Analysis of the Continuity of Outpatient among Adult Patients with hypertension and its Influential Factors in Korea (우리나라 성인 고혈압환자의 외래진료 지속성과 이에 영향을 미치는 요인 분석)

  • The administration data of the national health insurance and health insurance bills were utilized in this study. The data of 485,953 patients who were at the age of 30 and up and used the out-patient departments of every medical institution located in some regions involving two southern and northern provinces once or more during a 184-day period from July to December, 2008. As a result of analyzing their Continuity of Ambulatory Care and factors affecting it, the following findings were given: The continuity of ambulatory care among the adult patients with hypertension in our country turned out to be on a high level(MMCI $0.96{\pm}0.13$, MFPC $0.96{\pm}0.12$). Given examining the outpatient medical-cure continuity level according to index, the averagely medical-cure continuity level was calculated to be high level with MMCI, $0.96{\pm}0.13$, and MFPC $0.96{\pm}0.12$. Thus, the tendency of visiting only one medical provider was high. The findings of the study illustrated that the average continuity of ambulatory care among the adult patients with hypertension in our country was on a high level, and it seemed that special care should be provided to patients with a low-level continuity of ambulatory care, such as women and elderly people aged 64 and over. The findings of the study are expected to serve as one of the barometers for the health care of patients with hypertension and for the performance of national hypertension management plans.

Development of a Self-Management Adherence Instrument for Patients with Hypertension (고혈압 환자의 자가 관리 이행 도구개발)

  • Purpose: This study aimed to develop an instrument that measured adherence to self-management among patients with hypertension, and to verify the validity and reliability of the developed instrument. Methods: The 34 indicators that underwent the content validity test were provided to 202 subjects for investigation The collected data were analyzed using SPSS Ver. 22.0 and Amos Ver. 25.0. Results: Five factors, 'interaction,' 'lifestyle change,' 'continuing motivation,' 'medical care', and 'weight control' were extracted, and the total cumulative variance was shown to be 61.75%. The test statistic of the regression coefficient was statistically significant according to the confirmatory factor analysis. Conclusion: It is suggested that more research is needed to generalize the instrument to suit more candidates, while continuing to refine the reliability and validity of the instrument.

  • With the increasing number of chronic disease patients, the importance of everyday health care has grown more significant. The study researchers expected it would help for patients' health improvement and healthcare service expansion if users check their vital signs in their daily lives and send the results to a medical center's servers through a specific device automatically. This thesis, in line with this idea, seeks to develop a portable healthcare gateway. The gateway is designed in a USB type and can transmit standardized data, operating regardless of a user's location and Personal Healthcare Devices (PHDs). The developed portable healthcare gateway provides effective services in ubiquitous environments to customers, which will improve the health of chronic patients.

Study on Health Behavior of Hypertensive Patients and Compliance for Treatment of Antihypertensive Medication (고혈압 환자들의 순응도와 건강행태의 관계)

  • Objectives: To estimate the prevalence rate of hypertension, the changes of health behavior, and compliance for the drug treatment after diagnosed as hypertension. Methods: 7,030 persons who live in Cheonan City of Chungnam Province were selected by the cluster sampling method, and 5,372 persons were surveyed by questionnaire and health examination. This data is analyzed by Chi-square test on each variable. Results: 49.8%- of men and 38.8%- of women had been diagnosed as hypertension, and the prevalence rate of hypertension was significantly increased with aging in both gender. The prevalence rate tended to decrease in highly educated women group. Unemployed persons or obese persons showed relatively higher prevalence rate. The prevalence rate of hypertension increased in groups with higher total cholesterol levels over 240 mg/dl, and groups with glucose level over 200 mg/dl. 53.1%- of male patients and 66.6%- of female patients showed compliance for antihypertensive treatment. Compliance for treatment was higher in aged group or lower educated group in both gender. Among men, proportion of compliant subjects was higher in unemployed group(49.3%-), and lower in labor or primary industry than the others but among women, there was not any significant difference. And men with compliance for treatment had higher monthly income than the others, but women did not show any. Conclusion : This population had a high prevalence rate of hypertension which may lead to cardiovascular disease. Therefore health education programs and distribution of information must be emphasized in order to increase compliance to treatment and encourage the change of health behavior to promote health.

