인간공학 기술을 이용한 예부선 선원의 해상 작업안전 분석

  • 본 연구는 예부선 선원이 해상작업 시 발생할 수 있는 근골격계 부담 작업의 위험성을 평가 하기 위해 실행되었다. 근골격계 부담 작업은 예부선 선원들의 인터뷰, 작업 분석, 작업 자세 평가를 통해 분석을 통해 분석이 되었으며, 도출된 근골격계 부담 작업은 인간공학 분야에서 작업자세 평가 도구인 OWAS, RULA, REBA를 활용하여 분석을 실시하였다. 분석을 위해 국내 해운선사에서 운용중인 예선 A호와 부선 B호에서 작업을 하는 예부선 선원들의 각종 작업을 분석하였으며 해당 작업들은 근골격계 위험 수준인 3수준 이상으로 해당 작업이 근골격계에 부담되는 작업이며 작업자세 교정이 필요한 것으로 나타났다. 따라서 예부선 작업환경의 위험성이 외부환경에 대한 위협뿐만 아니라 작업자의 작업자세가 장기적인 위협을 보이고 있음을 보여주었다.

  • 본 연구는 예부선 선원이 해상작업 시 발생할 수 있는 근골격계 부담 작업의 위험성을 평가 하기위해 실행되었다. 근골격계 부담 작업은 예부선 선원들의 인터뷰, 작업 분석, 자세 평가를 통해 분석을 통해 분석이 되었으며, 도출된 근골격계 부담 작업은 인간공학 분야에서 작업자세 평가 도구인 OWAS, REBA를 활용하여 분석을 실시하였다. 분석을 위해 오일 탱커 부선에서 작업을 하는 예부선 선원들의 송유관 체결 작업과 모선과 접안을 위한 줄 잡기 작업을 분석하였으며 해당 작업들은 OWAS 점수 3점과 REBA 점수 9점 이상으로 해당 작업이 근골격계에 부담되는 작업이며 작업자세 교정이 필요한 것으로 나타났다. 따라서 예부선 작업환경의 위험성이 외부환경에 대한 위협뿐만 아니라 작업자의 작업자세가 장기적인 위협을 보이고 있음을 보여주었다.

Relations between Self-reported Symptoms of Industrial Workers' Work-related Musculoskeletal Disorders and Psychosocial Factors (산업체 근로자의 작업관련성 근골격계질환의 자각증상과 사회심리적 요인과의 관계)

  • This study aims to identify the relations between self-reported symptoms of industrial workers work-related musculoskeletal disorders and psychosocial factors. The subjects of the study were 120 male workers who were using the rehabilitation center of K company and they were voluntarily interviewed with a use of organized questionnaire. Except the responses from eighteen subjects whose responses were insincere and who had disorders in musculoskeletal areas, the responses of 102 ones were decided for the final analysis. The results showed that there were significant differences in self-reported symptoms of work-related musculoskeletal disorders between the subjects according to smoking, physical burden and self-efficacy. To prevent and control work-related musculoskeletal disorders, multi-characteristic aspects of how the disorders appeared and psychological factors, in particular, self-efficacy, should be carefully considered. Then, as psychosocial factors affect musculoskeletal disorders, relations between the disorders and factors should be analysed through long-term follow-up studies, psychosocial factors should be actively intervened, and the effect should be evaluated.

A Study on Effects of the Convergence of musculoskeletal disorders Women with Disabilities on Quality of life in Jeon-nam Area (근골격계질환 장애여성의 융복합이 삶의 질에 미치는 영향)

  • As we enter the era Convergence, commercialized convergence contents aimed at the disabled are giving a lot of help to them. This fusion research is aimed at providing necessary basic data and practical measures for these by analyzing the effect of influencing the quality of life for women with disabilities. This research was conducted with 250 disabled women living in Jeon-nam region from Jan,1 through April, 25 in 2015. After screening women aged 20-80 who had symptoms and musculoskeletal disorders and were using convergence contents, convergence projects were implemented with total 125 patients over 16 weeks. The result of the convergence projects showed that most of these experieced the decrease of musculoskeletal pain, the decrease of depressed levels, and the improvement of quality of life. Future studies should be done in terms of a long-term perspective and continuing support, and follow-up studies should also be done to evaluate its effectiveness.

