Management of Acute Gastroenteritis in Children: A Survey among Members of the Korean Society of Pediatric Gastroenterology, Hepatology, and Nutrition

  • Purpose: No national survey has yet described the guidelines followed by Korean pediatricians to treat acute gastroenteritis (AGE). An online survey was performed to investigate the management of AGE followed by members of The Korean Society of Pediatric Gastroenterology, Hepatology, and Nutrition, and the results were compared between pediatric gastroenterologists (PG) and general pediatricians (GP). Methods: Questionnaires were sent to pediatricians between June 2 and 4, 2018 regarding the type of hospital, indications for admission, antiemetic and antidiarrheal drugs and antibiotics prescribed, and dietary changes advised. Results: Among the 400 pediatricians approached, 141 pediatricians (35.3%) responded to the survey. PG comprised 39% of the respondents and 72.7% worked at a tertiary hospital. Both PG and GP considered diarrhea or vomiting to be the primary symptom. The most common indication for hospitalization was severe dehydration (98.8%). Most pediatricians managed dehydration with intravenous fluid infusions (PG 98.2%, GP 92.9%). Antiemetics were prescribed by 87.3% of PG and 96.6% of GP. Probiotics to manage diarrhea were prescribed by 89.1% of PG and 100.0% of GP. Antibiotics were used in children with blood in diarrheal stool or high fever. Dietary changes were more commonly recommended by GP (59.3%) than by PG (27.3%) (p<0.05). Tests to identify etiological agents were performed primarily in hospitalized children. Conclusion: This survey assessing the management of pediatric AGE showed that the indications for admission and rehydration were similar between GP and PG. Drug prescriptions for diarrhea and dietary changes were slightly commonly recommended by GP than by PG.

The Beneficial Effect of Platycodon grandiflorum on DSS-induced Colitis through Regulation of HIF-1α in Mice

  • Ulcerative colitis (UC) is a typical inflammatory colon disorder. Platycodon grandiflorum (PG) is known to exert various beneficial effects including anti-oxidative and anti-bacterial properties and improvements in liver function. However, the improving effect and mechanism of PG on intestinal inflammation are not fully understood. The present research was designed to investigate the effect of PG on the clinical signs of DSS-induced colitis in mice. The ameliorative effects of PG on inflammatory cytokine expression and the activation of hypoxia-inducible-factor (HIF)-1α in DSS-treated colon tissue were also determined. Our results showed that mice treated with DSS displayed the main clinical symptoms of colitis, including weight loss, bloody stools, decrease in colon length and diarrhea and PG treatment significantly improved the clinical features induced by DSS in mice. PG inhibited the increase in the levels of inflammatory cytokines caused by DSS in colon tissues. We also showed that the anti-inflammatory mechanism of PG involved suppressing the activation of HIF-1α in DSS-treated colon tissues. Collectively, the findings of this study indicate the prospect of developing new drugs from PG for UC treatment.

Identification of WAT1-like genes in Panax ginseng and functional analysis in secondary growth

  • The precise homeostatic regulation of local auxin accumulation in xylem precursors of cambium stem cell tissues is one of the most important mechanisms for plant vascular patterning and radial secondary growth. Walls are thin (WAT1), a novel intracellular auxin transporter, contributes directly to the auxin accumulation maxima in xylem precursors. According to recent research, the auxin signaling activated pathway-related gene network was significantly enriched during the secondary growth of Panax ginseng storage roots. These imply that during P. ginseng root secondary growth, specific signaling mechanisms for local auxin maxima in the vascular cambial cells are probably triggered. This study identified four WAT1-like genes, PgWAT1-1/-2 and PgWAT2-1/-2, in the P. ginseng genome. Their expression levels were greatly increased in nitratetreated storage roots stimulated for secondary root growth. PgWAT1-1 and PgWAT2-1 were similar to WAT1 from Arabidopsis and tomato plants in terms of their subcellular localization at a tonoplast and predicted transmembrane topology. We discovered that overexpression of PgWAT1-1 and PgWAT2-1 was sufficient to compensate for the secondary growth defects observed in slwat1-copi loss of function tomato mutants. This critical information from the PgWAT1-1 and PgWAT2-1 genes can potentially be used in future P. ginseng genetic engineering and breeding for increased crop yield.

