Serological Analysis and Epidemiologic Characteristics of Group A Streptococci in Seoul(1998-2000) (3년간(1998-2000년) A군 연구균 감염의 혈청학적 형분류와 역학적 특징)

  • Purpose : Group A streptococci have a cell wall which consists of M protein and T protein. T protein is known to be helpful in the understanding of the epidemiology of group A streptococci. To study the epidemiologic characteristics, we serotyped T protein of group A streptococci obtained from patients admitted to hospitals, or who visited OPD in five districts of Seoul the during last three years. Methods : Group A streptococci were obtained in five districts in north, northeast, central, northwest and south Seoul from 1998 through 2000. All isolated group A streptococci were serotyped with T protein antisera(Institute of Sera and Vaccine, Prague, Czech Republic). Results : In 1998, analysis of obtained total number of 92 strains revealed that T12, T4, and NT acounted for 72.2%. Among seven cases of scarlet fever, T12 was isolated in four cases and T4 was found in three cases. Two cases of tonsilar abscess produced T8 and NT. One case of cervical lymphadenitis showed T12. In 1999, 41 cases were studied showing that T12, T4, and T1 contributed 68%. Among five cases of scarlet fever, T12 and T4 make up three case. There were two cases of pneumonia(T4 and T1) and one case of cervical lymphadenitis(T8/25). In 2000, the study was performed in four districts except the central area. Among 83 isolates, T12, T4 and T1 accounted for 63.9%. There were three cases of scarlet fever(T12, T4, T5), one case of tonsillar abscess(T12), one case of pneumonia(NT) and one case of sepsis(T1). Conclusion : Serological analysis of T protein of group A streptococci shows no endemic specificity. The yearly pattern reveals that T12 had been decreasing but T1 had shown the opposite trend.

Preoperative Staging in Non-Small Cell Lung Cancer without Lymphadenopathy on Computed Tomogram (흉부 전산화 단층촬영상 임파절종대가 없는 비소세포암 환자에 있어서 술전 병기판정)

  • Objectives: Careful evaluation about mediastinal involvement is important in the management of patients with non-small cell lung cancer. Invasive staging procedure such as mediastinoscopy is advocated because of the unreliability of noninvasive staging methods such as CT, MRI. We compared differences between pre- and postoperative staging in non-small cell lung cancer without lymphadenopathy on CT scan and investigated the methods for more accurate preoperative staging. Methods & Results: 1) Records of a total of 41 patients with preoperative $T_{1-3}N_0M_0$ non-small cell lung cancer were reviewed and the histologic types of tumors were squamous cell carcinoma in 32 cases, adenocarcinoma in 6 cases and large cell carcinoma in 3 cases. Twenty-four cases were central lesions and seventeen cases were peripheral lesions. 2) Among the 32 cases with preoperative $T_2$, 2 cases were identified postoperatively as $T_3$ with invasion of chest wall and among 6 cases with preoperative $T_3$, 1 case was identified postoperatively as $T_4$ with invasion of aorta and pulmonary arteries. 3) After the operation of 35 cases with $T_{1-2}$, 5 cases were $N_1$ and 3 cases were $N_2$ postoperatively. After the operation of 6 cases with $T_3$, 2 cases were $N_1$ and 3 cases were $N_2$ postoperatively. Preoperative $T_3$ showed more intrathoracic lymph node metastases and higher $N_2/N_1$ involvement ratio than preoperative $T_{1-2}$. 4) Complete surgical resections were done in 34 out of 41 cases. Incomplete resection were done in all postoperative $N_2$ tumors. Conclusion: Invasive staging procedures such as mediastinoscopy should be considered in the case of preoperative $T_3$ non-small cell lung cancer even though mediastinal lymphadenopathy is not recognized on the CT scan of the chest.

