Effects of ankle foot orthoses on Center of Mass of children with spastic cerebral palsy during gait (경첩 발목 보조기 착용이 경직성 뇌성마비 아동의 신체 질량 중심점에 보행에 미치는 영향)

  • 본 연구에서는 경첩 발목 보조기(Hinged Ankle Foot Orthoses, HAFO)의 착용이 경직성 뇌성마비 아동의 보행 시 신체 질량 중심점(Center of Mass, COM)에 미치는 영향을 알아보고자 한다. 실험은 보조 기구의 사용 없이 독립적인 보행이 가능한 경직성 뇌성마비 아동(하지마비 아동 8명, 편마비 아동 5명) 13명이 참여하였다. 34개의 적외선 반사 마커를 신체에 직접 부착한 뒤, 6대의 동작분석 카메라로 맨발 보행과 HAFO착용 보행을 각 5회 촬영하였다. 전-후방, 내-외측, 수직 방향으로의 COM의 이동과 COM 속도를 계산하여 HAFO착용 전 후를 대응표본 T검정으로 분석하였다. HAFO 착용은 양하지 마비 아동 그룹은 COM의 전방 이동이 유의하게 증가하였고(p<0.01), COM 속도는 내-외측과 수직 방향에서 유의하게 증가하였다(p<0.01). 반면 편마비 아동 그룹은 COM 이동과 속도가 모든 방향에서 유의하게 증가하였다(p<0.01). 따라서 HAFO는 뇌성마비 아동의 보행 기능을 개선시킨다고 할 수 있다.

Effect of Self Care Training(based on International Classification of Functioning, Disability and Health) on Functional Independence in the Young Children with Spastic Cerebral Palsy (국제 기능 장애 건강분류의 구성요소에 기반을 둔 자기관리 훈련이 경직성 뇌성마비 아동의 기능적 독립성에 미치는 영향)

  • The purpose of this study was to investigate the effect of self-care training based on ICF(International Classification of functioning, Disability and Health) on functional independence in the young children with spastic cerebral palsy. Total of 43 young children(male=25, female=18; age range from 36month to 72month) with spastic cerebral palsy, classified at GMFCS(Gross Motor Function Classification System) levels III-IV. Total of 32sessions of a self-care training (eating, grooming, bathing, toileting) were given 4 times a week for 30minutes from August 1th to September 30th of 2008. Changes in the functional independence after the training obtained by Wee-FIM(Functional Independence Measure for Children). Results were as follows: Functional independence was significantly increased after the training. As a result, a self-care training should be applied as an effective intervention to improve the functional independence in the young children with spastic cerebral palsy.

The Effects of Functional Electrical Stimulation on Hand Function of Children With Spastic Cerebral Palsy (기능적 전기자극이 경직성 뇌성마비 아동의 상지 기능에 미치는 영향)

  • Objective: This study aims investigating the effects of functional electrical stimulation (FES) on hand function of children with spastic cerebral palsy. Methods: The participants of this study are 20 children with spastic cerebral palsy aged between 11 to 16 years old. All the subjects underwent 24 sessions of FES during 8 weeks. Each FES was 20 minutes per session, and 3 sessions of FES were provided in a week. Measurements used to assess hand function are Jebsen Hand Function Test, Modified Ashworth Scale and 3D Motion Analysis. After establishment of the baseline for each client by all the measurements, reevaluations were performed every 2 weeks using Jebsen Hand Function Test and the Modified Ashworth Scale. The 3D Motion Analysis was performed only before- and after the 8 weeks of FES treatment. Results: After the FES, there was significant decrease in completed time for the all 6 subtasks of Jebsen Hand Function Test were (p<.05) and also significant decrease in spasticity score of Modified Ashworth Scale as well (p<.05). 3D Motion Analysis showed that the hand tapping and the finger tapping has been significantly improved (p<.05), and the pronation-supination movement of lower arm has been significantly improved as well. Conclusion: Based on the results of this study, it is evidenced that functional electrical stimulation is effective treatment for hand function of children with cerebral palsy. For future research, it is recommended to examine various protocols of FES including impact of long-term application.

