급성기병원에서의 간호 · 간병통합서비스 운영 전후 낙상 및 욕창 발생 변화 (Performance of Comprehensive Nursing Care Service in an Acute Care Hospital: Focusing on Accidental Falls and Pressure Injuries)

  • Purpose: This study aimed to investigate whether the comprehensive nursing care service positively affected accidental falls and pressure injuries. Methods: This study was a retrospective study that analyzed the accidental falls and pressure injuries cases in an acute care hospital located in Seoul and compared the rates of accidental falls and pressure injuries before and after the comprehensive nursing care service was operated. Results: Comparing the accidental fall incidence rates per 100 person-months between a comprehensive nursing care ward and a general ward, it showed fewer accidental falls by 0.44 in comprehensive nursing care wards, but the result was not statistically significant. In the case of pressure ulcers, the incident rate per 100 person-month was 6.17 in general wards and 4.77 in comprehensive nursing care wards, which showed that the number of pressure ulcer patients was lower in comprehensive nursing care wards, however it was also not statistically significant. Conclusion: It is not confirmed that the operation of the comprehensive nursing care service contributes to the reduction of accidental hospital falls or pressure injuries. Follow-up studies are recommended to determine the effectiveness of comprehensive nursing services in quality indicators.

  • Purpose: The purpose of this study was to identify the influencing effects of job stress, professional autonomy, and reciprocity on the job embeddedness among comprehensive nursing care unit nurses. Methods: The participants in this study were 147 nurses who have worked for over 6 months in Comprehensive Nursing Care Unit. Data were collected from January 3 to January 31, 2022 from six general hospitals with more than 300 beds in three cities in G, G and P. Results: As the results of hierarchical regression analysis, job embeddedness was lower when nurses had clinical experiences for 1 to under 5 years (β=-.49, p<.001), 5 to under 10 years (β=-.27, p=.035), 10 to under 20 years (β=-.54, p<.001) compared to those who had clinical experiences for more than 20 years. Also, job embeddedness was higher when there was greater balance within team caregiving of reciprocality (β=.27, p<.001) and intrinsic reward (β=.22, p=.003), and lower role conflict (β =-.27, p<.001). Conclusion: The results of the study showed that job embededness would increase if the role conflict of comprehensive nursing care unit nurses reduces, if there was a culture that the members can make cooperative relationship with other health care professionals and, if there was an appropriate intrinsic reward depending on their work experience.

Mediating Effect of Job Burnout in the Relationship between Role Ambiguity and Organizational Effectiveness among Nurses in Comprehensive Nursing Care Service Units

  • 본 연구의 목적은 간호간병통합서비스 병동에 근무하는 간호사의 역할모호성, 직무소진과 조직유효성 간의 관계를 확인하고, 역할모호성과 조직유효성의 관계에서 직무소진의 매개효과를 파악하는데 있다. 대상자는 10개 종합병원에 근무 중인 149명의 간호사이다. 데이터는 자기보고식 설문지를 사용하여 수집하고 IBM SPSS Statistics 25.0 program과 PROCESS macro Program의 model 4를 이용하여 분석하였다. 분석 결과, 조직유효성은 역할모호성, 직무소진과 유의한 부(-)의 관계가 있는 것으로 나타났으며 직무소진은 역할모호성이 조직유효성에 미치는 영향을 완전매개 하는 것으로 확인되었다. 연구결과에 기초하여 직무소진을 관리하고 조직유효성을 제고하기 위한 방안을 제안하였다.