Gene Frequencies and Phenotypes of Transferrin C Subtypes and Haptoglobin in Korean Population (한국인집단의 Transferrin C Subtypes와 Haptoglogin Phenotypes의 분포와 유전자 빈도)

  • Genetic polymorphism of transferrin $(T_f)$ subtypes in Jeju population was studied by isoelectric focusing of human sera on polyacrylamide gels under high voltage, and haptolobin (Hp) polymorphism in Seoul and Jeju population was studied by polyacrylamide gel electrophoresis. Among 946 normal samples, three common types of transferrin, $T_{f}C_{1}, T_{f}C_{1}-C_{2} and T_{f}C_{2}$ were observed with some variants migrating slower than $T_{f}C$ subtypes, while among 139 patient (hepatitis) samples, only three common types were found. The gene frequencies were calculated as follows; in normal population, $T_{f}C^{1}$ was 0.7220; $T_{f}C^{2}, 0.2743; T_{f}D^{Jeju}, 0.0037$, and in patient population, $T_{f}C^{1} was 0.7194; T_{f}C^{2}, 0.2806$ respectively. Among 460 samples in Seoul and 502 in Jeju population, three types of haptoglobin, Hp 1-1, Hp 2-1 and Hp 2-2 were observed. The gene frequency of $Hp^1$ was 0.304, $Hp^2$, 0.696 in Seoul and in Jeju, $Hp^1$ was 0.269 and $Hp^2$, 0.731, respectively. The frequencies of the genes and the polymorphic phenotypes were discussed comparatively with the other populations.

Effects of Dietary Inuloprebiotics on Egg Production and on the Microbial Ecology and Blood Lipid Profile of Laying Hens (이눌로프리바이오틱스의 급여가 산란계의 혈액지질, 맹장 미생물, 계란 생산성에 미치는 효과)

  • This study was carried out to investigate the effects of inuloprebiotics (INPs), an alternative antibacterial growth promotor, from Jerusalem artichoke extract (Helianthus tuberosus L.) on egg production and quality in Hyline brown laying hens. The hens were divided randomly into four treatment groups and housed in individual cages for 10 weeks: a control group (0 ppm INP) (T1), 450 ppm (T2), 600 ppm (T3), and 750 ppm (T4). Egg production, egg weight, Haugh unit, eggshell thickness, and breaking strength were significantly higher in all of the INP-treated groups compared with the control (p<0.05). Egg cholesterol was highest in the T1 group and decreased with INP addition from 15.04 to 17.98% (p<0.05). Compared with the T1 group, triglycerides in the blood and in total cholesterol decreased significantly in groups T2, T3, and T4 by 21.71-24.07% and 27.17-30.36%, respectively (p<0.05). The growth of cecum Bifidobacterium and Lactobacillus was stimulated in groups T2, T3, and T4 compared with T1, whereas the growth of Escherichia and Salmonella was clearly inhibited (p<0.05). The results suggest that the addition of 450 ppm INP to the diet of laying hens can improve egg production and egg quality.

T1-, T2-weighted, and FLAIR Imaging: Clinical Application (T1, T2강조영상, FLAIR영상의 임상 적용)

  • T1-, and T2-weighted imagings and FLAIR (fluid attenuated inversion recovery) imaging are fundamental imaging methods in the brain. T1-weighted imaging is a spin-echo sequence with short TR and short TE and produces the tissue contrast by different T1 relaxation times. In other words, short TR maximizes the difference of the longituidinal magnetization recovery between the tissues. T2-weighted imaging is a spin-echo sequence with long TR and long TE and produces the tissue contrast by different T2 relaxation times. Long TE maximizes the difference of the transverse magnetization decay between the tissues. FLAIR is an inversion recovery sequence using 180 degree inversion pulse. 2500 msec of inversion time is applied to suppress the CSF signal.

