The aim of this study was to compare the morphological characteristics of two types of stainless steel permanent molar crowns using three-dimensional scanners and the morphological changes of these crowns after crimping. Two types of stainless steel permanent molar crowns, PO-96 and PERMACROWN were scanned using three-dimensional scanner. Crown size, crown index (ratio of buccolingual diameter to mesiodistal diameter at height of contour), cervical convergency of crown were measured. Stainless steel crowns were crimped and re-scanned with three-dimensional scanner. Morphological changes of stainless steel permanent molar crowns were analyzed. As for the crown index, maxillary PERMACROWN was larger buccolingually and smaller mesiodistally than maxillary PO-96 and mandibular PERMACROWN was smaller buccolingually and larger mesiodistally than mandibular PO-96. Maxillary PO-96 was more convergent to cervical mesiodistally than maxillary PERMACROWN and mandibular PO-96 was more convergent to the cervical mesiodistally, buccolingually than mandibular PERMACROWN. Both types of stainless steel permanent molar crowns showed reduction of cervical circumference after crimping. Two products were morphologically different in crown size, shape and cervical convergence. Although both types of stainless steel permanent molar crowns are pre-contoured type, additional crimping is needed to achieve better marginal adaptation.

Due to increasing demand for aesthetics, zirconia crowns have become a popular choice for treating primary molars. However, there is limited literature available comparing the survival rates of zirconia crowns with those of other restorative materials. The objective of this study was to compare the 36-month survival rates of zirconia crowns and stainless steel crowns for proximal caries, as well as to analyze failure types associated with each crown type. Electronic medical records and radiographs of 1,061 primary molars from 498 patients treated with 2 types of prefabricated crowns at Chosun University Dental Hospital and 2 private dental clinics between 2017 and 2019 were collected and analyzed. The survival rate of zirconia crowns was found to be lower compared to that of stainless steel crowns. Regarding the groups without pulp treatment, the survival rate of stainless steel crowns was significantly higher than that of zirconia crowns. However, in the groups that received pulp therapy, no significant difference in the survival rates was observed between the two preformed crowns. Notably, abnormal root resorption or periapical lesions were identified as the primary cause of restorative failure in stainless steel crowns, whereas loss of restoration was the predominant cause in zirconia crowns. This study holds valuable implications for clinicians when selecting preformed crowns for primary molars.

Whenever pediatric dentists choose the preformed stainless steel crowns for restoration of primary molars, we usually prefer the crown forms that most fit to the abutment teeth. Despite some previous reports on significant ethnic difference in the size and shape of tooth crowns in primary dentition, only the imported products have been available to date, which might show some discrepancy with tooth anatomy of Korean. However, a domestic product of preformed stainless steel crown has been marketed recently that is expected to show more accurate fit to the primary tooth of Korean. To investigate the morphological characteristics of primary molars of Korean children, their mesiodistal and buccolingual diameters from the dental casts of 263 children in Pusan and Kyungsangnamdo province were measured by digital calipers. The measured data were compared with those of the three types of preformed stainless steel crowns(3M ESPE Stainless Steel Primary Molar Crown, Kids Crown, Sankin Anatom Primary Crown) by statistical processes with regression analysis. The results were as follows: 1. In every primary molar crown of three brands, the bucco-lingual width to mesio-distal width ratio (MD-BL ratio) showed higher values compared with those of natural teeth. 2. While the products of 3M ESPE and Shinhung showed similar features, those of Sankin showed some difference from both, and these difference was more marked in mandibular primary molars. 3. The nearer is the mesio-distal width of the crown to the average, the better was the adaptation of the preformed crown, and the reverse was true. 4. The teeth which revealed more discrepancy between preformed crown and natural tooth were upper and lower first primary molars.

This study analyzed the longevity of preformed metal crowns (PMCs) in first permanent molars and evaluated factors influencing their survival during a long-term follow-up period. In all, 115 first permanent molars treated with PMCs between June 2008 and June 2018 were retrospectively analyzed. The overall combined success rate for the study group was 84.3%. The 5-year survival rate was 82.8%. Multivariate Cox regression analyses identified distal cavities and mandibular PMC placement as risk factors for restoration failure. Careful placement of PMCs at the final try-in stage augments the longevity of the crown.

Data derived from odontometric studies are useful in many areas. Especially crown diameter of primary teeth is important in not only anthropology but also clinical dentistry. The purposes of this study were to compare diameters of primary molars of Korean children with those of preformed stainless steel crown, and examine racial characteristics of primary molars of Korean children. The mesiodistal and buccolingual diameters of primary molars were measured on dental casts taken from 235 children(male 105, female 131), with digital calipers. And the data were compared with those of preformed stainless steel crowns. The results were as follows; 1. No significant difference was observed between the right and left members of antimeric teeth and all primary molars of male were larger than those of female. 2. The diameters of primary molars of Korean are smaller than those of Austrailian aborigines and there was significant difference between diameters of primary molars of Korean and those of other populations. 3. There were size differences between diameters of maxillary, mandibular first molar and preformed stainless steel crown than other primary molars, relatively.

