Monitoring of Methicillin Resistant Staphylococcus aureus from Medical Environment in Korea. (국내 의료 환경 중의 Methicillin 내성 Staphylococcus aureus의 모니터링에 관한 연구)

  • Methicillin-resistant Staphylococcus aureus (MRSA) is one of a major nosocomial pathogen worldwide and the emergence of this strain has become a major clinical problem. This study was performed for 13 hospitals with more than 400 beds in the country by collecting samples including hands and nasal cavities of doctors, nurses, guardians and patients. Also, additional 320 samples of hands and nasal cavities of 160 community resident in different locations and regions were collected. In all of medical environments and community resident, 625 strains of S. aureus were detected. Among 625 strains of S. aureus, 585 strains(93.6%) showed the resistance to at least one kind of antimicrobial and 112 strains (17.9%) showed multi-drug resistance with the resistance to 4 different types of antimicrobial. Total 152 MRSA strains (24.3%) were isolated from medical environment and community resident. In nasal cavity and hand, 49 MRSA (19.4%) and 103 (27.6%) MRSA were isolated, respectively Minimum inhibitory concentration(MIC) test is used to measure for susceptibility of MRSA isolated to oxacillin. At a concentration $16{\mu}g/ml$ of oxacillin, 11 strains were inhibited. 32 strains at $32{\mu}g/ml$, 41 strains at $64{\mu}g/ml$, 3 strains at $128{\mu}g/ml$, 25 stains at $256{\mu}g/ml$ and 40 strains at over $256{\mu}g/ml$ were inhibited. It was considered that medical environment showed higher than livestock and marine environments in MRSA detection rate.

A Study on Type of Clinical Experience of Adaptation of New Nursing Staffs in a General Hospital (일 종합병원 신규간호사의 실무적응 경험 유형)

  • This study designed to know subjective structure about clinical experience of adaptation of new nursing staffs in a general hospital and using Q-methodology. Using Q-methodology, 40 new nursing staffs sorted working a general hospital through personal interview. The statement positions on the sorting continuum were scored from +4(representing the highest level of agreement) through 0(at the neutral position on the continuum) through to -4(representing the highest level of disagreement). Factor analysis of the data was undertaken using PC QUANL program to determine if any patterns were discernible. The five types extracted all had eigenvalues greater than 1.0 and explained 62.38% of the variation in responses.: (1) New workload type, (2) Interest and responsibility type, (3) Improve the situation type, (4) Lack of knowledge and technology type, and (5) Excessive workload type. The significant differences found in the subjective structure of clinical experience of adaptation of new nursing staffs in ad general hospital. The findings of this study could contribute to the development of an effective programmes of education and personnel clinical practice strategies.

Study of Subjective View on the Meaning of Well-dying Held by Medical Practitioners and Nursing Students: Based on Q-Methodology (의료인과 간호학생의 웰다잉 의미에 관한 주관성 연구)

  • Purpose: This study was conducted to examine understanding of the meaning of well-dying and types of such views held by medical practitioners and nursing students. Methods: The Q-methodology was used to analyze the subjectivity of each item. The P-sample was made up of 22 medical practitioners or nursing students. The P-sample was instructed to rate 33 statements using a 7-point scale to obtain forced normal distribution. They were asked to make extra comments on the statements that were placed on both ends of the distribution curve. The PC-QUANL Program was used for the factor analysis of the collected data. Results: The participants had three types of meaning of well-dying. Total variance explained by these types was 57.97%" where type 1 was "reality-oriented", type 2 "relationship-oriented" and type 3 "obeying-the-nature". Conclusion: The participants' subjective views on well-dying influences their medical practice on patients who are facing death. Therefore, medical practitioners should have profound insights concerning life and death. To that end, a training program is needed to help medical practitioners develop a proper view on well-dying by subjectivity type.

PULMONARY EDEMA INDUCED BY BOSMINE SOAKED GAUZE IN ORTHOGNATHIC SURGER (악교정 수술시 보스민 거즈에 의해 나타난 폐부종 증례보고)

  • There are many vessels in oral and maxillofacial region. Therefore blood oozing is a frequent findings in this area when operation takes place. Beacuse of this, most oral and maxillofacial surgeons as well as operators of this region usually use bosmine soaked gauze for oozing control during operation. Theoretically overdose of systemic epiniephrine(bosmine) may lead to pulmonary edema due to secondary systemic volume overload. For this reason, oral and maxillofacial surgeons should give more attention to the use of bosmine soaked gauze. In spite of this possibility, epinephrine induced pulmonary edema using bosmine soaked gauze has not been reported. We experienced one case of pulmonary edema which was induced by epinephrine overdose by bosmine soaked gauze that has not been diluted by mistake in preparing bosmine soaked gauze. The authuors report a case with review of literature.

Geographic Mobility and Related Factors among Newly Graduated Nurses (신입간호사의 지역간 이동 양상과 영향요인)

  • Purpose: This study aimed to analyze the mobility of newly graduated nurses from regions where their nursing schools were located to regions where they took up their first jobs, and to identify factors influencing nurses' mobility. Methods: Data from the Graduates Occupational Mobility Survey, collected annually from 2010 to 2014 by the Korea Employment Information Service, were analyzed. The sample consisted of 1,488 graduates and 1,229 nurses who were employed on a full-time basis in hospitals. Multiple logistic regression analysis was conducted to identify factors associated with geographic mobility. Results: Among the nurses working in hospitals, 69.2% had their first jobs in their nursing school regions and 11.3% in their high school regions. Fifty-two percent of the nurses worked in the capital region; 47.2% thereof had moved from a non-capital region. Nurses were more likely to work in their nursing school region when they were female, were older, graduated from a high school located in their nursing school region, graduated from a college (vs. university), had a lower nursing school performance, and expected lower monthly wage, compared with those who left their nursing school region. Conclusion: Education and remuneration policies are required to reduce geographical mobility to the capital region.

