  • Sudan IV는 2시간 관류시키는 동안 약 30%의 농도감소를 보였다 Sudan IV자체는 물에 녹지않고 리포솜에 의해서 가용화 되는 물질이기 때문에 관류액중에서 리포솜으로부터 release out되지 않는다. 따라서 situation I의 가능성은 희박하며 또 그 자체로는 물에 거의 녹지않아 흡수되지 않을 것으로 예상되는 물질이므로 Sudan IV의 관류액으로부터의 소실은 곧바로 situation II를 반영할 것이다. 또 리포솜의 구성물질인 PC역시 관류액중에서 농도감소를 나타내었는데 이로부터 난용성약물을 봉입한 리포솜이 어떻게 장관으로 흡수되는 지 예상할 수 있었다.

Comparison of Regional Myocardial Perfusion, Left Ventricular Volume and Ejection Fraction between 8- and 16-Frame Gated SPECT (8-단위영상과 16-단위영상으로 얻은 게이트 심근관류 스펙트에서 국소 심근관류, 좌심실 용적과 구혈률의 비교)

  • 목적: 게이트 심근관류 스펙트는 심근의 관류와 좌심실 기능을 동시에 평가할 수 있는 유용한 방법이다. 현재, 게이트 영상 획득시 8 단위영상이 널리 사용되지만, 아직 가장 적절한 단위영상의 수에 대해서는 논의의 여지가 있다. 본 연구는 동일한 환자에서 8 단위영상과 16 단위영상으로 게이트 심근관류 스펙트를 2회 연속적으로 시행하여 국소적 심근관류, 좌심실 용적과 구혈률등을 비교하였다. 대상 및 방법: 42명(평균 연령: 55세, 남: 녀=26: 16)의 관상동맥 질환이 의심되거나 알려진 환자를 대상으로 안정시 Tc-99m MIBI를 이용한 게이트 스펙트를 8 단위영상과 16 단위영상으로 2회 연속하여 시행하였다. 좌심실의 확장기말 용적, 수축기말 용적과 구혈률을 각각 얻었으며 확장기말의 단층영상을 18 분절로 나누고 각 분절마다 심근의 관류정도를 4등급으로 나누어 국소적 심근관류를 평가하였다. 결과: 8 단위영상과 16 단위영상 게이트 스펙트에서 국소적 심근관류의 일치도는 85.7% (tau-b=0.786, p<0.001)로 우수하였다. 두 방법간에 1등급의 차이는 100 분절(13.2%), 2등급 차이는 8 분절(1.1 %)이 있었고 3등급 차이는 없었다. 좌심실의 확장기말 용적, 수축기말 용적, 구혈률은 두 방법간에 높은 상관성을 보였으며(r=0.993, 0.989, 0.935), 좌심실 구혈률은 8 단위영상을 사용할 때 평균 1.8% 낮게 측정되어 통계적인 유의성은 있었으나(p<0.05), 그 차이는 크지 않았다. 결론: 8 단위영상과 16 단위영상을 사용한 게이트 심근관류 스펙트에서 국소적 심근관류의 일치도는 우수하였으며, 좌심실의 기능적 계수의 차이도 크지 않았다. 비록 높은 단위영상을 사용했을 때 더 정확하고 자세한 정보를 얻을 수 있다 하더라도, 구혈률과 용적 등을 얻는 일상적인 임상검사에는 8 단위영상 또한 그 역할을 충분히 할 수 있을 것으로 사료된다.

Expression of Bcl-2 Protein in Ischemia-Reperfused Myocardium of Rabbit (가토 허혈-재관류 심근에서의 Bcl-2 단백의 발현)

  • Background: Myocardial cell death after myocardial infarction or reperfusion is classified into necrosis and apoptosis. Bcl-2 protein is a cytoplasmic protein, which inhibits apoptosis and is expressed in acute stage of myocardial infarction but not in normal heart. This study was performed to investigate whether Bcl-2 protein was expressed respectively to the reperfusion time. Materials and methods: Thirty nine New Zealand white rabbits weighing 1.5-4.8 kg (mean, 2.9kg) were alloted into 7 groups (n=5 in each group) which underwent left anterior descending coronary artery(LAD) occlusion for 30 minutes, followed by reperfusion. The animals were sacrificed at 1, 4, 8, 12, 24 hours, and 3, 7 days after occlusion. Ventricle was excised immediately after intervention. Tissues were fixed in 10% buffured formalin and embedded in paraffin. Bcl-2 protein was detected by immunohistochemical stain with using monoclonal antibody against Bcl-2 protein. Results: The positive immunohistochemical reactivity for Bcl-2 protein was observed in 12, 24 hours, and 3 days reperfusion groups. Bcl-2 protein was detected in salvaged myocytes surrounding the infarcted area. Conclusions: Bcl-2 protein is expressed at the late acute stage of infarct. Therefore, the expression of Bcl-2 protein may not protect acute cell death, but may play a role in the prevention of late cell death after myocardial is chemia-reperfusion.

