Establishment of I-131, Tc-99m Labeling Methods to In-house Anti-CEA Antibodies and Evaluation of the Immunological Characteristics (국산 항 CEA 항체의 I-131, Tc-99m 표지법 확립 및 면역학적 특성 분석)

  • Cancer cells have several tumor-associated antigens on the cell surfaces, and antibodies against these antigens have been developed by many investigators. Radiolabeled antibodies have been used as new methods to diagnose and treat malignant tumors. Especially anti-carcinoembryonic antigen (CEA) is the most popular antibody for these purposes. In this investigation, we tried to label $^{131}I$ and $^{99m}Tc $ to anti-CEA monoclonal antibodies which were developed in the Seoul National University College of Medicine. We found CEA-79 and CEA-92 antibodies had the better immunological characteristics among 8 anti-CEA monoclonal antibodies. And radioiodination of CEA-79 could be performed by chloramine-T method, while radioiodination of CEA-92 by iodogen method. To label these antibodies with $^{99m}Tc $, we used pretargeting transchelation as direct labeling method. At first, $^{99m}Tc $ was bound to glucaric acid, and monoclonal antibody was reduced by $\beta-mercaptoethanol$. When these were incubated together. $^{99m}Tc $ bound to glucarate was switched to monoclonal antibody because of higher affinity. We established conditions of several steps in this method. Anti-CEA monoclonal antibodies labeled with $^{131}I$ and $^{99m}Tc $ are expected to be used valuably in the detection and treatment of malignant tumors.

$^{99m}Tc-Labeling$ of Monoclonal Antibody to Carcinoembryonic Antigen and Biodistribution (항 암태아성항원에 대한 단세포군항체의 $^{99m}Tc$ 표지법개발 및 생체분포)

  • This study was designed to evaluate a direct method of $^{99m}Tc$ labeling using $\beta-mercaptoethanol$ as a reducing agent, and to investigate whether $^{99m}Tc$ labeled specific monoclonal antibody against carcinoembryonic antigen (CEA-92) can be used for the scintigraphic localization of human colon cancer xenograft. Purified CEA-92 IgG was fragmented into F $(ab')_2$ and then labeled with $^{99m}Tc$ by transchelation method using glucarate as a chelator. Labeling efficiency, immunological reactivity and in vitro stability of $^{99m}Tc$ CEA-92 F $(ab')_2$ were measured and then injected intravenously into nude mice bearing human colon cancer (SNU-C4). Scintigrams were obtained at 24 hour after injection. Then nude mice were sacrificed and the radioactivity was measured Labeling efficiency of injected $^{99m}Tc$ CEA-92 F $(ab')_2$, immunoreative fraction and in vitro stability at 24 hour of injected $^{99m}Tc$ CEA-92 F $(ab')_2$ was 45.2%, 32.8% and 57.4%, respectively. At 24 hour after injection, % ID/g in kidney (46.77) showed high uptake, but %ID/g in tumor (1.65) was significantly higher than spleen (0.69), muscle (0.16), intestine (0.45), stomach (0.75), heart (0.48) and blood (0.45). There was no significant difference between tumor and liver (1.81). Tumor contrast as quantitated by tumor to blood ratio of $^{99m}Tc$ CEA-92 F $(ab')_2$ was increased significantly (p<0.005) until 24 hours (3.70), and there was no statistical differece from tumor to blood ratio of I-131 CEA-92 F $(ab')_2$. The scintigram demonstrated localization of radioactivity over transplanted tumor, but significant background radioactivity was also noted over kidney and abdomen. It is concluded that CEA-92 F $(ab')_2$ can be labeled with $^{99m}Tc$ by a direct transchelation method using $\beta-mercaptoethanol$ as a reducing agent and $^{99m}Tc$ labeled CEA-92 F $(ab')_2$ can be used for the scintigraphic localization of human colon cancer xenograft in nude mice model.

Antioxidant and Anti-inflammatory Effects of Dioscorea japonica and Chenopodium album (참마와 명아주의 항산화 및 항염증 효과)

  • In this study, free radical scavenging activity and enzyme-linked immunosorbent assay (ELISA) experiments were carried out using Dioscorea japonica (D. japonica) and Chenopodium album (C. album) extracts to evaluate their anti-oxidative and anti-inflammatory effects. In the free radical (1,1-diphenyl-2-picrylhydrazyl, DPPH) scavenging activity, $EC_{50}$ of D. japonica and C. album were measured as 2.386 and 0.524 mg/mL, respectively. The free radical scavenging activity of the mixed sample of D. japonica and C. album was the highest when the D. japonica and C. album ratio was 2 : 1. The IL-6 and $TNF-{\alpha}$ ELISA assay showed that IL-6 in mouse spleen cells treated 1 mg/mL of samples, D. japonica and C. album decreased the production of IL-6 concentration by 27.17%, 72.30%. In the case of $TNF-{\alpha}$, D. japonica and C. album decreased 61.97% and 77.85% of $TNF-{\alpha}$ production, respectively. Through these results, we confirmed that D. japonica and C. album have antioxidant and anti - inflammatory effects and could be applied to natural medicine cosmetic having anti - inflammatory effects.

