difficulty or not made statistically significant differences to the dental utilization in Model 3 that involved oral health status. The above-mentioned findings illustrated that the predisposing factors, the enabling factors and the need factors exerted an influence on the elderly dental utilization. As there are a variety of factors to affect elderly dental utilization, its required to make an effort to boost the accessibility of the elderly to dental service in order to improve their oral health of the elderly.

A study on the recognition about national health insurance coverage of implant, denture of dental workers (치과의료종사자들의 임플란트, 노인 틀니 건강보험에 대한 인식도 연구)

  • Objectives: This study aims to provide data that will improve the scope of national health insurance coverage by surveying the awareness of health insurance benefits, specifically for implants and dentures, among dental workers. Methods: Information was collected through questionnaires completed by 194 dental workers at dental hospitals and clinics. The multiple logistic regression analysis was conducted to confirm influential factors in recognizing the health insurance benefits application for dentures and implants. Results: Regarding the awareness about the validity of health insurance benefits, satisfaction with the appropriateness on the subjects of the denture application and implant application are appeared to be high with each 3.369 and 3.673. Regarding satisfaction with the appropriateness of free post-maintenance for implants, the awareness level regarding the validity of health insurance benefits was indicated as high at 3.673. Conclusions: The categories and levels of appropriate insurance benefits must be restructured continuously.
