  • An implant-supported fixed dental prosthesis (ISFDP) or an implant-supported overdenture (IOD) are good options when treating a completely edentulous jaw opposing natural teeth. However, an ISFDP for a full arch requires sufficient bone quality and quantity, which limits its application. Meanwhile, using an ISFDP as an abutment of a removable partial denture has been considered recently. This clinical report discusses the treatments applied to two patients with edentulous maxillas and opposing natural teeth: one was treated with an IOD and the other was treated with an ISFDP and removable partial denture. Follow-up and management were performed for 8 years.

  • Nowadays, 3 dimentional (3D) printing, especially Direct Metal Laser Sintering (DMLS) system is used in dentistry. DMLS system has recently been introduced for fabrication metal framework for metal ceramic crowns to overcome the disadvantages of the casting method and computer aided design/computer aided manufacturing (CAD/CAM) milling system. DMLS system uses a high-temperature laser beam to selectively heat a substructure metal powder based on the CAD data with the framework design. A thin layer of the beamed area becomes fused, and the metal framework is completed by laminating these thin layers. Utilizing DMLS system to fabricate fixed prostheses is expected to achieve free-from shaping without mold and limitations from cutting tools, fabricate prostheses with complex geometry, prevent distortion and fabrication defects that inherent to conventional fabrication methods. The purpose of this case report is to demonstrate various fixed prostheses such as long span fixed prostheses, post to achieve satisfactory results in functional and esthetic aspects.

  • Functional and esthetic recovery through reconstruction of the oral and maxillofacial system is important for patients who underwent mandibulectomy due to lesions such as oral cancer. The fibula free flap, which is frequently used in the reconstruction of the mandible, is easy to prosthetic restoration with implant placement. However, patients with growing periods have difficulty in treatment with implants when the size of the fibula is small and residual growth remains. This case is an 11-year-old female patient who underwent mandibulectomy due to osteosarcoma and reconstruction of the mandible using a fibular free flap. Functional and esthetic recovery were obtained through prosthetic restoration using implant-supported fixed prostheses. Implantation site, number of implants, and type of prostheses were considered. Implant-supported fixed prostheses were delivered at 13 years old, and after an additional 1 year observation, stable results were obtained. However, observing changes in the maxillofacial system through further examinations is necessary.

  • Hemimandibulectomy and free fibular flap (FFF) in patients with squamous cell carcinoma could disturb the functional movement of the mandible. The muscular function in the sectioned side was compromised because of the incompetency of the muscle attachment to the mandible, leading to the unstable occlusal contact in the centric and eccentric occlusion. In this report, we present a case of a 63-year-old male who underwent cancer surgery and reconstruction with fibular graft, and he needed a fixed dental prosthesis to restore tooth loss in the anterior region. Occlusal contact change and instability were found according to the bite force in the centric relation and eccentric movement. This case report aimed to present the prosthetic procedure for fabricating the functionally optimized fixed prosthesis wherein the functionally generated path (FGP) technique and digital imaging method were applied to replicate mandibular movement and vertical dimension of the patient.

Finite Element Analysis of Bone Stress Caused by Horizontal Misfit of Implant Supported Three-Unit Fixed Prosthodontics (3차원 유한요소법에 의한 임플란트 지지 3본 고정성 가공 의치의 부적합도가 인접골 응력에 미치는 영향 분석)

