The Effect of Green Vegetable Drink Supplementation on Blood Pressure, Erythrocyte Antioxidant Enzyme Activities and Plasma Lipid Profiles of Korean Male Subjects (채소혼합음료의 섭취가 남자 성인의 혈압, 항산화 효소 및 혈장지질 양상에 미치는 영향)

  • The purpose of this study was to evaluate whether a daily regimen of green vegetable drink supplementation to male subjects improves the blood pressure and enhances other antioxidant enzyme status, lipid profiles and liver functionalities. Twenty nonsmokers and 19 smokers aged $23{\sim}60$ were given 240 mL of green vegetable drink every day for 8 weeks in addition to their normal diet, and blood samples were drawn before and after the intervention. The 8 weeks of green vegetable drink consumption resulted in a significant decrease of diastolic blood pressure in smokers (p<0.05) as well as in nonsmokers (p<0.01), and systolic blood pressure in smokers (p<0.05). Erythrocyte superoxide dismutase (SOD) activity was significantly increased in both nonsmokers and smokers after the trials. Plasma total cholesterol (TC) level was decreased in smokers and plasma TC and LDL-cholesterol (LDL-C) levels were decreased in nonsmokers while triglyceride (TG) and HDL-cholesterol (HDL-C) was not affected in both nonsmokers and smokers by green vegetable drink consumption. Plasma glutamic pyruvic transaminase (GPT), a marker of liver function was significantly reduced in both nonsmokers and smokers and plasma glutamic oxaloacetic transaminase (GOT) level was significantly reduced in nonsmokers after the supplementation. These results support the hypothesis that green vegetable drink exerts a blood pressure reducing effect and affects not only the activity of antioxidant enzymes, lipid profiles but also plasma GOT and/or GPT levels.

Nutrition Knowledge and Utilization of Information on Fast Food of Secondary School Students (대도시 중.고등학생의 패스트푸드에 대한 영양지식 및 정보 활용)

  • The purpose of this study was to evaluate nutrition knowledge and utilization of information on fast food of secondary school students in the two biggest cities, Seoul and Busan. Questionnaires were distributed to a total of 1,265 students (645 at 10 middle schools, 620 at 10 high schools). An average rate of correct answers on nutrition knowledge of fast food was 73.4%. General nutrition knowledge on fast food showed high percentage of correct answers, but the knowledge of vitamin and fiber on fast food was of low percentage. The scores of nutrition knowledge, the fast food (ie. chicken, pizza, gimbab, dukbokki, and ramyon) intake group with $'{\geq}1$ time/week' showed significantly (p<0.01) lower scores than those of the group with $'{\leq}1$ time/month'. The groups with higher interest in health and weight control had significantly (p<0.05) higher scores on nutrition knowledge than those of lower interest groups. Only 19.6% of the subjects had the experience of using the nutrition information for selecting fast food, but 66% of them wanted the developments of the internet-sites to provide information on fast food and on the relationship between fast food and health.

Public Perception and Acceptance of the National Strategy for Well-Dying (웰다잉 국가 전략에 대한 일반 국민들의 인식 및 수용도)

  • Purpose: Ten years have passed since the Korean government announced its plan to institutionally support hospice and palliative care in 2002. In line with that, this study aims to suggest future directions for Korea's hospice and palliative care policy. Methods: We conducted a survey on people's perception and acceptance of well-dying. Data were collected from 1,000 participants aged 19~69 years between June 1 and June 11, 2012 via computer-assisted telephone interviews. Results: The most important factor for well-dying was placing no burden of care on others (36.7%) and the second most important factor was staying with their family and loved ones (19.1%). Among nine suggestions of policy support for well-dying, the most popular was the promotion of voluntary care sharing (88.3%), followed by the palliative care training support for healthcare providers (83.7%) and the support for palliative care facilities instead of funeral halls (81.7%). The idea of formulating a five-year national plan for end-of-life care drew strong support (91%). According to the survey, the plan should be implemented by the central government (47.5%), the National Assembly (20.2%) or civic groups (10%). Conclusion: This study demonstrated the public consensus and their consistent direction toward policy support for well-dying. Results of this study may serve as a foundation for the establishment of policy support for people's well-dying and palliative care at the national-level.

