The Time Responses of Spirometric Values in Response to Single Doses of Inhaled Salbutamol (기관지확장제 사용 후 시간에 따른 폐활량 측정치의 변화)

  • Background : An assessment of the presence and the degree of reversibility of airflow obstruction is clinically important in patients with asthma or chronic obstructive pulmonary disease. However, the time responses of spirometric parameters in response to bronchodilator have not been well investigated. Methods: We studied 15 patients with asthma. Spirometric and mini-Wright peak expiratory flow measurements were performed at 15, 30, 45, and 60 minutes after using single dose($200{\mu}g$) of inhaled bronchodilator, salbutamol. Results : The mean values of forced expiratory volume in one second($FEV_1$) and forced vital capicaty(FVC) were significantly increased at 60 minutes after using bronchodilator in comparison to 15 minutes. And peak expiratory flow rate measured by either mass flow sensor or mini-Wright peak flow meter were significantly increased at 45 minutes after using bronchodilator in comparison to 15 minutes. Conclusions : To appropriate evaluation of the bronchodilator response in patients with reversible airflow limitation, it would be useful measuring either $FEV_1$ or PEF at the later time point 60 or 45 minutes in comparison to 15 minutes after using bronchodilator.

Comparison of Anti-asthmatic Activity by Native Codonopsis lanceolata Extract (자생 돌더덕 추출물에 의한 천식억제 활성의 분석)

  • Codonopsis lanceolata (Campanulaceae) has been widely used in traditional medicine and is considered to have medicinal properties to treat diseases and symptoms such as bronchitis, coughs, spasm, edema, hepatitis, colitis, and lung injury. In order to investigate whether native Codonopsis lanceolata extract alleviates ashmatic symptoms in vivo, we first carried out various antioxidant activities by 2,2-diphenyl-1-picrylhydrazyl (DPPH) radical scavenging, ferric reducing antioxidant power (FRAP), and cupric reducing antioxidant capacity (CUPRAC) assays. The antioxidant activities were increased by adding Codonopsis lanceolata extract in a concentration-dependent manner which compared to ascorbic acid as a positive control. Histological studies using an ovalbumin-induced animal model exhibited potent anti-inflammatory potential by decreasing immuno-responsive cells in the lung by the extract by confirming H&E and PAS staining. It is revelaed that further immunihistochemical analysis showed anti-ashmatic capabilities by assessing histamine, IL-31, and MMP-9 expressions. The level of IL-13 expression in Codonopsis lanceolata extract-treated group was decreased upto 73.7% compared to control, whereas that of total cells and eosinophil counting in Codonopsis lanceolata extract-treated group was diminished to 73.5% and 80.9%, respectively. These results collectively indicate that the C. lanceolata extract ameliorates asthmatic symptoms effectively in an ovalbumin-challenged mice model, in that the extract can be used for the development of an anti-asthmatic food ingredient.

  • We analysed mutation of ${\beta}_2$-adrenergic receptor gene that controls bronchial asthma by denaturing high performance liquid chromatography (DHPLC) according to ion-pair reversed-phase high performance liquid chromatography (IP-RP-HPLC). We extracted genomic DNA from 50 asthma patients, amplified DNA using PCR, and analysed PCR product by DHPLC. As a result, we obtained that mutation frequency was 15 (30%) among 50 cases. Consequently DHPLC mutation detection was confirmed that the result of direct sequencing was coincide exactly.