Musculoskeletal Injuries by Weapons in Korean Soldiers: Four-Year Follow-Up (총기 및 폭발물에 의한 군인의 근골격계 손상: 최근 4년간 분석)

  • Purpose: To date, studies of firearm and explosive injuries in the Korean military have been limited compared to its importance. To overcome this, this study examined the characteristics of musculoskeletal damages in soldiers who have suffered firearm and explosive injuries over the past four years. Materials and Methods: From January 2015 to July 2019, military forces who had suffered musculoskeletal injuries from firearms or explosive substances were included. The medical records and radiographs were reviewed retrospectively, and telephone surveys about Short Musculoskeletal Functional Assessment (SMFA) for this group were conducted. To compare the functional outcomes, statistical analysis was performed using a t-test for the types of weapons, and ANOVA for others. Results: Of the 61 patients treated for firearms and explosives injuries, 30 patients (49.2%) were included after undergoing orthopedic treatment due to musculoskeletal injury. The average age at injury was 26.4 years old (21-52 years old). The number of officers and soldiers was similar. Eleven were injured by gunshot and 19 by an explosive device. Sixteen were treated in the Armed Forces Capital Hospital and 10 at private hospitals. More than half of the 16 patients (53.3%) with a fracture had multiple fractures. The most common injury site was the hand (33.3%), followed by the lower leg (30.0%). There were 14 patients (46.7%) with Gustilo-Anderson classification 3B or higher who required a soft tissue reconstruction. Fifteen patients agreed to join the SMFA survey for the functional outcomes. Between officers and soldiers, officers had better scores in the Bother Index compared to soldiers (p=0.0045). Patients treated in the Armed Forces Capital Hospital had better scores in both the Dysfunction and Bother Index compared to private hospitals (p=0.0008, p=0.0149). Conclusion: This is the first study to analyze of weapons injuries in the Korean military. As a result of the study, the orthopedic burden was high in the treating patients with military weapon injuries. In addition, it is necessary to build a military trauma registry, including firearm and explosive injuries, for trauma treatment evaluation and development of military trauma system.

Analysis of Frequent Disease and Medical Expenses Structure of Patients Admitted in a Vaterans Hospital (일개 보훈병원 입원환자의 상병 및 진료비 구조분석)

  • Objectives: This study attempts to analyze the length of hospital stay and expenses of frequent disease admitted in a Vaterans Hospital. Methods: Data was collected from January 1, 2001 to December 31, 2003 from the Claim records of 9,640 patients in a Vaterans Hospital. Results: The results were as follows: 1. In age & sex distribution, there was male 70.9%, female 29.1%, and 35.8% of them is 70 age group. Frequency by insurance program was Health insurance 78.1%, Medical aid 14.2%, no insurance 4.1%, others 3.6%. Distribution of each department was internal medicine 28.3%, orthopedic surgery 21.3%, surgery 16.6%, neurosurgey 7.1%, pediatrics 5.9%. Also, in the veterans group, male to female patient ratio was 99.3% male to 0.7% female, them over 70 years old was 51.6%, and them which live in daejeon was 43.5%. 2. In frequency of disease, there was gastroenteritis 4.8%, pneumonia 3.8%, cartaract 3.7%, cerebral infarct 3.2%, hyperplasia of prostate 3.0%. In frequency of korean standard classification of diseases, there was injury and poisoning and certain other consequences of external causes 17.1%, diseases of digestive system 16.1%, diseases of musculoskeletal system and connective tissue 13.9%, diseases of respiratory system 9.4%, diseases of genitourinary system 8.6%. Also, in veterans group, frequency of them was diseases of musculoskeletal system and connective tissue 19.4%, diseases of digestive system 16.8%, injury and poisoning and certain other consequences of external causes 15.7%, diseases of genitourinary system 9.7%, diseases of circuatory system 8.2%. 3. Average length of hospital stay was 29.0 days for total patients, 51.8 days for the veterans group, 15.7 days for the non-veterans one. Average total expenses was 3,669,579 won, the veterans group 7,263,877 won, the non-veterans one 1,560,333 won. The ratio of insurer to insuree was 55.2 : 44.8, the ratio of amount paid by patient in the veterans group 61.7%, in the non-veterans one 33.0%. 4. In items of medical expenses, fee for hospital accommodation was 34.7%, fee for medication 13.2%(injection 7.8%, drug 5.4%), fee for service 48.6%(physical therapy 26.3%, operation 9.7%, laboratory examination 5.2%, radiological examination 3.1%, etc), others 3.4%. In them for the veterans group, fee for physical therapy was 35.3%, fee for hospital accommodation 35.2%, fee for injection 6.2%, fee for operation 5.9%, for the non-veterans one, fee for hospital accommodation 35.7%, fee for operation 16.4%, fee for injection 11.4%, fee for laboratory examination 8.3%. 5. In the comparison of the frequency by Korean standard classification of diseases and distance between the hospital and home, the region under 21.5Km was more frequent in symptoms, signs an abnormal clinical and laboratory findings 56.0%, injury and poisoning and certain other consequences of external causes 55.6%, diseases of the eye and adnexa 52.9%, the one over 21.5Km was more frequent in neoplasms 57.4%, diseases of musculoskeletal system and connective tissue 55.9%, diseases of genitourinary system 53.5%.

