Abstract
Objectives : This case study aims to report the effects of Korean medicine on a patient with idiopathic abducens nerve palsy and its associated symptoms, including strabismus, diplopia, nausea, and dizziness. Methods : A male patient with idiopathic abducens nerve palsy was treated using Korean medicine, which included herbal medicine, acupuncture, electroacupuncture, cupping, and pharmacopuncture. To evaluate the effects of this treatment, we employed the Visual Analog Scale (VAS), the Scott and Kraft score, photographs of eye movements, and a satisfaction survey for Korean medicine treatment. Results : After three months of treatment, all symptoms of abducens nerve palsy recovered to a VAS score of 0. Notably, the range of lateral gaze in the left eye increased from 1mm at the initial assessment to 7mm, now matching the range of the normal eye. No side effects were observed. Conclusions : The patient, who had not shown improvement with conventional treatment, exhibited rapid improvement after receiving Korean medicine. This demonstrates the effectiveness of Korean medicine in treating abducens nerve palsy.
본 논문은 특발성 좌측 외전신경 마비로 내사시, 복시, 어지럼증, 오심 등의 증상을 호소하는 환자에게 2024년 4월 2일부터 2024년 8월 28일까지 상지대학교 부속한방병원 한방안이비인후피부과에서 한약 치료, 침 치료, 황련해독탕(黃連解毒湯) 약침 치료, 전침 치료, 부항 치료를 시행한 치험례이다. 본 증례의 환자는 처음 증상 발생 후 약 4주간 양방 치료를 받았으나 전혀 호전 반응을 보이지 않아 본원에 내원하였다. 본원 초진 시 좌측 안구의 외전 범위 1mm로, Scott and Kraft score -3에 해당하는 심한 외전신경 마비 상태였다. 3개월 간 한의 치료를 시행한 후 복시, 어지럼증, 오심, 안구 불편감의 네 가지 증상이 모두 VAS 0으로 호전되었고, 좌측 안구의 외전 범위도 정상 측 안구와 동일한 7mm로 회복되었다. 이는 향후 외전신경 마비에 대한 한의 치료의 근거로 활용될 수 있을 것으로 사료되어 보고하는 바이다.