A Study for Developing Music Therapy Activity Program for Development of Rudimentary Movement Phase of Spastic Cerebral Palsied Infant : Applying the techniques of Neurological Music Therapy (경직형 뇌성마비 유아의 초보운동단계 발달을 위한 음악치료활동 프로그램 개발 - 신경학적 음악치료의 기법을 활용하여)

  • Cerebral palsy is a collection of motor disorders resulting from damage to the central nervous system that arise in multiple handicaps including cognitive disorders, speech disorders, epilepsy, perception disorders, and emotion disorders. Today spastic cerebral palsy has become more prevalent because intensive care for newborns has resulted in higher survival rates for very small premature babies. Since the children grow the fastest in order for a development during one year after birth, the therapeutic intervention is provided as early as possible to the children with cerebral palsy. After seven year old, there is no effect of intervention. So, the necessity of early intervention to spastic cerebral palsied infants is increasing. The purpose of this study is to develop the music therapy activity program using the techniques of neurological music therapy(NMT), the therapeutic application of music to dysfunctions due to neurologic disease of the human nervous system, for rudimentary movement phase of spastic cerebral palsied infant. This music therapy activity program was developed on the basis of the major developmental tasks of the rudimentary movement phase, the period that children can acquire the most basic movement function at the 0 to 2. Then the developmental characteristics of spastic cerebral palsy were applied to this music therapy activity program. This music therapy activity program was classified to three domains, those are stability, locomotion, and manipulation. This study has been consisted of three steps, those are the development of the activities, the evaluation of the activities by th panels, and the adjustment and complement of the activities. Reviewing literatures and interviews were done for the development of the activities, and the evaluation the activities was done by seven music therapists. In the evaluation steps, the questionnaire was used for estimating the content validity and application efficiency. The adjustment and complement of the activities were evaluated by the panels who were participating in the music therapy for cerebral palsied children in the clinical setting, and the results of the adjustment and complement were confirmed by the panels. The evaluation was presented in a mean value with the comment of the panels. In conclusion, the music therapy activity program for the spastic cerebral palsied infants using the techniques of NMT was developed on the basis of the major developmental tasks of the rudimentary movement phase. The program is comprised of 38 activities, those are 14 activities for developing the stability, 10 activities for developing the locomotion, and 14 activities for developing the manipulation. The programed activities would bring out the answers in the affirmative for the conformance with infants' development phase, the harmony between the objective and the activity, the conformance with the cerebral palsied infants, the properness of the music and the instruments, and the utility in the clinic field. This results mean that this developed music activity program is appropriate to help spastic cerebral palsied infants progress their movement development by stages.

The Effect of Intensive Weight Shift Training on Seated Postural Balance in Children With Spastic Cerebral Palsy: A Case Study (집중체중 이동훈련이 뇌성마비 아동의 체간조절 능력에 미치는 영향: 사례연구)

  • Objective : This study investigated the effect of intensive weight shift training (IWST) on the ability to balance in a sitting posture of children with spastic cerebral palsy (SCP). Methods : One child with spastic diplegia participated in this study for a total of 8 weeks using the ABA experimental design. For the pre-intervention period (A1), general physical therapy (GPT) for children with SCP was performed. The intervention period (B1 and B2), GPT and IWST were conducted for children with SCP, and GPT was conducted again for the post-intervention period (A'1 and A'2). Trunk control capacity was measured using the Korean Trunk Control Measurement Scale (TCMS-K) and Biorescue (RM Ingenierie, France). Results : All TCMS-K variables increased from A1 to B2, and decreased from B2 to A'2. The total area with limit of stability (LOS) increased from A1 to B2, and decreased from B2 to A'2. The ratio of the left/right (Lt/Rt) and anterior/posterior (Ant/Post) LOS area was closer to 1, meaning symmetry, in B2 than in A1. The ratio of the Lt/Rt LOS area decreased further from 1 in A'2 than in B2. The ratio of the Ant/Post LOS area was closer to 1 in A'2 than in B2. Conclusion : IWST had a positive effect on the improvement of balance in the sitting posture of a child with SCP. The results suggest that IWST might help to improve the balance abilities of children with SCP for independent sitting, postural control, and activities of daily living.

Evaluation of Balance and Activities of Daily Living in Children with Spastic Cerebral Palsy using Virtual Reality Program with Electronic Games (전자게임을 이용한 가상현실프로그램이 경직성 뇌성마비 아동의 균형과 일상생활활동에 미치는 영향)

  • The aim of this study were to examine the effects of virtual reality program on balance and activities of daily living in children with spastic cerebral palsy (CP) using the Pediatric Balance Scale (PBS) and the Functional Independence Measure for Children (WeeFIM), and to measure relationship between the PBS and the WeeFIM. For this, A total of 20 spastic CP classified as the Gross Motor Function Classification System (GMFCS) I and II were employed. The Participant's were allocated randomly to 2 groups: a virtual reality group (n=10) and the control group (n=10). Both groups received muscle strengthening exercise for 3 sessions, 30 minutes per week over a 12 week period. The virtual reality group practiced additional virtual reality program. The virtual reality group showed significant increases in balance (p<0.05) and activities of daily living (p<0.05). There were a significant correlation between the PBS and the WeeFIM (p<0.05). Application of the virtual reality program to treat the spastic CP will be feasible and suitable. And the PBS was a useful tool to predict activities of daily living in the spastic CP.