  • Background: Interleukin-4 plays an important role in pathogenesis of asthma, especially in developing atopy by means of switching B lymphocytes to produce IgE. It has been shown that there is polymorphism in the Interleukin-4 promoter region, transversion of cytosine to thymine at-598 from translation initiation site of IL-4 gene. There has also been quite a few works to reveal the role of the polymorphism of IL-4 gene in patients with asthma. We performed this investigation to determine the role of the polymorphism in the severity of symptoms of patients with asthma. We also examined the frequency and the type of the polymorphism in asthmatics compared with non-asthmatics as well. Method: The subjects enrolled in this study were 49 asthmatics and 33 non-asthmatics. All the asthmatics were classified as mild and moderate to severe by the NHLBI/WHO Workshop. DNA from both asthmatics and non-asthmatics was extracted, then performed ARMS(Amplification Refractory Mutation System) as well as RFLP using BsmFl restriction enzyme in order to confirm the polymorphism of Il-4 gene. Results: There was no significant difference in the occurrence of polymorphism of the IL-4 promoter sequence between asthm and non-asthma groups(P=0.7). Among those with polymorphisms, the number of C/C type was slightly more than C/T type in both asthmatics and non-asthmatics, 26 vs 21 in asthmatics and 18 vs 15 in non-asthmatics, which was, however, insignificant statistically. No significant relationship between the severity of asthma and the polymorphism was found(P=0.7). Conclusion: There was no significant difference between the severity of asthma and the IL-4 promoter polymorphism(P=0.709). Interestingly, the frequency of the polymorphism in both asthmatics as well as non-asthmatics was found to be even higher than that occurred in Caucasians. However, no significant difference in the frequency of the polymorphism was found in both groups.

  • Adenoid cystic carcinoma formerly called cylindroma is rare tracheal tumor. Characteristics of adenoid cystic carcinoma are infiltrative nature with local recurrence tendency and long natural course of the disease. Adenoid cystic carcinomas develop most commonly in the trachea. Primary resection and end-to-end anastomosis of the involved airway are treatment of choice. And postoperative radiation therapy might be useful, particularly when the surgical margins are not ample. We report two cases of adenoid cystic carcinoma of trachea diagnosed by flow-volume curve.

Symptom Experience and Quality of Life in Patients with Chronic Lung Disease - With a Special Reference to Bronchial Asthma and Bronchiectasis - (만성폐질환자의 증상 경험과 삶의 질 - 기관지천식과 기관지확장증 환자를 중심으로 -)

  • Purpose: In this study, symptom experience and quality of life(QoL) in patients with Bronchial asthma(BA) and Bronchiectasis(BRC) were examined to develop nursing strategies. Method: Data collection was done with 61 outpatients with BA and 43 outpatients with BRC in January and February, 2001. Data analyses were performed using SPSS Win 8.0. Result: The mean scores of symptom experience were 11.3 and 10.8 in patients with BA and BRC, respectively. The mean score of QoL was 18.0 in patients with BA and 19.2 in patients with BRC. There were statistically significant differences in the score of the symptom experience according to sex and occupation in patients with BA, and to the religion and occupation in patients with BRC. The mean score of QoL had significant difference according to the income in patients with BA, but had no significant differences in patients with BRC. The score of QoL had significantly correlated with the degree of symptom experience in patients with BA(p=.000) and with BRC(p=.022). Conclusion: The degree of symptom experience negatively correlated with QoL. Therefore, symptoms experienced by patients should be considered in development of nursing strategies for patients with BA and BRC.

  • Tracheal tumors are uncommon comprising less than 0.1% of all malignancies. Metastatic tracheal tumors, especially from the extrathoracic sites, are exceedingly rare. Ovarian cancer tends to metastasize to the serous cavities and the lymph nodes. One large autopsy study reported tracheal involvement in 1% of patients who had died from ovarian cancer. Other studies have not mentioned tracheal involvement at all. Since the main symptoms of cough, hemoptysis, or wheezing are nonspecific, patients may be initially treated for other conditions including asthma or bronchitis. Here we describe a metastatic tracheal tumor from an ovarian carcinoma that was initially treated for bronchial asthma.

