척추관 협착증은 감압술과 융합술 그리고 인공추간판 치환술 등이 있으며, 2000년대 중반부터는 인공추간판 치환술이 널리 시술되고 있다. 인공추간판의 연구는 추간판의 자유도 및 추체의 굴곡-신전, 측굴전, 축회전에 대한 해석이 핵심기술이지만, 시술 후 수년이 경과하여 발생하는 피로파손이 새롭게 큰 문제점으로 대두되고 있다. 따라서 인공추간판 연구는 슬라이딩 코어의 피로특성 및 내구성 향상에 집중되어야 한다. 본 연구에서는 세계적으로 가장 많이 사용되는 인공추간판 제품(SB Charit$\acute{e}$ III)을 기초로 유한요소모델을 제작하고, 슬라이딩 코어의 곡률반경과 마찰계수의 변화가 von-Mises 응력과 접촉압력에 미치는 영향을 평가하였다. 이와 같은 결과를 바탕으로 새로운 인공추간판 모델들 (Model-I, -II, -III)을 제안하고 일정수명 후 발생할 수 있는 슬라이딩 코어의 피로파손 거동에 대해 SB Charit$\acute{e}$ III의 결과와 비교 평가하였다.
제지 공정의 효율적인 운영을 위한 수단으로 보류 및 탈수성의 분석은 매우 중요한 작 업 이 다. 보류 및 탈수 특성 을 평 가하기 위 하여 SR(Shopper-Riegler), CSF(Canadian S Standard Freeness tester) 그리고 원형 수초지기 등의 비난류 상태의 자연탈수 특성 평가 장치와 동적 조건의 감압식 탈수성 분석기 등이 사용되고 있다. 하지만 이것들은 제지 공정 에 보다 더 근접한 탈수 및 보류 특성을 평가하기에는 부족한 점이 있다. 특히 wet-end 첨 가제들의 효능을 보다 정확히 분석하는 것이 불가능하다. 이런 이유로 난류 발생과 감압 탈 수 둥을 유도한 탈수 특성 평가 장치 개발의 필요성이 커지고 있다. R RDA - HSF(Retention Drainage Analyzer- Handsheet Former)는 지료조성, 감압탈수 및 습지의 성형이 자동화되어 보류도와 탈수성, 종이지합 및 강도를 종합 평가 할 수 있는 새 로운 실험실용 초지기이다. 보다 현장에 가까운 초지 조건을 구현하기 위해서 지료의 농도 를 높이고 진공 탈수를 실시함에 따라 기존의 탈수성 분석기나 보류도 측정기의 단점을 보 완하게 되 었다. 본 연 구에 서 는 Canadian Standard Freeness test와 Dynamic Drainage Jar test 및 원 형 수초지기를 활용한 기존의 탈수성, 보류도 분석 결과와 RDA-HSF를 비교하여, 자연 탈수를 이용하는 기존의 탈수 및 보류 설비에 비해 현장 공정을 보다 현실적으로 재현할 수 있는 감압 탈수 초지 설비로서 RDA-HSF의 활용 가능성을 분석하였다. 과실을 이용한 증류주 개발에 있어서 국내에서 생산량이 많고 과잉생산으로 인한 잉여과실 발생빈도가 높은 사과와 감귤을 선정하였으며, 외국에서 증류주 제조에 많이 사용하고 국내에 많이 자생하는 마가목 열매를 원료로 선정하여 본 연구를 수행하였다. 각 원료의 발효특성에 따라 맞춤형 효모를 선정하였고, 선정된 효모를 이용하여 발효한 술덧을 동재질의 상압다단식 증류기와 스테인리스 재질의 상압단식 증류기 및 감압단식증류기 등으로 각각 증류한 증류주의 주요 향기성분과 증류효율 등을 비교 평가하였다. 또한 상기 증류주의 품질평가를 위해 4개월 숙성 후 관능평가를 실시하였다. 연구결과 사과와 감귤의 경우 원료 사용비율에 관계없이 발효가 원활히 진행되었으나 마가목 열매의 경우 원료사용비율이 10 w/v% 이상일 경우 발효가 되지 않거나 미미하였으며, 사용비율이 8 w/v% 이하일 때에는 원활한 발효가 진행되었다. 원료별, 증류방식별로 초류, 본류, 후류로 나누어 증류한 증류주의 증류효율을 검토한 결과 과실 원료 차이에 따른 증류주의 수율 차이는 적었지만, 증류효율 면에서는 상압다단식 증류기, 상압단식, 감압단식 순으로 높게 나타났다. 과실증류주의 주요 향기성분 분석결과 증류방식별 다소의 차이를 나타내었으나 모든 원료에서 동재질의 상압다단식증류 방식에 의해 제조된 시제품이 스테인리스 재질의 상압단식 및 감압단식방식에 의해 제조된 증류주에 비해 아로마가 풍부한 것으로 나타났다. 결론적으로 사용된 원료에 관계없이 증류수율, 작업능률 및 품질측면에서 동재질의 상압다단식증류방식이 스테인리스 재질의 단식 또는 감압증류방식보다는 증류주제조에 더 적합한 것으로 나타났다. 쌀을 이용한 증류주 제조에만 국한되어 있는 국내 증류주시장을 고려하면 과실을 이용한 유럽스타일의 증류주 제조, 특히 마가목 열매를 이용한 과실증류주 상품화 기술은 향후 FTA 시대를 맞아 시장 다변화 및 우리술 경쟁력 강화에 기여할 것으로 보인다.