  • The use of music as a means of inducing positive emotions and subsequent relaxation has been extensively studied by researchers. A great deal of this research has focussed on the use of music as a means of reducing feelings of anxiety and stress as well as aiding in the relief of numerous pathologies. The purpose of this study was to evaluate this effect of music using laser doppler flowmeter that monitors relative changes in the muscular blood cell perfusion, concentration of moving blood cells, mean velocity of the myofascial pain disorder patients. Total of 30 subjects were selected. They were monitored by LDF(Laser Doppler Flowmeter) during music listening for 3 minutes. LDF probe was placed over the temporalis muscle. The obtained results were as follows: 1. The average blood perfusion for 3 minutes of music listening at surface of temporalis muscles were elevated on both affected and unaffected parts compared to rest time. 2. The average of blood perfusion elevated during music listening at surface of temporalis muscles were higher in affected part than unaffected part. 3. Increase of blood perfusion during music listening is proportioin to that before music listening. In summary, these results may encouraging start to the elucidation of the hemodynamic events occuring during music listening and aimed to serve as a base for further studies.

Effect of Reperfusion after 20 min Ligation of the Left Coronary Artery in Open-chest Bovine Heart: An Ultrastructural Study (재관류가 허혈 심근세포의 미세구조에 미치는 영향 : 재관류 손상에 관한 연구)

  • Background: It has been well documented that transient occlusion of the coronary artery causes myocardial ischemia and finally cell death when ischemia is sustained for more than 20 minutes. Extensive studies have revealed that ischemic myocardium cannot recover without reperfusion by adequate restoration of blood flow, however, reperfusion can cause long-lasting cardiac dysfunction and aggravation of structural damage. The author therefore attempted to examine the effect of postischemic reperfusion on myocardial ultrastructure and to determine the rationales for recanalization therapy to salvage ischemic myocardium. Materials and methods: Young Holstein-Friesian cows(130∼140 Kg body weight; n=40) of both sexes, maintained with nutritionally balanced diet and under constant conditions, were used. The left anterior descending coronary artery(LAD) was occluded by ligation with 4-0 silk snare for 20 minutes and recanalized by release of the ligation under continuous intravenous drip anesthesia with sodium pentobarbital(0.15 mg/Kg/min). Drill biopsies of the risk area (antero-lateral wall) were performed at just on reperfusion(5 minutes), 1-, 2-, 3-, 6-, 12-hours after recanalization, and at 1-hour assist(only with mechanical respiration and fluid replacement) after 12-hour recanalization. The materials were subdivided into subepicardial and subendocardial tissues. Tissue samples were examined with a transmission electron microscope (Philips EM 300) at the accelerating voltage of 60 KeV. Results: After a 20-minute ligation of the LAD, myocytes showed slight to moderate degree of ultrastructural changes including subsarcolemmal bleb formation, loss of nuclear matrix, clumping of chromatin and margination, mitochondrial destruction, and contracture of sarcomeres. However, microvascular structures were relatively well preserved. After 1-hour reperfusion, nuclear and mitochondrial matrices reappeared and intravascular plugging by polymorphonuclear leukocytes or platelets was observed. However, nucleoli and intramitochondrial granules reappeared within 3 hours of reperfusion and a large number of myocytes were recovered progressively within 6 hours of reperfusion. Recovery was apparent in the subepicardial myocytes and there were no distinct changes in the ultrastructure except narrowed lumen of the microvessels in the later period of reperfusion. Conclusions: It is likely that the ischemic myocardium could not be salvaged without adequate restoration of coronary flow and that the microvasculature is more resistant to reversible period of ischemia than subendocardium and subepicardium. Therefore, thrombolysis and/or angioplasty may be a rational method of therapy for coronarogenic myocardial ischemia. However, it may take a relatively longer period of time to recover from ischemic insult and reperfusion injury should be considered.

  • Malperfusion of major organs which frequently accompanies acute aortic dissection is one of the major causes of death. Malperfusion does not only develop before surgery, but also during or after surgery in various manifestations according to the aortic branches involved. Expeditious diagnostic and therapeutic measures based on high degree of clinical suspicion are mandatory for successful treatment. The authors report four cases of acute aortic dissection accompanied by malperfusion of various organs that were successfully treated.

