의료 두뇌영상의 익명성 (Anonymity of Medical Brain Images)

  • 현재 사용되고 있는 두뇌영상의 제거 방법은 비록 환자의 개인 정보를 보호하고 있으나, 과도한 제거로 정확한 두뇌영상의 무결성을 손실할 수 있다. 원래 두뇌의 영상과 동일한 두뇌 조직을 나타내면서 환자의 신원을 감출 수 있는 새로운 익명화 얼굴모델을 생성시키는 방법을 연구하였다. 제안방법은 두 단계로 구성되었다: 10명의 두뇌영상을 정규화시켜서 모조 두뇌 표본 영상을 생성하는 단계와 실험영상 두뇌의 외곽부를 모조 두뇌의 안면부로 대체시키는 단계이다. 전체 두뇌영상에서 두피와 두개골 영역을 분할하기 위하여 레벨셋 알고리즘을 적용하였다. 영역화된 모조 두뇌를 대상 두뇌영상에 동일하게 배치하고 정규화를 시켜서 익명화된 얼굴 모델을 생성하였다. 원래 영상과 변형된 영상의 두뇌 조직부의 밝기 변화를 비교하여 제안 알고리즘의 타당성을 실험하였다. 실험 결과 두 두뇌영상은 두뇌 조직에서 완전히 동일하면서 신원을 파악할 수 없는 것을 검증하였다.

Incidence and Risk Factors for Leptomeningeal Carcinomatosis in Breast Cancer Patients with Parenchymal Brain Metastases

  • Objective : The objective of study is to evaluate the incidence of leptomeningeal carcinomatosis (LMC) in breast cancer patients with parenchymal brain metastases (PBM) and clinical risk factors for the development of LMC. Methods : We retrospectively analyzed 27 patients who had undergone surgical resection (SR) and 156 patients with whole brain radiation therapy (WBRT) as an initial treatment for their PBM from breast cancer in our institution and compared the difference of incidence of LMC according to clinical factors. The diagnosis of LMC was made by cerebrospinal fluid cytology and/or magnetic resonance imaging. Results : A total of 27 patients (14%) in the study population developed LMC at a median of 6.0 months (range, 1.0-50). Ten of 27 patients (37%) developed LMC after SR, whereas 17 of 156 (11%) patients who received WBRT were diagnosed with LMC after the index procedure. The incidence of LMC was significantly higher in the SR group compared with the WBRT group and the hazard ratio was 2.95 (95% confidence interval; 1.33-6.54, p<0.01). Three additional factors were identified in the multivariable analysis : the younger age group (<40 years old), the progressing systemic disease showed significantly increased incidence of LMC, whereas the adjuvant chemotherapy reduce the incidence. Conclusion : There is an increased risk of LMC after SR for PBM from breast cancer compared with WBRT. The young age (<40) and systemic burden of cancer in terms of progressing systemic disease without adjuvant chemotherapy could be additional risk factors for the development of LMC.

쥐에서 임신기, 수유기 및 이유후에 식이로 섭취한 어유가 뇌조직의 DHA 분포량에 미치는 영향 (Influence of Dietary Supplementation of Fish Oil at Different Life Cycle on the Incorporation of DHA into Brain in Rats)