Differences of Symptom Improvement Between Different Diagnosis Classification Groups in Acute Cerebral Infarction Patient (급성기 뇌경색 환자의 변증군간 호전도 차이에 대한 연구)

  • Object : The aim of this study was to examine the differences of symptom improvement between different diagnosis classification groups in acute cerebral infarction patients. Methods : We studied inpatients within a month after the onset of cerebral infarction who were admitted at Kyunghee University Oriental Medical Center from May 2011 to October 2012. We compared the improvement of Motricity Index and Scandinavian Stroke Scale score between Fire-heat group(n=20), Yin deficiency group(n=31), Dampness-phlegm group(n=30), and Qi deficiency group(n=13). Results : Yin deficiency group patients with cerebral infarction showed the most improvement in MI and SSS scores, and patients in Fire-heat group showed the poorest improvement in MI and SSS scores. There was a significant difference between the two groups, but there were no significant differences between all four diagnosis classification groups. Conclusions : This study provides evidence that diagnosis classification could be considered as an important factor in predicting the prognosis of acute cerebral infarction.

A Study on Monitoring and Management of Invasive Alien Species Applied by Citizen Science in the Wetland Protected Areas(Inland Wetland) (시민과학을 활용한 습지보호지역의 생태계교란 식물 모니터링 및 관리방안 연구)

  • This study suggested a citizen science based model to enhance the efficacy of the managing invasive alien plants and examined whose applicability in 3 Wetland Protected Areas (Jangrok of Gwangju metropolitan city, Madongho of Goseong in South Gyeongsang Province, and Ungok of Gochang in North Jeolla Province). The process consists of (a) collecting citizen scientist including local residents of 3 protected areas and piling up information on the 4 species of invasive alien plants (Sicyos angulatus L., Solanum carolinense L., Ambrosia artemisiifolia L. and Solidago altissima L) in a information platform Ecological Information Bank (EcoBank) from September 18th to October 31th, (b) constructing distribution map containing the location and density (3 phases: individual-population-community) of target plants, (c) providing distribution map to Environment Agency and local government who is principal agent of managing invasive alien plants in 3 protected areas, and from whom (d) surveying applications of the distribution map and opinion for future supplement. As a result, citizen science based monitoring should be continued to complement the nationwide information for the field management of invasive alien plants with the expansion of target species (total 17 plants species that Ministry of Environment in South Korea designated) and period of monitoring in a year to increase the usability of surveyed information from citizen science. In the long run, effectiveness of the management of invasive alien species applied by citizen science should be reviewed including efficacy of field management process from citizen's participating in elimination project of invasive alien plants and time series distribution followed by the management of the species.

Landscape Characteristics of the Sacred Dangsan Forests in the Neighborhood of Gyeokpo-ri, Buan-gun as a Potential World Heritage-Sacred Natural Site (세계유산 자연성지 잠재지로서의 부안군 격포리 일원 당산숲의 경관특성)

  • UNESCO and IUCN established the term of 'Sacred Natural Sites' (areas of land or water having special spiritual significance to peoples and communities) for conservation of biological and cultural diversity. Dangsan forest, a traditional village forests of rural Korea is a representative 'Sacred Natural Site' with a history of more than several hundred years of Dangsan ritual. The Chungmak village, Gyeokpo-ri, Buan-gun is a small seashore village. It is an important place that has the largest ancient maritime ritual sites in Korea. Buan-gun have been tried to register the 'Chungmak-dong Ritual Site' for the World Heritage List. However, the fact that this 'Chungmak-dong Ritual Site'(5~6 century, Baekje of the Three-kingdom period) was located in the Dangsan forest, surrounding the shrine, is not much understood. In this study, the landscape characteristics and culture of the sacred Dangsan forest at Gyeokpo-ri, including Gyeokpo-ri, Dae-ri, Naesosa temple Seokpo-ri, Buan-gun and Dongho-ri, Gochang-gun were investigated. And, the potential of registering for World Heritage, 'Protected Area of Sacred Natural Sites' by linking the four site's Dangsan forests has been investigated. The sacred Dangsan forests in the neighborhood of Gyeokpo-ri have kept their landscape characteristics and retained Dangsan ritual. As a result of SWOT analysis for sacred natural sites, WT(weakness-threat) strategy has chosen as priority strategy. The reason is that there is few management scheme. The Dangsan forests at the neighborhood of Gyeokpo-ri need to be recognized by people in Korea, for their valuable landscape characteristics. The places should be managed and protected to remain as a sacred natural sites in order to be prepared for a World Heritage.

