Influence of general anesthesia on the postoperative sleep cycle in patients undergoing surgery and dental treatment: a scoping review on the incidence of postoperative sleep disturbance

  • General anesthesia may influence the postoperative sleep cycle; however, no clinical studies have fully evaluated whether anesthesia causes sleep disturbances during the postoperative period. In this scoping review, we explored the changes in postoperative sleep cycles during surgical procedures or dental treatment under general anesthesia. We compared and evaluated the influence of general anesthesia on sleep cycles and sleep disturbances during the postoperative period in adult and pediatric patients undergoing surgery and/or dental treatment. Literature was retrieved by searching eight public databases. Randomized clinical trials, observational studies, observational case-control studies, and cohort studies were included. Primary outcomes included the incidence of sleep, circadian cycle alterations, and/or sleep disturbances. The search strategy yielded six studies after duplicates were removed. Finally, six clinical trials with 1,044 patients were included. In conclusion, general anesthesia may cause sleep disturbances based on alterations in sleep or the circadian cycle in the postoperative period in patients scheduled for elective surgery.

2교대와 3교대 근무간호사의 수면과 피로도 (The Effects of 2 or 3 Shift Rotation on the Fatigue and Sleep)

  • The purpose of this study was to compare the sleep and fatigue between groups with two shift and three shift nurses. A total of 14 nurses working in Neonatal Intensive Care Unit(NIC) were included with the seven for two shifts and the other seven for three shifts. Data were collected by Korean version of sleep tool(Oh et al., 1998) and fatigue symptom tool(Kim et al., 2001) and analyzed using SPSS PC 12.0. The results of the study were as follows: 1) There was statistically a significant difference in the number of total sleep hour and no difference in fatigue between groups. 2) There were no significant differences found in both sleep and fatigue by demographic characteristics between groups. 3) There were no correlations between sleep and fatigue in both groups. Through the further future studies, the findings of this study were expected to provide an opportunities for exploring the information on 2-shift nurses as well as 3-shift nurses and to offer the fundamental data on an arrangement of work force according to the types of shift work.

병원간호사의 직무스트레스, 수면장애, 피로 (Hospital Nurses' Job Stress, Sleep Disturbance, and Fatigue)

  • Purpose: The purposes of this study were to investigate hospital nurses' job stress, sleep disturbance, and fatigue and to examine the relationships among them. Methods: The subjects were 119 registered nurses working in two hospitals. The data were collected from September 1 to September 30, 2015 using self-report questionnaires. The data were analyzed using SPSS 20.0 program. Results: Job stress levels of nurses were middle or higher level. Nurses levels of sleep disorders was higher than the median. It was the middle of the fatigue level of the nurses. There was a difference between sleep disorders and fatigue, depending on age and working department. There was a significant quantitative correlation among stress, sleep disorders, and fatigue. Conclusion: Therefore, it is necessary to develop programs to reduce job stress and sleep disorders and fatigue of hospital nurses. And it requires institutional improvements.

수면장애 노인 환자와 일반 노인 환자의 수면양상, 수면장애요인 및 수면증진행위 (Comparison of Sleep Pattern, Factors of Sleep Disturbance and Sleep Enhancement Behaviors between Sleep Disturbance and Non-Sleep Disturbance Aged Patients)

  • Purpose: This study was done to compare sleep patterns, factors of sleep disturbance and sleep enhancement behaviors between sleep disturbance and non-sleep disturbance elderly patients. Method: The participants were 117 patients over 65 years old who were hospitalized at 5 general hospitals in Seoul. There were 83 patients in the good sleeper group and 34 in the poor sleeper group. The survey questionnaires included questions on general characteristics, sleep patterns, sleep disturbing factors, and sleep enhancement behaviors of the patients. Results: For sleep patterns and environmental factors of sleep disturbance, the poor sleepers was significantly higher than that of the good sleepers. Significant differences between good sleepers and poor sleepers were also found on the following variables: Environmental factors of sleep disturbance -'Sound of other patient's or care-giver's', 'Discomfort of bed, linen, pillow and patient's gown', 'Light in the room', 'Emergency situations', 'Temperature of patients rooms too hot or too cold'. The significant physical factor of sleep disturbance was 'Hard to breath even without moving' and for sleep enhancement behaviors: 'Use of ear plugs or eye covers' Conclusion: Consequently it is necessary to develop positive and differentiated programs for sleep enhancement for the inactive and poor sleeper among the elderly patients.

대학생의 SNS 중독경향성이 특성불안과 수면의 질에 미치는 영향 (The Effect of SNS addiction tendency on Trait-Anxiety and Quality of Sleep in University students')

  • Purpose: This study investigates university students' SNS addiction tendency, trait-anxiety and quality of sleep. This study is going to offer basic resource that required to make the way that is able to reduce the anxiety with figuring out the relation between variable. Method: 213 people participated in this study is 20 to 30 age of university students who use SNS. Collecting resource period was April 4 to May 11, 2018. Analyses consisted of descriptive statistics, ANOVA, Pearson's correlation analysis, and stepwise multiple regression analysis. Results: The findings showed SNS addiction tendency and quality of sleep(r=.45, p<.000). Major Influence factor is predicted addiction tendency of SNS using time (F=21.21, p=.000) and trait-anxiety (F=23.55, p=.000). Of the highest trait-anxiety affects quality of sleep (F=24.82 p=.000). Conclusion: These findings indicate that it is necessary to develop and implement information programs and design appropriate SNS addiction prevention programs that can induce trait-anxiety and improve quality of sleep. Thus follow-up study should have diverse participants to investigate SNS addiction tendency, trait-anxiety and quality of sleep that influenced by overusing SNS.

