The Application of Vacuum-Assisted Closures According to the Texas Staging System in the Treatment of Infective Diabetic Foot Ulcers (감염성 당뇨 족부 궤양 치료에서 Texas 기준 체계에 따른 음압치료 적용)

  • Purpose: A group of patients who were hospitalized for diabetic foot ulcers was classified according to the University of Texas Staging System for Diabetic Foot Ulcers, and we attempted to evaluate whether this staging system could be a criterion for treatment success using vacuum-assisted closure (VAC) technique. Materials and Methods: A total of 32 patients were diagnosed with diabetic foot ulcers according to the University of Texas Staging System for Diabetic Foot Ulcers. Of these, 24 patients who were evaluated as stage B according to the staging system were classified as Group 1, and 8 patients in stage D were classified as Group 2. After applying VAC, the treatment success rate was compared by evaluating the size and severity of ulcers between the two groups. Results: The grade of granulation after VAC was on average 3.75±0.53 in Group 1 and 2.25±0.71 in Group 2. There was better granulation after VAC application in Group 1 (p<0.01). The success rate of the treatment was 22 cases (91.67%) in Group 1 and one case (12.5%) in Group 2. Thus there were statistically significant differences in the success rate of treatment between groups 1 and 2 (Pearson's chisquare test, p=0.01; odd ratio 77.00, 95% confidence interval [CI] 1.26~14.66; relative risk 4.30, 95% CI 1.26~14.66). Conclusion: These results suggest that there was a higher success rate of treatment with VAC in stage B patients. The University of Texas Staging System for Diabetic Foot Ulcers can thus be an index for applying VAC to patients with infective diabetic foot ulcers.

Anti-Colitic Effects of Herbal Medicine HPL-01 on Dextran Sulfate Sodium Induced Ulcerative Colitis in Rat (해동피를 포함한 한약제제 HPL-01의 Dextran sodium sulfate 유도 궤양성 대장염 완화 효과)

  • This study aimed to investigate the anti-inflammatory effects of herbal complex HPL-01 on dextran sodium sulfate(DSS) induced ulcerative colitis in rat. Adult male Sprague-Dawley rats were randomized and divided into six groups. Five groups, excluding the normal group, were orally administered orally HPL-01(50, 100, or 200 mg/kg) for 21 days, and acute colitis was induced during the last 7 days by 4% DSS in the drinking water. The HPL-01 administered DSS-treated rats exhibited significantly reduced colon macroscopic damage index and increased body weight and colonic length 7 days after DSS treatment. Additionally, these rats showed lower serum levels of the pro-inflammatory cytokines TNF-α and IL-6 than those treated only with DSS. HPL-01(100 or 200 mg/kg) also attenuated the DSS-induced increase in the number of white blood cells, granulocytes, and mid cells and improved intestinal damage. Taken together, these results suggest that HPL-01 is a promising anti-inflammatory agent that may be in the treatment of colitis.

A Case Study of Wound Dressing Treatment in a Patient with Diabetic Foot Ulcer (당뇨병성 족부 궤양 환자의 창상 드레싱 치료 증례)

  • Objectives : The purpose of this study is to report on the treatment of wound dressing in patients with diabetic foot ulcers in the dermatology clinic of Korean medicine. Methods : First, sufficient marginal resection of the ulcer and necrotic tissue on the foot of a patient with diabetic peripheral neuropathy was performed. Inflammation was treated with Anti-inflammatory pharmacopuncture solution, and dressings were applied for one month using Cornu Cervi Parvum pharmacopuncture solution, and Haeboo ointment for granulation tissue formation and skin regeneration. Results : After 10 days of the treatment, the formation of new granulation tissue was observed in the necrotic tissue area, and the extent of the lesion decreased sharply from the 14th day. On the 40th day, epithelialization progressed so that the treatment was terminated, and complete keratinization of the site was observed at follow-up 3 weeks after the end of the treatment. Conclusions : In the treatment of wound dressings in diabetic foot ulcer patients, marginal resection treatment with the application of pharmacopuncture solution and herbal ointment showed good effect.

  • Objectives : Ulcerative colitis (UC) is an inflammatory bowel disease, which is one of chronic gastrointestinal disorders. Orostachys japonicus (OJ) has been used as a traditional medicine for various diseases including gastric cancer, gastric ulcers and intoxication. However, the regulatory effect of OJ on intestinal inflammation has not been fully understood, yet. The aim of this study was to investigate the effect of OJ on dextran sulfate sodium (DSS)-induced colitis in mice. Methods : To ascertain the pharmacological effects of OJ, the colitis mice were induced by drinking water containing 5% DSS for 7 days. Mice were randomized into groups receiving OJ (100 mg/kg), sulfasalazine (150 mg/kg) as a positive control, or water as a negative control. We evaluated the effects of OJ on DSS-induced the clinical signs, measuring weight loss and colon length. In addition, the inhibitory effect of OJ on the tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$) level was determined by enzyme-linked immunosorbent assay in colitis tissue. Results : The results indicated that mice treated with DSS showed remarkable clinical signs, including weight loss, and reduced colon length. However, treatment with OJ significantly improved the weight loss and DAI as clinical symptoms. Moreover, OJ reduced the TNF-${\alpha}$ levels in DSS-treated colon tissues. Conclusions : Collectively, the findings of this study provide us with novel insights into the pharmacological actions of OJ as a potential medicine for use in the treatment of ulcerative colitis.

