Purpose: This study was to investigate that near gradient AC/A ratio could be used to prescribe a patient with distance exophoria, we compared the difference between distance gradient AC/A ratio and near gradient AC/A ratio. Also, this thesis was to understand the relationship between calculated AC/A ratio and gradient AC/A ratio. Methods: Objective and subjective refractive error were corrected and we used Howell (3 m) chart for distance phoria tests and Howell-Kim (40 cm) chart for near phoria tests. The near gradient AC/A ratio and calculated AC/A ratio were used by Howell-Kim (40 cm) combined with +1.00 D, -1.00 D, +2.00 D and -2.00 D. Results: The average value of distance exophoria was 1.17${\pm}$1.17 $\Delta$, and the average value of near exophoria was 3.71${\pm}$2.80 $\Delta$ (t-test. p<0.001). The correlation of distance phoria with near phoria was little higher (r = 0.59, p < 0.001). Gradient AC/A ratios depending on measuring distance and stimulus were higher (r = 0.11~0.53. P < 0.001), when distance was shorter and stimulus to accommodation was more. Also, stimulus to accommodation by plus lens was higher than stimulus to accommodation by minus lens (paired t-test. p < 0.001). There was negative correlation between calculated AC/A ratio and gradient AC/A ratio. As the calculated AC/A ratio was higher, gradient AC/A ratio was lesser. Near gradient AC/A ratio was slightly higher than distance gradient AC/A ratio. Distance and near gradient AC/A ratio taken through the subjective -1.00 D were 1.30 $\Delta$/D and 1.68 $\Delta$/D(t-test. t=1.67, p < 0.001). Conclusions: There is negative correlation between calculated AC/A ratio and gradient AC/A ratio. Also, there is subtle difference between near gradient AC/A ratio and distance gradient AC/A ratio. Therefore, we need to measure distance gradient AC/A ratio when a practitioner prescribe glasses for a patient with distance exophoria.
Purpose: The accommodative response and the near horizontal phoria were examined with additional spherical power to analyze the stimulus and response AC/A ratios that suggest reference data for the binocular vision. Methods: The open-field autorefractometer (Nvision-K 5001, Shin nippon) and modified thorington method (MIM card; Muscle Imbalance Measure card, Bernell) at 40 cm were utilized to measure the accommodative response and the near horizontal phoria for 81 persons ($20.89{\pm}1.92$ years old) with additional spherical power. The stimulus and the response AC/A ratios were calculated by gradient AC/A method. Results: The exophoria group showed the highest accommodative response ($1.92{\pm}0.26D$) at 40 cm, followed by orthophoria group and esophoria group($1.72{\pm}0.26D$ and $1.62{\pm}0.42D$, respectively) Meanwhile, the esophoria group showed the biggest ocular deviation for the near ($23.24{\Delta}$) followed by the orthophoria group and exophoria group ($19.76{\Delta}$ and $15.14{\Delta}$, respectively). The biggest difference of the stimulus and the response AC/A ratios was $1.72{\Delta}$ for the exophoria group with -2.00 D, while the one was $3.43{\Delta}$ for the esophoria group with +1.00 D. There was a significant difference between AC/A ratios for the exophoria group with -2.00 D, -1.00 D and the esophoria group with +3.00 D, +2.00D, +1.00D and -1.00D. Conclusions: The difference between stimulus and response AC/A was greater when increased minus spherical power for the exophoria group, while it was greater when increased plus spherical power for the esophoria group. Furthermore, the difference for the esophoria group was a greater than the one for the exophoria group.
