EFFECT OF RESTORATION TYPE ON THE STRESS DISTRIBUTION OF ENDODONTICALLY TREATED MAXILLARY PREMOLARS; THREE-DIMENSIONAL FINITE ELEMENT STUDY (수복물의 종류가 근관치료된 상악 제2소구치의 응력분포에 미치는 영향: 3차원 유한요소법적 연구)

  • The purpose of this study was to investigate the effects of four restorative materials under various occlusal loading conditions on the stress distribution at the CEJ of buccal. palatal surface and central groove of occlusal surface of endodontically treated maxillary second premolar, using a 3D finte element analysis. A 3D finite element model of human maxillary second premolar was endodontically treated. After endodontic treatment, access cavity was filled with Amalgam, resin, ceramic or gold of different mechanical properties. A static 500N forces were applied at the buccal (Load-1) and palatal cusp (Load-2) and a static 170N forces were applied at the mesial marginal ridge and palatal cusp simultaneously as centric occlusion (Load-3). Under 3-type Loading condition, the value of tensile stress was analyzed after 4-type restoration at the CEJ of buccal and palatal surface and central groove of occlusal surface Excessive high tensile stresses were observed along the palatal CEJ in Load-1 case and buccal CEJ in Load-2 in all of the restorations. There was no difference in magnitude of stress in relation to the type of restorations. Heavy tensile stress concentrations were observed around the loading point and along the central groove of occlusal surface in all of the restorations. There was slight difference in magnitude of stress between different types of restorations. High tensile stress concentrations around the loading points were observed and there was no difference in magnitude of stress between different types of restorations in Load-3.

  • 본 연구는 치수강 상아질 부위 및 상아질 접착제 종류에 따른 결합 강도를 측정하고 이들 사이의 상관관계를 구명하고자 시행되었다 45개의 대구치를 포매 후, 대조군에서는 교합면 법랑질 제거 후 #600 SiC paper까지 순차연마하여 상아질을 노출시켰고, 실험군에서는 치수강 개방 후 1시간동안 NaOCl에 보관 후 axial wall과 pulpal floor를 노출시켰다. 노출된 상아질 면에 상아질 접착제를 적용한 후 Z-100을 충전한 다음 40초간 광중합하였다. 사용된 상아질 접착제는 Scotchbond Multi-Purpose와 Single Bond, Clearfil SE Bond였다. $37^{\circ}C$ 증류수에 24시간 보관 후, 저속 diamond saw를 이용하여 0.7mm 두께로 수직절단하고 고속 diamond point(#104)로 단면적 $1{mm}^2$가 되도록 시편을 제작하고, Universal testing machine에서 미세인장 결합강도를 측정하였다. 1. 모든 상아질 접착제의 미세인장강도는 대조군, axial wall군, pulpal floor군 순으로 감소하는 경향을 보였다. 2. 대조군에서 SM과 BB는 SE에 비해 유의성 있게 높은 결합강도를 나타내었다(p<0.05). 3. SM과 SB는 대조군에 비해 axial wall군과 pulpal floor군에서 유의성있게 낮은 결합강도를 보였으나, SE에서는 pulpal floor군만이 유의성 있게 낮은 결합강도를 보였다(p<0.05). 4. Axial wall군과 pulpal floor군에서는 상아질 접착제의 종류에 따른 유의차를 보이지 않았다. 5. 전자현미경 소견에서는 대조군에 비해 axial wall군과 pulpal floor군이 더 부드러운 접착 계면을 나타내었다. 혼성층의 두께는 결합강도의 감소와는 관련이 없었다.

THE EFFECTS OF DENTIN BONDING AGENT THICKNESS ON STRESS DISTRIBUTION OF COMPOSITE-TOOTH INTERFACE : FINITE ELEMENT METHOD (상아질 접착제의 두께가 치아와 복합레진 경계의 응력발생에 미치는 영향에 관한 유한요소법 연구)

  • The aim of this study was to examine that thick dentin bonding agent application or low modulus composite restoration could reduce stresses on dentin bonding agent layer. A mandibular first premolar with abfraction lesion was modeled by finite element method. The lesion was restored by different composite resins with variable dentin bonding agent thickness ($50{\mu}m$, $100{\mu}m$, $150{\mu}m$). 170N of occlusal loading was applied buccally or lingually. Von Mises stress on dentin bonding agent layer were measured. When thickness of dentin bonding agent was increased von Mises stresses at dentin bonding agent were decreased in both composites. Lower elastic modulus composite restoration showed decreased von Mises stresses. On root dentin margin more stresses were generated than enamel margin. For occlusal stress relief at dentin boning agent layer to applicate thick dentin bonding agent or to choose low elastic modulus composite is recommended.

