THE TREATMENT OF CLASS III MALOCCLUSION USING FR-III (FR-III를 이용한 3급 부정교합 환자의 치험례)

  • In children with Class III malocclusion, it is important to identify whether the etiology is functional, dental, skeletal. FR-III developed by Rolf Frankel in 1970, has been used during deciduous, mixed, and early permanent dentition to correct class III malocclusion characterized by maxillary skeletal retrusion. According Frankel, the vestibular shields and upper labial pads act to counteract the forces of the surrounding musculature that restrict forward maxillary development and cause a retrusion in maxillary tooth position. This can achieve favorable developments with the basal bone, teeth and alveolar bone. We report FR-III that can be applicated in cases of early mixed dentition with mild maxillary deficiency and deepbite before the eruption of permanent maxillary incisor.

Three-dimensional analysis of the cusp variation patterns of mandibular second premolar in Koreans (일부 한국인 하악 제2소구치 교두 변이 양상에 대한 3차원적 분석)

  • Purpose: This study aims to investigate the cusp variation pattern of the Korean mandibular second premolar and to determine the difference in tooth diameter and surface area using a virtual three-dimensional model. Methods: Dental casts from 69 students were scanned as a virtual dental models with a three-dimensional dental model scanner. Tooth diameter, absolute and relative individual areas, total crown area, the number of lingual cusps and central groove pattern were analyzed using RapidForm 2004. The Mann-Whitney U-test and the Kruskal-Wallis test were performed to verify sexual dimorphism, the difference in tooth diameter and surface according to the cusp variation pattern (α=0.05). Results: There was no significant difference except in buccolingual diameter (p<0.05) and buccolingual diameter at the cervix (p<0.05). The relative surface area of the total clinical crown was 65.76% for the buccal half and 34.24% for the lingual half, with a ratio of 2:1. In the case of the presence of two lingual cusps, the ratio was 21.47% for the mesiolingual half and 14.12% for the distaolingual half, with a ratio of 3:2. The dominant central groove patterns of the second premolar were the H-pattern (42.0%), followed by the Y-pattern (37.7%), then the U-pattern (20.3%). The relative buccal half was largest in the U-pattern central groove and the relative lingual half was largest in the Y-pattern central groove (p<0.05). Conclusion: This study is significant in that it measured quantitative surface areas and the findings could be a meaningful reference to comprehend dental anatomy in Koreans.

  • Incontinentia pigmenti, also called Bloch-Sulzberger syndrome, is a rare X-linked inherited dominant disorder that affects females, but causes spontaneous abortion of prenatal males. Incontinentia pigmenti is a systemic disease with clinical features similar to ectodermal dysplasia, including congenitally missing teeth. The pathogenesis is related to gene mutations in NF-kappa-B essential modulator on chromosome Xq28. Incontinentia pigmenti is caused by a defect in the developmental stage of organs originating from the ectoderm or mesoderm and involves the skin, eyes, hair, teeth and central nervous system. This report discusses the management of three cases of 3 to 5 years old females with incontinentia pigmenti and accompanying multiple missing teeth. The cases had sparse hair, and showed oligodontia and anomalous crowns with supplementary cusps in the posterior teeth and conical anterior teeth. Removable space maintainers were applied, achieving improved esthetics, recovery of mastication and increased self-esteem in the patients.

Full mouth rehabilitation on the patient with class II jaw relation and posterior bite collapse using reestablishment of occlusal vertical dimension: a case report (구치부 교합지지가 상실된 II급 악간관계 환자의 교합 재설정을 통한 완전 구강회복 증례)

  • Loss of molar support and abnormal jaw relationship lead to occlusal disharmony and cause pathologic signs. Full mouth rehabilitations with reestablishment of occlusal schemes are needed. In this case, the 75 year-old female patient showed posterior bite collapse, irregular occlusal plane and Class II jaw relationship. By observing her profile and interocclusal distance, she was diagnosed as loss of occlusal vertical dimension. Treatment plan is to restore maxillay class I removable partial denture and mandibular fixed prosthesis and to establish vertical dimension and harmonious occlusal plane. Occlusal vertical dimension of 19 mm, which is obtained by 7.5 mm increase between maxillary right lateral incisor and mandibular canine, was established using temporary prosthesis via diagnostic wax-up. Patient adaptation with newly formed vertical dimension was verified during 8 week follow-up period. Within the information of interim prostheses, final restoration was constructed and delivered. The patient showed sound occlusal scheme and esthetic profile.