The aim of this study was to provide fundamental data for the factors influencing the asthenopia of emmetropia with phoria and alleviation of asthenopia. A total of 348 subjects, aged between 19 and 30 years old, who had no strabismus, an eye trouble or whole body disease, were examined using corrected visual acuity, corrected diopter, stereopsis and suppression tests from September of 2002 to September of 2004. We excluded 21 subjects for the following reasons: if they had an amblyopia affecting binocular vision or inaccurate data. After these exclusions, 327 subjects remained. We then individually measured the refractive error correction, pupillary distance, optical center distance, phoria, convergence, accommodation and the AC/A as well as the asthenopia during binocular vision using a questionnaire. After analysis of factors affecting asthenopia, we also examined the reductive effect of a prism on the asthenopia in subjects who had asthenopia. To determine the factors affecting asthenopia during binocular vision, statistic analyses were carried out using the Chi-square test and the multivariate Logistic regression model. The results of this study were as follow. For asthenopia during near binocular vision of emmetropia with phoria, in case of the lower the accommodation and convergence, a significantly higher rate of asthenopia was observed (p<0.001). When the AC/A is lower, the higher the rate of asthenopia was observed but not significantly and there was no association between phoria and asthenopia. When the multivariate logistic regression model was used to determine factors affecting binocular vision of emmetropia with phoria, in case of the lower accommodation and convergence, a significantly higher rate of asthenopia was observed. when the phoria is esophoria or higher exophoria, or when the AC/A is lower than normal, the higher the rate of asthenopia was observed but not significantly and there was no association between phoria. AC/A and asthenopia. Therefore accommodation and convergence could be predictive factors for asthenopia during near distance binocular vision. Prism was used among' subjects who had asthenopia during near distance binocular vision, the symptom of asthenopia was eased up to 74.2% in emmetropia with phoria.
For this study, we measured heterophoria, Calculated AC/A, Gradient AC/A of pre & post LASIK operation. In case of heterophoria, the results showed that orthophoria 2.78% decreased, exophoria 2.78% increased on pre & post op distance phoria and exophoria 5.55% increased, esophoria 5.55% decreased on pre & post op near phoria. In case of AC/A, Calculated AC/A significantly decreased as 0.36 at AC/A and accommodation decreased as 1.25D (p<0.05). It showed that Gradient AC/A decreased 0.34 and PC/A increased 0.09 but they didn't show significant difference.

Purpose: The aim of this study was to propose effective data for the clinical examinations of binocular vision by comparative analysis of measuring the near horizontal phoria according to accommodative stimulations. Methods: It carried out near horizontal phorias, targeting 104 college students (50 males and 54 females) aged between 19 and 24 ($20.27{\pm}1.31$). It made a comparative analysis of changes in near horizontal phoria according to accommodative stimulations by using the von Graefe, Maddox rod and the Howell phoria methods respectively. We have also investigated the AC/A ratio in all phoria groups. Results: As the additional lenses were changed to the negative (-) diopter, the near horizontal phoria changed to the esodeviation. At this time, the rate of change in the section signifies the AC/A ratio, and the values were not consistent based on the evaluating methods or on each section. The AC/A ratio of the esophoria group appeared the largest value in all groups. As a result of analyzing AC/A ratio at the exophoria group using the von Graefe method, the AC/A ratio was $1.568{\pm}1.937$${\Delta}/D$ on the additional lenses +2.00 D and $2.527{\pm}2.253$${\Delta}/D$ on the additional lenses -1.00 D and at the esophoria group using the Howell phoria method, the AC/A ratio was $5.521{\pm}1.337$${\Delta}/D$, $5.593{\pm}1.623$${\Delta}/D$ on the additional lenses +2.00 D, +1.00 D and $4.687{\pm}1.643$${\Delta}/D$ on the additional lenses -2.00 D. These were significant differences statistically. Conclusions: In the exophoria group, when the (-) lenses were added, the averages of the AC/A ratio were shown to be high but in the esophoria group, when the (+) lenses were added, the of AC/A ratio was high.
Purpose: This study was aimed to investigate effects of increased breath alcohol concentration (BrAC) which is the standard measurement of alcohol consumption in sobriety test under current laws on visual acuity and values of objective refraction. Methods: For twenty three males in 20s (average age $21.17{\pm}2.19$ years, body mass index (BMI) $22.09{\pm}2.16$) were selected. Distance and near visual test was performed at BrAC of 0%, 0.05% and 0.08%, and objective refraction with open-field auto-refractometer was also performed at different BrAC. Results: As breath alcohol concentration is increased, distance visual acuity was decreased, which was statistically significant, but near visual acuity was not changed. Also, values of objective refraction tended to be increased towards minus as breath alcohol concentration is increased. Conclusions: As breath alcohol concentration is increased, corrected visual acuity is decreased and refractive power is towards minus, it is necessary that visual acuity test and refraction measurement should be conducted under sober condition.
