When open reduction of maxilla fractures is postponed due to concurrent life-threatening injuries, delayed union may result with malunion or nonunion. If delayed malunion is occurred, significant facial deformity may result, including a dished-out face, irregular retromaxillism with Angle's class III malocclusion, open anterior bite, nasal collapse, telecanthus and malar flattening. The treatment planning for this problem includes cephalometric evaluation anterior and lateral tomograms, dental casts, orthodontic planning, dental planning and use of impression tray to rupture the fibrous tissue casts, orthodontic planning, dental planning and use of impression tray to rupture the fibrous tissue attachment at the fracture site. In this paper, one case presented a 58-year-old female patient with maxilla retrusion after comminuted fracture, who was treated with orthodontic methods of maxillary protraction headgear and Plaster headcap, whereas the other two cases were about male patients who were treated principally with surgically open reduction or Le Fort I-controlled transverse osteotomy with iliac bone graft.

Purpose: The aim of the present study was to investigate the effects of N-acetyl cysteine (NAC) loading on the bone formation surrounding sandblasted, large-grit and acid-etched (SLA) implants. Materials and methods: Implantation of NAC loaded SLA implants (NSI group) and SLA implants (SI group) was performed bilaterally in the mandible of 4 adult beagle dogs (each group, n = 8). The animals were sacrificed after a healing period of 3 and 6 weeks, respectively (n = 2 animal each). Dissected blocks were processed for histomorphometrical analysis. Bone to implant contact percentage (BIC%) and bone volume (BV%) were assessed histomorphometrically. Results: BIC% of NAC loaded SLA implants were about 50% higher than that of SLA implants at 3 weeks of bone healing, but not significantly (51.79 vs 35.43%; P=.185). BV% of NAC loaded SLA implants were significantly higher than that of SLA implants at 3 weeks of bone healing (45.09 vs 37.57 %; P=.044). At 6 weeks of bone healing, BIC% and BV% of two experimental groups were similar (P>.05). Conclusion: Within the limits of the present study, NAC loading have a positive effects on the early bone formation surrounding SLA implants. So, it might be concluded that NAC loading enhance the osseointegration and shorten the healing time after implantation of the SLA implants.

Recently, osseointegrated implants have been used to improve denture support, stability, and retention. Several studies have been revealed that mandibular two-implant overdentures significantly increase patient satisfaction with the function of their prostheses. Use of a wide variety of attachment systems, including stud, magnet, and bar attachments, has proven both clinically predictable and effective. This article presents that two-implant overdenture can become the standard of care for treatment of the edentulous mandible.
After dental implant are planted into their bony site among the various growth factors associated with bone formation. BMP is expressed in the bone surrounding the implant fixture. By taking a close look at BMP2, BMP4 which are growth factors that take put in bone formation, its histologic features and radiographic bone healing patterns we would like to examine the mechanism of osseointegration. We randomly used 8 male and female house rabbit amd used diameter 5 mm height spiral shaped implants(Ostem, Korea) for animal use handled as a resorbable blast machined(RBM) surface and machined surface. 2group were formed and each group had RBM surface and machined surface implant or a simple bone cavity. After 3, 7, 14 and 28 days post surgery 2 objects were sacrificed from each group and histologic specimens were acquired. RT-PCR analysis was conducted and after H&E staining the extent of osseointegration was measured applying a histologic feature and histomorphometric analysis program. Quanitity one -4.41(Bio-Rad, USA) was used after scanning the PCR product image of the growth factors manifested in each group. According to the histomorphometric features the RBM, Machined surface group showed increased contact between bone and implant surface at 3, 7, 14 and 28 days after surgery. The BMP2 level increased in both experiment groups but remained unchanged in the contrast group. BMP4 levels stayed steady after the early post implantation period for RBM but showed decreased in the machined surface group and contrast group. The amount of contact between bone and implant surface increased with the passage of time. BMP2, BMP4 were expressed in both experimental group and contrast group. These growth factors play a role in osseointegration of implant.
The purpose of this study was to present the causes and their mechanisms of eruption failure of teeth and to investigate the treatment modalities. There are so many reports about eruption failure, but most of them are dealing with local mechanical interferences. But, we have patients suffered from eruption failure of another causes. Many developmental failures show eruption problems of teeth, although in some cases, the primary failure of eruption (failure of the eruption mechanism itself) can be the primary cause. We have to know about the causes, differences, and the treatment modalities for those abnormalities.

