  • This study was carried out to investigate the effects of Opuntia ficus-indica complex (OF) on blood glucose, glucose tolerance, plasma insulin level and histopathological appearance of pancreatic islets in streptozotoxin (STZ)-induced diabetic rats. Thirty-two male Sprague-Daweley rats were divided into non-diabetic control (NC), diabetic control (DC), diabetic OF of 2% (OF-2) and diabetic OF of 5% (OF-5) and fed experimental diets for 3 weeks. Compared to the DC group fasting blood glucose levels in the OF-2 and OF-5 groups were significantly (p<0.05) reduced while fasting plasma insulin level in the OF-2 and OF-5 groups were significantly (p<0.05) increased. Glucose tolerance in the OF-2 and OF-5 groups were improved. Histopathological observation of pancreatic islets of the OF-2 and OF-5 groups showed hyperplasia which was very similar to NC. Numbers of ${\beta}$-cells in OF-2 ($47.81{\pm}0.92$) and OF-5 ($81.64{\pm}2.80$) were higher than numbers of ${\beta}$-cells in DC ($13.18{\pm}1.01$). These results imply that the intake of OF improves ${\beta}$-cell proliferation and prevents the death of ${\beta}$-cells in STZ-induced diabetic rats.

  • This study analyzed the distribution of the blood glucose level according to the fasting status. Moreover, a relationship was analyzed between fasting blood glucose level and glycemic control indicators. A total of 707 outpatients, who visited Dankook University Hospital, were included and classified into either the fasting group and the non-fasting group. The mean blood glucose level of each group was calculated and analyzed by sex, age, and clinic. In addition, blood glucose, HbA1c, fructosamine, and 1,5-AG were measured in 153 fasting health check-up patients, and the correlation between the blood glucose level and glycemic control indicators was evaluated. Blood glucose averages between the two groups (non-fasting 111.9 vs. fasting 103.6 mg/dL) were different (p<0.05); and the mean difference was lower in women (4.8 mg/dL) than in men (12.2 mg/dL). A significant difference of the median glucose values among the age groups was only observed in the non-fasting group (Kruskal-Wallis test, p<0.01), and not in the fasting group. A 1,5-Anhydroglucitol was estimated to be significantly correlated with the fast blood glucose level in the range of the criteria of impaired fasting glucose (IFG). We presented an assessment of the distribution of blood glucose level in accordance with the fasting status among outpatients, and estimated that 1,5-anhydroglucitol was well correlated with the fasting blood glucose than fructosamine and HbA1c, through the analysis of results of health screening subjects. It is suggested that the use of glycemic indicators that reflect short-term blood glucose control can be used together with the blood glucose measurement in the screening of diabetes mellitus.

  • The aim of the present study was to investigate antihyperglycemic effect of Gleditschiae Spina (GS) in streptozotocin (STZ)-nicotinamide (NA)-induced type 2 diabetic rats. The rats were divided into four groups: normal control (NC), diabetic control (DC), diabetic rats supplemented with acarbose (AC, 4 mg/kg), and with GS ethanol extracts (GSE, 50 mg/kg). Weekly fasting blood glucose (FBG) for 10 weeks and oral glucose tolerance test (OGTT) at 10th week were monitored using glucose oxidase-peroxidase reactive strips. The FBG level was significantly reduced in AC group after 8 weeks and in GSE group at the end of period. The AUCs for the glucose response from OGTT and blood glucose level after sacrifice were significantly lower in the AC and GSE groups than the DC group. GSE supplementation significantly increased plasma total radical-trapping antioxidant potential (TRAP) in STZ-NA-induced diabetic rats, compared with DC group. The present study indicates that GSE could ameliorate type 2 diabetes and be comparable to acarbose, a standard hypoglycemic drug. Also, we suggest that GSE may possess antioxidant activity against the STZ-NA-induced oxidative stress.

  • The purpose of this study is to investigate the effects of the silkworm powder on blood levels of glucose, Hb Alc, insulin, and lipids. Thirty-one NIDDM patients from Kyung Hee Medical Center were divided into two groups : patients with silkworm powder supplements and drug treatments(Drug diabetes) and patients with silkworm powder supplement only(Non-drug diabetes). For the control group, age-matched subjects were recruited. During the 4 weeks of the experimental period, silkworm powder(500mg/mea1) was given to the subjects right after each meal. Nutritional assessments and dietary education were carried out periodically, and body weight and blood pressure were measured when patients visited the hospital. Overnight fasting and 2-h postprandial blood glucose levels were measured at 2 week intervals. The blood levels of insulin, Hb Alc, and lipids were measured before and after the supplements. The mean ages of the three groups were 56.7-59.6 years old. The height, weight, and BMI did not differ among the groups. The fasting blood glucose levels were 138.1$\pm$22.0mg/dl for the Drug treated diabetes group, 175.0$\pm$32.0mg/dl for the Non-drug diabetes group, and 108. 3$\pm$16.gmg/dl for the control group at the begining of the supplement. After 4-wks of supplements, the blood levels of glucose tended to decrease in all three experimental groups. Before the supplements, the 2-h postprandial blood glucose levels of the Drug diabetes, Non drug diabetes, and control groups were 244.7$\pm$62.6mg/dl, 272.4$\pm$40.1mg/dl, and 147.7$\pm$28.0mg/dl, respectively. After the supplement, the levels were 197.2$\pm$30.gmg/dl, 208.6$\pm$ 56.6mg/dl, and 151.3$\pm$30.3mg/dl, respectively. This shows that silkworm powder tended to lower blood levels by 19.4% and 23.4% in NIDDM patient groups. However, the changes in the blood levels of insulin, Hb Alc, ind lipids were not observed after the supplement. In conclusion, the present study has demonstrated that silkworm powder has a tendency to decrease 2-h postprandial blood glucose levels, but it should be used with caution in controlling the diabetes. • This study examined the relationships among impaired fasting glucose (IFG), diabetes and periodontal disease. The data from 10,856 adults (aged over 19 years) was derived from the Fifth Korean National Health and Nutrition Examination Survey, which was conducted in 2010 and 2012. Adjusting the related confounders, multiple logistic regression analysis showed that periodontitis were related to gender, age, education level, smoking and diabetic status (p<0.001). These findings suggest that abnormal fasting glucose, which is a predisposing factor for diabetes mellitus, does not appear to be a risk indicator for periodontal disease. On the other hand, if patients do not take steps to prevent or delay diabetes, prediabetes is likely to develop into type 2 diabetes within 10 years. Therefore, patients with prediabetes need to undergo continuous examinations and management of periodontal disease.