A36 patients of a total of 60 persons show $2{\Delta}$ and less, and the rest of 24 patients show $6{\Delta}$ and less. The patient, who increase more in near distance than far distance in exophoria amount are 27 persons. The unchanging patients are 13 persons, and the patients who phoria amount decreasing are 20 persons. Like this, the patients who phoria amount unchange and decrease show more than the half. That is why convergence increases in near distance. The expected value of horizontal phoria in near distance is $3exo{\pm}3$. Only 13 patients get rid of its value, 47 satisfy its value. In AC/A value, 35 persons show under 3(1ow), 21 persons show 3~5(normal), and 4 persons show more than 5(high). By Percival's theory, 36 patients don't feel a slight asthenopia without correcting phoria. Finally, in near point of convergence 27 patients show 10cm and less, 24 patients show 11~15cm, 3 patients show 15~20cm, and 6 patients show 21~25cm. Most of them show high accommodation.
Purpose: The aim of this study was to compare binocular functions such as near point of convergence (NPC), AC/A ratios, heterophoria, accommodation, accommodation lag, positive relative accommodation (PRA), negative relative accommodation (NRA), positive relative convergence (PRC), negative relative convergence (NRC) in normal subjects and convergence insufficiency. Methods: 86 subject (male n=45, female n=41, mean $age{\pm}SD=23.27{\pm}2.85$ years) without amblyopia, strabismus, and ocular pathology were studied. Forty three patients each group were classified as normal subjects and convergence insufficiency group based on AC/A ratio and far and near phoria. Binocular function of the two groups was measured using phoropter. r. Results: The values between normal subjects and convergence insufficiency were 5.71 cm and 7.07 cm for NPC, 5.28 and 2.81 for Heterophria AC/A, 0.92 exo/3.36 exo and 2.17 exo/10.84 exo for far and near phoria, 15.49/23.30/13.30 and 13.50/20.02/9.09 for PRC, respectively. However, there were no significant differences between normal subjects and convergence insufficiency for accommodative amplitude, accommodative lag, PRA, NRA, BI vergence test, BO vergence test, NRC. Conclusions: Both groups were a significant difference for cover test, NPC, Heterophria AC/A, far and near phoria, break point of PRC, and recovery point of PRC.
Purpose: The current study was performed to compare the difference in binocular visual function depending on variable background colors at near work. Method: Fifty four adults (18 males, 34 females) who consented to the present study and had no ocular disease, ocular surgery history, strabismus and amblyopia with normal binocular vision were participated into this study. The subjects were asked to read the novels with black letter printed on white, red, green and blue background for 15 min. Then, their heterophoria, AC/A ratio, near point of convergence, accommodation facility, relative accommodation and vergence were measured before and after reading. The difference of measurements were compared. Result: Overall heterophoria was tended to decrease with regardless of background color. AC/A ratio showed a tendency of increase after reading the novels with all backgrounds except white background. Near point of convergence was significantly increased compared to before reading at all background color. Accommodative facility of dominant and non-dominant eyes were also significantly increased after reading however, binocular accommodative facility showed a tendency of decrease. Negative relative accommodation also decreased at all background colors however, the change of positive relative accommodation was not significantly different. In case of vergence, there was significant difference in break point of far BO and recovery point of far BI by the wavelength of background color. Conclusions: From the results, it was known there is convergence change depending on the wavelength of light even though same amount of accommodation and convergence is required when doing near work for certain period. Thus, it can be suggested that the adjustment of the near working environment which perception of various color was required, should be conducted according to the main wavelength.

Previous studies based on the theoretical evidence and based on the empirical data have been constantly revealing related factors affecting visual fatigue in viewing 3D images. however, the measurement data coupled with the factors that affect the viewers to feel fatigue is insufficient. this study research that the viewers feel the visual functional data is based on subjective fatigue such as IPD, AC/A ratio, and the degree of phoria and the effects on variance in these data, is to examine the correlation between these data. based on the average score of subjective fatigue which is $6.00{\pm}3.88$, total fatigue points from 0 up to 6 is classified as a group not appealing for the fatigue and from 7 up to 16, they were classified as the fatigue appealing group. the results showed no difference in satisfaction of the images by each subjective fatigue, IPD and far phoria. AC/A ratio was higher from the fatigue not appealing group, near phoria was higher from the fatigue appealing group. after watching 3D images, the fatigue not appealing group showed a tendency of higher increase of exophoria. correlations appeared in this experiment showed that AC/A ratio and near phoria are closely related to level of subjective fatigue which feel when watching 3D images. when watching 3D images based on two data appear to be able to predict the fatigue, even during content production fatigue reduce the data dimensions to be used as the main basis is expected.