Regulation of Professional Advertising: Focusing on Physician Advertising (전문직 표시·광고규제의 몇 가지 쟁점: 의료광고를 중심으로)

  • A commercial advertisement is not only a way of competition but also a medium of communication. Thus, it is under the constitutional protection of the freedom of business (article 15 of the Constitution) as well as the freedom of press [article 21 (1) of the Constitution]. In terms of the freedom of business or competition, it should be noted that an unfair advertising (false or misleading advertisement) can be regulated as an unfair competition, while any restraint on advertising other than unfair one might be doubted as an unjustifiable restraint of trade. In terms of the freedom of press or communication, it is important that article 21 (2) of the Constitution forbids any kind of (prior) censorship, and the Constitutional Court applies this restriction even to commercial advertising. In this article, the applicability of these schemes to advertising of the so-called learned professions, especially physician, are to be examined, and some proposals for the reformation of the current regulatory regime are to be made. Main arguments of this article can be summarized as follows: First, the current regime which requires advance review of physician advertising as prescribed in article 56 (2) no. 9 of Medical Act should be reformed. It does not mean that the current interpretation of article 21 of the Constitution is agreeable. Though a commercial advertising is a way of communication and can be protected by article 21 (1) of the Constitution, it should not be under the prohibition of censorship prescribed by article 21 (2) of the Constitution. The Constitutional Court adopts the opposite view, however. It is doubtful that physician advertising needs some prior restraint, also. Of course, there exists severe informational asymmetry between physicians and patients and medical treatment might harm the life and health of patients irrevocably, so that medical treatment can be discerned from other services. It is civil and criminal liability for medical malpractice and duty to inform and not regulation on physician advertising, to address these differences or problems. Advance review should be abandoned and repelled, or substituted by more unproblematic way of regulation such as an accreditation of reviewed advertising or a self-regulation preformed by physician association independently from the Ministry of Health and Welfare or any other governmental agencies. Second, the substantive criteria for unfair physician advertising also should correspond that of unfair advertising in general. Some might argue that a learned profession, especially medical practice, is totally different from other businesses. It is performed under the professional ethics and should not persue commercial interest; medical practice in Korea is governed by the National Health Insurance system, the stability of which might be endangered when commercial competition in medical practice be allowed. Medical Act as well as the condition of medical practice market do not exclude competition between physicians. The fact is quite the opposite. Physicians are competing even though under the professional ethics and obligations and all the restrictions provided by the National Health Insurance system. In this situation, regulation on physician advertising might constitute unjustifiable restraint of competition, especially a kind of entry barrier for 'new physicians.'

Bioavailability and Digestibility of Organic Calcium Sources by Bone Health Index (뼈건강 지표를 이용한 유기태 칼슘 급원의 생체이용성 및 소화율)

  • This study was carried out to evaluate the bioavailabilities and the digestibilities of oligopeptide chelated (peptide-Ca), anchovy bone (anchovy-Ca) and methionine hydroxyl analogue (MHA-Ca) calcium compared to those of calcium carbonate in rats. In exp1, $CaCO_3$, were added to the basal diet at level of 0, 30 and 60% calcium of the AIN-93G diet. In test groups, peptide-Ca, anchovy-Ca and MHA-Ca, were added to the basal diet to provide calcium at the level of 40% of AIN-93G. In exp1, the bioavailabilities were evaluated from the regression equation of the ratios of theological/ actual calcium intakes of each dietary treatment. In exp2, urine and feces was to evaluate the true- and apparent digestibility and apparent retention. In exp1, Ca-60% group had higher bone mineral density (BMD), bone mineral content (BMC) and bone breaking strength (BBS) than those of the other standard groups. The bone weight and ash content of the peptide-Ca and anchovy-Ca groups were significantly higher than those of the MHA-Ca. Bone calcium content were not significantly different from the test group. The bioavailability of the MHA-Ca group was shown higher BMD (71%), BS (38%) and BBS (27%) compared to another control group. But the regression coefficient for BMD, BS and BBS were lower compare with that of bone ash and BMC. In exp2, the true- and apparent digestibility of test groups were shown to over 90%. Peptide-Ca was not significantly different from other test group, but digestibility and retention were higher compare to other test groups. In conclusion, peptide-Ca, anchovy-Ca and MHA-Ca improved Ca bioavailability in the rats. The compounds were higher Ca digestibility compared with those of $CaCO_3$. It is assumed that difference of digestibility for test groups may be correlated to the bioavailability of test groups in BMD, BMC, BS, BBS and bone ash respectively.

Dietary Behavioral Correlates of Nutrition Label Use in Korean Women (한국 성인 여성에서 영양표시 사용과 식행동 요인과의 관계)

  • This study describes the demographic and diet-related psychosocial correlates of nutrition label use, and examines the relationship between label use and diet. Self-reported dada from a population-based cross-sectional survey of 2073 Korean women aged 20 to 60 years were collected to identify demographic and health-related characteristics, belief on diet-disease relationship, awareness on importance of healthy eating practice and diet quality associated with label use. Label users, who are in the stage of action and maintenance (31.6%), were more likely to have belief on nutrient-disease relationship (in sodium, cholesterol, sugar and trans fat) and were more likely to have higher awareness of the importance of healthy eating practice compared with label nonusers, who are in the stage of precontemplation, contemplation and preparation. Label users were more likely to have higher dietary quality compared with label nonusers [odds ratio (OR) = 2.01; 95% confidence interval (CI): 1.66, 2.44](P < 0.001). Also, label use appeared to be associated with the consumption of diets that were higher vegetables and fruits, and lower in cholesterol. The findings of this study suggests that reading nutrition labels on food packages may improve food choices and enable healthful dietary practices.