목적: 원발성 및 전이성 척추종양의 치료는 해부학적 특성상 근치적 광범위 절제술이 용이하지 않다. 원발성 및 전이성 척추종양에 대해 다양한 수술을 시도하였으며, 그 중 전 척추절제술이 양호한 임상결과를 보여, 본원에서 원발성 및 전이성 척추종양으로 전 척추절제술을 받은 5례를 대상으로 임상적, 방사선학적 결과를 분석하였다. 대상 및 방법: 1997년 6월부터 2006년 1월까지 본원 정형외과에서 치료하였던 환자 중 전 척추절제술을 받고 추시 관찰이 가능했던 원발성 척추종양 1례와 전이성 척추종양 4례를 대상으로 하였다. 전이성 척추종양의 원발 병소는 신장암 2례, 유방암 1례, 원발 병소 불명의 선암 1례였다. 전 척추절제술의 임상적 평가 방법으로 동통과 신경학적 증상을 McAfee의 4 point scale과 VAS(Visual Analogue Scale), 및 Frankel 분류를 이용하여 관찰하였다. 술 후 3개월마다 반복적인 이학적 검사를 시행하였으며, 단순 방사선촬영으로 국소 재발 및 골유합, 합병증을 관찰하였다. 결과: 동통의 평가는 McAfee scale에서 술 전 평균 3에서 술 후 평균 1.6으로 감소하였고, VAS는 술 전 평균 9.2에서 1.6으로 감소하였다. 술 후 신경학적 결손은 Frankel 분류 C에서 D로 호전되었다. 추시기간 중 제 4요추의 전이성 선암 1례에서 국소 재발이 관찰되었다. 결론: 전 척추절제술은 척수 신경을 완전하게 감압시키고 골격통을 신속하게 감소시키며 생존 기간 동안 삶의 질을 높이는 유용한 술식이다.
Purpose: We evaluated the effect of nerve decompression for restoration of plantar sweating and sensation in diabetic neuropathic feet, and we selected diabetic neuropathic patients with the possibility of overlying entrapmental neuropathy. Materials and Methods: From June 2002 to May 2003, we have investigated and follow-up examed 10 patients with diabetic neuropathic feet, with decreased sensation in their lower limb, who underwent peripheral nerve decompression. The surgical procedure was multiple neurolysis of the common peroneal nerve, posterior tibial nerve and its three branches of one limb. We compared the operated limb with the opposite, unoperated limb. We performed history taking, physical examination, sweat secretion test, touch sensory test using Semmes-Weinstein monofilaments and electrodiagnostic study, pre-operatively and post-operatively. Results: On 6 months after the operation, the post-operative tests showed that there were noticeable improvements to sensation, statistically (P<0.05), but there was no change in the sweat secretion test. According to the Cseuz criteria, 7 patients out of the 10 patients who received the multiple neurolysis showed excellent or good results. Conclusion: We observed that the peripheral nerve neurolysis could be benefit for improving sensation and alleviating pain of the diabetic neuropathic feet with nerve entrapmental symptoms, but there was no change in the sweat secretion on short-term follow-up. To identify whether the effect will be continued or not, additional follow-up will be required.
Appropriate clinical examination and imaging may lead to early diagnosis and treatment of the shoulder impingement syndrome, thus preventing progressing to a complete tear of rotator cuff. The impingement syndrome was caused by repeated entrapment and compression of supraspinatus tendon between the proximal end of humerus inferiorly, particullary its greater tuberosity. and one or m <)re component of coracoacromial arch superiorly. The purpose of this study is to critically, evaluate the result of twenty-five consecutive subacromial decompression with impingement syndrome and to assess the diagnostic accuracy of MR imaging by using oblique coronal and oblique sagittal plan. These patients were treated by arthroscopic subacromial decompression after their pains failed to improve with conservative therapy over three month. The average follow up was 25 month(range, 12 to 50). The mean age was 43 year old. The results were rated based on subjective response and the UCLA shoulder rating scale of the result. Ten patients(40%) were rated as excellent, 11patients(44%) were good. while four patients(16%) were fair. Radiologic evaluation suggested that the oblique sagittal plan of MRI can be helpful in evaluation of bony and soft-tissue structure of the coracoacromial arch and determining depth of bony resection. There were no infection or neurovascular injury. In reviewing our result, it appears that the arthroscopic subacromial decompression can be successful sugery for shoulder impingement syndrome and diagnostic accuracy of supplimentary oblique sagittal view of MRI was relatively higher than oblique coronal view alone for apprqpriate surgical plan.
