간호대학 임상실습 교육자의 역할 경험 (Lived Experience of Clinical Nursing Instructors' Role in Clinical Practicum)

  • Purpose: The purpose of this study was to explore the role of clinical nursing instructors' lived experience in clinical practicum. Methods: Data were collected from 11 clinical nursing instructors by in-depth interviews. The data were analyzed using content analysis of Downe-Wamboldt (1992). Results: Four themes and twelve subthemes were extracted. 1) Recognizing and conducting the roles of clinical nursing instructor: 'Helping and providing support for successful clinical practicum', 'Coordinating clinical activities in daily practice', 'Providing mentoring as an elder in life'; 2) Participating in improving integrative nursing competency: 'Helping to improve cognitive competency', 'Helping to improve functional competency', 'Contributing to form desirable professionalism', 'Helping to deliberate the nature of nursing'; 3) Experiencing difficulties in performing the role of clinical nursing instructor: 'Facing with difficulty from institutional limits', 'Recognizing difficulty from lack of personal knowledge and experience'; 4) Experiencing value of clinical nursing instructor and accomplishing personal growth: 'Making efforts to widen personal knowledge and experience', 'Developing one's own educational competency', 'Making a chance to reflect oneself'. Conclusion: Despite the distinctive features of clinical nursing instructors, little is known of the characteristics. The results of this study could be used as a reference to improve the quality of clinical nursing education.

종합병원 간호사의 간호역량, 전문성 활동에 따른 임상경력관리체계 수정모형 (Revised Clinical Ladder System Model Based on Nurses' Clinical Competence and Professional Activities for Nurses in General Hospitals)

  • Purpose: The purpose of this study was to propose a revised Clinical Ladder System(CLS) Model for nurses based on the evaluation of clinical competence and professional activities of nurses working in general hospitals. Methods: Data were collected between September 10 and October 30, 2017. Participants were 50 head nurses from 10 general hospitals with over 400 beds located in Seoul City and Gyeonggi Province. Each head nurse evaluated clinical competence, qualifications, and professional activities of 5 staff nurses at each of the 5 levels of CLS in her unit. The total number of the nurses evaluated was 245. Data were analyzed with descriptive statistics and t-test, one-way ANOVA, and $Scheff{\acute{e}}$. Results: Over 80% of the nurses were university graduates. As the CLS levels increased, clinical competence, qualifications, and professional activities also increased significantly. Education material development and quality improvement activities were carried out by nurses from level 2, research and evidence based practice activities were carried out from level 3, and nurses at level 4 or 5 participated in most of the professional activities as leaders. Conclusion: In order to retain excellent nurses in general hospitals, recognizing and rewarding nurses according to the revised model of the CLS are recommended.

말초 정맥주사 삽입 어려움 예측을 위한 노모그램 구축 (Construction of a Nomogram for Predicting Difficulty in Peripheral Intravenous Cannulation)

  • Purpose: The purpose of this study was to construct a nomogram for predicting difficulty in peripheral intravenous cannulation (DPIVC) for adult inpatients. Methods: This study conducted a secondary analysis of data from the intravenous cannulation cohort by intravenous specialist nurses at a tertiary hospital in Seoul. Overall, 504 patients were included; of these, 166 (32.9%) patients with failed cannulation in the first intravenous cannulation attempt were included in the case group, while the remaining 338 patients were included in the control group. The nomogram was built with the identified risk factors using a multiple logistic regression analysis. The model performance was analyzed using the Hosmer-Lemeshow test, area under the curve (AUC), and calibration plot. Results: Five factors, including vein diameter, vein visibility, chronic kidney disease, diabetes, and chemotherapy, were risk factors of DPIVC. The nomogram showed good discrimination with an AUC of 0.81 (95% confidence interval: 0.80-0.82) by the sample data and 0.79 (95% confidence interval: 0.74-0.84) by bootstrapping validation. The Hosmer-Lemeshow goodness-of-fit test showed a p-value of 0.694, and the calibration curve of the nomogram showed high coherence between the predicted and actual probabilities of DPIVC. Conclusion: This nomogram can be used in clinical practice by nurses to predict DPIVC probability. Future studies are required, including those on factors possibly affecting intravenous cannulation.

