Prevalence-based Interpretation of Predictive Values of Diagnostic Tests: An Example for Detection of Canine Heartworm Infection (진단키트 검사결과에 대한 유병율 위주 해석: 개 심장사상충의 예)

  • The use of screening tests as part of a diagnostic work-up is common in domestic canine practice, but understanding of the diagnostic test characteristics and factors affecting diagnostic accuracy is not clear among clinicians. This article was aimed to provide clinicians with a better understanding on the selection of test kits and with a proper interpretation of test results using an example from heartworm(Dirofilaria immitis) studies. From the literatures, diagnostic accuracy varied depending on the kits: percent average sensitivity and specificity of ELISA antigen-detecting kits were DiroChek(Synbiotics, USA) 78.1 and 95.2, SNAP(IDEXX, USA) 66.3 and 98.1, and Solo Step(Heska, Switzerland) 69.5 and 97.5, respectively, while the values for three hematological methods(Modified Knott's, direct smear and capillary tube) ranged from 38.4 to 81.8% and from 96.9 to 100%, respectively. Furthermore, it was also reported that the prevalence of heartworm disease in domestic dog populations varied depending on the regions studied: 2.5-22.8% for microfilarial test and 2.2-66.3% by ELISA. The values of predictive values for positive(PPV) and negative(NPV) provide useful information to clinicians on the probability of heartworm infection, but the PPV and NPV are greatly dependent on the heartworm prevalence. This suggests that PPV or NPV values of a test should be interpreted carefully in different clinical settings. Practical methods on the interpretation taking into account heartworm prevalence were discussed.

Development of Rule-based Expert System for Interpretative Report with Health Screening Tests (건강검진자를 대상으로 해석적 보고를 위한 전문가 시스템의 개발)

  • Background : Interpretative reporting is an important aspect of laboratory medicine. The large menu of laboratory tests available today makes it increasingly difficult for the non-specialist to order and interpret all laboratory tests. The aim of this study was to determine the usefulness of an expert system to interpret laboratory tests and help physicians order the appropriate tests. Materials and Methods : In order to interpret laboratory tests, a rules-based expert system was developed. In this module, if-then rules were used to interpret the given test result patterns (e.g. urinalysis, anemia, hepatitis B virus, hypercholesterolemia, glucose, syphilis, and tumor markers) and select matching text elements. The system was used to evaluate 535 subjects who visited a health-check program. Results : The overall abnormal rate was 50.5% in the expert system; 34% for cholesterol, 9.9% for urinalysis, 8.0% for anemia, 7.7% for thyroid function tests, 4.5% for tumor marker study, 4.7% for hepatitis virus antigen, 4.3% for serum glucose, and 1.1% for syphilis. Conclusion : These results indicate that the application of the expert system for the interpretation of laboratory tests may provide a useful method for the interpretation of reports. However more rules are needed for the application to in-patients.

  • Colorectal cancer produce focal mass or segmental thickening which can be detected with sonography. The purpose of this study was to describe sonographic findings of colorectal cancer. we reviewed sonograms of 51 patients with colorectal cancer in whom sonography was performed before colon study. In 51 patients who had more common coloretal cancer 27cases(53%) had 40 to 50 years of age(60%). Sonographic findings included segmental thickening 42cases, or irregular mass 9 cases. With careful examination, these findings can be detectable, and therefore bowel should be carefully examination in patients with sign and symptoms suggesting carcinoma of the colon and rectum.

  • Laboratory medicine is a specialized division that supports physicians in the care of patients by providing rapid and accurate in vitro diagnostic tests. Standardization of every component of a specific test is essential for producing accurate results. The Clinical and Laboratory Standards Institute (CLSI) was founded to develop a formal consensus process for standardization in 1968, and has been publishing standards and guidelines covering all aspects of clinical, research, and other laboratory work. CLSI guidelines are widely used around the world for standardization. The CLSI antimicrobial susceptibility testing subcommittee (AST SC) consists of 6 standing and many ad hoc working groups. Members of the AST SC review submitted proposals and suggestions, decide on approving these submissions in face-to-face meetings held twice a year, and revise CLSI documents accordingly. As these face-to-face meetings are open to anyone who registers to attend, I strongly encourage the members of our Society to attend and actively participate in document development.

  • Diffusion-weighted magnetic resonance imaging (DW MRI) is a fast unenhanced technique that shows promise as a stand-alone modality for cancer screening and characterization. Currently, DW MRI may have lower sensitivity than that of dynamic contrast-enhanced MRI as a standalone modality for breast cancer detection but superior to that of mammography, which may provide a useful alternative for supplemental screening. Standardized acquisition and interpretation of DW MRI can improve the image quality and reduce the variability of the results. Furthermore, high-resolution DW MRI, with advanced techniques and postprocessing, will facilitate better detection and characterization of subcentimeter cancers and reduce false-negatives and false-positives. Future results from ongoing prospective multicenter clinical trials using standardized and optimized protocols will facilitate the use of DW MRI as a stand-alone modality.

  • Studies that can find resolutions to problems of subjective psychiatric analysis must be performed and indeed they are in the process. However there still lies many problems in researches of mentality examination, which should be the foundation of finding potential resolutions. One of the biggest problems in the conventional system is that there are many different opinions by psychiatrists depending on their educations and experiences. There is no objective standard on the subjects and there is no effective psychiatric analysis method. Also, the characteristic of such examinations is that it cannot be applied to disabilities, foreigners and infants alyce the examination is ch examinconversation. The objective of this atudy is to standardize TAT(Thematic Apperception Test)analysiBallken index method so that subjective data from the examination can be excluded and the examination thus maklysithe examination objectified. Furthermore, objective result and patients' brain wave pattern is read with BCI(Brain Computer Interface) ch exaTherenvironment to Alsare it to brain wave characteristics vectors to reate brain-wave characteristics vector DB. Therefore, such DB can be utilize durlysithe examination and diagnosis to reate objective examination method and standardized diagnosis system. Thus, mentality examination can be performed only with brain-wave scans with BCI based TAT system.

