대구시 개원의사와 개국약사의 의약분업에 대한 인식과 태도 (Recognition and attitude to functional division between physicians and pharmacists of practising physicians and pharmacists in Taegu city)

  • 1989년 10월부터 실시되고 있는 약국의료보험과 의약분업제도에 관한 태도를 조사하기 위하여 대구시내 개원의사 및 재국약사를 계통적 표본추출법(systematic sampling)에 의한 표본을 선정하여 우편설문지법으로 1992년 4월 20일부터 5월 20일까지 조사하여 회신된 개원의사 184명, 개국약사 157명의 설문지를 분석한 결과를 요약하면 다음과 같다. 현재 시행중인 약국의료보험의 성과에 대해 개원의사는 71.2%가 '실패적'이라고 한 반면 개원약사는 13.4%가 '실패적'이라고 하였다. 개원의사의 50%는 약국의료보험을 폐지하고 의약분업을 도입해야 한다고 한 반면, 개국약사는 66.9%가 의약분업제도와는 관계없이 약국의료보험자체만으로도 성공적인 제도라고 하였다. 개국약사의 약국 1일 평균조제건수는 32.2회였으며, 약국의료보험 이용횟수는 6.2회로 조제건수의 20%에 불과했고, 의사처방전을 지참한 약국의료보험이용횟수는 조제건수의 0.7%였다. 그리고 개원의사의 원외처방전 발행경험자는 58.7%였다. 의약분업제도의 실시에 대해 개원의사는 59.2%가 찬성하였으며 27.7%가 반대하였으나 재국약사는 38.0%가 찬성, 45.5%가 반대 하였다. 그리고 약사가 의사보다 의약분업의 내용을 더 많이 안다고 하였다. 의약분업제도 실시의 찬성자중 찬성이유로 개원의사는 '의약품의 남오용 방지' (54.1%)를 많이 지적한 반면 개국약사는 '의사와 약사의 전문직능 발휘'(62.0%)를 많이 제기하였다. 그리고 분업찬성자에서 개원의사는 52.3%가 '완전강제분업'을 원한 반면, 개국약사는 81.7%가 '부분분업'을 원하였다. 의약분업제도 실시시에 처방전의 발행 방법에 대해서는 개원의사와 개국약사 모두 '일반명' 처방을 44.0%, 89.8%로 가장 많이 원하였고 개원의사에서는 '상품명' 처방도 35.3%나 차지하였다. 의약분업제도의 실시가 이루어지지 않은 이유에 대해 개원의사 및 개국약사 모두 '의사 약사단체 상호간의 업권문제'를 가장 많이 지적했으며 '국민들의 인식 및 관심 부족' '정부의 의지력 결여' 순으로 일치된 결과로 나타났다. 의약분업실시를 위한 선결조건으로는 '의료시설과 약국의 도시 농촌간의 균등분포'를 가장 많이 지적하였으며 의사는 '약사들의 수용태세 확립', 약사는 '의사의 수용태세 확립'을 그 다음으로 지적해 서로 상반된 결과가 나타났다. 이상의 결과를 종합해 보면 첫째, 조사대상 개원의사들은 현행 약국의료보험제도에 대해 부정적 견해를 보인 반면 개국약사들은 긍정적 견해를 보였으나 약국의료보험이용은 극히 저조하고 의사의 처방전 발행도 저조한 실정이다. 이에 약국의료보험제도에서 의약분업제도로의 제도적 전환이 고려되어야 할 것으로 생각된다. 둘째, 의사와 약사의 의약분업에 대한 의견이 상이한 점으로 미루어 유추할 수 있지만 의약분업제도 실시의 장애요인으로 의 약사단체 상호간의 업권문제와 의약사간의 갈등이 지적되는 바, 이들 모두를 만족 할 대안을 선택하는 것이 쉬운일 아닐 것이므로 국민의 건강보호차원에서 정부의 중립적 의지가 있어야 할 것으로 생각된다.