근거기반 중심정맥 주입요법 간호실무지침 개정 (Updates of Evidence-Based Nursing Practice Guidelines for Central Venous Infusion Therapy)

  • Purpose: This study was conducted to update nursing practice guidelines for intravenous infusion published in 2017. Methods: The guideline update process was carried out using 22 steps developed by NICE and SIGN. It was agreed to update domains related to central venous infusion therapy. Contents related to peripheral infusion would be updated later. Results: Updated guidelines for central venous infusion therapy consisted of 6 domains and 195 recommendations. The number of recommendations by domain was 11 for general instruction, 14 for central vascular access devices (CVAD) and add-on devices, 13 for nursing management before insertion of CVAD, 30 for management during insertion of CVAD, 51 for management after insertion of CVAD, and 76 for complications. A grade was 29 (14.9%), B grade was 87 (44.6%), and C grade was 79 (40.5%) in the strength of recommendations. A total of 37 (19.0%) recommendations were newly developed and 23 (12.3%) previous recommendations have been modified. The newly developed recommendations were mainly related to the infection control methods. Conclusion: The updated guideline is focused on safe maintenance of central venous infusion therapy. Through this guideline, it is hoped to minimize the occurrence of complications and improve the standardization and efficiency of nursing practice.

간호대학생의 재난간호역량에 미치는 영향요인 (Factors Influencing the Disaster Nursing Competency of Nursing College Students)

  • 본 연구는 간호대학생을 대상으로 재난에 대한 인식, 태도 및 재난준비도, 간호전문직관, 사회적 지지, 재난간호역량을 파악하고, 재난간호역량의 예측요인을 확인하여 재난간호역량 향상 방안을 마련하기 위한 서술적 조사연구이다. 자료수집은 2023년 3월 27일부터 4월 7일까지 G도 간호대학생 160명을 대상으로 수집하였으며, 총 158부를 최종 분석하였다. SPSS Win. 25.0 프로그램을 사용하여 기술통계, t-test, one-way ANOVA, Pearson correlation coefficient, hierarchial multiple regression으로 분석하였다. 연구결과, 4학년(β=.20, p=.027), 재난간호교육이 '있음'(β=.15, p=.041), 재난준비도(β=.27, p<.001), 간호전문직관(β=.21, p=.012)이 재난간호역량을 설명하는 유의한 예측요인으로 나타났다. 재난준비도가 재난간호역량을 가장 잘 예측하는 요인으로 나타났고, 그 다음 '간호전문직관, '4학년, 재난간호교육이 '있음'인 경우 순이었고, 설명력은 26.3%이었다. 본 연구결과를 통해 간호대학생의 재난간호역량을 높이기 위해서는 재난에 대한 인식, 재난에 대한 태도, 재난준비도, 간호전문직관, 사회적 지지를 강화시킬 수 있는 재난 간호 실습의 통합교육 프로그램 개발이 필요할 것으로 생각된다.

대학원 기본간호학 전공 교육과정 개발을 위한 연구 (A Study on Curriculum Development for Fundamentals of Nursing at the Graduate Level)

  • Purpose: This study was conducted to develop a curriculum model for fundamentals of nursing at the graduate level which reflects a consensus of educators and the students in Korean nursing society. Method: A survey was used with three groups to identify consensus on a curriculum model for fundamentals of nursing at the graduate level; 42 educators in fundamentals of nursing, 11 nurse educators whose major was not fundamentals of nursing and 70 nurses with a bachelor degree in nursing. Results: Consensus on a curriculum model at the graduate level were delineated from analysis of the survey, Based on these results, a curriculum model fur fundamentals of nursing at the graduate level is outlined: As one major area of nursing, fundamentals of nursing focuses on nursing practice based on basic human needs. Thus main focus of the curriculum for fundamentals of nursing at the graduate level is research performance and theory development on nursing interventions to meet basic human needs in clinical settings. Conclusions: A curriculum model for fundamentals of nursing at the graduate level suggests that its application may have a positive impact on development of distinct knowledge body for fundamentals of nursing that will differentiate the content of education for undergraduate and graduate levels of nursing. However, the suggestion for the curriculum model needs to be refined and developed for application.