A Short form of the Gray-Wheelwright Test (단축형 그레이-휠라이트 검사)

  • We investigated whether the 81 items of the Gray-Wheelwright test correctly measure the concept of Jung's typology and aimed to refine the test. Participants (n=431) completed the Gray-Wheelwright test, and the results were analyzed using factor analysis with the varimax rotation and the maximum likelihood extraction method. A pair of opposing attitudes, introversion/extroversion, or one of the two pairs of opposing functional types, thinking/feeling or intuition/sensation, was labeled to the extracted factor according to the majority type of the items in the factor. The minority items or items not included in any factors were excluded from making a short form of the Gray-Wheelwright test with 45 items. We used intraclass correlation (ICC) coefficient and Cronbach's alpha for the test-retest reliability and internal consistency of the test, respectively. The newly developed short form of the Gray-Wheelwright test measured the Jung's personality types well, which was comparable to the original one while reducing time and effort required for the testing.

  • PHEBUS FPT0 노내실험의 핵연료 다발에 대한 실험후 비파괴 검사 및 파괴 검사 결과를 분석하여 노심손상 후기과정을 정alf 분석하였다. 분석한 비파괴 검사결과는 gamma scanning, radiography, tomographies 였으며 파괴 검사 결과는 정밀사진, metallography, Electron Probe Micro Analysis(EPMA)였다. 그 결과, PHEBUS-FPT0 실험에 사용한 핵연료다발은 기존에 수행된 어떤 다른 노내실험의 핵연료다발보다 많이 용융되었으며 용융 pool 및 피막충의 형성, 용융물 내부의 자연대류 열전달과 이에 따른 shroud 물질 손상, 핵연료다발 물질들간의 eutectic 형성 등을 보여주었다. 특히 Ag-In-Cd 제어봉 물질과 stainless-steel이 핵연료봉 물질과 반응하여 이들의 용융온도를 낮게하여 실험 예측값보다 많이 핵연료다발이 손상되어 기존 중대사고 해석 전산코드의 개선이 요구되었다.

Novel Defect Testing of RF Front End Using Input Matching Measurement (입력 매칭 측정을 이용한 RF Front End의 새로운 결함 검사 방법)

  • 본 논문에서는 입력 매칭(input matching) BIST(Built-In Self-Test) 회로를 이용한 RF font end의 새로운 결함 검사방법을 제안한다. BIST 회로를 가진 RF front end는 1.8GHz LNA(Low Noise Amplifier: 저 잡음 증폭기)와 이중 대칭 구조의 Gilbert 셀 믹서로 구성되어 있으며, TSMC 0.25$\mu\textrm{m}$ CMOS 기술을 이용하여 설계되었다. catastrophic 결함 및 parametric 변동을 가진 RF front end와 결함을 갖지 않은 RF front end를 판별하기 위해 RF front end의 입력 전압 특성을 조사하였다. 본 방법에서는 DUT(Device Under Test: 검사대상이 되는 소자)와 BIST 회로가 동일한 칩 상에 설계되어 있기 때문에 측정할 때 단지 디지털 전자계와 고주파 전압 발생기만이 필요하며, 측정이 간단하고 비용이 저렴하다는 장점이 있다. BIST 회로가 차지하는 면적은 RF front end가 차지하는 전체면적의 약 10%에 불과하다. 본 논문에서 제안하는 검사기술을 이용하여 시뮬레이션해 본 결과 catastrophic 결함에 대해서는 100%, parametric 변동에 대해서는 약 79%의 결함을 검출할 수 있었다.

NORMOBARIC OXYGEN($O_2$) ADMINISTRATION EFFECT ON ATTENTION AND MEMORY FUNCTION IN TEENAGE ADOLESCENTS (10대 청소년의 주의력과 기억능력에 미치는 정상기압 산소흡입 효과)

  • Objectives:This study was conducted to investigate the effect of oxygen on attention and memory functions in healthy adolescents. Methods:The participant subjects were recruited from local advertisement. All subjects are students attending ordinary middle and high school. Their degree of achievement was average or below average. Before the study, its nature and purpose were fully explained to the patients and their parents, and a written informed consent was obtained from each child's parent and a written assent from each child for entire the procedure. The Ethics Committee and Clinical Research Committee of Gyeongsang National University Hospital approved the protocol. For baseline assessment, all subjects received tests for attention and memory. All tests were conducted by a certified psychologist. Stroop test, continuous performance test and trail making test A and B were used for evaluation of attention. As memory tests, we used memory assessment scale(MAS), standardized memory assessment tools. Ten to fourteen days after initial assessments, same tests was applied to the same subjects after prior 5 minute oxygen inhalation. Results:1) Attention test:Improved performances in trail making part B, and stroop test were found in normobaric oxygen inhalation group compared to air inhalation group. Improved reaction time in those tests seemed to reflect the enhanced executive prefrontal activity. 2) Memory test:More words and digits memorization were found in short-term memory subscale score in MAS in oxygen inhalation group compared to air inhalation group. This finding suggested the improved working memory function after oxygen inhalation. Conclusion:Though interpreted cautiously, these results suggested that normobaric oxygen inhalation could enhance executive function and working memory of prefrontal lobe. Further study, however, should be performed to investigate the mechanism of effects of oxygen on cognitive enhancement.