The Effect of Balance and Function in Children with Spastic Cerebral Palsy using Motor Learning training with Treadmill (트레드밀 운동학습 훈련이 경직성 뇌성마비 아동의 기능과 균형에 미치는 영향)

  • The purpose of this study was to apply treadmill training through motor learning to cerebral palsy children and examine its effects on their motor Functions and balance. The subjects of this study were 16 spastic diplegia children who had difficulty in independent gait, and GMFCS level III, IV. The participant's were allocated randomy to 2 groups: a motor learning group(n=8) and the control group(n=8), Both groups received muscle strengthening exercise for 3 session, 30 minutes per week over 7 weeks period. Data collected from the 16 spastic diplegia children the results were as follows. The motor learning group showed significant increase in motor function(p<.05). The motor learning group showed significant increase in balance(p<.05). Between motor learning group and control group, motor functions and balance was a statistically significant difference(p<.05).

Correlation Between Selective Motor Control Test and Functional Performance Evaluation in Children With Spastic Cerebral Palsy (선택적 운동 조절 척도와 기능적 수행도 평가 간의 상관: 경직형 뇌성마비 아동을 대상으로)

  • The purpose of this study was to investigate the relationship among functional evaluation systems, the Selective Motor Control Scale (SMC scale), the Gross Motor Function Classification System (GMFCS), the Gross Motor Function Measure (GMFM) and Activities of daily living in children with spastic cerebral palsy and to provide the foundation data about SMC scle for evaluation system of abilities of selective motor control in children with spastic cerebral palsy. For this, sixty eight children with spastic cerebral palsy were participated in this study. The children were evaluated by using the SMC scale for their selective motor control ability and by using the GMFCS and GMFM for their gross motor function. The activities of daily living were assessed by using the Functional Independence Measure of Children (WeeFIM). There were a significant correlation between the SMC scale and the GMFCS (r = -.485, p < .05). The good correlation between the SMC scale and GMFM was found (r = .482, p < .05). The activities of daily living were not a significant correlation with SMC scale (r = .019, p > .05). The SMC scale in practice will provide usefulness for assessment of abilities of selective motor control in children with spastic cerebral palsy.

Effects of Combined Postural Correction Exercises on Selected Muscle Activity and Stiffness in Children With Cerebral Spastic Diplegia: Case Study (융합적 자세교정치료가 뇌성경직양측마비 아동의 선택된 근육 활성도와 강직도에 미치는 영향: 사례연구)

  • The aim of this study was to investigate the short-term effects of combined postural correction exercises (CPCE) on muscle stiffness and muscle activity in children with cerebral spastic diplegia(CSD). The subjects were 3 children (2 male and 1 female) with CSD from 4 to 7 years old. The measurement sites were upper trapezius muscle(UT), rectus abdominis muscle(RA), and gluteus maximus muscle(Gmax), muscle stiffness and muscle activity were measured using Myoton and surface EMG, and the symmetry of both sides was also examined. As a result, children with severe compensatory action showed decreased muscle stiffness and muscle activity in UT and RA and increase in Gmax after CPCE intervention. Especially, the left and right symmetry of the muscles was decreased. In this study, despite some limitations, CPCE have shown a positive effect in posture correction of children with CSD.

A study on relationships between the Initial Food Consumption Ability of Articulation Production and Intelligibility in Children with Spastic Cerebral Palsy (경직형 뇌성마비 아동의 초기 섭식능력과 조음기관 구조평가 및 말 명료도와의 관련성 연구)

  • This study is to help fine some factors of how the Initial food consumption ability affects articulation production and intelligibility in children at aged 4-15 with cerebral palsy. According to the factor analysis of articulation production, here are some positive articulation productions; the ability of mumbling and swallowing food, biting and swallowing and liquid diet, the experience of food consumption therapy, the use of specific methods or tools for food consumption, the behavior-problem during a meal and the beginning of the neck control. Therefore, the study finds that the Initial food consumption ability in children with spastic cerebral palsy may affect articulation production and intelligibility.