  • This study aimed to investigate the differences in the upper and lower limits of the 95% distribution of fasting blood glucose (FBG) by age groups. We also analyzed the changes in the mean values and dispersion of the data using the Korean Blood Glucose Reference Standard raw data published by the National Health Insurance Service (NHIS). Furthermore, the trends among 16 administrative districts were analyzed and any gender differences were determined. We also assessed whether the study results correlated with the relative standard uncertainty, as published by the NHIS. On the dispersion analysis using the differences between the upper and lower limits of the 95% distribution of FBG by age group, there were significant differences across gender and administrative districts (P<0.05). The gender differences in FBG measurements, as published by the NHIS, were significant across different administrative districts and age groups (P<0.001). This confirmed the need to recalculate the blood glucose reference standards for men and women. No significant correlation was observed between the relative standard uncertainty, as published by NHIS, and the dispersion and number of measurements analyzed in this study. However, it showed a high correlation with the measured mean value (R2=0.95). Therefore, further research on the reference standard and uncertainty is needed.

  • This study was conducted to evaluate the association between periodontal disease and diabetes mellitus. The final analysis population of this study was composed of 4,830 adults with diabetes mellitus or periodontal disease and aged 19 years or older, based on the third-edition data of the 4th Korea National Health and Nutrition Examination Survey (KNHANES) (in 2009). Diabetic status and potential confounders were used in questionnaire materials and physical examination materials, and the presence of periodontal disease was used in the materials for oral health examination by a dentist. For diabetic status, fasting plasma glucose (FPG) levels <100 mg/dl were subcategorized into normal group and FPG levels ${\geq}100mg/dl$ into impaired fasting glucose group; glycosylated hemoglobin (HbA1c) levels <7% into normal group and HbA1c ${\geq}7%$ into diabetes group, on the basis of the American Diabetes Association. According to the 2009 Korea Health Statistics, the case where the pocket depth is 3 mm or more was defined as periodontal disease. The association between the two diseases was evaluated through $x^2$-test and logistic regression analysis using R-commander 2.14. In impaired fasting glucose group, community periodontal index (CPI) groups 3 to 4 had higher risks for periodontal disease 1.23 times (95% confidence interval, 1.07~1.42) than those of CPI groups CPI 0~2, even after adjustment for several confounders. In addition, periodontal disease and diabetes mellitus showed statistically significant differences depending on age, sex, income level, educational background, smoking status, alcohol consumption, and snack intake. The analyses of the third-edition data of the 4th KNHANES showed that there was a statistically significant association between periodontal disease and diabetes mellitus as examined by means of CPI in this study.

  • Purpose : This study aimed to assess the prevalence of obesity, hypertension, and hyperlipidemia in children from low-income families in Gunpo and to evaluate whether economic status affects the prevalence of obesity. Methods : Between October 2007 and March 2008, 341 children (167 girls and 174 boys; age, 6 to 13 years) were enrolled in this study. All these children came from families who earned minimum wages and who were supported by government. We measured height, weight, and blood pressure and performed laboratory examinations, including total cholesterol, high-density lipoprotein cholesterol, and triglycerides. Body mass index (BMI) was calculated using weight and height. We compared the prevalence of obesity, hypertension, and hyperlipidemia in children from low-income families with the prevalence of these maladies found in other studies. Results : Prevalence of obesity ($BMI{\geq}95^{th}$) was 7.1%. Prevalence of hypertension and hyperlipidemia was 8.2% and 16.7%, respectively. In the obese group, prevalence of hypertension and hyperlipidemia was 25.0% and 45.8%, respectively. Obesity and hyperlipidemia were slightly more frequent in our study than those found in other reports, and the prevalence of hypertension in the obese group was very high compared with statistics from the Korean National Health and Nutrition Examination Survey (25% vs. 19.5%) and those from other reports. Conclusion : Low-income status was associated with an increased incidence of obesity, hyperlipidemia, and hypertension. Hypertension showed an especially strong association with economic status, which seemed to correlate with genetic, environmental, and dietary effects.
