Correlation between Visceral Adipose Tissue and Stagnation of the Liver Qi (gan-yu, 肝鬱) in Korean Perimenopausal Women (한국 갱년기 여성의 간울과 내장비만의 상관성)

  • 목적 : 갱년기 여성은 내장비만의 위험이 높으며 또한 심리적 변화를 겪는 시기이다. 갱년기 여성의 복부지방, 내장지방, 피하지방 및 이와 관련된 단순비만지표, 간울증, 스트레스, 우울, 자존감 등을 조사하여 갱년기 건강의 위협이 되는 신체 심리 인자를 알아보고자 이 연구를 시행하였다. 방법 : 병원공고를 통해 체질량지수 $23kg/m^2$ 이상 비흡연자인 만 45세 이상 55세 이하의 폐경 전후의 갱년기 여성 환자를 모집하여 2007년 8월 20일부터 24일까지 47명이 모집되었으며 이중 체질량지수 $25kg/m^2$ 이상이면서 복부 전산화 단층촬영 상 내장지방이 $100cm^2$ 이상인 20명의 단순비만지표, 체성분검사, 혈액검사, 복부 전산화 단층촬영 및 한방비만변증 (간울) 및 스트레스, 우울, 자존감 설문검사 결과의 상관성을 분석하였다. 본 연구는 경희대학교 동서신의학병원의 임상연구심사위원회의 승인을 받았다. 결과 : 1. 갱년기 비만여성에서 복부지방 면적, 피하지방 면적은 체질량지수, 체지방률, 허리둘레/신장비 등의 단순비만지표와 유의한 상관성을 보인 반면 (p < 0.01), 내장지방 면적은 단순비만지표와 상관성을 보이지 않았다. 2. 내장지방 면적은 사회 재적응 평가척도 (${\gamma}=0.577$, p < 0.01)와 유의한 상관성이 있었고, 선형회귀분석에서 유의한 결과를 나타냈다. $VAT(cm^2)=116.1+0.101{\times}(SRRS\;score)$ (${\gamma}^2=0.332$) 3. 피하지방은 식이태도 점수와, 총복부지방은 식이태도 점수 및 갱년기지수 중 혈관운동증상과 상관성이 있었다. 4. 간울증은 스트레스 반응척도, 우울지수, 갱년기지수 및 그 하부항목 중 혈관운동, 정신, 운동, 소화, 전신증상과 양의 상관성을, 자존감척도와는 음의 상관성을 나타냈으며, 내장지방과 직접적인 상관성을 보이지 않았다. 결론 : 간울증은 높은 스트레스, 우울, 갱년기 증상 및 낮은 자존감과 관련되는 것으로 나타났다. 폐경전후 비만여성에서 내장지방 면적은 간울증과 유의한 상관성을 보이지 않았으나, 생활 스트레스 사건이 많을수록 높아지는 것으로 나타나 갱년기 여성 내장비만에서 스트레스에 대한 대처가 중요할 것으로 사료된다. 추후 임상적 연계성 및 설문 보완에 대한 연구가 필요하리라 생각된다.

The Usefulness of Visceral Fat Thickness Measured by Ultrasonography as an Abdominal Obesity Index (초음파로 측정된 내장지방두께의 복부 비만지표로서의 유용성)