A Characteristics of Bearing Capacity by PG Pile on Waste Landfill (폐기물 매립지반에서 PG Pile의 지반지지력 특성)

  • Waste landfill is so loose that it may cause the insufficient bearing capacity and the differential settlement. And so, characteristics and conditions of the ground should be considered in applications of ground improvement in waste landfill. In this paper, analysis of field tests as the static loading test and the bearing capacity test were performed. In result, PG(Pack Grouting) pile method is proved effective and economic, because it could bring about the increase of end bearing capacity, the prevention of differential settlement and increase of density by expansion of pile.

  • Currently, CIF is used widely as a common data base to represent IC layouts. The CIF2PG program is developed using C language to obtain electromask or mann 3000 PG file from the CIF input file. By obtaining the PG data, which can realize the actual mask layout, the construction of IC Design Automation System became possible using a general purpose computer.

Effect of Pyunggangaeuljihyul-tang (Pinggankaiyuzhixue-tang) on Toxic Agent Induced Liver Cell Damage (평간개울지혈탕이 독성약물에 의한 간조직 손상에 미치는 영향)

  • Objective : This study was undertaken to determine if Pyunggangaeuljihyul-tang (Pinggankaiyuzhixue-tang, PG) has a protective effect against cell injury induced by various toxic agents in rabbit liver, Methods : Cell injury in vitro was estimated by measuring lactate dehydrogenase (LDH), and that in vivo was estimated by measuring alanine aminotransferase (ALT) and aspartate aminotransferase (AST) activity in serum. Lipid peroxidation was examined by measuring malondialdehyde, a product of lipid per oxidation. Results : PG prevented the LDH release by $CCl_4$, mercury, menadione, and tert-butyl hydroperoxide treatment in vitro in liver slices. The extent of protection by 2% PG was similar to that of $10{\mu\textrm{M}}$ N,N'-diphenyl-p-phenylenedianline, a potent antioxidant, in tert-butyl hydroperoxide-induced LDH release. PG also prevented lipid peroxidation and depletion of cellular ATP induced by Hg. Hg causes motphological changes including cell necrosis and its effect was significantly prevented by PG. When rats were treated intraperitoneatly with 0.5 ml/kg of $CCl_4$, serum alanine aminotransferase and aspartate aminotransferase activities were increased compared with the control, which was significantly inhibited by pretreatment of PG. PG also prevented reduction in GSH and lipid peroxidation induced by $CCl_4$ Conclusion : These results suggest that PG exerts aprotective effect against various toxic agents by its antioxidant action in liver tissues. Thus, PG may be used in prevention and treatment of drug-induced liver cell injury. However, the precise mechanisms of PG protection remain to be determined.

Systemic Expression of Vascular Endothelial Growth Factor in Patients with Cerebral Cavernous Malformation Treated by Stereotactic Radiosurgery

  • Objective : Increased expression of angiogenic factors, such as vascular endothelial growth factor (VEGF), is associated with the pathogenesis of cerebral cavernous malformations (CCMs). The purpose of this study was to investigate plasma levels of VEGF in normal subjects and in patients with CCM and to evaluate change in these levels following stereotactic radiosurgery (SRS). Methods : Peripheral venous blood was collected from 6 patients with CCM before SRS using Gamma Knife and at the 1 week, 1 month, 3month, and 6 month follow-up visits. Plasma VEGF levels were measured using commercially available enzyme-linked immunosorbent assay kits. Peripheral blood samples were obtained from 10 healthy volunteers as controls. Results : Mean plasma VEGF level of 41.9 pg/mL (range, 11.7-114.9 pg/mL) in patients with CCM at baseline was higher than that of the healthy controls (29.3 pg/mL, range, 9.2-64.3 pg/mL), without significant differences between CCM patients and controls (p=0.828). Plasma VEGF level following SRS dropped to 24.6 pg/mL after 1 week, and decreased to 18.5 pg/mL after 1 month, then increased to 24.3 pg/mL after 3 months, and 32.6 pg/mL after 6 months. Two patients suffering from rebleeding after SRS showed a higher level of VEGF at 6 months after SRS than their pretreatment level. Conclusion : Plasma VEGF levels in patients with CCM were elevated over controls at baseline, and decreased from baseline to 1 month after SRS and increased further for up to 6 months. Theses results indicated that anti-angiogenic effect of SRS might play a role in the treatment of CCMs.