기능성화장품의 마케팅전략에 관한 연구 -약국을 중심으로- (A Study on the Marketing Strategy for Cosmeceuticals in Pharmacies)

  • 본 연구의 목적은 기능성화장품을 취급하고 있는 개국 약사들을 대상으로 기능성화장품에 대한 인식, 취급실태, 중요성과 만족도를 조사하고 이를 토대로 약국에서 기능성화장품의 마케팅 활성화방안을 마련하는데 있다. 설문조사결과 약사들의 기능성 화장품에 대한 인식은 아직 저조한 편으로 전문지식을 제고할 필요가 있는 것으로 나타났다. 약국에서 기능성화장품의 마케팅을 활성화하기 위해서는 기능성화장품의 정의 확대, 기능성화장품 심사제도 개선, 다양한 약국 전문 기능성화장품 개발 및 유통, 약사의 전문성 제고를 위한 교육 실시, 기업의 적극적 홍보 및 소비자 네트워크 중시, 의약 공동협력 마케팅 추진, 건강보조식품과 기능성화장품의 연계마케팅, 약국 공간 확보 및 전문적 디스플레이, 제품에 대한 전문적 상담과 적극적 자세 등이 필요하다. 본 연구는 의약분업과 같은 약국환경변화에 따라 약국의 경영개선을 위한 방안으로서 기능성화장품의 마케팅 강화 필요성과 구체적인 방안을 설문조사결과를 통해 제시하였다는데 의의가 있다. 본 연구결과는 개국 약사들의 기능성화장품에 대한 판매 전략과 기능성화장품제조 및 유통기업의 기능성화장품에 대한 교육 및 유통전략에 기초를 제공하는데 커다란 도움이 되리라 본다.

개국약사의 적정조제건수 산출 (Estimation of the Number of Optimal Dispensing Cases for the Community Pharmacist)

  • Separation of prescribing and dispensing practice in Korea has changed the service pattern of the pharmacy. The prescription dispensing activities, however, are concentrated excessively on the pharmacies near hospitals or clinics. Thus this study was conducted to estimate the number of optimal dispensing cases for the community pharmacy. Forty-six pharmacies were selected using systematic stratified random sampling method, and ninety-five pharmacists were interviewed on their workload of dispensing and other activities at pharmacies. One hundred and seventy prescriptions were chosen based on the length of drug administration and drug dosage form, and the dispensing time was measured by time-watch method. Also pharmacy benefit claims data were analyzed to identify the characteristics of the pharmacies which performed more than optimal dispensing cases. According to the study results, the average work time per pharmacist per day was found to be 10hours 32minutes and the dispensing activities occupied 7hours 36minutes. It took 5.72minutes on average for each dispensing case. The optimal dispensing case was estimated as 75 cases under the condition of 10hours 32minutes work time and 6% allowance rate. Even though the pharmacies near hospitals or clinics participated dispensing services actively, only pharmacies near clinics dealt with more than optimal dispensing cases. For the pharmacies near hospitals they dealt with less than optimal cases, but drug administration period per prescription was almost 3 times longer than that of pharmacies near clinics. Thus the intensity of dispensing activities such as drug administration period is to be considered to estimate optimal dispensing cases more accurately.

약국업무시 고충에 관한 개국약국 약사의 인식에 관한 연구 (Community Pharmacists' Perception of Barriers to Pharmacy Work)

  • The purpose of this study was to evaluate barriers to pharmacists' routine work in community setting. A survey was given to 281 pharmacists who enrolled in 16-week clinical pharmacy educating program in continuous education center for advanced pharmacy at Seoul National University. Three main questions that pharmacists were asked to answer were: (1) difficulties on dispensing prescription, (2) difficulties on communicating with doctors, (3) most difficult disease on patients counseling. The response rates for the survey were more than 60 % for each three questions (62.63 %, 63.7 %, and 64.41 %, respectively). The top three barriers to dispensing prescription were lack of professional knowledge about medications, prescription error and its solving ability and patient counseling. The top three barriers to communicating with doctors were lacking of opportunity to discuss about patients' medication due to unavailability of doctors, doctors' attitude using authoritative manner, and a pharmacist's lack of knowledge. The top 4 most difficult diseases on patients counseling were cardiovascular disorders, dermatologic disorders, endocrinologic disorders, and psychiatric disorders.

약물유해반응 보고에 대한 개국약국 약사들의 태도 및 지식에 관한 연구 (Attitude and Knowledge of Community Pharmacists to Adverse Drug Reaction Reporting)

  • The purpose of this study was to evaluate the attitude and knowledge of community pharmacists to spontaneous adverse drug reaction (ADR) reporting. A survey was conducted in 103 community pharmacists in Seoul and Kyung-Gi Province from September to October, 2009. Almost half (53, 51.5%) and three-fourths (79, 76.7%) of respondents knew they could participate in ADR reporting in the Regional Pharmacovigilance Center (RPC) or Korean Food and Drug Administration (KFDA). However, only three of them have reported an ADR to the RPC and none of them have reported to KFDA. A majority of the respondents (96, 93.2%) felt that ADR reporting was a professional obligation for pharmacists. The purpose of an ADR reporting was incorrectly identified by most pharmacists. ADR reporting was encouraged if the reaction was serious and a new product. The most frequently mentioned barriers to reporting were: uncertainty concerning the causal relationship (86.3%) and not knowing how to report an ADR (83.2%). Only 19 (18.4%) respondents had received education on how to report ADR. Education was the most recognized method of improving ADR reporting. This study showed the knowledge of ADR reporting among community pharmacists is inadequate. Education and training should be reinforced in order to improve ADR reporting by community pharmacists.