A cyst is a pathologic lesion characterized by a cavity filled with fluid, celluar products, air, or a combination of these. Dentigerous cysts were formed around the crown of unerupted teeth. The reduced enamel epithelium persists around the crown after it has formed. Proliferation of the epithelium in a fluid-filled sac may be induced by osmotic pressure. In the first decade the most frequent location is the premolar site. In each subsequent decade the largest number of cysts are in the mandibular third molar site, with the second most frequent site being the maxillary canine. The treatment of odontogenic cyst can be mostly classified into three types of cyst enucleation, marsupialization and decompression. We should consider age of patient, anatomic structure, location and size for choosing a treatment method. Advantage of cyst enucleation is fast healing, but a injury of a surrounding structure is highly. Marsupialization is conservative treatment that can reduce the damage of a adjacent structure, but it is only limited at superficial lesion. Decompression also is conservative treatment, but it has the difficulty of the oral hygiene and the troublesome of the lavage. We present the possibility that reduces the defect of decompression and cures the lesion efficiently. We report a male patient with the dentigerous cyst developed at left mandibular third molar in this study. We used the decompression for a treatment and created special appliance to treat the lesion efficiently. We report a case of the cyst treatment that is association with efficiency of decompression appliance.
Objectives : Many surgical procedures have been introduced to a symptomatic lumbar stenosis. Most of these procedures still have been regarded as an extensive surgical intervention with respect to normal aging process of the lumbar spine. We adopted a microsurgical decompression procedure via unilateral exposure as a minimally invasive intervention for symptomatic lumbar stenosis without instability. Materials and Methods : Fifty-seven patients with symptomatic lumbar stenosis underwent microsurgical decompression via unilateral laminotomy between March 1998 and December 1999. The conceptual modification and technical refinements were added to the previously reported microsurgical decompression procedure. Bilateral decompression through a unilateral laminotomy hole was performed in 11 patients. These patients profile also included 9 cases of degenerative spondylolisthesis(Grade I) without instability. Results : Preoperative neurogenic intermittent claudication(NIC) was more notably improved than low back pain, 60% to 82% during the follow-up period. Overall clinical results were excellent in 20(35%), good in 29(51%), fair in 6(11%) and poor in 2(3%). Conclusions : Microsurgical decompression for lumbar stenosis with stable spine provided a satisfactory symptomatic improvement without extensive destruction of the weight-bearing structures and functional mobile segments, even bilateral symptoms existed.
Background: Radicular pain, associated with herniated intervertebral disc, has been treated with either conservative treatments or a traditional surgical discectomy. Several modalities for minimally invasive percutaneous procedures have been developed as alternatives to a surgical discectomy. Percutaneous decompression using a $Dekompressor^{(R)}$ probe has been recently introduced. Herein, we report the outcome of percutaneous decompression with a $Dekompressor^{(R)}$ for the treatment of a herniated intervertebral disc. Methods: Between August 2004 and April 2005, percutaneous decompression was performed using a $Dekompressor^{(R)}$, 1.5-mm percutaneous lumbar discectomy probe in 17 patients with a herniated lumbar intervertebral disc resistant to conservative treatments, with the results reviewed retrospectively. The procedure was performed under fluoroscopic guidance after local anesthesia. Disc access was gained with a posterolateral approach on the symptomatic side and intradiscal placement of the discectomy probe in the herniated disc confirmed from the anteroposterior and lateral views on the fluoroscopy. Results: We obtained satisfactory clinical results in 14 patients with a decrease in the initial Visual analogue scale (VAS) of more than 55% and the elimination or reduction of analgesic medication, with a follow-up of 3 to 11 months. Conclusion: We concluded that a percutaneous discectomy with a $Dekompressor^{(R)}$ probe might be an effective alternative for the treatments of painful disc herniations resistant to conservative managements when performed under proper selection criteria.
Background: Discogenic leg pain is a major cause of health problems, often due to herniation of the intervertebral disc, and has traditionally been treated conservatively or with an open surgical discectomy. Conventional open surgery has many complications, such as nerve root injury, discitis and a relatively high mortality rate; failure of conservative treatments is also common. Recently, the $Dekompressor^{(R)}$ Percutaneous Lumbar Discectomy probe was developed. Herein, we present the early results for a percutaneous lumbar discectomy in herniated lumbar disc disease. Methods: Eleven patients, including 8 men and 3 women, with ages ranging from 22 to 78 years, were enrolled in this study. Those patients with a previous history of back surgery were not excluded from the study. All patients were postoperatively evaluated for their clinical outcomes, such as visual analogue scale (VAS) for pain after 1 and 3 months, reduction in analgesics, functional improvement and overall satisfaction. Results: The percutaneous lumbar discectomy was completed in 11 patients (17 levels), with average reductions in pre-VAS of 61.3 and 60.2% at 1 and 3 months, respectively. Also, 72.7% of patients reported functional improvement, with 81.1% expressing overall satisfaction. There were no procedural related complications. Conclusions: We concluded that a percutaneous lumbar discectomy is a safe and effective treatment modality for a herniated lumbar disc.

회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
