조헌영의 『동양의학총서(東洋醫學叢書)』에 대한 의사학적 연구 (A Historical Study on Cho Heon-yeong's 『Eastern Medicine Series (東洋醫學叢書)』)

  • This study is about Cho Heon-yeong's 『Eastern Medicine Series (東洋醫學叢書)』. It consists of 5 clinical books, 『Therapy for People (民衆醫術理療法)』, 『Tuberculosis Eastern Medical Treatment (肺病漢方治療法)』, 『Neurasthenia Treatment (神經衰弱症治療法)』, 『Gastrointestinal Disease Treatment (胃腸病治療法)』 and 『Gynecological Treatment (婦人病治療法)』, which were published between 1935-1941. This series mainly succeeded 『Donguibogam (東醫寶鑑)』, and was influenced by 『Kyeongakjeonseo (景岳全書)』, and 『Hwangjenegyeong (黃帝內經)』. Cho Heon-yeong's medical philosophy appears in two ways. First, he emphasized invigoration in treatment for the people who lacked nutrition and medical care at the time. Second, eclecticism of Korean Medicine and Western medicine is specifically revealed through this series. He aimed for a comprehensive medicine that consists mainly of Korean medicine and includes only a part of Western medicine.

일부 한.양방병원 뇌혈관질환 환자의 진료결과 및 만족도의 비교연구 -한양방협진 진료프로토콜의 적용을 중심으로- (The Clinical Interchange between Western Medicine and Oriental Medicine: with the Stroke Patient Outcomes Research)

  • Objectives : This study was done to assess the effects of the clinical interchange between the Western Medicine and the Oriental Medicine for ischemic stroke patients. The patient outcomes include changes in neurologic function by modified NIH stoke scale, stroke pattern identification scale, and patient satisfaction, Methods : For the assessment of effects, this study was performed with 178 inpatients who had undergone the stroke care at three hospitals (W Hospital adopted western therapy, S Oriental Hospital adopted Sasang constitution medicine therapy, and H Oriental Hospital adopted mixed therapy according to a joint protocol on Western Oriental medical care) from November 1997 to December 1998. Patients were interviewed or written with self-entered questionnaire forms, and clinical data were obtained, Physicians or oriental doctors wrote clinical questionnaire forms according to the care process. Results : The patient outcomes within three hospitals at 2 stages (at admission and discharge in the modified NIH stroke scale. at admission and second weeks during admission in the stroke pattern identification scale) were found to be decreased, Especially in the results of hierarchical multiple regression analysis, the degree of improvement of modified NIH stroke scale of the stroke patients at W Hospital was significant large than it at S Oriental Hospital. Also, the degree of improvement of stroke pattern identification scale at W Hospital was significantly large than it at other two hospitals. However, the patient's satisfaction score at three hospitals wasn't significantly different. Conclusions : The result of this study suggested that the joint clinical research of Western & Oriental medical practitioners was possible even if there was a conflict between Western Medicine and Oriental Medicine. Therefore Western & Oriental medical practitioners share a mutual responsibility to apply evidence-based practice, to seek scientific empirical proof through randomized clinical trials between the multicenter.

의과대학부속병원 의사들의 양.한방 협진에 대한 인식 (Recognition of Medical Doctors Employed to University Hospital on the Cooperative Medical System between Korean Traditional and Western Medicines)