The Clinical and Cost Effectiveness of Medical Nutrition Therapy for Patients with Type 2 Diabetes Mellitus (제2형 당뇨병환자에서 임상영앙치료의 임상적 효과와 비용효과 연구)

  • Medical nutrition therapy (MNT) is considered a keystone of medical treatment of chronic diseases. However, only few studies have evaluated medical and economical outcome of MNT. The study was performed on the patient with type 2 diabetes mellitus to evaluate the effect of clinical and cost-effective outcomes of MNT. Subjects from two general hospitals were randomly assigned to two different groups; One receiving basic nutritional education (BE) (n = 35), and the other receiving intensive nutritional education (IE) (n = 32) for a 6-month clinical trial. The group which received BE had a single visit with a dietitian, while the other group which received IE had an initial visit with a dietitian addition to two visits during the first 4 weeks of the study periods. Anthropometric parameters, blood components, and dietary intake were measures at the beginning of study period and after 6 month. Cost-effective analysis included direct labor costs, educational materials and medication cost difference during 6 months. After 6 month, subjects from IE group showed significant reduction of body weight (p <0.05) and systolic blood pressure (p <0.05), whereas BE group did not show any significant changes. Result from biochemical indices showed glycated hemoglobin concentration was significantly reduced by 0.7% (p <0.05) only in the IE group. The ratio of energy intake to prescribed energy intake decreased significantly in both groups (p <0.05). Mean time taken for a dietitian to educate the subject was 67.9 ${\pm}$ 9.3 min/person for BE group, while 96.4 ${\pm}$ 12.2 min/person for IE group. Mean number of educational materials was 1.9 ${\pm}$ 0.7/person for BE group and 2.5 ${\pm}$ 0.7/person for IE group. Change in glycated hemoglobin level along the 6 month period of study can be achieved with an investment of \88,510/% by implementing BE and \53,691/% by implementing IE. Considering the net cost-effect of blood glucose control and HbA Ic, IE which provides MNT by dietitian had a cost efficiency advantage than that of BE. According to this study, MNT provided by dietitian had a significant improvements in medical and clinical outcomes compared to that of BE intervention. Therefore, MNT protocol should be performed by systemic intensive nutrition care by dietitian in clinical setting to achieve good therapeutic results of DM with lower cost.

A Comparative Analysis of the Level of Occupational Health : Before and After the Subsidiary Program on Health Care Management of Small Scale Industries (영세사업장 보건관리 지원사업 실시 전후의 산업보건수준 비교 분석)

  • The small scale industries which have less than 30 employees occupy 86.5% of total number of industries in Korea. And though they have higher accident rate and lower environmental condition than big industries, it has been not mandatory to appointing health care manager at factory. So, from 1993, government subsidizes to the health care management of small industries. The purpose of this study is to identify the real feature of health care status in small industries, and to evaluate the level of health care management, before and after the subsidiary program. 65 small plating industries which have been managed by the same health care management support institution in 1993 were selected for study. Of the 65 industries, 3 which have not taken both environmental evaluation and health screening in 1994, and 9 which have closed were excluded from study sample. And the remaining 53 were analyzed by using the results of environmental evaluation and health screening, reported to the Ministry of Labor, before and after the subsidiary program, the analysis was done by the comparison of the two year paired data of the same industry. Over-permissible-limit rate, health screening implementation rate, above grade C rate were calculated and compared. The status of health care management ; 1. Of the sample industries, 96.9% provide protective equipment and 80.0% set up ventilating system. Protective gloves (89.2%) and protective clothing (80.0%) are widely provided, but ear plugs (4.6%) are rarely provided. 21.5% of the protective equipment are well put on, and 40.4% of the ventilating systems function well. 2. In 1993, 35 industries, 53.8% of the sample, checked working environment twice. Over-permissible-limit rates of heavy metal (12.2%), suspended particle (11.1%), noise (5.5%) were high. To put on protective equipment and to set up local ventilating system were pointed out by the examiners. 3. General health screening was done at 63.1% of the sample industries and 35.3% of total workers were examined. Specific health screening was done at 93.8% of the sample industries and 75.4% of workers were examined. 15.5% of workers was provided to be above grade C and to have digestive system disease (43.3%), circulatory disease (18.9%), and hematopoietic disease (14.2%), etc. 4. In 1993, the subsidiary program of health care management was provided in forms of health education, health counseling, and rounding check of working field. And 61.5%, 83.0%, 55.4% of sample industries respectively received it. The average visit per industry was 1.8. Comparisons of the level of occupational health before and after the subsidiary program ; 1. Over-permissible-limit rates of hazardous factors of 1993 and that of 1994 were compared. The rates of suspended particle, noise, organic solvent of 1994 (37.5%, 13.4%, 24.2% respectively) were higher than that of 1993 (25.0%, 6.0%, 6.3% respectively). In the case of acid, there was no difference between the rate of 1993 and that of 1994. Only the rate of heavy metal decreased from 12.9% in 1993 to 3.0% in 1994. 2. General health screening was done at 38.7% of the sample industries in 1993 and at 44.6% in 1994. But the implementation rate of specific health screening decreased from 72.4% in 1993 to 64.6% in 1994. 3. The implementation rate of specific health screening was analyzed by some health factors. The rate of suspended particle increased from 61.8% in 1993 to 91.2% in 1994. But the rates of the others-noise, organic solvent, heavy metal, specific chemical substances-decreased. 4. Above grade C rate in health screening increased from 27.8% in 1993 to 35.5% in 1994. But that of endocrine disorders and pulmonary disease decreased.