  • Purpose : Abdominal obesity with visceral fat accumulation have been known to be intimately associated with the development of metabolic syndrome. Therefore, it is important to estimate the precise amount of visceral fat. Ultrasonography has been reported that it is a simple and noninvasive method for visceral fat evaluation. Purpose of this study is to evaluate the association of ultrasonographic visceral fat thickness, anthropometric indexes, and risk factor of metabolic syndrome, and to investigate the cut-off value of abdominal visceral fat thickness leading to increased risk of metabolic syndrome. Materials and methods : The subject included 200 men and 200 women who visited D healthcare center in Daejeon from January to April 2008. The subcutaneous fat thickness and visceral fat thickness were measured by ultrasonograph. As anthropometric index, we measured body mass index, waist circumference and waist/height ratio. As for the risk factor of metabolic syndrome, we measured blood pressure, high density lipoprotein cholesterol, triglyceride and fasting serum glucose. Results : VFT was significantly correlated with waist circumference, (r=0.683/M, r=0.604/F), waist to height ratio (r=0.633/M, r=0.593/F) and BMI (r=0.621/M, r=0.534/F) in both men and women. In addition it was significantly correlated with Systolic blood pressure (r=0.229/M, r=0.232/F), Diastolic blood pressure ((r=0.285/M, r=0.254/F), high density cholesterol (r=-0.254/M, r=-0.254/F), Triglyceride (r=0.475/M, r=0.411/F), and Fasting blood sugar (r=0.158/M, r=0.234/F) in both men and women. The cut-off value of visceral fat thickness leading to the increased risk of metabolic syndrome was 4.58cm (sensitivity89.2%, specificity 71.2%) in men and 3.50cm (sensitivity61.2% specificity 80.8%) in women respectively. The odds ratio of the risk of metabolic syndrome was dramatically increased with the abdominal visceral fat thickness level over 6cm in men and 5cm in women. Conclusion : The visceral fat thickness using ultrasonography was significantly correlated with anthropometric indexes and risk factors of metabolic syndrome in both men and women. The cut-off value of visceral fat thickness leading to the increased risk of metabolic syndrome was 4.58cm in men and 3.50cm in women.

The Effect of Nutrition Education on Visceral Fat Reduction and Diet Quality in Postmenopausal Women (폐경 여성의 내장지방 및 식사의 질에 미치는 영양 교육의 효과)

  • This study investigated the effects of the nutrition education on body weight, visceral fat and diet quality in the postmenopausal women. The subjects (n = 101) were randomly divided into two groups: Nutrition education + Exercise (NEE) group (n = 51) and Exercise only (EO) group (n = 50). Nutrition education was consisted of counseling in portion control, food selection for low carbohydrate, high fiber food items and for the improvement in micronutrient intakes and diet quality. After 6 months, the reduction in the body weight and visceral fat area was significantly greater in the NEE than in the EO group. The NEE subjects were further divided into two groups according to the amount of visceral fat area reduction; high visceral fat area loss (HVL) group with a visceral fat area reduction 2.35% or greater and low visceral fat area loss (LVL) group with a reduction less than 2.35%. In the HVL group, the reduction in body weight, BMI, percent body fat, waist to hip ratio and visceral fat area was significantly greater than that in the LVL group. We observed a significant increase in the serum HDL-cholesterol level and a decrease in systolic blood pressure, fasting blood sucrose, total and LDL-cholesterol levels in the HVL group compared to the LVL group. The energyadjusted protein, fiber, calcium, vitamin $B_6$, vitamin C, vitamin E intakes were significantly increased in the HVL compared to LVL group. The index of nutritional quality (INQ) and mean adequacy ratio (MAR) were also increased in the HVL group compared to the LVL group. These results show that our nutrition education program was an effective intervention measure for the reduction of body weight and visceral fat, blood pressure, glucose and lipid levels in the blood and also for the improvement of nutrient intake and diet quality in postmenopausal women who are overweight.

Correlation of Body Mass Index, Body Fat Distribution, Aminotranferases and Computed Tomography in Obese Children with Fatty Liver (비만을 동반한 소아 지방간 환아에서 체질량 지수, 체지방 분포, 간 효소치 및 복부 CT 소견과의 상관관계에 대한 연구)