Estimation of the Cost of Hypertension Disease Loss in 2010-2017 Using Cohort at Diagnosis Age and Treatment Time (진단나이 및 치료시점 코호트를 활용한 2010~2017년 고혈압 질환 손실비 추계)

  • The rise in chronic disease not only has a negative effect on people's lives, but it also increases the cost of medical care owing to the increased usage of medical care as health and medical technology improves, life expectancy rises, and rapid population ageing. In such context, this study examined the difference in the disease cost of hypertension according to demographic information and the effect of the initial diagnosis age and treatment period on the cost. This study used the Korean Health Panel Survey from 2010 to 2017, and selected subjects based on health insurance beneficiaries between the ages of 30 and under 80. With the selected data, the direct and indirect costs of disease loss were calculated according to the cost of illness approach, and we constructed a disease-loss ratio cohort considering the age of diagnosis and time of treatment for hypertension. From the results of the study, the annual cost of disease loss for hypertensive patients differed by gender by 110,107 won, and it was found that the cost increased by 1.8 times as the treatment time increased. In addition, when comparing disease loss ratios between the same age groups, it was found that the disease loss ratios between those in their 60s and 70s were affected by treatment time. This study confirmed that hypertension significantly affects the cost of the disease, and not only requires early diagnosis and management, but also preventive efforts to lower the incidence of hypertensive disease must be strengthened.

A Study on Health Behaviors of the Young Adults and the Elderly with Hypertension: A Secondary Analysis of the 2018 Korea Health Panel Data (고혈압이 있는 청장년층과 노년층의 건강행위 분석: 2018년 한국의료패널조사 자료활용)

  • The purpose of this study was to investigate the health behavior performance between the young adults and the elderly with hypertension. The research data were analyzed using 2018 Korea Medical Panel data. The subjects of this study were 3,117 persons without disabilities or activity disorders among those diagnosed with hypertension. As a result of the study, first, it was found that the medical utilization rate and drug adherence were high overall. Second, the average body mass index(BMI) and obesity over 25 kg/m2 were found to be higher among the young adults. Third, smoking and drinking were higher in young adults in terms of experience and smoking and alcohol consumption. Fifth, the subjective health status was found to be perceived more positively by the young adults. Based on the results of this study, it is necessary to raise awareness of the need for customized health care from a young age and to develop sustainable and effective programs.

  • In recent years, health management services have been increased according to the increase in interests on health and the development of information technology (IT) based on increases in advanced ages and chronic disease patients. Thus, it requires the monitoring of health conditions and the specialized healthcare services not only in a hospital but also their own home. This study provides the specific notification services related to the context information based on users' bio signal data and the notification services of specific patients and attempts to develop a hypertension monitoring system and a notification service system according to indexes. Because this system considers the context of users by differing it from the conventional monitoring services, it makes possible to obtain more exact measurement values. In addition, it is able to reduce certain health risks through managing specific patients and based on living indexes. Also, it can provide more customized services to users due to the exact and finely classified services.

Effect of Community-Based Interventions for Registering and Managing Diabetes Patients in Rural Areas of Korea: Focusing on Medication Adherence by Difference in Difference Regression Analysis (한 농촌 지역사회 기반 당뇨병 환자의 등록관리 중재의 효과: 투약순응도에 대한 이중차이분석을 중심으로)