초등학교 보건교사의 문화적 역량 영향요인 (Factors Affecting Cultural Competency of Elementary School Nurses)

  • Purpose: The aim of this study was to investigate the factors affecting the cultural competency of elementary school nurses. Methods: Data were collected from a convenience sample of 109 school nurses who were recruited from elementary schools across three large cities in South Korea. The daata were analyzed with independent t-tests, a one-way ANOVA, Pearson's correlation coefficients, and stepwise multiple regression. Results: School nurses who had experience caring for multicultural students showed a significant difference in the cultural competency (t=8.75, p=.004). Factors affecting their cultural competency were awareness of multicultural families (${\beta}=.17$, p=.006) and transcultural self-efficacy (${\beta}=.69$, p<.001), which explained 58.6% of the total variance. Conclusion: The findings suggest that it is necessary to provide more training and educational programs for school nurses to improve their awareness of multiculcural families, including interventions to enhance transcultural self-efficacy, in order to enhance their cultural competence and to help them provide tailored care for multicultural students with diverse cultural backgrounds.

간호대학생이 임상실습에서 경험하는 무례함, 극복력, 사회적 지지가 소진에 미치는 영향 (Effect of Incivility, Resilience, and Social Support Experienced by Nursing Students on Burnout in Clinical Practice)

  • Purpose: The purpose of the study was to determine effect of incivility, resilience, and social support experienced by nursing students on burnout in clinical practice. Methods: Subjects were 140 nursing students who agreed to participate in this study. Collected data were analyzed using descriptive statistics, t-test, analysis of variance, Pearson's correlation and stepwise multiple regression with SPSS WIN 23.0 program. Results: Burnout showed significantly positive correlation with incivility but significantly negative correlations with resilience and social support. Factors affecting burnout were satisfaction with major-dissatisfaction, satisfaction with major-average, social support, grade, and relationship with peers. Satisfaction with major (dissatisfaction) had the greatest effect on burnout, explaining 41% of the total variance. Conclusion: According to this study, dissatisfaction with major was identified as the most significant factor influencing burnout of nursing students in clinical practice. Therefore, it is important to develop and implement programs that can reduce dissatisfaction with major and increase social support and relationship with peers in order to lower burnout of nursing students. In addition, a systemic management of fourth-grade students with a high level of clinical practice is necessary to reduce the level of clinical practice. 국민안심병원에서 근무하는 간호사들의 직무스트레스와 회복탄력성이 소진에 미치는 영향 (Influence of Job Stress and Resilience on the Burnout of Nurses Who Works at the Designated Public Relief Hospital)

  • 본 연구는 장기화된 팬데믹 상황에서 COVID-19관련 환자를 돌보는 간호사들의 직무스트레스와 회복탄력성이 소진에 미치는 영향을 알아보는 것을 목적으로 하였다. 간호사 162명을 대상으로 구조화된 설문지를 배포하여 분석하였고, 직무스트레스의 보상부적절, 직무요구, 직장문화, 회복탄력성의 철학적 패턴, 관계적 패턴이 통계적으로 유의하게 나타났다. 직무스트레스와 회복탄력성이 소진에 미치는 효과를 단계적다중회귀분석 방법으로 파악한 결과 최종적으로 유의한 변수는 직무스트레스였다. 간호사의 소진은 간호서비스 질 저하, 병원 경쟁력의 감소, 나아가 방역체제에 영향을 줄 수 있다. 이에, 본 연구는 간호사의 처우 및 근무환경 개선 등 적절한 보상의 정책적인 지원이 필요함을 제언한다.

중장년층의 근력운동 실천유무가 대사증후군에 미치는 영향: 제8기 1차 년도 국민건강영양조사(2019년) 자료를 이용하여 (Effect of the Muscle Strength Exercise in Middle Aged People on Risk Factors for Metabolic Syndrome: Using Data from the 8th First Years National Health and Nutrition Examination Survey (2019))

  • Purpose: Metabolic syndrome is known as a factor that increases the incidence of chronic diseases, such as diabetes and cardiovascular disease. In particular, the metabolic syndrome among a middle-aged population is rapidly increasing from 15.6% to 31.9%. The purpose of this study was to investigate the effect of muscle strength exercise on the metabolic syndrome in middle aged. Methods: This study was a secondary data analysis using National Health and Nutrition Survey 8th, including 2,739 middle aged people (40~64 years old). We used multivariate logistic regression to identify risk factors associated with metabolic syndrome. Statistical analysis was performed using the SPSS 23.0 program. Results: There were 772 patients in the group with metabolic syndrome and 1,967 patients in the non-metabolic syndrome group. The risk of metabolic syndrome was 1.29 times higher in those who did not do muscle strength exercise than those who did exercise (OR=1.29, 95% CI=1.01~1.66). Conclusion: We have found that muscle strength exercise was effective in lowering the risk of developing metabolic syndrome in middle aged. Thus, it is necessary to develop practical muscle strength exercise and education programs.