A MORPHOMETRIC STUDY OF THE MAXILLARY PRIMARY FIRST MOLARS USING THREE-DIMENSIONAL SCANNER (3차원 스캐너를 이용한 상악 제1유구치 치관의 크기와 형태에 관한 연구)

  • The purpose of this study was to elucidate the size and morphologic characteristics of maxillary primary first molars in Korean children using three-dimensional laser scanner and compare three-dimensional image with preformed stainless steel crown. Scanned three-dimensional images of dental cast taken from 132 children(male 62, female 70) by three-dimensional laser scanner(Breuckmann opto-Top HE100, INUS, Korea) were used. Mesiodistal diameter, buccolingual diameter, occlusogingival height and crown shape of each image were calculated by Rapidform 2004 program(INUS, Korea). The values were statistically compared by independent samples t-test with 95% of significant level. The results were as follows : 1. No significant difference in crown size was found between left and right maxillay primary first molar(p>0.05). 2, Significant difference in mesiodistal diameter, buccolingual diameter, buccal occlusogingival height was found between male and female (p<0.05), and crown size of male was bigger than that of female. 3. Average image of maxillay primary first molar was shaped three-dimensionally and measured. In comparison with 3M stainless steel crown, this image was similar with No.4 or No.5 SS crown in male, No.4 in female. In comparison with ILSUNG SS crown, this image was similar with No.5 in male, No.4 in female. 4 Mesiolingual line angle area, distolingual line angle area and buccogingival ridge were more obvious in average image than 3M stainless steel crown. ILSUNG SS crown was more square and had longer mesiodistal diameter than average 3D image.

A COMPARATIVE ANALYSIS OF CLASS II DIVISION 1 TREATMENTS : ADOLESCENTS CONTRASTED WITH ADULTS (성장기 아동과 성인에서의 II급 1류 부정교합치료양상에 관한 두부방사선 계측학적 비교연구)

  • The purpose of this study was to quantitate differences in the nature of the correction of Angle's Class II div 1 malocclusion dependent on the patient's age at the time of treatment. The sample consisted of 27 female patients in the adolescent group with a mean initial records age of 11.8 years and 25 female patients in the adult group with a mean starting age of 21.1 yrs. Lateral cephalometric head films were taken before and after orthodontic treatment with four bicuspid extraction. The results were obtained as follows. 1. None of maxillary skeletal parameters exhibited a significantly different in treatment change between adolescents and adults. But, in mandibular skeletal measurements, there were significant differences between two groups. (P<0.05) 2. Measures of vertical dimension in the adults remained unchanged during treatment, reflecting the effective absence of growth. 3. The steepness of occlusal plane in the adults changed significantly.(P<0.05) In contrast, the adolescents displayed stability of the occlusal plane. 4. According to the Johnston analysis, there was a significant difference in the total molar correction between two groups.(P<0.05) 5. According to the Johnston analysis, differential mandibular growth in the adolescents contrubuted $63\%$ of the total molar correction, with orthodontic tooth movement accounting for the remaining $37\%$. In the adults, dental movement comprised $99\%$ of the correction.

  • It is well known that there are different craniofacial skeletal groups in the same malocclusion types. The present study was performed to determine morphologic sub-groups in Korean children with Class III malocclusions, and to find out morphological differences between subgroups by means of a cross-sectional longitudinal cephalometric study. In this study, 135 children aged 6 to 14 year-old with untreated Class III malocclusions were selected. The samples were divided into two groups depending on the angulation of middle cranial fossa (MCF). That is, when the MCF of an individual was measured smaller than 40.3, he/she was tossed into mandibular protrusive-effect group(MREG), while when an individual was measured larger than 40.3, he/she was tossed into mandibular retrusive-effect group(MREG). Thereafter, the grouped samples were divided into 4 age groups(7, 9, 11 and 13 year-old). Thirty four linear and angular measurements on the tracings of lateral cephaloradiographs were measured, and the morphological characteristics and differences were compared and analysed by means of Wilcoxon test. It was found that Korean children with Class III malocclusions were divided into two groups, in which 39.3% were belonged in the MREG and 60.7% were in the MREG. In the MREG, anterior-posterior length of cranial base, nasomaxillary complex, maxilla and mandible were larger than the MREG. And although there was no difference in the total length of mandible (Co-Gn), mandibular body length (Go-Gn) was larger in the MREG during the majority of the observed periods. These results would suggest that a majority of the samples, 60.7%, demonstrated many of configurations of craniofacial skeletal relationships that can be found in the leptoprosopic faceform.