  • In the latest medical world, the attempt of reconstructing anatomical human body system using computer graphics technology awakes people's interests. Actually, this trial has been made in dentistry too. There are a lot of practicable technology fields using computer graphics in dentistry For example, 3D visualization and measurement of dental data, detection of implant location, surface reconstruction for restoring artificial teeth in prostheses and relocation of teeth in orthodontics can be applied. In this paper, we propose methods for definitely detecting the geometric features of teeth such as cusp, ridge, fissure and pit, which have been used as most important characteristics in dental applications. The proposed methods are based on the approximate curvatures that are measured on a 3D tooth model made by scanning an impression. We also give examples of the geometric features detected by using the proposed methods. Comparing to other traditional methods visually, the methods are very useful in detecting more accurate geometric features.

  • Attrition is the loss of tooth hard tissue due to contact between teeth, and in severe cases, dentin is exposed, accompanied by selective corrosion and excessive wear of teeth, which is called cupping. If these lesions are left untreated, the size of the lesion gradually increases, breaking the unsupported enamel, resulting in a decrease in aesthetics and chewing function. In this case report, patients with cupping and enamel fracture due to severe attrition were directly restored using a resin with soft properties containing organic fillers. In the follow-up observation six years later, most of the filling of the occlusal surface was eliminated, but the filling on the buccal surfaces remained relatively intact, and it was confirmed that this type of resin was suitable for the area where the occlusal force was relatively weak rather than the area where the occlusal force was greatly applied.

  • Purpose: The purpose of this study was to compare the stress distribution characteristics of four different abutment connections on SS-$III^{(R)}$ fixture under occlusal loading, using 3-dimensional finite element method. Materials and methods: The fixture of SS-$III^{(R)}$ (Osstem, Korea) with 4 mm diameter and 11.5 mm length and 4 types of abutments were analyzed; Solid, Com-Octa, ComOcta Gold, and Octa abutment. The models were placed in the area of first molar in the mandible. The 4 loading conditions were; (1) the vertical loading of 100 N on the central fossa, (2) the vertical loading of 100 N on the buccal cusp, (3) the $30^{\circ}$ inclined loading of 100 N to lingual side on the central fossa, and (4) the $30^{\circ}$ inclined loading of 100 N to the lingual side on the buccal cusp. The 3G.Author program was used, the von-Mises stress was calculated and the stress contours were plotted on each part of the implant systems and the surrounding bone structures. Results: Regardless of abutment types and loading conditions, higher stress concentration was observed at the cortical bone. In cancellous bone, the highest stress was observed at apical portion and the maximum stress occurred at the implant neck. The higher internal stress was observed in the fixtures than in the bone. The lowest stress was observed at loading condition 1 and the stress concentration was also lower than any other loading conditions. Conclusion: Within the limitation of the result of this study, it seems that the abutment connection type does not affect much on the stress distribution of bone structure.

