A Relationship between Burden of Clinical Nursing Instructor and Teaching Effectiveness (임상실습 지도자의 부담감과 교수 효율성과의 관계)

  • The purpose of this study was to identify the correlation between burden and teaching effectiveness on clinical nursing practice. The subjects were collected 135 clinical nurses who have taught nursing students and worked at hospitals which have over 400 beds in Seoul, Inchon, and Kyoung-gi Do. The instruments used in this study were : the burden on clinical nursing education was measured by Montgomery (1985) developed and visual analogue scale, and effectiveness of clinical instruction was measured by Reeve (1994) developed. The results of this study were as follows. 1) Seeing that the general characteristics of participants : average ages we 32.8 years old, 29.6% of them have Catholics in religions. 75.6% of them were graduated from 3-year nursing college. 20.7% of nurses are working at the surgery ward and I.C.U in each. Clinical nursing career is average 10.03 years, clinical instructional career is 5.22 years, and clinical teaching time is 5.26 hours in a day. Contents of clinical teaching were composed of basic nursing skills 80.7%, orientation 78.5%, inspection(making rounds) 71.9%. 2) The mean score of the burden on clinical nursing instructor was 2.42 by Montgomery's scale and 4.69 by the visual analogue scale. Theses scores represented that subjects were not affected burden highly. The mean score of leaching effectiveness on clinical nursing education was 3.47 and the almost items were found to have higher level. 3) There is no statistically significant differences in the burden according to general characteristics. And the teaching effective ness on clinical nursing education according to general characteristics regarding the age, job position, clinical career and clinical educational time were shown statistically significant differences. 4) There is a negative correlation between the burden and teachin effectiveness on clinical nursing education with a correlation efficient(r=-0.396, p<0.01). Further study is recommended to explore the meaning of burden experiences of clinical instructor deeply and to identify the correlation between the burden of clinical instructor and teaching effectiveness as the job position, and to analysis differences in teaching effectiveness as subcategories.

A Study on the Content of Fundamentals of Nursing in Korea -Focused on Nursing Professors and Clinical Educators- (기본간호학 교과내용 개선을 위한 일 조사연구-간호학 교수와 임상실무교육 간호사를 중심으로-)

  • This study analyzed the necessity of detailed items of fundamentals of nursing education in college by soliciting answers from clinical nursing educators in hospitals and nursing professors. The questionnaire consisted of four parts. The first part included questions about the general demographic characteristics of respondents, the second part. questions about the general necessity of fundamentals of nursing, the third part, questions about the necessity of 26 items in fundamentals of nursing in knowledge education and about the necessity of 81 items in fundamentals of nursing in practice education, the fourth part solicited free description of ideas about the problems education in fundamentals of nursing in Korea. Thirty professors from universities, 30 professors from community. and 30 clinical nursing educators were randomly sampled and the questionnaires were sent by mail. Seventy people returned completed questionnaires. Ninety three percent evaluated fundamentals of nursing as necessary for nursing not only in knowledge education but also in practice education. They also generally agreed on what items are necessary for fundamentals of nursing. However. opinions about some items were split between nursing scholars and clinical nursing educators. Clinical educators wanted fundamentals of nursing to be more practical and to incorporate recent developments and changes in clinical settings. They described several problems in fundamentals of nursing. One was that some the content of fundamentals of nursing overlaps with other subjects, especially with adult nursing. Some respondents also thought that fundamentals of nursing included too many topics. These problems make it difficult for students, scholars, and nurses identify what fundamentals of nursing is. This causes an identity problem in fundamentals of nursing. Some disparity between clinical nursing and knowledge education in fundamentals of nursing was also reported. This was also related to problems lack of clinical experience in teachers of fundamentals of nursing. Some respondents suggested requiring clinical experience for professors or establishing a system of clinical professorship. Problems of teaching material were also pointed out. Fundamental nursing skills and knowledge in teaching materials are often old or not appropriate for the Korean nursing situation. The respondents urged the development of teaching materials appropriate for Korean nursing. In order to solve these problems, the authors suggest forming an ad hoc committee which can reformulate and standardize education in fundamentals of nursing in Korea.

Mixed Method Research Investigating Evidence-Based Practice Self-efficacy, Course Needs, Barriers, and Facilitators: From the Academic Faculty and Clinical Nurse Preceptors (근거기반실무(Evidence-based Practice) 자기효능감, 교과목 요구도, 장애요인 및 촉진요인 탐색을 위한 혼합연구: 교수 및 임상실습 프리셉터를 중심으로)

