A Study on Change of Plantar Fascia Thickness in Chronic Stroke Patient Based on Spasticity (만성 뇌졸중 환자에서 경직에 따른 족저근막의 두께 변화에 관한 연구)

  • The purpose of this study was to investigate the imbalance of muscle tone and frequent exposure to excessive gait training in patients with chronic spasticity due to stroke, the thickness of the plantar fascia to change and to evaluate. The subjects of this study was in 54 patients with chronic stroke from 18 patients Group I(MAS G0), 18 patients Group II(MAS G1), and 18 patients Group III(MAS G2) were selected. Measurement of clinical symptoms and physical examination, MAS(Modified Ashworth Scale), ultrasonographic, ROM(Range of Motion), VAS(Visual Analogue Scale), TUG(Timed Up and Go test) was measured. The study results were each group between the unaffected side and the affected side on plantar fascia thickness was statistically significantly thicker(p<.001). Each group between the unaffected side and the affected side on ankle dorsiflexion ROM was statistically significantly decrease(p<.001), VAS(p<.001), TUG(p<.001) statistically significantly increase(p<.001). In this study, the plantar fascia pathokinesiology ever presented by the contents of gait training in stroke patients is one of the information that you need to consider when presented.

  • Macrophage activation syndrome (MAS) is a rare complication in systemic lupus erythematosus (SLE) that can be triggered by infections. Due to the fact that MAS may mimic clinical features of underlying rheumatic disease, or be confused with an infectious complication, its detection can prove challenging. This is particularly true when there is an unknown/undiagnosed disease; and could turn into an even greater challenge if MAS and SLE are combined with a viral infection. A-14-year-old female came to the hospital with an ongoing fever for 2 weeks and a painful facial skin rash. Hepatomegaly, pancytopenia, increased aspartate aminotransferase, elevated serum ferritin and lactate dehydrogenase were reported. No hemophagocytic infiltration of bone marrow was reported. The patient was suspected for hemophagocytic lymphohistiocytosis. Her skin rashes were eczema herpeticum, which is usually associated with immune compromised conditions. With the history of oral ulcers and malar rash, positive ANA and low C3, C4 and the evidence of hemolytic anemia, she was diagnosed as SLE. According to the diagnostic guideline for MAS in SLE, she was diagnosed MAS as well, activated by acute HSV infection. After administering steroids and antiviral agent, the fever and skin rash disappeared, and the abnormal laboratory findings normalized. Therefore, we are reporting a rare case of MAS triggered by acute HSV infection as the first manifestation of SLE.

Effects of the long-term use of maxillary protraction facemasks with skeletal anchorage on pharyngeal airway dimensions in growing patients with cleft lip and palate

  • Objective: To investigate the effects of the long-term use of a maxillary protraction facemask with miniplate (FM-MP) on pharyngeal airway dimensions in growing patients with cleft lip and palate (CLP). Methods: The study included 24 boys with CLP (mean age, 12.2 years; mean duration of FM-MP therapy, 4.9 years), divided into two groups according to the amount of A point advancement to the vertical reference plane (VRP): Group 1, > 4 mm; Group 2, < 2 mm; n = 12/group. After evaluating the skeletodental and airway variables using lateral cephalograms acquired before and after FM-MP therapy, statistical analyses were performed. Results: Group 1 showed greater forward and downward displacements of the posterior maxilla (posterior nasal spine [PNS]-horizontal reference plane [HRP]; PNS-VRP), greater increase in ANB, more forward tongue position (tongue tip-Pt vertical line to Frankfort horizontal plane), and greater increase in the oropharynx (superior posterior airway space [SPAS]; middle airway space [MAS]) and upper nasopharynx (PNS-adenoid2) than did Group 2. While maxillary advancement (A-VRP and PNS-VRP) correlated with increases in SPAS, MAS, and PNS-adenoid2, downward displacement of the PNS (PNS-HRP) correlated with increases in SPAS, MAS, PNS-adenoid1, and PNS-adenoid2, and with a decrease in vertical airway length (VAL). Mandibular forward displacement and decrease in mandibular plane correlated with increases in MAS. Conclusions: FM-MP therapy had positive effects on the oropharyngeal and nasopharyngeal airway spaces without increases in VAL in Group 1 rather than in Group 2. However, further validation using an untreated control group is necessary.

Aspartate-glutamate carrier 2 (citrin): a role in glucose and amino acid metabolism in the liver

  • Aspartate-glutamate carrier 2 (AGC2, citrin) is a mitochondrial carrier expressed in the liver that transports aspartate from mitochondria into the cytosol in exchange for glutamate. The AGC2 is the main component of the malate-aspartate shuttle (MAS) that ensures indirect transport of NADH produced in the cytosol during glycolysis, lactate oxidation to pyruvate, and ethanol oxidation to acetaldehyde into mitochondria. Through MAS, AGC2 is necessary to maintain intracellular redox balance, mitochondrial respiration, and ATP synthesis. Through elevated cytosolic Ca2+ level, the AGC2 is stimulated by catecholamines and glucagon during starvation, exercise, and muscle wasting disorders. In these conditions, AGC2 increases aspartate input to the urea cycle, where aspartate is a source of one of two nitrogen atoms in the urea molecule (the other is ammonia), and a substrate for the synthesis of fumarate that is gradually converted to oxaloacetate, the starting substrate for gluconeogenesis. Furthermore, aspartate is a substrate for the synthesis of asparagine, nucleotides, and proteins. It is concluded that AGC2 plays a fundamental role in the compartmentalization of aspartate and glutamate metabolism and linkage of the reactions of MAS, glycolysis, gluconeogenesis, amino acid catabolism, urea cycle, protein synthesis, and cell proliferation. Targeting of AGC genes may represent a new therapeutic strategy to fight cancer.