  • Background: A chronic disease management program including patient education, recall and remind service, and reduction of out-of-pocket payment was implemented in Korea through a chronic care model. This study aimed to assess the effect of a community-based intervention program for improving medication adherence of patients with diabetes mellitus in rural areas of Korea. Methods: We applied a non-equivalent control group design using Korean National Health Insurance Big Data. Hongcheon County has been continuously adopting this program since 2012 as an intervention region. Hoengseong County did not adopt such program. It was used as a control region. Subjects were a cohort of patients with diabetes mellitus aged more than 65 years but less than 85 years among residents for 11 years from 2010 to 2020. After 1:1 matching, there were 368 subjects in the intervention region and 368 in the control region. Indirect indicators were analyzed using the difference-in-difference regression according to Andersen's medical use model. Results: The increasing percent point of diabetic patients who continuously received insurance benefits for more than 240 days from 2010 to 2014 and from 2010 to 2020 were 2.6%p and 2.7%p in the intervention region and 3.0%p and 3.9%p in the control region, respectively. The number of dispensations per prescription of diabetic patient in the intervention region increased by approximately 4.61% by month compared to that in the control region. Conclusion: The intervention program encouraged older people with diabetes mellitus to receive continuous care for overcoming the rule of halves in the community. More research is needed to determine whether further improvement in the continuity of comprehensive care can prevent the progression of cardiovascular diseases.

Analysis of the medication compliance of hypertensives and its influential factors (고혈압 환자의 투약순응도와 영향 요인 분석)

  • The administration data of the national health insurance and health insurance bills were utilized in this study. The data of 432,915 patients who were at the age of 30 and up and used the out-patient departments of every medical institution located in some regions involving two southern and northern provinces once or more during a 184-day period from July to December. As a result of analyzing their prescription compliance and factors affecting it, the following findings were given: The average rate of the prescription compliance of the patients stood at 61.5 percent. It denoted that they were prescribed to take medicine for approximately 113 days during the six-month(184 days) period of time, and the rate of the patients who complied with the prescriptions just stood at 13.0 percent. They used out-patient department for 4.3 days on the average due to hypertension and they visited 1.1 medical institutions on the average. 94.9 percent just used a medical institution. The largest group (11.6%) suffered from diabetes as co-morbidity, and 23.3 percent of the hypertensives had co-morbidity. Concerning the relationship between their characteristics and prescription compliance, those who were male, who were beneficiaries of the national health insurance, who mainly used general hospitals and who suffered from co-morbidity complied better with the prescription they got. Their prescription compliance got better at the age of 65 to 74 and got worse afterwards. As for factors affecting their prescription compliance, the patients who were male, who were aged between 55 and 64 and who were beneficiaries of the national health insurance, who mainly used specialized general hospitals, general hospitals and public health centers and who had heart diseases and diabetes as co-morbidity complied better with the prescriptions. The above-mentioned findings of the study suggested that it's needed to make a factor analysis of the poor prescription compliance of patients from diverse angles, and that existing hypertension care plans should carefully be reviewed to improve the prescription compliance of patients and to find a feasible alternative. As hypertensives are easily likely to develop co-morbidity like diabetes, systematic health education should be provided for them to get into the right life habits such as taking low-salt meals or quitting smoking. In addition, the development of health care programs is required.

A Study on the Food Habits and Attitudes of Cerebrovascular Accident Patients in Daegu S Medical Center (대구 S병원 뇌졸중 환자의 생활 습관 및 식습관에 관한 조사 연구)

  • Stroke is a serious disease despite recent improvements in the medical treatments available. Thirty-six stroke patients were interviewed as case and 36 non-stroke patients were interviewed as controls between February 2005 and August 2005 at Daegu S Medical Center. Information regarding the subjects' food habits was collected using the recall method from 5 years before the onset of stroke. Using interview techniques, we investigated how dietary habits and attitude influence nutrient intake. These results were analyzed by the $X^2$, Student's t-test and Chi-square test using the SPSS 12.0 program. This case-control study was performed to demonstrate the relationships among general quality factors (BMI, WHR, and family history of stroke) and lifestyle factors (smoking, exercise and alcohol drinking). The results were as follows. Analysis of the percentage of nutrient intake in terms of meal pattern showed that WHR was significantly higher in the case group than in the control group. Hypertension and diabetes were the most common preceding diseases associated with the risk of stroke. The frequencies of smoking, alcohol consumption (p<0.001) and exercise (p<0.05) were higher in the cases than in the controls. Thus, the findings of this study are consistent with those of previous studies and suggest that people should be advised to control hypertension, smoking, alcohol drinking and obesity in order to prevent the occurrence of stroke as these factors are major risk factors for stroke.
