불안정 지면에서 복합 균형 운동이 무릎 전치환술 환자의 기능적 능력과 일상생활능력에 미치는 영향 (The effect of complex balance exercise on unstable surfaces on functional ability and daily living ability in patients with total knee arthroplasty)

  • Purpose: Patients with total knee arthroplasty (TKA) have impaired balance and movement control. Exercise interventions have not targeted these impairments in this population. This study aimed to investigate the effect of complex balance exercises on unstable ground, on the gait, balance, and daily living ability of patients with total knee arthroplasty. Design: Randomized controlled trial. Methods: The participants consisted of 30 patients placed into two groups of 15 each: a experimental group (complex balance exercise) and a control group (physical therapy exercise). Both group exercise was applied for 3 times a week for 30 minutes for four weeks. Force plate for balance ability and Timed up and go (TUG) test were the primary outcome measures. The secondary outcome measures included 10-m walk test (10MWT) and the daily living ability using the Knee Outcome Survey Activities of Daily Living scale (KOS-ADL). Results: The result of this study showed that the experimental group had a significant difference in TUG and 10MWT than the control group, and balance was significantly different in CEA, CPL, and CAV. There was a significant difference in daily living ability between the experimental and control groups. This study confirms that the physical therapy with complex balance exercise on unstable surfaces has positive effects on balance, gait and daily living ability in patients with total knee arthroplasty. Conclusion: As a result of this study, complex balance exercise on unstable surface was more effective in improvement gait, balance and daily living ability in total knee arthroplasty. From this study, physical therapy with complex balance exercises on unstable ground may be proposed as and effective intervention method for improving gait, balance, and daily living ability in patients with early total knee arthroplasty.

Effects of Progressive Core and Ankle Muscle Strengthening Exercises Using Thera-Band on Body Balance

  • Purpose: This study aims to compare the effect on balance during core and ankle muscle strengthening exercises using the Thera-Band. Methods: 21 healthy college students were recruited. The participants were divided into a core strengthening group (CSG), an ankle strengthening group (ASG), and a non-exercise group (NEG). CSG and ASG were performed twice a week for a total of 4 weeks, and static and dynamic balance were measured before and after the intervention. The static balance were measured as stability index and weight distribution index using Tetrax®. The dynamic balance was measured in each direction by the Y balance test. The Thera-Band intensity was increased after 2 weeks of exercise, and the exercise was subsequently performed. Results: Participants showed static balance with Tetrax®, a significant difference was noted between normal eye closes and pillow with eye closes in ASG (p<0.05). In the case of dynamic balance with the Y balance test, a significant difference was observed in posterolateral direction (PL) and composite score (CS) between each group for the pre- and post-intervention differences (p<0.05). A significant difference was observed between PL and CS in CSG (p<0.05). Conclusion: These findings show that the progressive Thera-Band exercise had a positive effect on balance abilities. It was confirmed that core strengthening was effective for dynamic balance, and ankle strengthening was effective for static balance.

바른체형 운동프로그램이 여자고등학생의 자세 및 발의 균형감에 미치는 영향 (Effects of the Upright Body Type Exercise Program on Postures and Foot Balance in Female High School Students)

  • The purpose of this study was to investigate the degree of improvement of posture and foot balance between the exercise group and the control group after upright body type exercise program of 12 weeks with 14 female students in B female high school. According to the procedure of this study, following conclusion came out. First, The change of head's posture became near a centerline and a significant decrease from $7.57{\pm}13.95mm$ to $2.71{\pm}5.23mm$ in experimental group and there was a statistically significant difference (p<.002). The change of left foot balance was near the perfect balance (50%) from $48.93{\pm}3.87%$ to $49.74{\pm}2.95%$ and right foot balance was from $51.07{\pm}3.87%$ to $50.26{\pm}2.95%$ in experimental group and there were near the perfect balance (50%). The change of left foot balance of control group fall away the perfect balance (50%) from $49.97{\pm}2.67%$ to $49.08{\pm}1.41%$, right foot balance also fall away the perfect balance (50%) from $50.03{\pm}2.67%$ to $50.92{\pm}1.41%$. As a conclusion, upright body type exercise program considered to effect posture and foot balance in female high school girls. Consequently It was suggested that upright body type exercise program with improvement and prevention for posture's unbalance of female high school girls. This program could be utilized for improvement of youth posture and foot balance.