  • Purpose : Visceral fat accumulation plays a major role in metabolic complications of obesity. It is known that nonalcoholic fatty liver in obese adults is associated with visceral fat accumulation. Body mass index(BMI) is used as the index of obesity in children. The aim of this study is to evaluate the correlation of BMI and visceral adipose tissue(VAT), and the correlation of BMI, body fat distribution, aminotransferases, and severity of fatty liver. Methods : Twenty three obese children with fatty liver diagnosed by non-contrast abdominal computed tomography(CT) were included in this study. Data on BMI, aminotransferase levels were collected from clinical records. Visceral adipose area was evaluated with CT. Results : BMI had a singnificant correlation with VAT(r=0.51719, P=0.0115). The severity of fatty liver had no significant correlations with BMI(r=-0.11938, P=0.5876), VAT(r=-0.31234, P=0.1468), aspartate aminotransferase(AST)(r=0.12729, P=0.5628) or alanine aminotransferase(ALT)(r=-0.00179, P=0.9935). Conclusion : BMI in obese children was correlated with VAT. But the severity of fatty liver cannot be assessed by BMI, VAT or aminotransferase levels.

Relationship between Visceral Adiposity Index, Insulin Resistance and Pancreatic Beta Cell Function According to the Prevalence of Metabolic Syndrome in Korean Obese Adults (한국 비만성인의 대사증후군 유병에 따른 내장지방지수와 인슐린저항성, 췌장 베타세포기능과의 관련성)

  • The purpose of this study was to investigate the relationship between VAI, insulin resistance, and pancreatic beta cell function according to the prevalence of metabolic syndrome in obese adults. From 2017 to 2019, 1,797 obese adults who received medical checkups at a general hospital in Bundang. Diagnosis of metabolic syndrome is NCEP-ATP III. HOMA index was used for insulin resistance and pancreatic beta cell function. VAI was higher in the metabolic syndrome than in the control(p<.001). As the number of risk factors for metabolic syndrome increased, the VAI value was higher(p<.001). The prevalence of metabolic syndrome increased as the VAI quartile increased(p<.001). VAI was also shown to be related to HOMA-IR and HOMA-β in the control, but not in the metabolic syndrome.

Effects of Aerobic Exercise on Serum Blood Lipids, Leptin, Ghrelin, and HOMA-IR Factors in Postmenopausal Obese Women (유산소 운동이 폐경 후 비만여성의 혈청지질, 렙틴, 그렐린 및 인슐린저항성지수에 미치는 영향)

  • The aim of this study was to analyze the effects of aerobic exercise on the blood lipids, leptin, ghrelin, and HOMA-IR factors in obese postmenopausal Korean women. Thirty-six healthy postmenopausal women (mean age, $54.47{\pm}2.50$ years) with >32 % body fat were assigned randomly to an aerobic exercise group (n = 18) or to a "no exercise" control group (n = 18). The subjects' body composition, blood lipid, leptin, ghrelin levels, and HOMA-IR were measured before and after a 16-week line-dancing program. The exercise group showed a significant decrease in body weight, percent body fat, body mass index, visceral fat area, leptin, insulin, glucose, HOMA-IR, total cholesterol, triglycerides, apolipoprotein B, low-density lipoprotein cholesterol, and systolic and diastolic blood pressure. In addition, this group exhibited a significant increase in the apolipoprotein A-I, ghrelin, and high-density lipoprotein cholesterol levels. The energy metabolic factors that influenced the visceral fat included ghrelin, leptin, insulin, glucose, and HOMA-IR. The t-value, which determined the statistical significance of the independent variables, was significant for ghrelin, glucose, insulin, and HOMA-IR (p < 0.05). Regular and continuous aerobic exercise (e.g., line dancing) effectively improved the body composition, visceral fat, serum blood lipids, leptin, ghrelin, and HOMA-IR factors in obese postmenopausal Korean women.

Anti-Visceral Obesity Effect of Apios americana Medikus in Diet-Induced Obese Mice (식이로 유도한 비만마우스에서 아피오스의 내장지방 감소 효과)

  • This study examined the effects of apios (Apios americana Medikus) supplementation on visceral obesity in high-fat diet-induced obese mice. C57BL/6N mice were fed a high-fat diet (40% calories from fat) with or without apios powder (10%, w/w) for 12 weeks. At the end of the experiment, apios supplementation reduced visceral fat mass significantly by 14.3% compared to the control group. Apios decreased significantly the atherogenic index, serum leptin level, hepatic lipid (free fatty acid and triglyceride) content, and lipid droplets, whereas it increased the serum high density lipoprotein-cholesterol/total cholesterol ratio. Hepatic lipogenic gene expression of peroxisome proliferator activated receptor gamma, fatty acid synthase, and diacylglycerol O-acyltransferase 2 was down-regulated by apios supplementation. These results suggest that apios is a healthy food for preventing high-fat diet-induced visceral obesity and fatty liver.