  • This study is to assess the effect of horizontal misfit of an implant supported 3-unit fixed prosthodontics on the stress development at the marginal cortical bone surrounding implant neck. Two finite element models consisting of a three unit fixed prosthodontics and an implant/bone complex were constructed on a three dimensional basis. The three unit fixed prosthodontics were designed either shorter (d=17.8mm model) or longer (d=18.0mm model) by 0.1mm than the span of two implants placed at the mandibular second premolar and second molar areas 17.9mm apart. Fitting of the fixed prosthodontics onto the implant abutments was simulated by a total of 6 steps, that is to say, 0.1mm displacement per each step, using DEFORM 3D (ver 6.1, SFTC, Columbus, OH, USA) program. Stresses in the fixed prosthodontics and implants were evaluated using von-Mises stress, maximum compressive stress, and radial stress as necessary. The d=17.8mm model assembled successfully on to the implant abutments while d=18.0mm model did not. Regardless if the fixed prosthodontics fitted onto the abutments or not, excessively higher stresses developed during the course of assembly trial and thereafter. On the marginal cortical bone around implants during the assembly, the peak tensile and compressive stresses were as high as 186.9MPa and 114.1MPa, respectively, even after the final sitting of the fixed prosthodontics (for d=17.8mm model). For this case, the area of marginal bone subject to compressive stresses above 55MPa, equivalent of the $4,000{\mu}{\varepsilon}$, i.e. the reported threshold strain to inhibit physiological remodeling of human cortical bone, extended up to 2mm away from implant during the assembly. Horizontal misfit of 0.1mm can produce excessively high stresses on the marginal cortical bone not only during the fixed prosthodontics assembly but also thereafter.

  • Lipases from Candida rogosa and Rhizopus arrhizus were immobilized by entrapment with photo-crosslinkable resin prepolymer for the study of fat splitting and interesterification in isooctane-two phase system. Dioctylsulfosuccinate was selected as the most suitable surfactant during the immobilization. Lipase entrapped with hydrophobic photo-crosslinkable resin prepolymer(ENTP-3000) exhibited the highest activity, whereas lipase entrapped with hydrophilic gel(ENT-4000) was more stable in organic solvent. As the degree of hydrophobicity of the immobilization matrix was increased, Vm(app) of the lipase entrapped was increased, but Km(app) was approximately constant. While the optimum pH of the lipases entrapped on hydrophilic gel (ENT-4000) were around pH 7.0 for Candida lipase and Rhizopus lipase, the reaction rate of the lipases entrapped on hydrophobic gel were less dependent on pH variations for short reaction time. However, for longer reaction time, the lipnses from C. rugosa and R. arrhizus entrapped on hydrophobic gel yielded maximum rate at pH 6.0 and 6.5, respectively, Entrapment method endowed the lipase with thermal stability.

Analysis of longevity and success rate of fixed, removable, and implant prostheses treated in Korea (국내에서 치료된 고정성, 가철성, 그리고 임플란트 보철물의 수명 및 성공률 분석)

  • Purpose: The purpose of this study is to analyze the factors affecting the longevity of failed prosthesis and the success rate of the prosthesis based on the data evaluated with the newly developed Korean Academy of Prosthodontics (KAP) criteria. Materials and methods: Evaluation was performed in the restored prosthesis for patients who visited the prosthodontics department of the 13 dental university hospitals and general hospitals. The status of the prosthesis was classified into four categories: Good, Fair, Bad, Worst. The success was recorded if only the category was classified in 'good'. The mean duration of failed prostheses and the success rate through Kaplan-Meier method were analyzed. Results: A total of 1,804 cases of prosthesis were evaluated: 810 cases of fixed dental prostheses (FDP), 519 cases of Removable Dental Prostheses (RDP), and 475 cases of implant prosthesis. The mean duration of failed FDP was $11.41{\pm}0.30years$ and the median was 10 years. The mean duration of failed RDP was $8.18{\pm}0.29years$ and the median was 7 years. The mean duration of failed implant prosthesis was $7.99{\pm}0.30years$ and the median was 7 years. The factors related to the failure were as follows: number of units, abutments, abutments treated with root canal, and plaque index in FDPs; treated and opposing dentition in RDPs; the number of implants, duration of use, and plaque index in implant prostheses. Conclusion: The average duration of failed prosthesis was 11.41 years for FDPs, 8.18 years for RDPs, and 7.99 years for implant prosthesis, according to the evaluation with newly developed KAP criteria.