깔창의 높이에 따른 전후 압력 분포와 좌우 균형에 관한 연구 (Investigation about anterior.posterior plantar pressure and right.left body balance by insole height)

  • Purpose : The purpose of this study was to investigate the effect of body balance according to insole height and to provide basic information about body balance by insole height. Method : We examed 40 university students who had healthy body without balance impairment. Plantar pressure was measured by EMED system and the measurement of MTD balance used the MTD-balance master in static stance posture. Both of equipments are various measurement method. We measured plantar pressure and MTD balance each three different height insole(0cm, 3cm, 7cm) and each trial was 30 second in duration. Result : The results were as follows : 1) It showed significantly differences between bare foot and height insole. The anterior plantar pressure with 3cm or 7cm insole were more higher than bare foot(P<.05). 2) There were no significantly differences between barefoot and height insole with MTD-balance master(P<.05). Conclusion : In conclusion, the measurement of MTD balance showed right and left balance ability didn't change by insole height, but plantar pressure was moved on anterior side of foot so we could know insole's height cause the effect to anterior and posterior balance ability.

학령전 아동의 아연흡수에 관한 연구 (Zinc Absorption of Preschool Children)

  • 학령전 아동의 zinc absorption, excretion, balance에 대하여 연구한 결과는 다음과 같다. Zinc absorption은 남자아동의 경우 29.7%, 여자아동의 경우 23.3%, 평균 25.8%로 나타났다. 대상자기 zinc absorption은 zinc intake와는 상관성이 나타나지 않았으며, zinc balance와 유의적 상관(p<0.01)을 보였다. Zinc balance는 남자아동의 경우 평균 3.25mg/day , 여자아동의 경우 1.78mg/day , 평균 2.34mg/day 로 1명의 대상자를 제외하고 모두 positive balance를 보였다. Fecal zinc loss는 평균 6.31mg/day, urinary ziilc loss는 평균 0.16mg/day 였다. zinc intake는 focal zinc loss및 zinc balance에 유의적 영향을 미치는 것으로 나타났으며(p<0.01), urinary zinc는 intake의 영향을 받지 않았다. 우리나라 학령전 아동의 zinc absorption 및 balance 는 양호한 것으로 나타났다. 그러나 이 방면의 연구 자료가 많지 않아, 성장기 아동을 대상으로 한 zinc nutrition에 대한 구체적이고 지속적인 연구가 필요하다.

Effects of trunk control robot training on balance and gait abilities in persons with chronic stroke

  • Objective: To investigate the effects of training using a trunk control robot (TCR) system combined with conventional therapy (CT) on balance and gait abilities in persons with chronic stroke. Design: Two-group pretest-posttest design. Methods: Thirty-five subjects with chronic stroke were randomly assigned to either the TCR group (n=17) or the trunk extension-training (TET) group (n=18). Both groups performed CT for 30 minutes, after which the TCR group performed TCR training and the TET group performed trunk extension training for 20 minutes. Both groups performed the therapeutic interventions 3 days per week for 6 weeks. Balance ability was evaluated using the Berg Balance Scale (BBS), and the Timed Up-and-Go (TUG) test. Gait ability was measured using the 10 m Walk Test (10MWT) and the NeuroCom Smart Balance Master. Results: TCR group showed significant improvements in static balance (weight bearing) and dynamic balance (weight shifting speed, weight shifting direction, BBS, and TUG), 10MWT, gait speed, and step width (p<0.05); step length was not significant. The TET group showed a significant partial improvement of dynamic balance (weight shifting speed, weight shifting direction, BBS, and 10MWT (p<0.05), but the improvements in static balance, TUG, gait speed, and step width and step length was not significant. Additionally, significant differences in static balance, dynamic balance (weight shifting speed, weight shifting direction, BBS, and TUG), 10MWT, gait speed, and step width were detected between groups (p<0.05). Conclusions: TCR training combined with CT is effective in improving static and dynamic balance, as well as gait abilities in persons with chronic stroke.

동적 균형과 발목 근육 특성 사이의 관계 (Association between dynamic balance and ankle muscle characteristics)

  • 균형과 발목 근육 특성 (근력과 유연성) 사이의 관계는 낙상 위험의 증가에 대한 규명과 낙상방지 훈련프로그램을 발전시키기 위해서 중요할 것이다. 발목 근육 특성(근력과 유연성)과 균형사이의 관계는 이전에 연구되지 않았다. 이 연구의 목적은 발목근육특성과 균형사이의 관계를 평가하는 것이다. 16명의 건강한 참가자들이 본 연구에 자원하였다. 동적 균형은 와이-발란스 (Y-balance) 키트를 이용하여 측정하였다. 발목 근육 특성 (근력과 유연성)은 악력계와 고니어미터를 사용하여 측정하였다. 발목내번근력과 와이-발란스 (Y-balance) 측정 수행능력 사이에 양의 상관관계를 나타냈다. 임상가들은 동적균형의 발전을 위해서 치료적 중재를 하는 동안 발목 근력을 고려해야 한다.