Effects of ADMIRE Algorithms on Fat Measurements Using Computed Tomography (CT) (CT를 이용한 지방측정에 ADMIRE 알고리즘이 미치는 영향)

  • To investigate the effects of iterative reconstruction algorithms on fat measurements using computed tomography (CT), we comparatively and quantitatively analyzed the ratios of visceral, subcutaneous, and visceral-subcutaneous fat areas as well as the variations of HU and noise of visceral and subcutaneous fat using ADMIRE strength and attempted to identify any difference between them. Experimental results showed that no statistically significant difference existed among the visceral, subcutaneous, and visceral-subcutaneous fat area ratios HU of visceral fat area and HU of subcutaneous fat area when applying ADMIRE as compared with existing conventional filtered back projection algorithms. However, as the ADMIRE strength increases, the noise of visceral and subcutaneous fat decreases by up to 12.1% and 19.2%, respectively. In conclusion, iterative reconstruction algorithms have no effect on the visceral, subcutaneous, and visceral-subcutaneous fat area ratios, which are indicators of fat measurement using CT.

A Study on Obesity-related Factors Through the Convergence of Body Mass Index (BMI) and Fat-CT in Middle-aged Women Living in Incheon (체질량지수(BMI)와 복부지방단층촬영(Fat-CT)의 융합을 통한 인천지역 중년 여성의 비만 검사연구)

  • To figure out the causes of obesity against middle-aged women, this study aimed to provide basic data for setting an obesity-related policy through analysis on diverse related factors. Against the healthy middle-aged women who visited 'N' Hospital Health Center in Incheon from April to November 2014, anthropometric assessment, body composition test and abdominal fat distribution test were conducted using Fat-CT. They were carried out against 159 women who agreed with the purpose of the study, using BMD, Q-CT. According to Fat-CT, subcutaneous and visceral types accounted for 39% (76 women) and 61% (119 respondents) respectively. In terms of BMI, 'underweight ($18.5kg/m^2$)' and 'overweight ($25kg/m^2$ or above)' were 13 women (6.7%) and 60 women (30.8%) each with the average of $23.5{\pm}4.11kg/m^2$. Mean while, waist circumference, diastolic blood pressure, systolic blood pressure and neutral fat were statistically significant at the 5% significance level. As getting older, muscle weakness offsets increase in body weight with abdominal obesity. A group with low BMI should also be considered due to increase in waist circumference.

Effects of Hatha Yoga Exercise on Body Composition, Serum Lipids, and Health-Related Fitness of Obese Middle-Aged Women (하타요가 운동이 비만중년여성의 신체조성, 혈청지질 및 건강체력에 미치는 영향)

  • The purpose of this study was to analyze the effects of Hatha Yoga exercise on the body composition, serum lipids, and health-related fitness of obese middle-aged women. The subjects in the Hatha Yoga exercise group (n=9) were 18 obese middle-aged women that trained for 16 weeks and a control group (n=9). Body composition, serum lipids, and health-related fitness were measured in all of the subjects before the start of the training program and at the end of the 16 weeks. The findings of this study were as follows. Body weight, %BF, BMI, WHR, VFA, TC, TG, LDL-C, FFA, and HOMA-IR significantly decreased, whereas LBM, HDL-C, grip strength, back strength, push-ups, sit-ups, Harvard step test, and sitting trunk flexion significantly increased in the Hatha Yoga exercise group after 16 weeks. Therefore, regular and continuous Yoga exercise was effective in improving body composition, serum lipids, and health-related fitness. Consequently, Yoga exercise could be effective for preventing cardiovascular disease caused by obesity in middle-aged women.