  • Shipping Along with the aging of the total population, its importance is greater cause of stroke, which is one of the diseases are listed cerebral perfusion is about 80%,.ie that the flow of blood flowing to the brain is reduced as the cause. One of the most typical prophylaxis and treatment of these stroke is to enhance cerebral perfusion. However, since the method of enhancing conventional cerebral perfusion have been conducted mainly in invasive method, giving a burden to the patient, the problem of inducing a number of complications were noted. Therefore, in this paper, an attempt to supplement the shortcomings of such invasive brain perfusion increase method, to provide a device to enhance the non-invasively brain perfusion.

Consideration of Normal Variation of Perfusion Measurements in the Quantitative Analysis of Myocardial Perfusion SPECT: Usefulness in Assessment of Viable Myocardium (심근관류 SPECT의 정량적 분석에서 관류정량값 정상변이의 고려: 생존심근 평가에서의 유용성)

  • Purpose: Although automatic quantification software of myocardial perfusion SPECT provides highly objective and reproducible quantitative measurements, there is still some limitation in the direct use of quantitative measurements. In this study we derived parameters using normal variation of perfusion measurements, and tried to test the usefulness of these parameters. Materials and Methods: In order to calculate normal variation of perfusion measurements on myocardial perfusion SPECT, 55 patients (M:F = 28:27) of low-likelihood for coronary artery disease were enrolled and $^{201}TI$ rest/$^{99m}Tc$-MIBI stress SPECT studies were performed. Using 20-segment model, mean (m) and standard deviation (SD) of perfusion were calculated in each segment. As a myocardial viability assessment group, another 48 patients with known coronary artery disease, who underwent coronary artery bypass graft surgery (CABG) were enrolled. $^{201}TI$ rest/$^{99m}Tc$-MIBI stress / $^{201}TI$ 24-hr delayed SPECT was performed before CABG and SPECT was followed up 3 months after CABG. From the preoperative 24-hr delayed SPECT, $Q_{delay}$ (perfusion measurement), ${\Delta}_{delay}$ ($Q_{delay}$ - m) and $Z_{delay}$ (($Q_{delay}$ - m)/SD) were defined and diagnostic performances of them for myocardial viability were evaluated using area under curve (AUC) on receiver operating characteristic (ROC) curve analysis. Results: Segmental perfusion measurements showed considerable normal variations among segments. In men, the lowest segmental perfusion measurement was $51.8{\pm}6.5$ and the highest segmental perfusion was $87.0{\pm}5.9$, and they are $58.7{\pm}8.1$ and $87.3{\pm}6.0$, respectively in women. In the viability assessment $Q_{delay}$ showed AUC of 0.633, while those for ${\Delta}_{delay}$ and $Z_{delay}$ were 0.735 and 0.716, respectively. The AUCs of ${\Delta}_{delay}$ and $Z_{delay}$ were significantly higher than that of $Q_{delay}$ (p = 0.001 and 0.018, respectively). The diagnostic performance of ${\Delta}_{delay}$, which showed highest AUC, was 85% of sensitivity and 53% of specificity at the optimal cutoff of -24.7. Conclusion: On automatic quantification of myocardial perfusion SPECT, the normal variation of perfusion measurements were considerable among segments. In the viability assessment, the parameters considering normal variation showed better diagnostic performance than the direct perfusion measurement. This study suggests that consideration of normal variation is important in the analysis of measurements on quantitative myocardial perfusion SPECT.

Amelioration Effects of Irrigation-Aspiration on Renal Ischemia-Reperfusion Injury in Canine Model (개에서 신장의 허혈-재관류 손상에 대한관류-흡인의 감소효과)

  • Renal ischemia-reperfusion injury is great clinical important because viability of the transplanted organ depends on the tolerance of the graft to ischemia-reperfusion injury, an inevitable processing during surgery. The purpose of this study was to investigate the effects of irrigation-aspiration in ischemia-reperfusion injury model induced by cross-clamping of renal vessels. Blood samples were collected from these dogs for measurement of kidney function and antioxidant enzyme activity, and RI at the intrarenal artery was measured at different time intervals. And the kidneys were taken for histopathologic evaluation at day 14. Kidney function (Cr and BUN) showed a significant increasing in untreated group compared to treated group. Resistive index of intrarenal artery was no significant difference among the groups. Activity of antioxidant enzymes in plasma was significant decrease in untreated group compare to control group while in treated group was no significant difference compared to control group. In histopathologic finding, treated group was showed less damage than that of untreated group. This result suggests that the processing of irrigation-aspiration is useful to reducing ischemia-reperfusion injury.