A Review on the of External Ointment Treatment for Diabetic Foot Ulcer (당뇨병성 족부 궤양의 외용 연고 처치에 대한 고찰)

  • Objectives : Diabetic Foot Ulcer(DFU) is one of the common complications of diabetes. DFU is difficult to treat compared to other chronic wounds and clinically effective treatments are limited. The purpose of this study is to review the of external ointment treatment for DFU. Methods : 8 databases such as PubMed, EMBASE, CENTRAL, CNKI, NDSL, RISS, KISS, OASIS including English, Korean and Chinese were searched by structured search strategies that consist of terms as 'diabetic foot', 'diabetic foot ulcer(DFU)' and 'randomized' from January 2001 to January 2021. All randomized controlled trials(RCTs) involving treatment group as external ointment or in combination with conventional treatment were included. Results: A total of 20 RCTs was identified and analyzed. In treatment group, ointment was applied based on conventional treatment. A total of 16 different ointments were used, and the frequency of use was highest in the order of Heat-clearing drug, Blood-activating and stasis-dispelling drug, Tonifying and Replenishing drug and Orifice-opening drug. The most used herbs were in the order of Coptis chinensis Franch., Phellodendron chinense Schneid., Borneolum syntheticum., Angelica sinensis(Oliv.) Diels. In treatment group, a clinically effective effect was obtained compared to the contrast group. Conclusions : The result of this study suggest that external ointment treatment based on conventional treatment can be applied to DFU.

  • Objective: This case study reports on the results of the Korean medical treatment of a major aphthous oral ulcer patient. Methods: A 19-year-old male Korean patient with a major aphthous oral ulcer received acupuncture, herbal medicine, and moxibustion for three weeks in a hospital. Results: After treatment, changes were observed in the numeric rating scale (NRS) from 8 to 5, World Health Organization oral toxicity scale (WHO OTS) from grades 3 to 2, oral perception guide from 11 to 15, and Oral Health Impact Profile-14 (OHIP-14) from 19 to 34. Conclusion: It is worth examining the progress of Korean medical treatment for a major aphthous oral ulcer patient.

Effect of Hwanggeumjackyak-tang (HJT) on the DSS-induced ulcerative colitis mouse model : including changes in intestinal metabolites (황금작약탕이 DSS로 유발된 궤양성 대장염 생쥐 모델에 미치는 영향 : 장내 대사물질 변화를 포함하여)

  • Background : To investigate the effect of Hwanggeumjackyak-tang (HJT) on Dextran sulfate sodium (DSS) induced ulcerative colitis. Methods : The experimental animals were divided into three groups; group 1, normal group(Normal); group 2, DSS-induced colitis and untreated group(UT+DSS); group 3, DSS-induced colitis and HJT 200 mg-treated group(HJT200+DSS). We evaluated cytotoxicity after HJT administration and confirmed the anti-inflammatory effect by histological changes in the intestine and genetic analysis of mucosal cells after HJT administration for each group. In addition, microbiological weapons and metabolites in faeces were examined, and the correlation between gut microbiome and metabolites was also investigated. Result : HJT was not observed to be cytotoxic, even at relatively high concentrations, and was effective in protecting the barrier and preventing intestinal inflammation by suppressing the increase in mucus secretion and the expression of inflammatory factors in mucosal cells. HJT treatment affected the increase in the amount and diversity of the gut microbiome in faeces and the increase in metabolites thought to be involved in alleviating inflammation in the gut. Conclusion : This study demonstrates the therapeutic potential of HJT in ulcerative colitis. Further studies should be carried out to confirm our findings.

  • Aspergillus tracheobronchitis, an uncommon form of invasive pulmonary aspergillosis, is characterized by the development of a pseudomembrane, ulcers, or an obstruction that is predominantly confined to the tracheobronchial tree. Pseudomembranous Aspergillus tracheobronchitis is the most severe form of Aspergillus tracheobronchitis, and only a few cases have been reported in Korea. We report the characteristic chest CT findings in a patient diagnosed with pseudomembranous Aspergillus tracheobronchitis after bronchoscopy and successfully treated by proper antifungal treatment.

  • The below-the-knee arterial tree is the thinnest of all the leg vessels and is an important path for blood flow to the foot. Hence, lesions including stenosis, especially obstruction, may lead to critical limb ischemia which represents the most severe clinical manifestation of peripheral arterial disease. It is characterized by the presence of ischemic rest pain, ischemic lesions, or gangrene attributable to the objectively proven arterial occlusive disease. Typically, the atherosclerotic disease process involving the below-the-knee arterial tree is diffuse in the majority of patients. The cornerstone of therapy is vascular reconstruction and limb salvage. Revascularization should be attempted whenever technically possible, without delay, in patients presenting critical limb ischemia and when the clinical status is not hopelessly non-ambulatory. Therefore, endovascular treatment can become the gold standard for the full range of patients including below-the-knee, limiting the clinical role of the classically trained surgeons.

  • Cases of undifferentiated pleomorphic sarcoma of the thoracic aorta are rare, and usually present with embolic events, renovascular hypertension, or back pain. Mural-based undifferentiated pleomorphic sarcomas that present as ruptured saccular aneurysms are extremely rare and are difficult to differentiate from mycotic aneurysms or penetrating atherosclerotic ulcers. Herein, we report a case of histopathologically proven undifferentiated pleomorphic sarcoma arising from the wall of the descending thoracic aorta that manifested as a mass after thoracic endovascular aortic repair for the treatment of a ruptured saccular aneurysm. We present findings obtained by CT and PET to provide helpful information for the accurate diagnosis and appropriate treatment of future cases.