가상현실기반과 고전적 스쿼트 운동 방법에 따른 정적, 동적 균형지수 간 상관분석 (The Correlation between Static and Dynamic Balance Index according to the Virtual Reality-Based Squat and Conventional Squat Exercise)

  • Purpose : The purpose of this study was to examine the correlation between static and dynamic balance according to the virtual reality-based squat and conventional squat exercise. Methods : Twenty four participants were randomly assigned to the virtual reality-based squat (VRS) group (n=12) or conventional squat (CS) group (n=12). The static balance (C90 area, C90 angle, trace length, sway average velocity) and dynamic balance (forward, rearward, leftward, rightward) were measured using a force plate by BT4. The VRS group used the virtual reality system during 4 weeks, while the CS group underwent classical squat training. Independent t-test was used to test the homogeneity of the general characteristics of the subjects. The collected data was analyzed using the paired t-test for static and dynamic balance comparisons before and after exercise in both groups and Pearson's test for the correlation between static and dynamic balance according to the measured time. The significance level was set to 0.05. Results : There was no significant correlation between group and static and dynamic balance related variables (p>.05). There was a significant correlation between measurement time and static and dynamic balance related variables (p<.05). According to the measurement time, the static balance parameter C90 area in the VRS group after exercise was significantly decreased (p<.05). The values of forward, leftward and rightward in the VRS group were significantly increased after exercise (p<.05). Conclusion : It is suggested that 20 normal healthy adult men and women who have normal balance ability can improve their ability to control their posture by improving the balance ability when applying virtual reality-based squat exercise.

The Effects of Balance Training with Functional Electrical Stimulation on Balance and Gait in patients with chronic stroke

  • Objective: The purpose of this study was to examine the effects of balance training with Functional Electrical Stimulation (FES) on balance and gait in patients with chronic stroke. Design: A cross over design Methods: Nine patients with stroke were recruited into this study. They were measuring their balance ability and gait ability. The intervention "A" included 4 weeks of balance training with Functional Electrical Stimulation (FES) for 40 m/d, 3 d/wk. Intervention "B" included 4 weeks of balance training with placebo Functional Electrical Stimulation (FES) for 40 m/d, 3 d/wk. Of the 9 patients who completed the study, 5 were randomly assigned to" group A-B", and 4 to group B-A. The crossover occurred after 4 weeks. Results: Following are the specific results of balance training with Functional Electrical Stimulation (FES) on patients with chronic stroke. First, patients who received treatment A showed improvement compared with patients who received treatment B in static balance. There were significant decreases in anterioposterior, mediolateral postural sway extension and velocity moment (p<0.05) with their eyes opened and closed conditions. Second, they had significantly improved in dynamic balance (p<0.05). Lastly, there were also improvement in their gait velocity and cadence (p<0.05). Conclusions: These findings suggest that, the Functional Electrical Stimulation (FES) combined with balance training more effectively improves the balance and gait ability, I'm convinced that it could be actively used in clinics added to the conventional physical therapy in the future.

Effect of Muscle Taping and Joint Taping on Static and Dynamic Balance in Normal Adults with Chronic Ankle Instability

  • Purpose : This study was conducted to investigate the effect of muscle taping and joint taping on static and dynamic balance in normal adults with chronic ankle instability. Methods : The subjects of this study were 32 people who met the inclusion criteria. This cross-sectional study was conducted using the Kinesio tape, an elastic tape, was used. Subjects were randomized to exclude the effect of sequence, and no taping, joint taping, and muscle taping were applied as taping interventions. One-leg standing test and a Functional reach test were conducted to measure static balance, and Y-balance test was conducted to measure dynamic balance. One way repeated ANOVA was performed to investigate the difference in balance ability according to the taping intervention. If there was a significant difference, a post-hoc was performed using the Bonferroni method. Results : In the case of static balance, joint taping showed more significant results than did no taping and muscle taping (p<.05), and muscle taping showed more significant results than did no taping (p<.05). In the case of dynamic balance, muscle taping showed significantly larger results than did no taping and joint taping (p<.05) and joint taping showed significantly larger results than did no taping (p<.05). Conclusion : This study found that mechanical stimulation of muscles and joint compression by elastic taping increased ankle stability and improved static and dynamic balance. In particular, for static balance, joint taping was more effective than muscle taping, and for dynamic balance, muscle taping was more effective than joint taping. Applying the appropriate taping method to individual subjects has the advantage of maximizing the therapeutic effect for the recovery of balance ability. Similarly, the application of various tapings to subjects with ankle instability will have a positive effect on functional improvement.