Exhaled Nitric Oxide Concentration in Children with Asthma and Allergic Rhinitis : Association with Atopy and Bronchial Hyperresponsiveness (천식 및 알레르기성 비염 환아에서의 호기 일산화질소 농도 : 아토피 및 기도과민성과의 연관성)

  • Purpose : A new airway inflammatory marker, exhaled nitric oxide(ENO) has been reported to correlate with bronchial hyperresponsiveness(BHR) and atopy. The purpose of this study was to analyze the relationship of ENO with BHR or atopy in patients with asthma and with allergic rhinitis. Methods : The subjects consisted of 55 children with asthma, 17 with allergic rhinitis, and 14 healthy controls. The asthma group was subdivided into the atopic asthma group(n=37) and the nonatopic asthma group(n=18) and the allergic rhinitis group into BHR group(n=7) and non-BHR group(n=10). All were investigated with spirometry and measurements of ENO concentration. The correlations between ENO concentration and both methacholine $PC_{20}$(provocative concentration causing a 20% decrease in forced expiratory volume in one second) and the number of allergen skin test positivity were analyzed. Results : ENO concentrations of both asthma and allergic rhinitis groups were significantly greater than that of control(P<0.01). ENO concentration of atopic asthma was significantly greater than that of nonatopic asthma(P<0.01). In allergic rhinitis, ENO concentration did not differ according to the presence or absence of BHR(P=0.50). ENO concentrations correlated significantly with the number of skin test positivity(r=0.32, P=0.02) or methacholine $PC_{20}$(r=-0.38, P<0.01) in asthma group, but not in the allergic rhinitis group(r=0.42, P=0.09; r=-0.06, P=0.83). Conclusion : In asthma patients, some pathogenetic mechanisms associated with atopy and BHR seem to influence ENO concentration. In allergic rhinitis patients, some factors other than BHR may be important in determining ENO concentration.

Lutein Modulates Th2 Immune Response in Ovalbumin-Induced Airway Inflammation (Ovalbumin으로 유도한 천식 생쥐모델에서 lutein의 Th2 면역반응 연구)

  • The general term flavonoids is often used to categorize a family of natural compounds that are highly abundant in all higher plants, and which in recent years have attracted scientific interest as therapeutics. Lutein is a xanthophyll and one of 600 known naturally occurring carotenoids. It is found in green vegetables such as spinach and kale, and has been demonstrated to exert anti-inflammatory activities. However, its anti-allergic effect in the Th1/Th2 immune response is poorly understood. In this study, we attempt to determine whether lutein regulates inflammatory mediators in an ovalbumin (OVA)-induced murine asthma model. To address this, mice were sensitized and challenged with OVA, and then treated with lutein before the last OVA challenge. Administration of lutein significantly suppressed the OVA-induced airway hyper-responsiveness. It also resulted in a significant alleviation of the infiltration of inflammatory cells into the bronchoalveolar lavage. Additionally, lutein attenuated the increased expression of Th2 responses in OVA-challenged mice. These results demonstrate that lutein is a potent inhibitor that reduces Th2 immune responses. Furthermore, they show that the immunopharmacological function is mediated by a pathway that involves and is regulated by Th2 immune response.

Ventilatory Dynamics According to Bronchial Stenosis in Bronchial Anthracofibrosis (기관지 탄분 섬유화증에서 동반된 기관지 협착에 따른 환기역학)

  • Background : Bronchial anthracofibrosis usually manifest as a form of obstructive airway disease, and can be accompanied by parenchymal diseases such as pneumonia, and pulmonary tuberculosis. This study investigated the ventilatory dynamics according to the severity of bronchial stenosis in patients with bronchial anthracofibrosis. Method : One hundred and thirteen patients with bronchial anthracofibrosis that was confirmed by bronchoscopy and who had undergone a pulmonary function test were enrolled in this study group. The correlation coefficients between the pulmonary functional parameters and the number of lobes with bronchial stenosis were investigated. Results : The incidence of ventilatory dysfunction was 56(49.6%) for obstructive, 8(7.1%) for restrictive, 2(1.8%) for mixed, and 47(41.6%) for a normal pattern. The $FEV_1/FVC$, $FEF_{25{\sim}75%}$, $FEF_{25%}$, $FEF_{50%}$, $FEF_{75%}$, and PEF showed a significant negative correlation (p<0.05) and the Raw had a significant positive correlation with the number of lobes with bronchial stenosis(p<0.001). Conclusion : These findings suggest that the most common abnormality of the ventilatory function in bronchial anthracofibrosis is an obstructive pattern with a small airway dysfunction according to the severity of bronchial stenosis.