Peri-implantitis is the most common reason for a late failure and can occur even after years of successful osseointegration. The role of microbial plaque accumulation in the development of peri-implantitis has been well documented. On the other hand, the ideal method of implant surface decontamination to re-establish the health of peri-implant tissue remains to be determined. Removal of bacterial deposits is essential in the treatment of peri-implant infections, and various therapeutic approaches have been described in the literature, including mechanical debridement, disinfection with chemotherapeutic agents, and laser therapy. Recently, there has been a plenitude of scientific data regarding the use of laser irradiation to achieve titanium surface decontamination. Thus, research is focusing on lasers' potential use in the treatment of peri-implantitis. The aim of this literature review is to analyze and evaluate the efficacy of laser therapy for the treatment of peri-implantitis.
The purpose of this study was to investigate the influence of early functional load around osseointegrated titanium implants. 24 titanium plasma spray coated implants (ITI HS-type) were placed into the previously extracted site in the mandible of six adult dogs. The implants were divided into three groups : the control group was the implants without abutment during the experimental period; the experimental group I was loaded by connecting the contoured abutment after 6 weeks of healing; the experimental group II was loaded after 12 weeks of healing: and the mandibular second premolar and surrounding tissues were selected for natural tooth group to compare the implanted group. All dogs were injected intravenously tetracycline, alizarin red S, and calcein for bone labeling. After the experimental period of 18 weeks, the dogs were sacrificed and longitudinal sections of the bone-implant interface were cut and observed using light microscope, scanning electron microscope, and fluorescence microscope. The results of the study were as follows: 1. Light and scanning electron microscopically, all implant surfaces were well contact with bone tissue at the cortical layer, but some areas of cancellous bone were not contact directly. 2. Fluorescence microscopically, number and size of the new secondary osteons around the implant were increased than those of the natural tooth. 3. Fluorescence microscopically, linear and concentrical fluorescence was observed at or near the surface of all implants, and the bone formation and remodeling of the implants loaded after 6 week of healing were great, and unloaded implants were worst. 4. Fluorescence microscopically, endosteal bone formation was greater than periosteal bone formation at or near the implants. 5. Fluorescence microscopically, number and size of linear and concentric fluorescence was increased at the lingual side than the buccal side of the loaded implants. The result of the study indicate the possibility of the early load to the implant via a prosthesis.
This paper reports marginal bone loss around osseointegrated implants after loading in partially edentulous patients in dental hospital, Yonsei University. Two types of implants($Br{\aa}nemark^{TM},\;IMZ^{TM}$) were used. Through the digital measurement on periapical radiograph around 37 implants in human subjects, marginal bone loss was observed for 24 months after delivery of prostheses. The results were as follows; 1. According to experimental periods marginal bone loss in total implants was 1.775 mm at 12 months, 1.921 mm at 24 months after delivery of prostheses(p<0.05). 2. Marginal bone loss in the $Br{\aa}nemark$ implants was 1.831 mm at 12 months, 1.833 mm at 24 months after delivery of prostheses(p<0.05). 3. Marginal bone loss in the IMZ implants was 1.578 mm at 12 months, 2.907 mm at 23 months after delivery of prostheses(p<0.05). 4. During the first year after loading, the IMZ implants showed less marginal bone loss than the $Br{\aa}nemark$ implants but, during the next the $Br{\aa}nemark$ implants showed less than the IMZ implants(p>0.05). These results indicate that marginal bone loss around osseointegrated implants occurs within the first 12 months after delivery of prostheses and stabilizes thereafter, so it is necessary to be careful of using dental implants for the first year after delivery of prostheses.
The purpose of this study was to evaluate the mechanical effects when one implant fixture was connected to the natural teeth with reduced alveolar bone height. This study also examined the effects of increasing the number of abutment teeth and the effects of the intramobile connector and the titanium connector as they were inserted between the implant superstructure and the fixture. The distribution and concentration load was applied to the fixed partial denture(FPD) supported by implant and the natural teeth with reduced alveolar bone height. The stress and displacement of each element was observed and compared by the two-dimensional finite element method. The following results were obtained : 1. The greater the loss of alveolar bone in natural teeth area, the greater the displacement of FPD and the stress concentration in alveolar bone around implant, especially at the stress concentration in the mesial alveolar bone crest around implant fixture. 2. The displacement of FPD was increased more and that of implants fixture was decreased more when intramobile connector was used than titanium connector was used. Also the stress concentration in alveolar bone around implant fixture was greater when intramobile connector than titanium connector. One implication of this finding was that the difference in stiffness of implant and the natural teeth with reduced alveolar bone height could be partially compensated in case of the POM intramobile connector. 3. The amount and direction of displacement and the stress distribution of the 4-unit FPD was better than those of the 3-unit FPD. It implied that the difference of stiffness of implant and natural teeth with reduced alveolar bone height could be partially compensated in case of the 4 unit FPD.
Objective: The objective of this study was to evaluate the osseointegration of narrow-diameter implant with trapezoid-shape and to evaluate the effect of coronal microthreads on trapezoid-shape narrow-diameter implant. Materials and Methods: The experimental narrow-diameter implants were classified into two groups according to absence or presence of coronal microthreads: trapezoid-shape narrow diameter implant (TN group) and trapezoid-shape narrow-diameter implant with microthreads (TNM group). They were installed alternately in bilateral mandible in three dogs. After 8 weeks, the animals were sacrificed. Resonance frequency analysis, removal torque test, and histometric analysis were performed. Results: Statistically higher implant stability quotient (ISQ) values were observed in TNM group than in TN group at the time of implant installation. However, significant ISQ values difference was not observed between groups at 8 weeks. Both groups showed significantly increased ISQ values at 8 weeks, compared to the time of implant installation. There was no significant difference between groups in removal torque test. Bone-implant contact ratio also showed no significant difference between groups in total and coronal part. Conclusion: Within the limitation of this study, it could be concluded that the trapezoid-shape design on narrow-diameter implant showed successful ossointegration, and the microthreads on coronal part did not result in significant bone-implant contact and biomechanical stability at 8 weeks.