  • Purpose: The current challenges faced by nurses in providing high quality and evidence-based practice (EBP) supported care require profound changes in nursing education. To understand the changes needed to strengthen EBP education, the researchers examined EBP self-efficacy, course needs, barriers, and facilitators for academic faculty and clinical nurse preceptors to teach EBP in undergraduate nursing curricula. Methods: For this study, mixed-method approach was used with survey data collected from 73 academic faculty members from 54 universities. Further, 17 clinical nurse preceptors in three academic hospitals provided qualitative data for exploration of barriers and facilitators to teaching EBP. Data analysis used SPSS/WIN 21.0 and content analysis. Results: Quantitative data showed that although the overall level of self-efficacy among faculty was moderate, the implementation levels were relatively low. Most faculty members agreed with the need to integrate EBP courses into undergraduate nursing curricula. The qualitative data showed that the barriers to teaching EBP were lack of knowledge, skill, and initial investment for teaching EBP; hierarchical, rules-oriented nursing culture; potential learner overloads in processing EBP; limited research dissemination and application. Facilitators were identified as the importance of EBP to the profession of nursing; collaboration in schools and hospitals; and continuing education in teaching/utilizing EBP. Conclusion: The findings indicate that for successful integration of EBP ni nursing education there is a need for faculty training and integrated EBP courses.

The Status of Blood and Body Fluid Exposure and Affecting Factors among Nursing Students including Knowledge, Performance regarding Standard Precautions (간호대학생의 임상실습중 혈액 및 체액 노출현황과 영향 요인 - 감염관리 지식 및 수행도 중심으로-)

  • Objectives: The purpose of this study was to investigate the status of exposure to blood and body fluid and factors affecting on blood and body fluid exposure of nursing students including knowledge, performance regarding standard precautions. Methods: 280 nursing students in D-city and S-city participated in this study. Data collection was conducted from January to March 2012. Students' knowledge and performance on standard precautions of infection control in hospitals were measured using the measurement developed by Hong(2010). Data analysis was performed using descriptive statistics, t-test, one way-ANOVA, ${\chi}^2-test$, Pearson correlation coeficient, logistic regression by the SPSS WIN 18.0 program. Results: 58.6 percent of subjects experienced a exposure to blood and body fluid. The level of nursing students' knowledge level regarding standard precautions was $22.44{\pm}2.11$ and performance level was $4.31{\pm}0.50$. The factors affecting on blood and body fluid exposure of nursing students were knowledge level and education type about infection control. Conclusions: Based on the findings of this study, to decrease the exposure to blood and body fluid of nursing students, a proper education need to be stressed to improve nursing students' infection control knowledge and awareness.

The Effectiveness of Assertive Training on the State Anxiety and Assertive Behavior of Nursing Students Experiencing Clinical Practice Training (주장훈련이 임상실습시 간호학생이 느끼는 불안과 주장행동에 미치는 효과)

  • This study was tried to investigate the effectiveness of assertive training on the state anxiety and assertive behavior which nursing students had experienced during clinical practice training from April 29, to June 5, 2000. Subjects for this study were thirty-six nursing students of practicing in a general hospital. Eighteen nursing students were assigned for the experimental group and trained by assertive training program during 4 weeks and eighteen nursing students were assigned for the control group. The assertive training program was composed of cognitive, behavioral and emotional assertive training. Post- test assessment was administrated to all subjects on a week later after the last session. The data were analyzed with SPSS PC programs. The results of this study were summarized as follows: 1) There were no significant differences between the experimental group and the control group on post- intervention comparisons of the state anxiety score (p=0.230), but in the experimental group, the anxiety score more decreased after the assertive training than that in the control group with significant differences(p=0.019). 2) There were no significant differences between the experimental group and the control group on post- intervention comparisons of assertive behavior (p=0.530). However, after the assertive training, the experimental group became more assertive than the control group with no significant differences(p=0.721). 3) The hypothesis that the more the assertive behavior, the less the state anxiety experienced by nursing students of clinical practice training was supported(r=-0.397, p=0.017). On the basis of this study results, the assertive training is effective for the reduction of the state anxiety experienced by nursing students of clinical practice training.

Current Status of Women's Health Nursing Practicum and Direction (여성건강간호학 실습교육의 현황과 과제)

  • Purpose: This study was done to examine current status of women's health nursing practicum and identify necessary core nursing skills in this practicum area. Moreover, one syllabus and evaluation sheets for women's health nursing clinical practicum at one university were reviewed. Methods: A survey design was used with 81 educators who were teaching maternity or women's health nursing and its practicum. Results: Most clinical sites for practicum were university hospitals (43.0%), women's hospitals (32.7%), or general hospitals (17.3%); but the majority (77.8%) of educators expressed difficulty in finding appropriate practicum places. Common teaching and learning methods were clinical guides for practicum (44.6%), e-learning content (30.2%), and simulation (23.6%). Core nursing skills for this practicum included assessment of stages of labor, preparation of uterine-fetal monitoring devices and interpretation of results, monitoring uterus and fetal activity, and performing Leopold's maneuver. For postpartum care, the following were included; postpartum fundal massage, assessment of breast engorgement, fundus height, and episiotomy sites, inserting urinary catheter, and teaching the use of patient-controlled analgesia. Conclusion: To improve the quality of clinical practicum, development of a clear course syllabus, standardized clinical guidebook, and core nursing skills is required and should be shared with all relevant nurse educators.