Effect of Protein Deprivation on Subsequent Efficiency of Dietary Protein Utilization in Finishing Pigs

  • A nitrogen (N) balance trial was conducted to examine the effect of N deprivation on subsequent N retention, blood urea nitrogen (BUN) and IGF-I levels and the ratio of IGF binding protein (IGFBP)-3 to IGFBP-l and -2. Pigs in treatment (T) 1 were given diet A (2.39% N) and those in T2 and T3 were given diet B (1.31% N) and excreta were collected (period 1 (P1)). Pigs in T1 continued to receive diet A while diets for T2 and T3 were changed to diets A and C (2.74% N), respectively. The excreta were collected for two more periods (P2 and P3). During P1, pigs in T2 and T3 retained 50% less N (p<0.001) than those in T1. However, pigs provided T2 (p<0.01) and T3 (p<0.05) retained more N than those assigned to T1 during P2. Pigs in T3 tended to retain more (p=0.10) N than those receiving T2 for the same period. The BUN values were lower (p<0.05) for pigs assigned to T2 and T3 than T1 during P1 and P2. Both IGF-I and IGFBP ratios of pigs assigned to T1 were higher (p<0.05) than those given T2 and T3 during P1 but no differences were found during P2 and P3.

Serial Survey on Group A beta-Hemolytic Streptococcal Carrier Rate and Serotyping in Elementary School Children in 1996~1998 (3년간(1996~1998) 초등학생의 A군 연쇄구균 보균율과 혈청학적 분류에 관한 연구)

  • Purpose : The accuracy of bacteriologic diagnosis of beta-hemolytic streptococcal pharyngotonsillitis depends on the degree of carrier rate in that area where the throat swabs are obtained and the evaluation of serological T typing as an epidemiologic marker is important to understand epidemiology of streptococcal infection. The purpose of this study is to know the carrier rates of group A streptococcus in normal children form four different areas and to find out the epidemiologic characteristic in distribution of the serotypes for 3 years. Method : Throat swabs were obtained from the tonsillar fossa of normal school children in four different areas(Uljin, Seoul, Osan, Kunsan) from March to May 1996, in Uljin in April 1997, and in Uljin in April 1998. The samples were plated on a 5% sheep blood agar plate and incubated overnight at $37^{\circ}C$ before examination for the presence of beta-hemolytic colonies. All isolated beta-hemolytic streptococcus were grouped and serotyped by T agglutination. Results : The carrier rate of beta-hemolytic streptococci and group A streptococci in 1996 were 27.6%, 18.6% at Uljin; 16.4%, 2.7% at Seoul; 33.0%, 26.0% at Osan; 20.0%, 12.3% at Kunsan, respectively. Among 1,192 normal school children from 4 different areas, we obtained 179 strains of group A streptococci. Fifty two percent of the strains were typable by T agglutination in 1996. Common T-type in 1996 were NT, T1, T3, T2 at Uljin; T12, T25 at Seoul; NT, T6, T28 at Osan; T25, T4, NT, T5 at Kunsan, in decreasing order, respectively. At Uljin, T1, T3, T25 accounted for 69% of strains in 1996, T1, T12, T25 accounted for 70% in 1997, and T12, T4 accounted for 88% in 1998. Conclusion : Higher carrier rates were found in Uljin and Osan, where there are a lower population density with scanty of medical facilities compared with another areas. We supposed that low carrier rates is likely to be related to antibiotic abuse or some epidemiologic factor. The periodic and seasonal serotyping analysis is important in monitoring and understanding the epidemiologic patterns of group A streptococci.

Quantitative T1 Mapping for Detecting Microvascular Obstruction in Reperfused Acute Myocardial Infarction: Comparison with Late Gadolinium Enhancement Imaging