Studies on the Hemodynamic Changes in Cirrhosis of the Liver (간경변증(肝硬變症)에서의 혈역학적(血力學的) 변화(變化)에 관(關)한 연구(硏究))

  • Cardiac output, plasma volume and renal plasma flow were determined to evaluate hemodynamic changes in 29 patients with cirrhosis of the liver. The results obtained were as follows. 1. The mean plasma volume was 3793+895ml and it was significantly higher than the normal controls. The mean blood volume ($5266{\pm}1222ml$) and blood volume per kg body weight ($95.7{\pm}23.41ml$) were also increased significantly. The mean plasma volume per kg body weight ($69.1{\pm}19.1ml$) showed increased tendency and the mean difference between blood volume and plasma volume per kg body weight ($26.4{\pm}7.05ml$) was in lower limit of normal range. 2. The mean cardiac output was $7708{\pm}2652ml/min$ and it was significantly increased. The mean cardiac index ($4924{\pm}1998ml/min/M^2$), stroke volume ($96.2{\pm}34.2ml/beat$), stroke index ($62.3{\pm}27.34ml/M^2$) and fractional cardiac index ($1.54{\pm}0.577$) were also increased significantly. The mean total -peripheral resistance was $1664{\pm}753.8\;dynes\;sec\;cm^{-5}M^2$ and it was significantly lower than the normal controls. 3. The mean renal plasma flow was $537{\pm}146.8ml/min/1.73M^2$ and it was normal to decreased tendency. The mean endogenous creatinine clearance ($66.7{\pm}23.0ml/min/1.73M^2$) was significantly decreased. Filtration fraction was variable, but it was slightly lower than normal in most cases. The mean renal fraction of cardiac output ($11.4{\pm}6.27%$) was relatively decreased. 4. Although renal plasma flow was normal or decreased in general, it was definitely diminished in patients with creatinine clearance less than $60ml/min/1.73M^2$, resistant ascites, and signs of azotemia (elevated BUN and serum creatinine). 5. Diminished glomrular filtration rate with low filtration fraction and decreased renal fraction of cardiac output observed strongly supported increased renal afferent arteriolar resistance. 6. Renal circulatory impairment preceded azotemia or oroliguria in cirrhosis. 7. Clinical findigns and liver function were not correlated with hemodynamic changes, except for esophageal varices associated with high cardiac output obsedved. 8. No definite correlation of renal hemodynamics with plasma volume or cardiac output was found.

Efficacy of Bee-venom Acupuncture on Central Post Stroke Pain -Single-blind Randomized Controlled Trial- (중풍 후 중추성 통증환자에 대한 봉독약침의 임상효과 -단일맹검 무작위배정 환자 대조군 비교연구-)

  • Objectives : This study was designed to evaluate objectively the efficacy of bee-venom acupuncture on Central post-stroke patients. Methods : After screening, 25 patients recruited with eligible criteria. Among them 1 dropped out in treatment group, 4 in control group. We applied bee-venom acupuncture 6 points of body($LI_{15}$, $GB_{21}$, $LI_{11}$, $GB_{31}$, $ST_{36}$, $GB_{39}$) 2 times a week for 3 weeks for treatment group, and normal saline for control group in the same way. After 3 weeks treatment and 2 weeks follow up we measured VAS, categorial rating scale as a pain assessment also MBI, Modified Rankin Scale as a stroke recovery. Results : 1. Treatment group showed significant difference in VAS, categorial rating scale after treatment and 2 weeks follow up, compared with baseline. The control group showed significant difference between baseline and after treatment But the 2 weeks follow up, there is no significant difference compared with baseline. Also there's no significant difference in categorial rating scale. 2. MBI showed significant difference in treatment group between baseline and after treatment, but the control group did not have any difference. There's no significant difference in Modified Rankin Scale for treatment group and control group. 3. There's no significant difference between control group and treatment group. This is because of the small sample size in the study, Thus it seems to reflect individual difference strongly. Conclusions : There are positive effect on CPSP patients in pain and stroke recovery treating with bee-venom acupuncture than control group.

The Effect and Side Effect of Dipyridamole in Myocardial SPECT (Dipyridamole 부하 심근 SPECT에서 Dipyridamole의 작용과 부작용)

  • Dipyridamole is an agent that may be used to noninvasively evaluate coronary artery disease. The effect of dipyridamole infusion its generally related to its induced peripheral vasodilatory effect. In normal person, heart rate is generally increased slightly while blood pressure decrease, but the achieved double product and related myocardial oxygen consumption have no significant change. The purpose of this study is to examine the effect and side effect of dipyridamle, and to compare different response to dipyridamole among the patients. We evaluated 847 patients who underwent dipyridamole stress myocardial SPECT. 93.6% of them had induced hypotension, 0.9% showed no change of blood pressure, 5.5% had increased blood pressure. 8.3% had no change of pulse rate more than 10% of basal pulse rate. Among diabetes, 16.9% was not change of pulse rate, 6.7% in non-diabetes. There was no significant correlation between age and rate pressure product rest(RPPr), in patiens without perfusion defects on SPECT(y=7.1x+ 48.4 r=0.13 p>0.01). As increasing age, RPPs/RPPr was declined(y= -11.6x+68.9 r=0.17 p<0.01), similar results were obtained in patients with perfusion defect. The size of perfusion defect on myocardial SPECT have no correlation between RPPr and RPPs/RPPr. The side effects of dipyridamole included chest pain and chest tightness, headache, abdominal pain, dizzness, nausea, and dyspnea. As increasing age, dipyridamole-induced cardiac work at rest was increased, cardiac response to dipyridamole was decreased.