This study was conducted for the purpose of evaluating the stainless steel crowns on extracted primary molars and thus identifying frequent errors and defects. Visual assessment and micro-computed tomography (micro-CT) image analysis were performed on 97 primary molars for evaluation of the state of marginal adaptation, cement loss, secondary caries, ledge formation, attritive perforation and marginal polishing defect. The results were as follows: In the examination of object teeth by evaluation criteria, cement loss was found most frequently (98%), followed by secondary caries (42.3%), marginal polishing defect (41.2%), ledge formation (29.9%) and attritive perforation (17.5%), in this order. The cement loss at the margins showed a significant relationship with marginal gap and secondary caries: the larger the marginal gap is, the more frequent is the cement loss (p < 0.05). The average marginal gap was $0.31{\pm}0.26mm$ and showed the highest value in the maxillary 2nd primary molars. The location of the crown margin above the cementoenamel junction was found most frequently and it was found that the higher the crown margin is located, the less the marginal gap becomes (p < 0.05). In conclusion, it is thought very desirable to pay closer attention to crown margins and shapes for stainless steel crown restoration in order to minimize the marginal gaps and consequent cement loss.

The use of stainless steel crowns are indicated for restoration of primary or permanent molars with proximal dental caries, extensive dental caries, or previous pulp treatment with increased danger of tooth fracture. Stainless steel crowns were introduced by Humphrey in 1950. For their improved durability, longevity, and success rate, they have been strongly considered for restoring extensive and multi-surfaced dental caries of molars in pediatric dentistry. However, they also have shortcomings, such as possibility of pulpal exposure or damaging proximal surface of adjacent teeth. In addition, when oversized stainless steel crowns are used, eruption of the adjacent permanent teeth may be disturbed by their prominent margin. As a means to compensate the shortcomings of stainless steel crowns, use of orthodontics bands may be considered. It is an alternative restoration method, where an orthodontic band is placed on a tooth first and cavity is restored with filling material, such as composite resin, glass ionomer, or amalgam. The use of an orthodontic band is indicated for molar restoration with cervical dental caries, extensive dental caries, enamel hypoplasia, or previous pulp treatment. Because it requires shorter chair time compared to stainless steel crown, its application is very useful for children with poor behavior. However, restoration using an orthodontic band requires good oral hygiene after its application. This case report illustrates the conservative restoration of primary molars and permanent molars with extensive dental caries using orthodontic bands.

The purpose of this study was to elucidate the size and morphologic characteristics of maxillary primary first molars in Korean children using three-dimensional laser scanner and compare three-dimensional image with preformed stainless steel crown. Scanned three-dimensional images of dental cast taken from 132 children(male 62, female 70) by three-dimensional laser scanner(Breuckmann opto-Top HE100, INUS, Korea) were used. Mesiodistal diameter, buccolingual diameter, occlusogingival height and crown shape of each image were calculated by Rapidform 2004 program(INUS, Korea). The values were statistically compared by independent samples t-test with 95% of significant level. The results were as follows : 1. No significant difference in crown size was found between left and right maxillay primary first molar(p>0.05). 2, Significant difference in mesiodistal diameter, buccolingual diameter, buccal occlusogingival height was found between male and female (p<0.05), and crown size of male was bigger than that of female. 3. Average image of maxillay primary first molar was shaped three-dimensionally and measured. In comparison with 3M stainless steel crown, this image was similar with No.4 or No.5 SS crown in male, No.4 in female. In comparison with ILSUNG SS crown, this image was similar with No.5 in male, No.4 in female. 4 Mesiolingual line angle area, distolingual line angle area and buccogingival ridge were more obvious in average image than 3M stainless steel crown. ILSUNG SS crown was more square and had longer mesiodistal diameter than average 3D image.

The purpose of this study is to evaluate changes of occlusion following the placement of preformed metal crown in the permanent first molar affected by severe caries or hypomineralization. The study was conducted on 24 patients who met the criteria among the patients with occlusion in the permanent first molar reaching the occlusal plane. Restorative treatment was performed on one first molar using preformed metal crown. Before treatment, immediately after treatment, after 4 weeks, and after 8 weeks, the T-scan III was used for measuring distribution of bite force at the maximum intercuspal position, and overbite was measured with digital vernier calipers and change of dental midline was surveyed. After 8 weeks, questionnaires survey was conducted. In most cases, immediately after treatment, the distribution of bite force between the restored side and non-restored side of arch was reversed compared to before treatment, and the overbite was decreased. However, after 4 weeks, bite force of the left and right side was balanced and the distribution of bite force on the restored and non-restored permanent first molars had similar values. Also, the changed overbite and midline were returned to pre-treatment status and there was no significant temporomandibular joint and masticatory discomfort after treatment. In this study, it was confirmed that spontaneous occlusal equilibrium was achieved one month after the placement of preformed metal crown.

Although the stainless steel crowns have been recognized as the most effective and durable form of restoration for primary molars, they have been regarded by many dentists as having definite demerits such as invasive nature of procedural complexity and behavioral aspects of children. As an alternative to conventional technique of stainless steel crown restoration, the Hall technique was first introduced in 1988, which is characterized by just pushing the pre-contoured, cement filled crown form onto the abutment molar with no local anesthesia, no caries removal, no tooth preparation. According to several reports, this can slow, arrest, or even reverse the progress of caries. In addition, its atraumatic feature gives less discomfort and stress to children than conventional one, which is thought excellent especially in younger children. Also, It has been reported to be effective and acceptable to dentist, child patients and their parents. In this case study, three children with age of 4 years 5 months, 4 years 10 months, 6 years 4 months were treated with stainless steel crowns using Hall technique on first primary molar respectively. The teeth were free from pulpal, periapical pathology. After follow up of about 3 to 6 months period, the results showed clinically successful outcomes without any marked complication in pulp, tooth or soft tissue till now. But, it should be kept in mind that this technique is not proper to every child, every carious molar, or every dentist. Thorough distinction of indicated cases and continuous follow-up check is highly required. Conclusively, Hall technique might be an effective and realistic minimally invasive alternative for the carious primary molars especially in younger or disabled children, despite potential doubts on its efficacy and some definite limitations.