  • Objective: To compare native and post-contrast T1 mapping with late gadolinium enhancement (LGE) imaging for detecting and measuring the microvascular obstruction (MVO) area in reperfused acute myocardial infarction (MI). Materials and Methods: This study included 20 patients with acute MI who had undergone 1.5T cardiovascular magnetic resonance imaging (CMR) after reperfusion therapy. CMR included cine imaging, LGE, and T1 mapping (modified look-locker inversion recovery). MI size was calculated from LGE by full-width at half-maximum technique. MVO was defined as an area with low signal intensity (LGE) or as a region of visually distinguishable T1 values (T1 maps) within infarcted myocardium. Regional T1 values were measured in MVO, infarcted, and remote myocardium on T1 maps. MVO area was measured on and compared among LGE, native, and post-contrast T1 maps. Results: The mean MI size was 27.1 ± 9.7% of the left ventricular mass. Of the 20 identified MVOs, 18 (90%) were detected on native T1 maps, while 10 (50%) were recognized on post-contrast T1 maps. The mean native T1 values of MVO, infarcted, and remote myocardium were 1013.5 ± 58.5, 1240.9 ± 55.8 (p < 0.001), and 1062.2 ± 55.8 ms (p = 0.169), respectively, while the mean post-contrast T1 values were 466.7 ± 26.8, 399.1 ± 21.3, and 585.2 ± 21.3 ms, respectively (p < 0.001). The mean MVO areas on LGE, native, and post-contrast T1 maps were 134.1 ± 81.2, 133.7 ± 80.4, and 117.1 ± 53.3 mm2, respectively. The median (interquartile range) MVO areas on LGE, native, and post-contrast T1 maps were 128.0 (58.1-215.4), 110.5 (67.7-227.9), and 143.0 (76.7-155.3) mm2, respectively (p = 0.002). Concordance correlation coefficients for the MVO area between LGE and native T1 maps, LGE and post-contrast T1 maps, and native and post-contrast T1 maps were 0.770, 0.375, and 0.565, respectively. Conclusion: MVO areas were accurately delineated on native T1 maps and showed high concordance with the areas measured on LGE. However, post-contrast T1 maps had low detection rates and underestimated MVO areas. Collectively, native T1 mapping is a useful tool for detecting MVO within the infarcted myocardium.

Age-related Changes in Luteinizing Hormone and Testosterone Levels in Korean Men (한국 남성의 혈중 Luteinizing Hormone과 Testosterone 수준의 연령-관련 변화)

  • Changes in luteinizing hormone (LH), serum testosterone (T), and salivary T levels with age were examined in Korean men. Serum was obtained from 167 Korean men of different ages ($20{\sim}69\;y$), and the serum LH and T levels were measured. Saliva samples were also obtained, and the salivary T level was determined. The LH levels did not change considerably until 40 y of age (20s, $2.5{\pm}1.0$; 30s, $2.7{\pm}1.5$; and 40s, $2.5{\pm}1.8\;mIU/mL$) but increased significantly around 50 y (50s, $3.7{\pm}1.8$ and 60s, $3.1{\pm}1.7\;mIU/mL$). Further, the serum T levels also did not change until 40 y of age (20s, $5.3{\pm}2.6$, 30s, $4.4{\pm}1.4$, 40s, $4.1{\pm}1.5\;ng/mL$) but decreased significantly at 50 y (50s, $3.4{\pm}1.5$; 60s, $2.6{\pm}0.8\;ng/mL$). The salivary T levels also showed small changes until the age of 40 y ($20s{\sim}40s$, $0.11{\pm}0.015\;ng/mL$) but decreased significantly at 50 y ($0.08{\pm}0.03\;ng/mL$). Thus, the relative ratio of salivary T to serum T levels did not change significantly in all the ages examined ($2.4{\pm}0.9%$). Linear regression analysis predicted that the LH levels increased 1.5%/y while the serum and salivary T levels decreased 1%/y and 0.8%/y, respectively. The serum T/LH ratio did not change considerably until the age of 40 y ($20s{\sim}40s$, $2.27{\pm}0.14$) but decreased significantly ($1.2{\pm}1.0$) at 50 y. Age-related changes in the salivary T/LH ratio were very similar to those in the serum T/LH ratio. These results demonstrated that LH and T levels in serum or saliva did not change considerably until 40 y of age; instead, in Korean men, from 50 y of age, the LH level increased, while the T level decreased. This suggests that primary testicular failure that occurred due to aging (approximately 50 y) and caused this phenomenon. The present study also shows that the salivary T level can be an indicator of the free T level in serum although the salivary T level correlates weakly with the total T level in serum (r=0.53). Thus, information regarding salivary T levels may be useful for studying the age-related changes occurring in male testicular physiology.