임상간호사 분포에 영향을 미치는 요인 (Factors Affecting the Distribution of Practicing Nurses)

  • 연구배경: 현행 간호인력정책이 간호사의 불균형 분포를 심화시킨다는 논란이 지속되고 있으나, 이를 확인한 연구는 매우 드물다. 이에 본 연구는 간호정책과 관련된 변수를 포함하여 지역수준에서 임상간호사 분포에 영향을 미치는 요인을 규명하고자 하였다. 방법: 2020년 기준 250개 시군구의 총 225,462명의 임상간호사 분포를 대상으로 분석하였다. 2020년 시군구별 국가통계자료를 수집하여 회귀분석을 실시하였다. 결과: 임상간호사 분포에 영향을 미치는 요인은 연간 입원일수, 간호간병통합서비스 병상 수, 공공보건의료기관 수, 상급종합병원 수, 종합병원 수 등으로 나타났다. 비수도권 지역에서는 간호학과 졸업생 수와 간호1, 2등급 기관 수도 임상간호사 분포에 영향을 미쳤다. 결론: 간호사 불균형 분포가 심화되지 않도록 간호간병통합서비스 병상의 확대 지역을 신중하게 결정해야 한다. 또한 간호사가 부족한 비수도권 지역을 중심으로 간호학과 학생을 늘리고, 비수도권 간호사의 임금과 근무환경을 우선적으로 개선해야 한다.

  • Purpose: This is a retrospective secondary data analysis study based on real-world data to analyze the level of nursing care needs of patients in a comprehensive nursing care service ward, and identify factors influencing nursing needs. Methods: Study participants included patients admitted to two comprehensive nursing care service wards at a tertiary general hospital located in Gyeonggi-do, Korea. After obtaining permission from the health and medical information team of the target hospital, data were collected from their electronic medical records. Nursing care needs were measured using seven items on nursing activity and four items on daily living activities developed by the National Health Insurance Service (NHIS). The collected data were analyzed using the SPSS version 29.0 with frequency and percentage, mean and standard deviation, minimum and maximum values, t-test, ANOVA, Pearson's correlation coefficients, and multiple linear regression. Results: The level of nursing care needs of patients in comprehensive nursing care service wards was found to be higher for patients with pressure sores (β=.33), older patients (β=.26), patients who underwent procedures (β=.15), patients with present guardians (β=.15), and patients with more comorbidities (β=.10). The total explanatory power was 51.0%. Conclusion: It is necessary to accurately identify patients' nursing care needs and provide nursing care according to priority by considering the characteristics of patients in comprehensive nursing care service wards.

말기암환자 가족간병인의 미충족 의료 분석 (Unmet Medical Service Needs in Family Caregivers of Terminal Cancer Patients)

  • 목적: 말기암환자들에게 간병인의 미충족 의료는 매우 중요하지만 간병인들이 필요로 하는 통증 조절, 삶의 질 보조, 가족간의 문제, 심리 사회적 문제, 자택 요양, 세심한 돌봄, 보완 대체 의학 같은 건강의 질을 유지할 수 있게 하는 관리 정보를 간병인에게 전부 제공하기에는 한계가 있고, 이와 관련된 연구는 아직 부족한 상태이다. 이번 연구를 통해 간병인의 특성과 미충족 의료를 밝히고, 특성에 따른 미충족 의료가 어떻게 다른지 알아보고자 하였다. 방법: 설문조사는 4개 호스피스 완화의료 병동에 입원한 말기암환자의 가족간병인 109명을 대상으로 시행되었다. 자료는 2014년 3월 1일부터 2014년 12월 31일까지 구조화된 설문지를 이용하여 수집되었다. 기존의 설문지를 수정하여 삶의 질 향상에 필요할 것으로 생각되는 14개 항목에 관해 필요성과 충족도를 알아보았다. 또, 통계는 단변량 분석으로 카이제곱 검정을, 다변량 분석으로 이중 회귀 분석을 사용하였다. 결과: 보완대체요법 및 건강기능식품 상담, 성기능 상담, 암 검진, 가족 및 대인관계 상담, 보험 상담, 직업 상담, 경제적 지원 등 7개의 항목에서 의미 있는 결과가 나타났다. 말기암환자의 가족간병인의 삶의 질 향상과 관련된 요구 중 충족도가 미충족도보다 더 높은 요구는 14개 중 피로 관리, 건강 체중 관리 2개였다. 결론: 본 연구 결과를 통하여, 말기암환자의 가족간병인의 미충족 의료를 이해하고 미충족 원인과 효과적인 중재방안에 대한 연구를 통해 요구를 충족시킬 수 있는 방안을 마련할 것을 제안한다.