The Effect of Ego-resilience, Stress Coping Styles, Teaching, Effectiveness, and Family Support on Satisfaction of Clinical Practice in Nursing Students by AMOS Structural Equation Model (간호대학생의 자아탄력성, 스트레스 대처방식, 교수효율성, 가족지지가 임상실습만족도에 미치는 효과: 구조방정식 모형구축)

  • Purpose: This study aimedto develop a structural equation model on the relationship among ego-resilience, and teaching effectiveness on clinical education, stress coping style, perceived family support, and satisfaction of clinical practice in nursing students in order to increase the satisfaction of clinical practice. Methods: The subjects were 399 undergraduate nursing students enrolled in 7 universities and participating in 10 clinical practice setting areas in 5 cities in South Korea. The research was conducted from December 5th, 2009 to February 20th, 2010. The structural equation model was used to perform the analysis with the statistics program of SPSS win 17.0 and AMOS 5.0. Results: Teaching effectiveness and family support showed a direct effect on satisfaction of clinical practice, while ego-resilience and stress coping styles showed an indirect effect. The biggest total effect on satisfaction of clinical practice was teaching effectiveness followed by family support, ego-resilience, and stress coping styles respectively, which accounted for 50.9%. Conclusion: Based on outcomes of this study, the proposed model allows better understanding of the satisfaction of clinical practice. This result implies that strategies or intervention programs enhancing extrinsic protection factors, motivation factors, and intrinsic protection factors should be considered. In addition, a structural support system for increasing the satisfaction level of clinical practice in nursing students should also be considered.

Needs of Training Programs for Practice Courses in Advanced Practice Nurse Education (전문간호사를 위한 임상 전문실습교육프로그램 요구)

  • Purpose: This study aimed to assess needs of training programs for APN practice courses and continuing education programs for APNs to provide baseline data to improve APN education and practice. Methods: A total of 98 questionnaires were distributed to professors in 38 nursing institutions and 64 (65.3%) were returned. Data collection was performed in November and December, 2007 through email. Results: Professors in charge of geriatric and home care nurse practitioner programs constituted 43.7% of the study participants. Forty-one percent answered that they were satisfied with the practice programs they currently have operated, while 36.5% responded to have difficulty in appointment of institutions for practice programs and 56.3% had problems in assignment of preceptorship. In case of developing training programs for APN practice courses, 58.7% answered that they are willing to participate, and 90.6% felt needs for development of continuing education programs for APNs. Regarding liability insurance for APN students, only 11.5% provided insurance policy. Conclusion: The study results indicated that standardized training programs are needed to assure quality of APN education, and continuing education programs are required to provide opportunities for APNs to improve competencies.

The Impact of Nursing Students' Learning Satisfaction on Motivation to Transfer in the Practicum of Psychiatric Nursing Convergence Simulation Using Standardized Patients: Mediating Effect of Self-Efficacy in learning (표준화환자 활용 정신간호학 융합시뮬레이션 실습에 대한 간호학생의 학습만족도가 전이동기에 미치는 영향: 학습자기효능감의 매개효과)

  • The study was to examine the mediating effect of self-efficacy in learning in the relationship between the learning satisfaction and motivation to transfer of nursing students who received the psychiatric nursing convergence simulation practicum using standardized patients. Participants were 144 third grade nursing students. Data were analyzed descriptive statistics, t-test, one-way ANOVA, Pearson's correlation coefficient analysis, and multiple regression following the Baron and Kenny's method and Sobel test for mediation. There were significant correlations between learning satisfaction and self-efficacy in learning(r=.686, p<.001), learning satisfaction and motivation to transfer(r=.633, p<.001) and self-efficacy in learning and motivation to transfer(r=.804, p<.001). Self-efficacy in learning showed partial mediating effects in the relationship between learning satisfaction and motivation to transfer(Z=7.63, p<.001). To increase the motivation to transfer, strategies to enhance the self-efficacy of nursing students are required.

Effects of a Standardized Patient Simulation Program for Nursing Students on Nursing Competence, Communication Skill, Self-efficacy and Critical Thinking Ability for Blood Transfusion (표준화환자 시뮬레이션 실습교육이 간호학생의 수혈 간호수행능력, 의사소통능력, 자기효능감과 비판적 사고능력에 미치는 효과)

  • Purpose: This study was conducted to identify the effects of a standardized patients (SP) simulation program for nursing students on nursing competence, communication skill, self-efficacy and critical thinking ability for blood transfusion. Methods: A nonequivalent control group non-synchronized design study was used and included as participants 96 junior nursing students at C University. The SP group (n=48) participated in the teaching class using a SP, while the control group (n=48) received conventional practice education. The outcome measurements were nursing competence, communication skill, self-efficacy, and critical thinking ability for transfusion. Results: Nursing competence, communication skill, self-efficacy, and critical thinking ability improved for students in both groups after training ($2.01{\leq}t{\leq}13.03$, p<.05). Self-efficacy showed greater improvement in students in the SP group compared to the control group (t=3.36, p<.001). Conclusion: SP simulation practice may be more effective in enhancing self-efficacy than that of conventional practice education. Whether self-efficacy will contribute to enhancing learning motivation for nursing students needs further examination.
