Absorbed and effective dose for periapical radiography using portable and wall type dental X-ray machines (이동형 구내방사선촬영기와 벽걸이 구내방사선촬영기로 촬영한 치근단 방사선촬영에서 환자의 흡수선량과 유효선량 평가)

  • Purpose: The purpose of this study was to measure the absorbed dose and to calculate the effective dose for one periapical radiography using the portable and wall type dental X-ray machines. Materials and methods: Thermoluminescent chips were placed at 25 sites throughout the layers of the head and neck of a tissue-equivalent human skull phantom. The man phantom was exposed with the portable and wall type dental X-ray machines. For one periapical radiography taken by portable dental X-ray machine, the exposure setting was 60 kVp, 2 mA and 0.2 seconds, while for one periapical radiography taken by wall type dental X-ray machine, exposure setting was 70 kVp, 8 mA and 0.074 seconds. Absorbed dose measurements were performed and equivalent doses to individual organs were summed using ICRP 103 to calculate effective dose. Results: In the upper anterior periapical radiography using portable dental X-ray machine and in the lower posterior periapical radiography using both machines, the highest absorbed dose was recorded at the mandible body. The effective dose in upper anterior periapical radiography using portable and wall type dental X-ray machines was $4{\mu}Sv$, $2{\mu}Sv$, respectively. In the lower posterior periapical radiography, the effective dose for each portable and wall type dental X-ray machines was $6{\mu}Sv$, $2{\mu}Sv$. Conclusion: It was recommended that the operator use prudently potable dental X-ray machine because that the effective dose in the periapical radiography using wall type dental X-ray machine was lower than that in the periapical radiography using portable dental X-ray machine.

  • 전남대학교 치과진료처에 내원한 환자의 구내방사선사진중 1단계 (Cl: 30개 병소)또는 2단계(C2: 213.8병소)의 인접면 치아우식을 가진 방사선사진 168장을 연구대상으로 하였다. 이들 방사선사진은 코닥사의 Ektaspeed(E group) 필름을 사용하여 평행촬영법으로 촬영하였다. 168장의 방사선사진에 대하여 1단계 및 2단계 치아우식증을 4명의 구강악안면방사선학을 전공한 치과의사들이 같이 판독하여 그 결과를 기록하였다. 또한 168장의 방사선사진을 필름-비디오 변환장치를 이용하여 디지털 영상으로 전환하였다. 이때 486 DX PC에 PCVision과 frame grabber를 이용하였다. 각각의 구내방사선사 진영상은 17"크기의 1280×768픽셀의 해상도를 갖는 모니터상에서 256단계 흑화도를 갖는 700×480픽셀로 제시되었다. 모니터상에서의 치아우식 판독 결과를 구내방사선사진 판독 결과와 비교하였다. 그 결과 sensitivity는 0.98이었다. 1단계 치아우식은 0.87, 2단계 치아우식 은 1.00의 sensitivity를 나타내었다.

Evaluation of alveolar bone density by intraoral periapical radiography (구강 내 치근단 방사선 영상을 이용한 치조골 골밀도 측정의 유용성 평가)

  • Purpose: A method detecting change of jaw or alveolar bone density may be helpful in periodontal care, implant dentistry and evaluation of bone density of whole body. Materials and methods: In this study, bone density of intraoral periapical radiography using phantom-integrated XCP is compared with that of quantitative computed tomography (QCT). Results: Bone density of intraoral periapical radiography and the one measured by QCT showed high correlation (correlation coefficient = 0.92, P<.001) in alveolar bone, and relatively high correlation (0.73, P<.001) in cancellous bone. Conclusion: This study revealed possibility of scoring of bone density by intraoral periapical radiography.

Feasibility study of insertable miniature x-ray source for dental imaging (치과 영상용 삽입형 초소형 X선 튜브의 가능성 연구)

  • Conventional periphery radiography system has a various problem such as patient dose and the pain of X-ray examination. In this paper, to solve these problems, we suggested insertional miniature x-ray system and we verified the feasibility of this system. First, we performed the Geant4 x-ray simulation to design x-ray collimator and filter to use miniature x-ray tube and we decided optimized thickness of filter and collimator. Also, we measured x-ray spectrum using CdTe detector and PX4 module to verify simulation results. Also, we acquired teeth image of fabricated phantom using conventional dental x-ray and prosed miniature x-ray system. As a results, our system has good image quality as compared to those of conventional systems. Our evaluation of the proposed system indicates that it can be potentially very useful for dental imaging.

Comparison between QraypenTM Imaging and the Conventional Methods of Visual Inspection and Periapical Radiography for Proximal Caries Detection in Primary Molars: An In Vivo Study (유구치 인접면 우식 병소 진단에 있어 QraypenTM과 시진 및 구내 치근단 방사선의 비교)

  • The purpose of this study was to evaluate the efficacy of the newly-developed $Qraypen^{TM}$ (All In One Bio, Korea) system for the diagnosis of early proximal caries by comparing it with the conventional methods of visual inspection and periapical radiography. This study was carried out from July 2015 to April 2016 targeting 32 children aged 7~12 years who visited Y-Dental Clinic for school oral health examinations. Two investigators selected and examined a total of 153 primary molars that had not undergone restorative treatment. Comparisons were carried out between visual inspections, readings of posterior periapical radiography images, and readings of $Qraypen^{TM}$ images. This study revealed that the percentage of interproximal surfaces of primary molar teeth without caries incidence was 83.7% using $Qraypen^{TM}$ imaging and 84.9% using visual inspection and periapical radiography. The differences between the two methods were not statistically significant. Thus, $Qraypen^{TM}$ is expected to be a useful and convenient auxiliary diagnostic device that can facilitate the detection of hidden proximal caries in primary molars.