복약지도 실태와 환자 소비자 요구에 대한 조사 - 간호과 학생에 의한 간접경험을 중심으로 - (Medication Counseling and the Patient Customers' Demands)

  • Purpose: The study examines the condition of medication counseling to determine the patient customers' demands, with the purpose of providing a basic data to improve the condition of medication counseling for phamacists and clinical nurses. Method: Questionnaire was completed by 150 nursing students. For data analysis, the SPSS win 10.0 program was used. Result: The contents of medication counseling given by the pharmacists in order of frequency were($100\%$ means 'always'): way of use$(78.7\%)$, period of taking medicine $(63.3\%)$, dosage$(46.7\%)$, effect$(27.3\%)$, drug name$(18.7\%)$, diet$(12.7\%)$, question of taking other drug$(12.7\%)$, giving a notice$(12.0\%)$, way of storage$(7.3\%)$, side effects$(6.0\%)$, drug interaction$(3.3\%)$' The needs and demands desired by the patients in order of frequency were('the most important' is given 10 points, followed by the others in numerically decreasing order until 0): effect(8.68), way of use(7.60), side effect(7.20), giving a notice(6.38), dosage (5.95), drug name(5.67), a period of taking medicine(5.60), drug interaction(4.37), diet(3.63), the way of storage(3.03). Most of the respondents were satisfied with the pharmacists' kindness$(62.7\%)$. Some recognized the medication counseling was quite good$(16.7\%)$, but the majority felt it was modest$(50.8\%)$' Nearly half of them felt the medication counseling time was quiet short$(47.3\%)$' The communication was mostly oral$(83.3\%)$, while others used sticker$(6.0\%)$ or leaflet$(6.0\%)$ additionally. There was a significant correlation between pharmacists' kindness and medication counseling content(p<,01), and also recognition(p<.001). The correlation between the number of medication counseling which gave to the patient customer and the recognition was also significant (p<.001). As the number of medication counseling increased, the patient felt the medication counseling time was adequate(p<.05). The correlation between the medication counseling recognition and the adequate time was also very significant(p<.001). Conclusion: These results revealed the situation of medication counseling to be poor and to be differ from the patient customers' demands. Therefore optimal medication counseling, especially regarding possible side effects and drug interaction should be emphasized. Pharmacists and clinical nurses should give qualitative medication counseling which reflects the patients' demands.

부산시 개원 의사와 개국 약사의 의약분업 정책목적 달성도와 손익에 대한 인식도 (Physicians and Pharmacists' Perceptions about the Goal Achievement of the Separation Policy of Drug Prescribing and Dispensing, and Benefit and Loss Caused by the Policy in Busan)

  • The purpose of this study was to identify health care providers' perceptions about the goal achievement and benefit/loss caused by the separation policy of drug prescribing and dispensing after the policy implemented on July 1, 2001. Uslng stratified sampling method based on the administration area, Ku, 315 physicians and pharmacists were sampled from the rosters of physician and pharmacist association in the city of Busan on 2001. There were 122 and 115 responses from physician and pharmacist sample, respectively. 78.3% of physicians and 50.4% of pharmacists evaluated that the goal of the policy was not achieved. Moreover, 75.3% of physicians and 40.7% of pharmacists did not support the policy. Most physicians and pharmacists considered preventing the citizens with drug abuse and misuses as the most important benefit derived from the policy. However, physicians and pharmacists concerned over raising health care cost that could be patients' burden. The most important physicians' benefit derived from the policy was free choice of all possible medicine that might result in effectiveness of medication. In physicians' the most important loss, most physicians worried about that breaking traditional patient and physician relationship might cause physicians' authority in treating diseases to be damaged. Pharmacists considered the most important policy benefit as hiked social status resulted from enforcement of profession due to the policy whereas they considered the most significant loss as expected financial problems of small pharmacies compared to that of large pharmacies or pharmacies adjacent to hospitals. In the current problems of the policy, physician and pharmacists blamed the government for inadequate preparations of the policy implementation. Physicians and pharmacists also considered citizens' mature attitudes toward the policy as a crucial success factor.