  • The incorporation of docosahexaenoic acid(DHA) and arachidonic acid(AA) into brain and liver lipid has been compared in male pups from binth to 10 weeks old by feeding DHA-rich experimental diets or chow diets to dams from pregnancy in rats. The experimental DHA-rich diets contained 7g fish oil and 3g corn oil per 100g diet. There were three experimental groups, FO-I : Dams were fed DHA-rich diet during pregnancy and lactation, and their it pups fed the same diet until 10 weeks old. FO-II Dams fed chow diet during pregnancy and DHA-diet during lactation, and their pups fed the same DHA-diet until 10 weeks. FO-III : Dams fed chow diet during gestation and lactation, and then the pups fed DHA-diet after weaning. The relative % of DHA in hepatic lipid was about 12% with chow diets, but increased rapidly to 20-25% level when DHA-rich diets were supplied after weaning. The AA(%) of FO-III group was relatively high when a chow diet containing higher amount of linoleic acid was given, but there was no significant difference between the groups after feeding on a DHA-rich diet. When the DHA-rich diet was supplied from pregnancy(FO-I), the relative % of DHA in brain lipid was 13.7% at birth and continuously increased to a maximum level(17.2%) at 3-weeks and then was sustained until 5 weeks old. Similar levels of DHA incorporation were observed when DHA-rich diet was supplied from lactation(FO-II). However, the pups of FO-III group showed significantly lower levels of DHA incorporation(72%) at birth. These livels slowly increased and reached an 87% level of FO-I at 10 weeks when the pups ate DHA-rich diets after weaning. The relative % of AA in brain lipid was 10.4% in the FO-I group at birth, which was significantly lower than those of other groups, but there was no significant difference between groups after feeding DHA-rich diets in all groups. The Ah(%) level increased to maximum(11-12%) at 3-weeks and then was slightly reduced and was sustained at about 10% after S-weeks. Total amounts of DNA in the whole brain rapidly reached maximum level at 3-weeks and then was sustained at a constant level after S-weeks. DNA content was not significantly different between groups at birth, but it was significantly higher in FO-I and FO-II groups than in FO-III group at 3-weeks. However, DNA content in FO-III group was continuously increased to 80% level of FO-I at 10-weeks after feeding DHA-rich diet since weaning. In conclusion, the DHA(%) in whole brain was most effectively deposited when DHA-rich diet had been supplied during pregnancy and lactation in rats. However, DHA supplementation after weaning also improved the incorporaton of DHA into brain and content of DNA even though brain development was almost completed, which suggests that DHA supplementation might be necessary to improve brain development in humans during infancy as well as pregnancy and lactation.   • Rapid object recognition is one of the main stream research themes focusing to reveal how human recognizes object and interacts with environment in natural world. This field of study is of consequence in that it is highly important in evolutionary perspective to quickly see the external objects and judge their characteristics to plan future reactions. In this study, we investigated how human detect natural scene objects and categorize them in a limited time frame. We applied Magnetoencepahlogram (MEG) while participants were performing detection (e.g. object vs. texture) or basic-level categorization (e.g. cars vs. dogs) tasks to track the dynamic interaction in human brain for rapid object recognition process. The results revealed that detection and categorization involves different temporal and functional connections that correlated for the successful recognition process as a whole. These results imply that dynamics in the brain are important for our interaction with environment. The implication from this study can be further extended to investigate the effect of subconscious emotional factors on the dynamics of brain interactions during the rapid recognition process.

간호교육에서 뇌기반학습의 교수전략을 위한 통합적 고찰 (Brain-based Teaching Strategies for Nurse Educators: An Integrative Review)

  • Purpose: Brain-based learning has become increasingly important in nursing education. The purpose of this study is to (a) synthesize the literature on brain-based learning in nursing education using Whittemore and Knafl's integrative review method, and (b) discuss teaching strategies for nursing educators. Method: Searches were made through the CINAHL, OVID, PubMED, and SCOPUS databases using the terms $nurs^*$ AND (brain based OR neuroscience) AND ($educa^*$ OR $learn^*$ OR $teach^*$). Included were original articles in the domain of undergraduate nursing education written in English in peer-reviewed journals between January 1984 and December 2013. Twenty-four papers met the criteria. Results: Three themes were retrieved: (a) activate whole-brain functions, (b) establish supportive educational environments, and (c) encourage learners to be more active. Conclusion: Brain-based learning enhances the learning capabilities of undergraduate nursing students.

Gefitinib Alone or with Concomitant Whole Brain Radiotherapy for Patients with Brain Metastasis from Non-small-cell Lung Cancer: A Retrospective Study