A absorbed and effective dose from the full-mouth periapical radiography using portable dental x-ray machine and panoramic radiography (ORIGINAL ARTICLE - 이동형 구내방사선촬영기로 촬영한 치근단 방사선촬영과 파노라마방사선촬영의 흡수선량과 유효선량 평가)

  • Purpose: The purpose of this study was to measure the absorbed dose and to calculate the effective dose for full-mouth periapical radiography using the portable dental x-ray machine and panoramic radiography Material and Method: Thermoluminescent chips were placed at 25sites throughout the layers of the head and neck of a tissue-equivalent human skull phantom. The man phantom was exposed with the portable dental x-ray machine and panoramic unit. During full-mouth periapical radiography the exposure setting was 60 kVp, 2 mA and 0.15 ~ 0.25 seconds, while during panoramic radiography the selected exposure setting was 72 kVp, 8 mA and 18 seconds. Absorbed dose measurements were obtained and equivalent doses to individual organs were summed using ICRP 103 to calculate of effective dose. Result: In the full-mouth periapical radiography, the highest absorbed dose was recorded at the mandible body follow with submandibular glands and cheek. Using panoramic unit, the highest absorbed dose was parotid glands and the following was back of neck and submandibular glands. The effective dose in full-mouth periapical radiography using portable dental x-ray machine was 46 ${\mu}Sv$. In panoramic radiography, the effective dose was 38 ${\mu}pSv$. Conclusion: It was recommended to panoramic radiography for general check in the head and neck area because that the effect dose in the panoramic radiography was lower than the dose in the full-mouth periapical radiography using portable dental x-ray machine.

The Actual State and the Utilization for Dental Radiography in Korea (국내 치과방사선의 현황 및 이용 실태)

  • The purpose of this study was first to analyze the utilization of dental examination through questionnaire to develop a diagnostic reference level of patient doses for dental radiography in korea. 77 dental institutions were classified into three groups: A group for the dental hospitals of the college of dentistry (11 institutions), B group for dental hospitals (30 institutions) and C group for dental clinics (36 institutions). The results were as follows : The mean numbers of unit chairs and medical staffs were 140.2, 15.3 and 5.8 sets, 112.6, 7.3 and 1.7 dentists, 3.1, 0.5 and no one radiologic technologists, and 19.7, 12.5 and 3.3 dental hygienists in A, B and C groups, respectively. The mean numbers of dental X-ray equipments were 14.64, 3.21 and 2.19 in A, B and C groups, respectively. Intraoral dental X-ray unit was used the most, the following equipments were panoramic, cephalometric, and cone-beam CT units. The most used X-ray imaging system was also digital system (above 50%) in all three groups. Insight dental film (Kodak, USA) having high sensitivity was routinely used for periapical radiography. The automatic processor was not used in many dental institutions, but the film-holding device was used in many dental institutions. The utilization rates of PACS in A, B and C groups were 90.9%, 83.3% and 16.7% respectively, and the PACS software program was used the most PiView STAR (Infinitt, Korea). The annual mean number of radiographic cases in one dental institution in 2008 for A group was 6.8 times and 21.2 times more than those for B and C groups, and periapical and panoramic radiographs were taken mostly. Tube voltage (kVp) and tube current (mA) for periapical radiography were similar in all three groups, but exposure time in C group was 12.0 times and 3.5 times longer than those in B and C groups. The amount of radiation exposure in C group, in which dental hygienists take dental radiographs, was more than those in other groups. The exposure parameters for panoramic radiography were similar in all three groups. In conclusion, the exposure parameters in dental radiography should be determined with reference level, not past experiences. Use of automatic processor and film-holding devices reduces the radiation exposure in film system. The quality assurance of dental equipments are necessary for the reduction of the patient dose and the improvement of X-ray image quality.

  • Purpose: The purpose of this study was to measure the absorbed dose and to calculate the effective dose for periapical radiography done by portable intraoral x-ray machines. Materials and Methods: 14 full mouth, upper posterior and lower posterior periapical radiographs were taken by wall-type 1 and portable type 3 intraoral x-ray machines. Thermoluminescent dosemeters were placed at 23 sites at the layers of the tissue-equivalent ART woman phantom for dosimetry. Average tissue absorbed dose and radiation weighted dose were calculated for each major anatomical site. Effective dose was calculated using 2005 ICRP tissue weighted factors. Results: On 14 full mouth periapical radiographs, the effective dose for wall-type x-ray machine was 30 Sv; for portable x-ray machines were 30 Sv, 22 Sv, 36 Sv. On upper posterior radiograph, the effective dose for wall-type x-ray machine was 4 Sv; for portable x-ray machines doses were 4 Sv, 3 Sv, 5 Sv. On lower posterior radiograph, the effective dose for wall type x-ray machine was 5 Sv; for portable x-ray machines doses were 4 Sv, 4 Sv, 5 Sv. Conclusion: Effective doses for periapical radiographs performed by portable intraoral x-ray machines were similar to doses for periapical radiographs taken by wall type intraoral x-ray machines.