  • Erdheim Chester disease (ECD) is very rare non-Langerhans cell histiocytosis (LCH) which occurs in the skeletal system and multiple organs. As it is progressive, sometimes it causes fatal results. However, it is often misdiagnosed as LCH or multiple bone metastasis and, thus, is very difficult to diagnose. In Korea, only 10 cases were first reported in 1999. In particular, there have been a few orthopedic approaches or reports in English-speaking literatures, and no report has been issued in Korea. The authors performed bone biopsy in patients with knee and lower extremity pain who were referred for the integrated treatment. We attempts to report this diagnosis experience with literature review.

A Comparative Study on the Balance of Musculoskelectal System between Long-Term Employed Male Taxi Drivers and General People - through Moire Topography (장기근속 남성 택시기사와 일반인의 근골격계 균형에 관한 비교 연구 - 모아레 체형측정법을 통해)

  • Objectives : The purpose of this study was to investigate effects of long-term taxi driving posture on the balance of musculoskelectal system. Methods : The author practiced Moire topography by using IBS-2000 for 30 male taxi drivers and general people. Then we measured difference of contour lines, difference of shoulder joint height, interval between vertical baseline of pelvis and vertical baseline of neck, ratio of thoracic curve and lumbar curve, difference of width between right and left through Moire topography. After we statistically analyzed difference of Moire topography's data between long-term employed male taxi drivers and general people. Results : 1. Taxi drivers, difference of contour lines in scapular, lumbar, gluteal region was bigger than general people and significant difference(p<0.05) was observed between subject group. 2. Taxi drivers, diference of shoulder joint height was more large than general people and significant difference(p<0.01) was observed between subject group. 3. Taxi drivers, diference of interval between vertical baseline of pelvis and vertical baseline of neck was more large than general people and significant difference((p<0.05) was observed between subject group. 4. Taxi drivers, ratio of thoracic curve was more large than general people and ratio of lumbar curve was more less than general people and significant difference(p<0.05) was observed between subject group. Conclusions : According to above results long-term taxi driving posture might cause musculoskelectal system unbalance.

Determining the Degree of Disability in Workers with Upper Limb Musculoskeletal Disorders using DASH (DASH로 평가한 상지 근골격계 질환자의 기능 제한)

  • This study aimed to examine the development and characteristics of the workers with upper limb musculoskeletal symptoms and disorders and to analyze the upper limb musculoskeletal symptoms and disorders for its relationship with the individual socio-demographic characteristics. This study investigated the effect on the limitations of physical activities using standardized surveillance tool and clinical diagnosis. Musculoskeletal symptoms and the limitations of physical activities were examined. The clinical diagnosis of musculoskeletal disorders were carried out by physical examination, radiological examination and electromyography-electroneuronography for 22 workers in kitchen hood assembly process and 50 workers in toggle process of leather product manufacturing. The proportion of workers with musculoskeletal disorders was higher and the DASH score was also statistically higher in female and aged workers with longer working hours, longer household working hours, less leisure/hobby activity and higher physical load. Physical activities component score increased in the following order: workers in normal health, workers with musculoskeletal symptoms, and workers with musculoskeletal disorders as clinically diagnosed. Score for each DASH component increased in the following order: sports/performing arts ability, social activities, specific physical functional activities, work or other regular daily activities, work ability, psychological activities, insomnia and upper limb symptoms. The overall and each component DASH scores were higher in workers with symptoms of status praesens and of more severity, and receiving medical intervention. Musculoskeletal symptoms and disorders are associated with individual socio-demographic characteristics, and DASH score for physical activities of upper limb was higher in workers with musculoskeletal disorders. Musculoskeletal symptoms and disorders have a remarkable epidemiological significance for physical activities, social activities, work or other regular daily activities, upper limb symptoms and insomnia, where work ability, sports/performing arts ability and preventive measure is needed.