An Extended Service Filtering Technique for Mass Calling-Type Services Using Intelligent Peripheral in an SCP-Bound Network

  • This paper proposes an extended service filtering technique to prevent overload in service control point (SCP) due to televoting (VOT) or mass calling (MAS) services with the heavy traffic characteristics. Also, this paper compares this extended technique with the existing overload control techniques, and calculates steady state call blocking probabilities in intelligent network (IN) under overload conditions. The proposed technique considers SCP overload and IN Capability Set (CS)-1 services (such as VOT or MAS service) that have to use the specialized resources of intelligent peripheral (IP). This technique uses first an activating step in which SCP requests service filtering to service switching point (SSP). Then, in the filtering step, SSP sends filtering results to SCP periodically or each N-calls. Also, when filtering time-out expires, SSP stops service filtering, and sends service filtering response to SCP in the deactivating step. This paper applies this technique to VOT/MAS service, and calculates SCP and SSP-IP (circuit) call blocking probabilities by using an analytical VOT/MAS service model. With the modeling and analyzing of this new technique, it shows that this technique reduces the traffic flow into SCP from SSP and IP prominently.

자율해운을 위한 원격제어 해상항만의 개념 설계

  • 자율해운(Autonomous Shipping)은 자율운항선박(Maritime Autonomous Surface Ships, MASS)의 태동과 함께 해양산업분야에서 창출되고 있는 해상운송의 새로운 개념이다. 자율해운은 MASS의 장점을 극대화하여 경제적 편익을 추구하기 위한 것이다. 한편, MASS는 기존 육상에 주로 구축된 항만시설을 이용한 해상운송 행위에 많은 제약이 따른다. 본 연구는 향후 전개될 것으로 예상되는 자율해운의 선점을 위한 것으로, 원격으로 제어되는 수준 2-3 사이의 MASS를 원격으로 제어하는 해상항만(Remote Controlled Sea Port)을 이용하여 경제적 편익을 극대화함과 동시에 안전하면서도 친환경적인 자율해운을 도모하기 위한 것이다. 본 연구의 목적은 해상 부유체를 이용한 원격제어 해상항만과 원격제어 MASS를 운영하기 위한 시설과 장비 및 운영과 관리 방법 등에 관한 기초 개념을 설계하기 위한 것이다. 글로벌 협력을 바탕으로 전개될 수 있는 자율해운을 정의하고, 자율해운에 현실적으로 적용 가능한 원격제어 해상항만의 구현 방법과 운영 방법을 논의하며, 이러한 원격제어 해상항만을 이용한 원격제어 자율운항선박의 제어 및 운용방법 등을 검토하였다. 본 연구는 세계적인 자율운항선박 관련 해양산업에서의 자율해운 시장의 선점에 기여할 것으로 고려된다.

MAS-NMR Studies in Mixed Alkali Silicate Glasses with low Fe2O3 Content (미소량의 Fe2O3를 첨가시킨 혼합 알칼리 Silicate 유리들의 MAS-NMR 연구)

  • The mixed alkali silicate glasses doped 0.1 mol% $Fe_2O_3$ are fabricated for studying the effect of transition metal oxides, which is related to the $Li^-$ ion movement in Magic-Angle-Spinning NMR. We have investigated the spin-lattice relaxation times in the room temperature and measured the $^7Li$ MAS-NMR spectrum with temperature. When the $Fe_2O_3$ oxides are added in alkali silicate glasses, the width of spectrum is changed a little but the line shape is hardly varied. For this reason, we can think that the mixed alkali effects are shown sufficiently in the spin-spin relaxation processes. However, it is not mixed alkali effects in this case. The activation energy of $Li^-$ ions are diminished in mixed alkali glasses. From the analysis of $Li^-$ diffusion environment, spin-lattice relaxation time and the nuclear magnetization, it is confirmed that the alkali mixed effects are not shown in $^7Li$ spin-lattice relaxation processes.

  • Miscibility and morphology in blends of bisphenol-A type polycarbonate and poly (ester-ether) elastomer with different compositions are studied by solid-state NMR spectroscopy. $^{13}$ C Solid-state NMR of CP/MAS/TOSS/DD, CP/MAS/DD, inversion recovery CP/MAS/DD, and 2D rotor driven spin diffusion techniques are used to identify the miscibility, morphology, and transesterification in blends. The blends of PC /BT elastomer with 15% to 42% of soft segment seem to be single phase miscible mixing and those of PC/PBT and PC/PBT elastomer with 62% of soft segment are cocontinuous two phase immiscible mixing. No significant transesterification reactions are observed in blends with different compositions.

  • This paper describes an efficient coding method for Korean characters using a three-state transition graph. To our knowledge, this is the first achievement of its kind. This new method, called the Paralle Hangul Dynamic Coding Method(PHDCM), compresses about 3.5 bits per a Korean character, which is more than 1 bit shorter than the conventional codes introduced thus far to achieve extensive code compression. When we ran the method on a MasPar machine, which is on SIMD SM (EFEW-PRAM)., it achieved a 49.314-fold speedup with 64 processors having 10 million Korean characters.