MORPHOLOGICAL FEATURES OF THE CROWNS OF MAXILLARY SECOND PRIMARY MOLAR AND FIRST PERMANENT MOLAR: AN ODONTOMETRIC STUDY (상악 제2유구치와 상악 제1대구치의 치관 형태: 치아계측학적 연구)

  • The basic structure of the primary crowns usually resemble their succeeding permanent teeth. However, maxillary second primary molars resemble ipsilateral first permanent molars. Accordingly in this study, odontometric data of the two teeth was obtained, then analyzed to verify the morphological relationship and sex difference between the two teeth. Dental study casts were examined for their mesiodistal and buccolingual width of the crowns, diameter of each cusps, and intercuspal distances. Photographs of the crowns were taken to measure the angles between each cusp tip. The results are as follows : 1. In boys, PrI, PaI, DBC angle, and MBC angle did not show any statistically significant difference between the two teeth(p>0.05), and moderate level of correlations were observed. 2. In girls, crown index, DBC angle, and MBC angle of the two teeth did not show any statistically significant difference(p>0.05), and moderate level of correlations were recognized. 3. Measurements that did not show statistically significant difference between the two teeth in both boys and girls were DBC angle and MBC angle(p>0.05). In DLC angle, however, statistically significant difference was observed(p<0.01). 4. Most of the measurements showed sex differences, except DBC angle, which did not show any sex difference in both teeth(p>0.05).

INFLUENCE OF THREE DIFFERENT PREPARATION DESIGNS ON THE MARGINAL AND INTERNAL GAPS OF CEREC3 CAD/CAM INLAYS (세 가지 다른 인레이 와동 형태가 CEREC3 CAD/CAM의 변연 및 내면 간극에 미치는 영향)

  • The aim of this study was to evaluate the marginal and internal gaps in CEREC3 CAD/CAM inlays of three different preparation designs. CEREC3 Inlays of three different preparation designs (n=10) were fabricated according to Group I-conventional functional cusp capping/shoulder preparation, Group II-horizontal reduction of cusps and Group III-complete reduction of cusps/shoulder preparation. After cementation of inlays. the bucco-lingual cross section was performed through the center of tooth. Cross section images of 20 magnifications were obtained through the stereomicroscope. The gaps were measured using the Leica application suite software at each reference point. Statistical analysis was performed using one-way ANOVA and Tukey's test (${\alpha}<0.05$). The marginal gaps ranged from 80.0 to $97.8{\mu}m$ for Group I, 42.0 to $194.8{\mu}m$ for Group II, 51.0 to $80.2{\mu}m$ for Group III. The internal gaps ranged from 90.5 to $304.1{\mu}m$ for Group I, 80.0 to $274.8{\mu}m$ for Group II, 79.7 to $296.7{\mu}m$ for Group III. The gaps of each group were the smallest on the margin and the largest on the horizontal wall. For the CEREC3 CAD/CAM inlays, the simplified designs (groups II and III) did not demonstrate superior results compared to the traditional cusp capping design (group I).

  • The purpose of this study was to investigate the effects of composite resin restorations on the stress distribution of notch shaped noncarious cervical lesion using three-dimensional (3D) finite element analysis (FEA). Extracted maxillary second premolar was scanned serially with Micro-CT (SkyScan1072 ; SkyScan, Aartselaar, Belgium). The 3D images were processed by 3D-DOCTOR (Able Software Co., Lexington, MA, USA). ANSYS (Swanson Analysis Systems, Inc., Houston, USA) was used to mesh and analyze 3D FE model. Notch shaped cavity was filled with hybrid or flowable resin and each restoration was simulated with adhesive layer thickness ($40{\mu}m$) A static load of 500 N was applied on a point load condition at buccal cusp (loading A) and palatal cusp (loading B). The principal stresses in the lesion apex (internal line angle of cavity) and middle vertical wall were analyzed using ANSYS. The results were as follows 1. Under loading A, compressive stress is created in the unrestored and restored cavity. Under loading B, tensile stress is created. And the peak stress concentration is seen at near mesial corner of the cavity under each load condition. 2. Compared to the unrestored cavity, the principal stresses at the cemeto-enamel junction (CEJ) and internal line angle of the cavity were more reduced in the restored cavity on both load con ditions. 3. In teeth restored with hybrid composite, the principal stresses at the CEJ and internal line angle of the cavity were more reduced than flowable resin.