Effects of Facemask Therapy for Class III Malocclusions in Patients with Different Vertical Skeletal Patterns (3급 부정교합 환자의 수직적 골격 양상에 따른 facemask 치료 효과 비교)

  • The purpose of this study was to evaluate the skeletal and dentoalveolar effects of facemask therapy and to compare the anchorage of a bonded expander in patients with Class III malocclusion and different vertical skeletal patterns. Twenty subjects with Class III malocclusion were included in this study and were treated with a facemask and bonded expander. Based on the FMA, subjects were divided into two groups of 10 patients each: a high vertical group (HV; mean FMA $33.56^{\circ}$) and an average vertical group (AV; mean FMA $24.88^{\circ}$). Lateral cephalograms were taken and evaluated before and after treatment. In both groups, forward movement of the maxilla and backward rotation of the mandible were observed after treatment, with no statistical differences between the groups. Vertical skeletal variables increased in both groups, but the increase of FMA was significantly larger in the HV group than the AV group. Mesial movement of maxillary molars and proclination of maxillary incisors which indicate anchorage loss of bonded expander were observed in both groups, with no significant differences between the groups. In conclusion, facemask therapy resulted in effective maxillary protraction in both HV and AV groups. However, the open bite tendency was increased more in the HV group.

The Effect of Thickness of Porcelain on Shear Bond Strength Between Heat-Pressed Porcelain and Non-Precious Metal (도재의 두께가 하부금속과의 전단결합강도에 미치는 영향)

  • The purpose of this study was to investigate the effect of various thickness of porcelain on shear bond strength between metal coping and porcelain. So, various thickness of feldspathic porcelain and heat-pressed porcelain were built up and compared. 120 metal cube($4{\times}4{\times}4mm$) specimens were prepared. 60 specimens were applied to feldspathic porcelain and the others were applied to heat-pressed porcelain by 1mm, 1.5mm, 2mm, 2.5mm, 3mm and 3.5mm thickness. The measurement of shear bond strength was performed by Instron universal testing machine. The following results were obtained from this study. 1. As thickness of feldspathic porcelain increases, shear bond strength has decreased. Feldspathic specimens with 1mm porcelain thickness were significantly stronger than other feldspathic subgroups. 2. There was no significant difference of shear bond strength according to porcelain thickness in heat-pressed porcelain group. 3. In comparison between subgroups with same thickness, feldspathic porcelain group had stronger shear bond strength than heat-pressed porcelain. There were significant difference between 1mm and 3mm porcelain thickness group. 4. In almost cases, fracture surface was found on both metal and porcelain surfaces. As thickness of porcelain was increased, metal exposure was decreased.

FRACTURE STRENGTH OF THE IPS EMPRESS CROWN: THE EFFECT OF OCCLUSAL DEPTH AND AXIAL INCLINATION ON UPPER FIRST MOLAR (IPS Empress 도재관의 파절강도 : 상악 제1대구치에서 교합면 두께와 축면경사도에 따른 영향)

  • The purpose of this study was compare the fracture strength of the IPS Empress ceramic crown according to the occlusal depth (1.5mm, 2.0mm, 2.5mm) and axial inclination ($4^{\circ},\;8^{\circ},\;12^{\circ}$) of the upper first molar. After 10 metal dies were made for each group, the IPS Empress ceramic crowns were fabricated and were cemented with resin cement. The cemented crowns mounted on the testing jig were inclined 30 degrees and a universal testing machine was used to measure the fracture strength. The results of this study were as follows: 1. The fracture strength of the ceramic crown with 2.5mm depth and $8^{\circ}$ inclination was the highest (1393N). Crowns of 1.5mm depth and $4^{\circ}$ inclination had the lowest strength (1015N) 2. There were no significant differences of the fracture strength according to occlusal depth and axial inclination. 3. Most fracture lines began at the loading area and extended through proximal surface perpendicular to the margin, irrespective of occlusal depth. 4. There was positive correlation between the fracture strength and the fracture surface area of crowns.