치매노인환자 간병지원을 위한 스마트 돌봄 플랫폼 설계 (The Design of Smart Care-platform for Supporting Care of Elderly Patients with Dementia)

  • In modern society, dementia patients are increasing due to busy daily life. Due to the nature of dementia, family members are having a hard time with their caring activities. Dementia diseases reduce the quality of life not only of the patient, but also of the family, and nursing activities lead to social problems such as unemployment or frequent turnover. This study aims to reduce the difficulty and stress of the nursing process by supporting daily life of dementia patients and nursing activities through smart care platform. Ultimately, we will reduce the cost of treatment for people with dementia and delay the worsening of symptoms through a systematic care program for people with dementia. The smart dementia care platform features smart technology to help the dementia patients' daily lives and support their caregivers.

간병범죄에 관한 연구 (A Study on nursing crime)

  • According to the provisions of the United Nations, the proportion was 65 years of age or older population is 7% more than in the total population aged society. Due to the rapid decrease fertility and medical technologies and the improvement of living standards, the average life expectancy has emerged as an aging society. In the case of our country, and in 2011 the proportion of the population aged 65 and over 11.4%, the future is even faster aging rate is expected to launch in 2026, the super aged society. The days went by relying on caring for personal piety. Nursing crime is often due to the patriarchal family culture and afford. To care for patients with severe human or plant in the home, not to assume that society. Expanding the scope of long-term care insurance service person and the family burden reduced through measures to apply for health insurance, nursing care can prevent crime.

노인병원 간병인의 직무 스트레스와 소진이 이직의도에 미치는 영향 (Influence of Occupational Stress and Burnout on Turnover Intention of Caregivers in Geriatric Hospitals)

  • Purpose: The purpose of this study was identify the influences of occupational stress and burnout on turnover intention of caregivers in geriatric hospitals. Methods: The data were collected from 176 caregivers working at three geriatric hospitals located in Gyeonggi province. A self-reported questionnaires was used to assess the level of occupational stress, burnout and turnover intention. The data were analyzed with descriptive statistics, t-test, ANOVA and Pearson correlation coefficients and multiple regression by SPSS/WIN 18.0 program. Results: The turnover intention was positively correlated with occupational stress(r=.438, p<.01) and burnout(r=.418, p<.01). The influencing factors of turnover intention were occupational stress(${\beta}$=.290, p<.001), burnout(${\beta}$=.253, p=.001) and education level(${\beta}$=.182, p=.006). Conclusion: The results of this study indicate that it is necessary to develop stress management program so that caregivers in geriatric hospitals can reduce occupational stress and burnout.

노인요양병원 간병인의 소진에 대한 영향요인 (Factors Affecting Burnout of Caregivers in Geriatric Hospitals)

  • Purpose: This study was conducted to identify the factors affecting burnout of caregivers in geriatric hospitals. Methods: Data were collected from 266 formal caregivers working at Korean geriatric hospitals using self-report questionnaires between May 16 and June 30, 2016. The measured variables were burnout, emotional labor, job stress, job satisfaction, and self-efficacy experienced by caregivers. T-tests, ANOVA, Pearson correlation coefficients, and multiple regression were employed for data analysis. Results: The results indicated that emotional labor and job stress significantly influenced burnout of caregivers in geriatric hospitals; the explanatory power of the model was 27.3%. Conclusion: Based on the study results, we suggest further investigation of factors influencing burnout, and the development of strategies to prevent burnout by reducing emotional labor and job stress experienced by caregivers.
