Factors influencing the return of spontaneous circulation of patients with out-of-hospital cardiac arrest (병원외 심정지 환자의 자발적 순환 회복에 영향을 미치는 요인)

  • Out-of-hospital cardiac arrest is a major public health problem in Korea. The survival rate to discharge remains at approximately 3.5% and only 1% have good neurological function. To increase the survival rate, prehospital care should restore spontaneous circulation. The purpose of this study was to analyze the factors associated with return of spontaneous circulation(ROSC) after out-of-hospital cardiac arrest. Data used for this study were collected from KCDC Out-of-Hospital Cardiac Arrest Surveillance 2009. As for the results of decision tree analysis, it is clear that prehospital CPR, cardiac arrest witness, activity, past history(cancer/heart disease/stroke), place, bystander CPR, response time, age, etc are significant contributing factors in ROSC. Among 16 cardiac arrest types from decision tree classification, the ROSC rate of type 1 is the highest(29.6%). Also notable is the fact that bystander CPR was strongly correlated with ROSC of patents with cardiac arrest occurring in non-public places. Community resources should be concentrated on increasing bystander CPR and early prehospital emergency care.

  • The purpose of this article is to cope with the abuse of antibiotics in the clinic, to determine the necessity of antibiotic administration, to share information on the selection and proper use of appropriate antibiotics, and to increase the appropriateness of antibiotic prescription through continuous monitoring. In line with the latest research and guidelines trends of various agencies, we will supplement the antibiotic prescription guidelines and use them for the treatment in Apple Tree Dental Hospital. Specially, by history taking and the penicillin allergy test, amoxicillin is prescribed as a primary selective antibiotic for 1 day. The complaints and treatment effects of the first antibiotic should be evaluated at the next visit. If the primary antibiotic was ineffective, we replaced it with a broad-spectrum antibiotic. If there was no improvement in symptoms, the patient would be referred to upper grade hospital. The staff of the Apple Tree Dental Hospital regularly monitored and educated antibiotic prescriptions. The current guidelines should be supplemented continually and positively affect the abuse of antibiotics and the habit of dental practice.

Effects of Lichen Colonization of Dirinaria applanata on the Death of Rhododendron schlippenbachii (철쭉나무(Rhododendron schlippenbachii)에 대한 엽상 지의류 Dirinaria applanata의 병원성 조사)

  • Lichen thalli of Dirinaria applanata heavily colonized on the twigs of dead or dying Rhododendron trees in Solok island, Jeonnam province in Korea. Pathogenesis of the lichen on the trees was investigated to find out the possibility of lichen as a causal agent. Histological examination of the lichen colonized twigs was attempted with differential staining technique. Lichen-forming fungus colonized only on the surface of bark and there was no direct penetration of fungal hyphae into the plant tissues. Symbiotic algae of the lichen was also examined. The isolated algal cells were inoculated on artificially induced wounds of the healthy trees. Histological examination of the inoculated tissues showed that some algal cells were successfully colonized inside the tissues without any pathogenic symptoms, even 2 months later, The extract of the lichen thalli was also examined using 10% of DMSO solution. Treated tissues showed no pathogenic symptoms, even 4 weeks later. The results suggested that the lichen was not directly involved in the death of the trees.

Occurrence of Leaf Blight on Sweet Persimmon Tree by Pestalotiopsis theae (Pestalotiopsis theae에 의한 단감나무 둥근갈색 무늬병(가칭)의 발생)

  • 1990이래 단감재배 면적이 확대되고 있는 경주 및 그 인근의 포장으로부터 이미 알려진 병징과는 전혀 다른 병징을 나타내는 병을 단감의 주요 품종인 부유의 이병 잎과 신초로부터 병원균을 분리하고 그 병원 곰팡이의 병원학적 특징을 조사하였다. 이 병은 신초선단부 잎에서 발생하여 아래잎으로 진전되며, 잎에는 잎가장자리나, 중앙부 등 일정한 부위가 없이 발병하여 회갈색의 둥근 무늬가 확대되기도 하고, 심하면 잎이 고사탈락하기도 한다. 심한 경우 병반의 크기가 전체 잎의 1/3이상을 차지할 정도로 크게 확대되었다. 신초는 병의 진전이 심해지면 고사함으로서 나무 생육이 급격히 중지되거나, 나무 수세가 떨어져 수확에도 지장을 주고 있다. 발생시기는 6월 중.하순~7월, 9~10월 초순까지 발병하며, 병든 잎과 가지를 채집하여 병원균을 순수 분리하여 Guba(1949년), Sutton(1969)와 Steyaert(1971)의 분류기준에 따라 동정한 결과는 다음과 같았다. 병원균의 분생포자의 크기는 23.1$\times$7.1 $\mu\textrm{m}$, color cell의 크기와 수는 15.65 $\mu\textrm{m}$, 3개이었고, 포자의 윗부분은 다갈색이었으며 아래부분은 올리브색이었다. Setrlae의 크기와 개수는 23.6 $\mu\textrm{m}$, 2~4(3)개이고, pedicle의 길이는 5.17 $\mu\textrm{m}$였다. 이상의 결과에 근거하여 이 병원균을 Pestalotiopsis theae로 동정하고 국내미기록병으로 보고한다.