Multi Analysis of Fumigants in Soil and Water (물과 토양에서 훈증제의 동시분석법 확립)

  • Emission of methyl bromide (MeBr) from soil was implicated in stratospheric ozone depletion. To determine multi analysis of alternatives fumigants for MeBr, this paper describes the methods of analysis in water and soil. The MeBr, methyl iodide (Mel), propargyl bromide (PBr), cis 1,3-dichloropropene (cis 1,3-D), trans 1,3-dichloropropene (trans 1,3-D) and chloropicrin(CP) are separated on the base line on GC-ECD at three column of AT+DB+DB (90m) with temperature programing of $35^{\circ}C{\rightarrow}110^{\circ}C$ on GC-ECD. The relative retention time for MeBr, Mel, PBr, cis 1,3-D, trans 1,3-D and CP is 1.0, 1.4, 2.3, 3.2, 3.6 and 3.7, respertively. The detection limit for MeBr, Mel, PBr, cis 1,3-D, trans 1,3-D and CP is 469 pg, 5 pg, 21 pg, 79 pg, 101 PE and 5pg, respectively. Recovery of MeBr Mel, PBr, cis 1,3-D, trans 1,3-D and CP in water added 150 ppm fumigants were 81%, 96%, 95%, 97%, 98% and 99%, respectively. Recovery of MeBr, MeI, PBr, cis 1, 3-D, trans 1,3-D and CP in soil added 150ppm fumigants were 56%, 84%), 85%, 81%, 87% and 88%, respectively.

Cell Growth Inhibitory Effect of Tissue Cultured Root of Wild Panax ginseng C.A. Mayer Extract on Various Cancer Cell Lines

  • This study was performed to investigate the cell growth inhibitory effect of tissue cultured root of wild Panax ginseng C.A. Mayer (tcwPG). The human stomach carcinoma cell line, MKN 74, was incubated with 70% EtOH extract of tcwPG or Panax ginseng C.A. Mayer (PG) for 24 hrs. tcwPG inhibited cell growth at a concentration of $250{\mu}g/ml$. However, Panax ginseng extract did not inhibit cell growth at the same concentration. We also tested the ethyl acetate and $H_2O$ fractions of tcwPG. The inhibitory effect of the ethyl acetate fraction on cell proliferation in MKN 74 cells was more potent than that of the crude extract, and the inhibitory effect of the $H_2O$ fraction was less than that of the ethyl acetate fraction. When we separated tcwPG into polar and non-polar saponin fractions and then measured cell growth inhibition, the non-polar saponin in tcwPG exhibited cytotoxicity. To compare the effects of tcwPG on various cancer cell lines, we measured cytotoxicity in MKN 74 (stomach cancer cell line), SW 620 (colon cancer cell line) and PC 3 (prostate cancer cell line). All three cell lines showed cell growth inhibition, and the cell growth inhibitory effects were not quite different in the various cell lines. The non-polar saponins of tcwPG arrested PC 3 cells at G1-phase as did Panax ginseng.

The Removal of Dioxins and the Formation of 2, 3, 7, 8-TeCDF in Drinking Water Treatment in Japan (정수처리에서의 다이옥신 제거 및 2, 3, 7, 8-TeCDF 생성)

  • To evaluate homologue patterns and removal efficiency before and after water treatment, the concentrations of dibenzo-p-dioxins (PCDDs), polychlorinated dibenzofurans (PCDFs) and coplanar polychlorinated biphenyls (Co-PCBs) were determined in 122 samples from 42 drinking water treatment plants throughout Japan over a two year period. The mean concentrations and toxic equivalent (TEQ) values of dioxins in raw and treated waters were 60.24 pg/L (0.14 pg-WHO-TEQ/L) and 4.15 pg/L (0.016 pg-WHO-TEQ/L), respectively. The dioxins contribution ratio of drinking water in relation to dioxins tolerable daily intake (TDI, 4 pg-TEQ/kg/day) was 0.016%. The mean TEQ removal rate of dioxins by drinking water treatment was over 88%. However, the mean removal rate of 2, 3, 7, 8-TeCDF (tetrachlorodibenzofuran) by water treatment in the 122 samples was minus 17%. Therefore, to identify which process affected the level of 2, 3, 7, 8-TeCDF, the removal efficiencies at both the advanced and conventional water treatment plants were investigated. For the TEQ removal rate across the processes, the dioxin congeners, TeCDF and non-ortho-PCBs remarkably indicated minus values after chlorination in both the advanced and conventional water treatments plant. From this study, the level of 2, 3, 7, 8-TeCDF was found to be increased as a result of chlorination.