The Fixation Effects in Immunohistochemistry and Electron Microscopy Using Low Energy of Microwave (LEM) in Human Gastric Adenocarcinoma and HeLa Cell (사람 위선암과 HeLa 세포에 관한 저에너지 마이크로파 고정효과의 조직화학 및 전자현미경적 연구)

  • Human gastric adenocarcinomas are fixated with low energy of microwave (LEM) to study fixation effects in level of ultrastructure and antigenicity of the cancer. For the Ag-Ab reactions , the LEM fixated sdenocarcinomas are incorporated with monoclonal mouse anti-human p53 (IgG2b, kappa) and rabbit anti human cerbB-2. The retrieval of antigenicity are easily recognizable in the LEM fixated sections compared with that of frozen sections which show often diffused colour reactions. And the LEM fixation methods have preserved ultrastructures of the adenocarcinoma, but it was often difficult to maintain constancy in fixation effects. For the constancy, LEM was coupled with low concentration of chemical fixatives, such as glutaraldehyde (<1%) and $OsO_4$ (<0.5%). The results were acceptable, but there are tendencies that the adenocarcinoma requisitioned rather weak microwave energy to come into the optimal fixation effects. Therefore , cultured HeLa cells were fixated with lower energy of microwave than that used to the adenocarcinoma. The ultrastructures of the single HeLa cell have been preserved. The results may imply that a different energy levels of microwave are requisitioned in accordance with kinds of cells and tissues for the optimal fixation effects. It is reported and discussed that the fixation methods of LEM used in this work could be applied routinely to conceal a insufficient diffusion rate of chemical fixatives into some kinds of cancer without compromising the ultrastructures as well as to improve antigenic quality of frozen sections.

Development of Continuous Clean Bioprocess for Kasugamycin Production (Kasugamycin 생산을 위한 연속 청정생물공정 개발)

  • Continuous immobilized-cell culture was carried out for the production of kasugamycin, a secondary metabolite by a filamentous bacteria, Streptomyces kasugaensis, with an intention of reducing waste generation. A sporulation medium was developed for production of bulk amounts of spores, and the spores were entrapped into celite biosupports for immobilization. It was possible to effectively keep the immobilized-cells inside the reactor during the continuous culture by an efficient immobilized cell separator of decantor type on the outlet of the fermentor. Using this continuous immobilized-cell fermentor system, we investigated the effects of feed substrate and phosphate concentrations on kasugamycin production and chemical oxygen demand(COD). Comparing with the conventional suspended-cell batch culture, the kasugamycin productivity was observed to increase by 2.5 times, whereas COD per unit kasugamycin production decreased by 2.3 times in the continuous immobilized-cell culture. Based on these results, the continuous immobilized-cell system was considered to be a cleaner bioprocess than the conventional batch suspended-cell system.

Implant restoration considering maintenance for a patient with excessive crown height space (과도한 치관높이 공간을 가진 환자에서 유지관리를 고려한 임플란트 수복증례)

  • Complications increases when crown height space is excessive and one should be careful of choosing type of fixed prosthesis in case crown height space is over 12 mm. In this condition, tooth seems to be longer, therefore, it is needed to use gingival shade in aesthetically important area for fixed prosthesis. In this case, fixed prosthesis which has inappropriate maintenance and hygiene could hold food, plaque, calculus, thus increase incidence of inflammation and infection. Moreover, it could lead to chronic inflammation and infection and in that case surgical improvement could be necessary. In present case report, gingival inflammation and swelling developed around prosthesis after having made it considering articulation and aesthetic view in patient who had excessive crown height space. Finding out that design of prosthesis is a factor to make it hard to maintain oral hygiene, a temporary prosthesis which has enough interproximal space and proper emergence profile was fabricated. After two months of observation, significant abnormal symptoms are not found and oral reconstruction is performed using fixed prosthesis with zircornia infrastructure and porcelain build-up based on convenient self hygiene design. Through serial treatment, inappropriate soft tissue response is not observed and satisfactory result in functional aspect comes out.