성인에서의 주간 수면과다증의 유병률 및 관련 요인 (Prevalence and Associated Factors of Excessive Daytime Sleepiness in Adults)

  • Purpose: The purpose of the present study was to identify prevalence of excessive daytime sleepiness(EDS) and its associations with sleep habits, sleep problems, depression, subjective health status and obesity in community dwelling adults. Method: Data was collected from adults aged 20-59 years by random sampling. Subjects completed a questionnaire which was composed of the Epworth Sleepiness Scale, Center for Epidemiologic Studies Depression Scale, and questions that included items about sleep habits, sleep problems, subjective health status and sociodemographic characteristics. Height and weight were measured for calculation of body mass index. The statistical analyses was based on 3,302 adults (51.5% males and 48.5% females). Descriptive statistics, univariate logistic regression and multiple logistic regression were used. Result: The prevalence of EDS was 17.1% Multiple logistic regression showed that the associated factors of EDS were depression, obesity, dissatisfaction with sleep time, irregular sleep, and habitual snoring. Depression was the most significant associated factor(adjusted odds ratio for severe depression=2.27, 95% Confidence Interval=1.73-2.96). Conclusion: EDS is a common symptom in adults. Our finding suggested that persons with a complaint of EDS should be completely assessed for depression and obesity as well as sleep problems.

외과계 중환자의 수면질에 영향을 미치는 요인 (Factors Influencing on Quality of Sleep among Patients at Surgical Intensive Care Units)

  • Purpose: This study was conducted to evaluate quality of sleep and to assess the factors that influence quality of sleep in surgical ICU. Methods: The subject of the study were consisted 109 adult patients who admitted to surgical ICU. The data were collected from May 20 to December 10, 2007 by structured questionnaires. The data were analyzed with descriptive analysis, paired t-test, Pearson correlation coefficient and stepwise multiple regression. Results: The score of quality of sleep was 4.57 point. The main sleep disturbance factors related to quality of sleep in surgical ICU inpatient were sleep time, machinery alarm and noise(adjusted $R^2$=33.2). Conclusion: Based on the finding of this study, it is needed to develop a nursing intervention program that including to promote quality of sleep and to decrease machinery alarm and noise in surgical ICU.

UWB 레이더를 이용한 비접촉 생체신호 검출에 관한 연구 (A Study on the Detecting of Noncontact Biosignal using UWB Radar)

  • This study relates to acquiring biological signal without attaching directly to the user using UWB(Ultra Wide Band) radar. The collected information is the respiratory rate, heart rate, and the degree of movement during sleep, and this information is used to measure the sleep state. A breathing measurement algorithm and a sleep state detection algorithm were developed to graph the measured data. Information about the sleep state will be used as a personalized diagnosis by connecting with the medical institution and contribute to the prevention of sleep related diseases. In addition, biological signal will be linked to various sensors in the era of the 4th industrial revolution, leading to smart healthcare, which will make human life more enriching.

복지관 이용노인의 신체활동과 수면 (Physical Activity and Sleep Patterns in Elderly Who Visited a Community Senior Center)

  • Purpose: The purpose of this study was to identify the relationship between physical activity and sleep patterns of the elderly. Methods: The subjects of this study were 154 elderly who visited a community senior center in Korea. Data was collected by an interview and a self reported questionnaire, during the period from June to October, 2006. Physical activity was measured by IPAQ Korean version(2006), and sleep pattern by Korean Sleep Scale A developed by Oh et al.(1998). Results: The prevalence of chronic illness in the subjects was 73.4 %. The mean time of vigorous activity was $6.62{\pm}31.27$ minutes/day during the past week. Moderate activity time was $28.85{\pm}50.31$ minutes/day and walking time was $28.85{\pm}50.3$ minutes/day. The total sleep time was $397.63{\pm}111.53$ minutes/day. Physical activity of the elderly significantly correlated with sex, chronic illness, job, and sleep patterns of the elderly. Stepwise multiple regression analysis revealed that the most powerful predictor of sleep pattern was the satisfaction of sleep. A combination of the number of chronic illnesses, moderate physical activities(MET), and total time of sleep accounted for 44.4% of the sleep pattern. Conclusions: Moderate physical activity is more effective than vigorous physical activity for improving the sleep quality of the elderly.

백내장 수술 후 수면 변화에 관한 통합적 고찰 (An Integrative Review on Alterations of Sleep after Cataract Surgery)

  • Purpose: The purpose of this study was to review previous literature with an aim to explore the sleep disturbance level after a patient undergoes cataract surgery using an integrative review. Methods: We used the key words, 'cataract surgery', 'sleep', and 'intraocular lens' to find peer-reviewed publications in seven databases. Among 450 searched articles, eight articles were selected after exclusion of articles that did not meet the criteria. Results: Five of the articles submitted that subjective sleep quality of the patients with blue-filtering intraocular lens (BF-IOL) implant improved as compared to that before surgery. The change of saliva melatonin concentration after BF-IOL implant did not coincide in two of the articles. Two of the articles reported an increased level of intrinsically photosensitive retinal ganglion cells response after BF-IOL implantation. Conclusion: The published studies stated that BF-IOL implant did not have any negative impact on quality of sleep among cataract patients suffering with poor sleep. Rregardless of intraocular lens type, cataract surgery may increase photoreception of intrinsically photosensitive retinal ganglion cells one year after surgery although the mechanism was not clear. It is necessary to identify various factors influencing the quality of sleep such as gender and activities among cataract patients with BF-IOL implant in the future.