  • 최근 Human 지놈 프로젝트를 포함한 다양한 종의 지놈 프로젝트가 수행되고 수많은 지놈정보가 생산되고 있으며 이를 해석하고 서로 연관성를 찾기 위한 다양한 연구가 진행되고 있다. 즉 최신 생명공학과 관련된 연구방향이 DNA의 구조적 해석에서 기능 해석과 유전자들의 상호연관성을 규명하는 방향으로 변화하고 있으며 이를 위한 강력한 도구로서 DNA microarray (DNA chip)는 방대한 양의 지놈 정보를 이용하여 단시간에 대량으로 고속처리하여 효율적으로 유전자 기능을 분석할 수 있는 주목받고 있는 방법이다. DNA microarray 실험과 분석에 있어 데이터분석, 재현성, 종간의 비교, 확인실험 및 비용 등의 문제가 있지만 유전자발현양상 데이터로부터 정확한 환자의 예후를 예측할 수 있는 비교적 적은 유전자 그룹의 진단마커를 찾거나, 하나의 유전자가 아니라 mouse 전체 지놈의 유전자발현 패턴을 인간의 암을 위시한 각종 질병 연구를 위한 발현 신호나 변화 등을 발견하여 신약개발 등에 활용하고자 하는 시도가 활발히 진행되고 있다. 서로 다른 종간에 비슷한 phenotype의 유전자발현도 진화적으로 보존되었다는 전제 하에서 지놈 sequence의 비교연구가 가능하고 DNA microarray 발현 데이터에 근거하여 독립적으로 각 종간의 유전자발현패턴을 비교함으로써 난치병 등을 새롭게 분류할 수 있다. 즉, 암세포 등에서 유전자발현 양상은 유전학적, 환경적 alteration들이 잘 반영되어 있다고 간주하고, 이러한 양상을 바탕으로 인간의 암을 위시한 다양한 질병 연구를 위한 최적의 mouse 모델을 찾을 수 있고, 이는 결국 새로운 치료 방법 개발이나 맞춤의학 실현에 중요한 역할을 할 것으로 기대된다. 특히 pathway 타겟으로 하는 치료를 위해서는 Human-mouse 비교를 통한 발현 신호를 찾는 것이 진단에서는 매우 유용한 방법이다. 이를 위한 고성능의 분석방법이나 시스템의 개발이 중요하게 된다.. 관류의 정도와 조영증강정도를 중심으로 관류 MR 영상소견과 조직학적 소견을 관련지어 분석하였다. 결과: 조영증강 T1강조MR영상에서 환상조영증강을 보이는 다형성 교보세포종 2예에서는 변연부 외륜이 고관류를, 중심부의 괴사부위는 저관류로 나타났다. 저등급 교종은 경계가 불분명한 저관류부위로 보였다. 뇌농양 2예는 변연부 외륜이 경도의 고관류를, 중심부는 저관류로 나타났다. 뇌수막종은 미만성의 균일한 중등도 혹은 고도의 고관류로 보였으며, 임파종과 배아종은 경계가 명확한 저관류부위로 나타났다. 신경세포종은 종괴\ulcorner 일부에 중등도 혹은 고도의 고관류부위가 관찰되었고, 전이암은 다수병변중 일부에서 중등도의 고관류를 보였다. 방사선괴사는 저관류부위내에 국소적 고관류부위를 보였다. 결론: 관류 MR영상은 뇌종양의 관류상태를 비교적 잘 반영하며, 조직학적 특성을 예측하는데에 도움을 주 수 있을 것으로 기대된다. 뇌종야에서의 관류MR영상의 분명한 역할을 규명하기 위해서는 앞으로 더 많은 임상적 연구가 필요할 것으로 생각된다.조증 환자의 자극성 전타액내 lactobacilli양은 peroxidase system을 함유한 세치제를 사용한 군에서 대조군에 비해 상대적으로 낮게 나타났으나(p = 0.067) 통계학적 유의성은 없었다.같은 예에서 찾아 볼 수 있다. 첫째, 발음상으로 동사의 변화형에서 "porte[$p{\jmath}rte$](들다: 현재형), porte[$p{\jmath}rte$](과거분사형), porta[$p{\jmath}rte$](단순과거형)"등이 대립되며, 이휘 "Porto[$p{\jmath}rte$](포르토)"와도 대립된다. 둘째, 어휘적 대립 "le haut[$l{\partial}o$](위)/l'eau[lo](물)"와 형태론적 대립 "le[$l{\partial}$](정관사, 남성단수)/l