Canker of Pomegranate (Punica granatum) Caused by Botryosphaeria dothidea (Botryosphaeria dothidea에 의한 석류나무 줄기썩음병)

  • Canker farmed on a pomegranate tree(Punica granatum) with yellowing leaves and death of some twigs was founded at a garden of Gyungbuk Agricultural Technology Administration. Botryosphaeria dothidea was isolated from the canker and pathogenisity of the isolate was confirmed by artificial inoculation to the pome-granate twig, which showed the same symptom with naturally infected twig. This is the first report of a natural infection of pomegranate tree by Botryosphaeria dothidea in Korea.

Studies on the White rot and Blister Canker in Apple Trees caused by Botryosphaeria berengeriana (사과나무의 겹무늬병(윤문병) 및 사마귀병 (우피병)의 병원균과 병원성에 관한 연구)

  • Fruit rot and blister canker, a disease of apple occurring severely in Korea has been studied for correct identification of the syndrome In fruit and apple trees. Among the fungi isolated from blister cankers, rough barks or fruits showing rotting of 7 different host species were Botryosphaeria berengeriana (pycnidial stage. Dethiorella mali), Penicillium expansum and Alternaria sp. from apple rots and Phomopsis sp. from pear fruit rots. The most dominant isolates were B. berengeriana. Ten isolates of D. mali were grouped in to two conidial types based up mycelial growth rate, growth habits and mycelial coloration on PDA. None of 10 isolates was chromogenic. Pycnidia in apple stems, stromatic, dark brown, globose or subglobose and the measuring were $103.5-287.5{\mu}\times92.0-287.5\mu$. The pycnidia contained hyaline, nonseptate, fusiform conidia. The sizes of pycnidiospore of isolates obtained from apple twig were $4.3-7.2{\mu}\times20.0-31.5{\mu}(average\;5.9\times25.4\mu)$. Some conidia of this fungus from apple, pear, peach and ornamental cherry showed 1-,2-,3-septate before or during germination. Microconidia were observed in pycnidia on PDA and fruit lesion of inoculated host. Symptoms on leaves and fruits were contoured brown spots when inoculated. Wart-like protuberance were formed on the surface of apple and pear. Canker appeared on branches of peach and ornamental cherry inoculated.

A Study on the Investigation and Application of Microbial Pathogens of Major Insect Pests of Forest in Korea (중요산림해충의 병원미생물 개발에 관한 연구)

  • The study has been carried to investigate a possibility to control several major insect pest of forest by microbial pathogens existing in nature as one of the biological control measure. Microorganisms including polyhedral virus isolated from diseased fall webworm were total of 4 kinds pathogenic microbes among these 4 kinds were polyhedral virus and Bacillus .species. Control effect of these two pathogens appeared to be $70.6\%$ and $49.5\%$, respectively, when they were compared with those of control plot that was $27.8\%$. Each one of bacterium species and fungus species were isolated from diseased Japanese alder leaf beetle. Pathogenisity to the healthy beetle was recognized by the fungus species, while the bacterium showed none of pathogenisity. The fungus was identified as Beauveria sp. and its effect on the beetle control was $96.2\%$ while untreated plot showed $49.2\%$ of dead beetles in the same period. Fifteen species of microbes were isolated from diseased larvae of pine gall midge. Six species out of 15 showed certain level of insecticidal effect to the larvae of the insects. The highest efficiency was showed by a fungus species, Fusarium sp. and was followed by Bacillus SP. I, Spicaria sp. pathogens isolated from larvae of pine gall midge did not affected to both of Japanese alder leaf beetles and fall webworms in any means.

Decision Tree Induction with Imbalanced Data Set: A Case of Health Insurance Bill Audit in a General Hospital (불균형 데이터 집합에서의 의사결정나무 추론: 종합 병원의 건강 보험료 청구 심사 사례)

  • In medical industry, health insurance bill audit is unique and essential process in general hospitals. The health insurance bill audit process is very important because not only for hospital's profit but also hospital's reputation. Particularly, at the large general hospitals many related workers including analysts, nurses, and etc. have engaged in the health insurance bill audit process. This paper introduces a case of health insurance bill audit for finding reducible health insurance bill cases using decision tree induction techniques at a large general hospital in Korea. When supervised learning methods had been tried to be applied, one of major problems was data imbalance problem in the health insurance bill audit data. In other words, there were many normal(passing) cases and relatively small number of reduction cases in a bill audit dataset. To resolve the problem, in this study, well-known methods for imbalanced data sets including over sampling of rare cases, under sampling of major cases, and adjusting the misclassification cost are combined in several ways to find appropriate decision trees that satisfy required conditions in health insurance bill audit situation.

A Predictive Model of Turnover among Nurses in a Tertiary Hospital: Decision Tree Analysis (의사결정나무 분석기법을 이용한 상급종합병원 간호사의 이직 예측모형 구축)

  • Purpose: The purposes of this study were to develop a predictive model and evaluate this model of turnover in hospital nurses. Methods: Participants were 1,565 nurses from a tertiary hospital in South Korea. Descriptive statistics and a decision-tree analysis were performed using the SPSS WIN 23.0 program. Results: The turnover groups were presented in eleven different pathways by decision tree analysis. There were three high-risk groups with a higher turnover rate than the average, and eight low-risk groups with a lower turnover rate. Among them, two low-risk groups had a 0% turnover rate. The groups were classified according to general characteristics such as position, period of temporary position, clinical career at last working unit, total clinical career, and period of leave of absence. The accuracy of the model was 83.2%, sensitivity 63.7%, and specificity 98.1%. Conclusion: This predictive model of turnover may be used to screen the turnover risk groups and contribute for decreasing the turnover of hospital nurses in South Korea.