Factors Associated with Body Mass Index(BMI) Among Older Adults: A Comparison Study of the U.S., Japan, and Korea (노인의 체질량지수에 관련된 요인 연구: 미국, 일본, 한국 비교를 중심으로)

  • This study examined BMI distributions among older adults in three different countries: the U.S., Japan, and Korea. The paper also explored differences in the factors predicting BMI in the three countries using three data sets: the U.S. Longitudinal Study of Aging (LSOA II, 8,589 persons), the Nihon University Japanese Longitudinal Study of Aging (NUJLSOA, 2,888 persons), and the Korean Longitudinal Study of Ageing (KLoSA, 2,397 persons). Descriptive analysis and multiple regression were performed. Japanese older adults were somewhat lighter than Koreans with fewer people at the upper end of the BMI distribution. Distributions of BMI among both Koreans and Japanese are shifted leftward relative to Americans. There is less dispersion in the distribution of BMI for Koreans and Japanese than among Americans. The association between socioeconomic variables and BMI is stronger in the U.S. and Japan than in Korea. Demographic variables are strong predictors of BMI in Korea. In Japan, all health behaviors have significant effects on BMI. It is concluded that the relationships between behavioral, demographical, and socioeconomic factors and BMI are not the same across countries. Results have policy implications for the involvement of health practitioners in helping older adults to control weight.

  • The purpose of this study was to investigate the Manpower status of nurse in the Health Centers from 1985 year to 1986 year, Pusu and Kyung Nam area. As the results through the analysis of collected data, the following conclusion has been reached. 1. The number of nurses who worked in the Family Planning in Pusan was largest and the number of nurses in Kyung Nam who worked for Consultation Clinic was the largest both in 1985 and in 1986. 2. Population per Health Center by year; Population per Health Center in Pusan area increased from 351,681 persons 1985 year to 357,884 persons, 1986 year In Kyung Nam area population per Health Center increased from 130,247 persons, 1985 to 130,252 persons,1986 year. 3. Population per Public Health Nurses by year; Population per PHN in Pusan area increased from 30,058 persons, 1985 to 31,120 persons, 1986. But in Kyung Nam area population per PHN decreased from 22,500 persons, 1985 to 16,747 persons, 1986. 4. Activities of Public Health Nurses by clinic; The order of priority activities by PHNs was family Planning in Pusan area and M.C.H in Kyung Nam area. 5. The target population per Public Health Nurse; The target population of M.C.H was the most in Pusan, 1985. In Kyung Nam area the most target population was M.C.H., 1985 year. 6. Number of service per Public Health Nurse; In number of service per PHN, the hightest health nursing service activity in Pusan and was M.C.H in Kyung Nam area.

A Study on the difference in the sharpness of venous images between individual algorithms and combinations (개별 알고리즘과 조합 간 정맥 영상의 선명화 차이에 관한 연구)

  • Intravenous infusion therapy is a standard nursing procedure in medical institutions that provides patients with drugs, fluids, blood, and nutrients into the patient's mucus. It is mainly performed and managed by nurses. Additionally, it is an injection method that injects drugs directly into the blood vessels, and is used to achieve rapid results in emergency situations, and quick and accurate effects can be expected. Even experienced nurses through education and training often make mistakes, which can not only cause discomfort to patients but also cause various problems that threaten patient safety. Various studies are being conducted to reduce the pain caused by these mistakes. This paper acquired images of veins on the back of the hands of three subjects through an image detection device and conducted a study to derive an algorithm to provide clear vein images through image processing of the collected images. To sharpen the acquired vein images, existing algorithms Histogram Equalization, CLAHE, and Unsharp Masking were selected and combined. A histogram graph was used to compare images derived by applying individual algorithms and algorithm combinations to images. The histogram graph was checked by calculating the difference between the minimum and maximum values of distributed pixels and averaging them. The algorithm combination presented in this paper was 209.1, which was higher than the average values of individual algorithms of 138.7, 132.3, and 126.2, and it was confirmed that visibility was good even in actual images.

Types of Perception toward End-of-Life Medical Decision-making of Clinical Nurses: Q-Methodological Approach (말기환자의 의료적 의사결정에 관한 임상간호사의 인식: Q 방법론적 접근)

  • Purpose: We analyzed how clinical nurses in Korea perceive terminally ill patients' medical decision-making. Methods: The Q-methodology which analyzes the subjectivity of each item was used. We selected 34 Q-statements among those provided by each of 37 subjects and grouped them into a shape of normal distribution using a 9 point scale. The collected data were analyzed using a QUANL PC program. Results: Four types of perception toward medical decision-making were identified. Type I focuses on patient participation, and Type II emphasizes the role of health professionals. Type III is characterized by an open-minded culture toward death, and Type IV values the role of family members. Conclusion: The results of this study indicate the need for development of a multi-disciplinary curriculum medical decision-making and death for medical and nursing students.

Factors Influencing Type D Personality of Female Undergraduate Students Majoring in Nursing (간호학전공 여대생의 D유형 성격에 영향을 미치는 요인)

  • The aim of this study was to clarify the variables influencing the Type D personality of female university students majoring in nursing. The subjects were 353 female students attending a nursing college, and multiple regression analysis was performed using the PASW 18.0 program. The distribution of Type D personality in the subjects was 34.8%. The factors influencing the Type D personality included the support of family and friends, satisfaction with their major, self-esteem, life stress, and resilience. The factor with the greatest influence among the variables was life stress, and the explanatory power of this model was 36.3%. Intervention programs should be developed to reduce the level of Type D personality among nursing college students to provide them with the appropriate expertise as prospective nurses.