  • Background: Gefitinib, a tyrosine kinase inhibitor (TKI) of epidermal growth factor receptor (EGFR), is used both as a single drug and concurrently with whole brain radiotherapy (WBRT) the standard treatment for brain metastases (BM), and is reported to be effective in a few small studies of patients with BM from non-small-cell lung cancer (NSCLC). However, no study has compared the two treatment modalities. This retrospective analysis was conducted to compare the efficacy of gefitinib alone with gefitinib plus concomitant WBRT in treatment of BM from NSCLC. Methods: We retrospectively reviewed 90 patients with BM from NSCLC who received gefitinib alone (250mg/day, gefitinib group) or with concomitant WBRT (40Gy/20f/4w, gefitinib-WBRT group) between September 2005 and September 2009 at Sun Yat-Sen University Cancer Center. Forty-five patients were in each group. Results: The objective response rate of BM was significantly higher in gefitinib-WBRT group (64.4%) compared with gefitinib group (26.7%, P<0.001). The disease control rate of BM was 71.1% in gefitinib-WBRT group and 42.2% in gefitinib group (P=0.006). The median time to progression of BM was 10.6 months in gefitinib-WBRT group and 6.57 months in gefitinib group (P<0.001). The median overall survival(OS) of gefitinib-WBRT and gefitinib alone group was 23.40 months and 14.83 months, respectively (HR, 0.432, P=0.002). Conclusion: Gefitinib plus concomitant WBRT had higher response rate of BM and significant improvement in OS compared with gefitinib alone in treatment of BM from NSCLC.

Analysis and Design of Whole-Head Magnetic Brain Stimulators: A Simulation Study

  • This paper proposes a helmet-type whole-head brain stimulator system considering a realistic head geometry. For more accurate design and computer simulations, a realistic volume conductor model was adopted and the current evoked on human cerebral cortex was analyzed using the boundary element method (BEM). To obtain a more focalized evoked current around the target points, various coil configurations were tested and an average targeting error of about 10 mm was obtained.

Primary Central Nervous System Lymphoma in Organ Recipient

  • We report a case of primary central nervous system(CNS) lymphoma in an organ recipient. A 33-years-old man who underwent a renal transplantation 3years previously presented with headache and vomiting. In Brain computed tomography scans and magnetic resonance images showed multiple periventricular cystic rim enhancing masses. Pathologic diagnosis by stereotactic biopsy revealed malignant non-Hodgkins B-cell lymphoma. After pathologic confirmation, methotrexate chemotherapy and whole brain radiation therapy were done. Having experienced such a case, the authors strongly recommend to add primary CNS lymphoma as one of the differential diagnoses to brain abscess, metastatic brain tumor and glioblastoma multiforme in cases of multiple ring enhancing periventricular lesions of immunocompromised patient or organ recipient.

전이성 뇌종양의 방사선치료 성적 (Radiation Therapy for Brain Metastases)

  • One hundred and twenty patients with brain metastases were seen and evaluated in the Dept. of Therapeutic Radiology, Seoul National University Hospital between 1979 and 1983. Of these, 90 Patients received whole brain irradiation with 2,000 ra4 in 1 week or 3,000 rad in 2 weeks for Palliative Purpose and 30 patients failed to complete the planned treatment. Carcinoma of the lung(44 cases), choriocarcinoma(11 cases), breast(8 cases) were common Primary tumors of 90 patients receiving planned treatment. Symptomatic subjective response was obtained in $92\%$ of Patients and meurologic functional improvement was obtained in $42\%$ of patients. Median survival was 6.4 months in patients with complete treatment an·d less than 2 months in Patients with incomplete treatment, overall survival rate at 1 year and 2 year were $26\%,\;16\%$ in Patients with complete treatment and $8\%,\;0\%$ in patients with incomplete treatment. Primary site, extent of metastases and interval from diagnosis of primary tumor to brain metastases were identified as prognostic factors.

기능적 자기공명영상을 이용한 뇌활성화 연구 (Neuroactivation studies using Functional Brain MRI)

  • Functional MRI (fMRI) provides an indirect mapping of cerebral activity, based on the detection of the local blood flow and oxygenation changes following neuronal activity (Blood Oxygenation Level Dependent). fMRI allows us to study noninvasively the normal and pathological aspects of functional cortical organization. Each fMRI study compares two different states of activity. Echo-Planar Imaging is the technique that makes it possible to study the whole brain at a rapid pace. Activation maps are calculated from a statistical analysis of the local signal changes. fMRI is now becoming an essential tool in the neurofunctional evaluation of normal volunteers and many neurological patients as well as the reference method to image normal or pathologic functional brain organization.