Analysis of Consumption Status of Cooked Rice with Different Grains and Related Factors in a Korean Population: Based on Data from 2011 Korean National Health and Nutritional Examination Survey (KNHANES) (한국인의 잡곡밥 섭취 실태 및 관련 요인 분석 - 2011년 국민건강영양조사 자료를 이용하여 -)

  • This study aimed to investigate consumption status of cooked rice with different grains as well as examine related factors of Koreans. Based on the Korea National Health and Nutrition Examination Survey of the second year (2011) in the fifth stage, we extracted data on 7,190 people aged 1 year and higher who consumed cooked rice with one or more grains and legumes for at least one meal per day. A total of 15,250 meals were analyzed. Approximately 67.6% of subjects consumed cooked rice with different grains for more than one meal. Age was the strongest influencing factor for consuming cooked rice with different grains. Compared with people in their 40s, those aged 19~29 and in their 30s showed lower probabilities (0.5 and 0.6, respectively) of intake of cooked rice with different grains, whereas those in their 50s and 60s showed relatively higher probabilities (1.9 and 5.0, respectively). The intake ratio of cooked rice with different grains was higher in subjects who were females (64.5%), aged 1~18, over 50s, and residing in a metropolis (62.0%). Breakfast (69.6%) showed a higher intake ratio of cooked rice with different grains compared to lunch (49.1%) and dinner (57.9%). Intake of cooked rice with different grains was highest at home (71.2%), whereas it was lowest in restaurants (11.2%). The findings of the current study may be useful in planning dietary guidelines for intake of cooked rice with different grains.

Differences between Species and Seasonal Changes in Cambial Electrical Resistance of Twenty Ornamental Tree Species (20개(個) 조경수종(造景樹種)의 형성층전기저항치(形成層電氣抵抗値)의 수종간(樹種間) 차이(差異)와 계절적(季節的) 변화(變化))

  • The objectives of this study were to obtain standardized basic data on cambial electrical resistance (CER) of ornamental trees to be used for estimation of tree vigor and to compare CER between shrubs, deciduous, and coniferous trees in relation to bud opening, flowering, and growth cessation. Eighty healthy trees with four trees each for twenty ornamental tree species growing at an university campus in Suwon were selected and their CER was measured using a Shigometer every week from March to May, once a month from June to October and every two weeks in November, 1996. The CER of all tree species was high in March, started to decrease in April, maintained minimum in May through August, began to increase in September, and returned in November to the similar high level to March. Among the 20 species, Metasequoia glyptostroboides showed the lowest($5.5k{\Omega}$) annual average CER, while Cercis chinensis and Ligustrum obtusifolium had the highest($22.7k{\Omega}$, $22.9k{\Omega}$) annual average CER. The lowest CER($2.4k{\Omega}$) in summer(June through August) was obtained from Wisteria floribunda, second lowest($2.5k{\Omega}$) from Metasequoia glyptostroboides, the highest CER($46.8k{\Omega}$) during dormant season from Euonymus japonica, and second highest($45.0k{\Omega}$) obtained from Ligustrum obtusifolium. The CER of most species, particularly shrub species, started to decrease with bud opening, and many species, particularly shrubs and deciduous trees with large amount of flowers, showed sharp decrease with flowering. When CER was compared between shrubs and trees, shrubs showed higher average CER than trees, and seasonal difference in CER of evergreen species was smaller than that of deciduous species. It is interesting to note that the deciduous Metasequoia glyptostroboides had the lowest annual average CER.