  • Objectives : This study was aimed to promote the cooperative system of Korean and Western medicine in the dual health care system through a survey of physicians on recognition, problems and solution of the cooperative system. Methods : The research took place at Dongguk University Hospital from May 25 to 27, 2009 with 44 professors, residents, and interns employed by the hospital. Results : Of total 44 surveyed doctors, positive and moderate responses on the cooperative system between Korean and Western medicines were 40.9% and 43.2%, respectively. They scored it positive (62.5%) and moderate (31.3%) based on their experiences. These results can be supposed to represent the environment for the interdisciplinary medicine. Even in the interdisciplinary hospital of Korean and Western medicine, 68.2% of responders had no experience of the cooperative medical system. Expected interdisciplinary efficient departments were ordered rehabilitation medicine, neurology and orthopedics and associated with musculoskeletal disorders, the most frequent diseases treated by Korean traditional medicine. Conclusions : Korean and Western medical doctors, as medical personnel, intellectual persons as well as specialists, need a recognition and attitude to understand and respect each others' medicine. However, both groups also realize there are many complicated issues in the treatment of patients. It is difficult to require a change of Western doctors' perceptions and attitude toward Korean traditional medicine only with results of a survey. For the efficient cooperative system in the medical field, Korean medical doctors will need to study and consider specific problems mentioned by Western medicine.

요양병원의 한방치료에 대한 분석 연구 (Analysis Study of the Korean Medical Treatments in Care Hospital)

  • Objectives : The purpose of this study is to analyze the Korean medical treatments in care hospitals and to provide the fundamental information for Korean medical policy. Methods : Data were collected from the patients who had been taken Korean Medicine in 00 Care Hospital from Jan. 1. 2011 to Dec. 31 2012, and their medical records werere viewed retrospectively. In addition, 42 inpatients were recruited and responded to the survey. Results : Major results of the analyses are as follows; 91.0 % of Western medical inpatients were treated with Korean medicine. And medium medical care group is 38.8 % among the inpatients that are treated with Korean medicine. Especially, musculoskeletal disease and stroke disease are treated more frequently at the rate of 43.9 %, 41.2 %. 92.8 % of patients responded to the survey were content to the Korean medical treatments. Conclusions : The result of this study shows that patients prefer Korean medicine, and it is necessary to supplement the Korean medical policy.

동북아시아 4개국의 양.한방 의료협진체계 비교 (A comparative Study on the Combined Oriental and Western Medicine(COWM) in Four Northeast Countries)

  • Since 1990s, the use of Complementary and Alternative Medicine(CAM) has been rising rapidly all of the world. In 1983, WHO recommended that the traditional medicine actively be utilized. At the end of 20th century, as chronic and intractable diseases increased in western countries, traditional medicine has attracted considerable attention. COWM shows possibilities of new approaches for these intractable diseases. Thus, we try to show our proper approach of COWM through the international comparative study. In order to fulfill the objectives, we applied the following methodology: 1) Literature review on previous study, 2) Local survey using self-administered questionnaire, and 3) FGI(Focus Group Interview) with local experts. The results were as follows : Three Asian countries, China, Korea and Taiwan, are very active in implementing COWM policy. Japan, however, has independent system of unified medicine. In regards to the combined care policy and system, China has the most advanced COWM system among four countries. In respect to combined care education, it is needed to increase the COWM education contents and the amount of cross educational curriculum. Based on the current COWM system, Chinese, Japanese and Taiwanese doctors can prescribe both oriental and western drugs. But, Korean medical law prohibits western doctors and oriental doctors from prescribing the counterpart´s medicine. So, the revision of current medical law is urgent for COWM in Korea. And when it comes to patient satisfaction, more than fifty percent responded positively in China, Korea and Taiwan. To achieve the goal of COWM ; 1) mutual understanding and recognition of COWM is essential. 2) institutional and legal support system for COWM is desperately urgent. 3) possible international collaboration and cooperation should be sought to untangle these complex cultural dilemmas.

Has Income-related Inequity in Health Care Utilization and Expenditures Been Improved? Evidence From the Korean National Health and Nutrition Examination Survey of 2005 and 2010

  • Objectives: The purpose of this study is to examine and explain the extent of income-related inequity in health care utilization and expenditures to compare the extent in 2005 and 2010 in Korea. Methods: We employed the concentration indices and the horizontal inequity index proposed by Wagstaff and van Doorslaer based on one- and two-part models. This study was conducted using data from the 2005 and 2010 Korean National Health and Nutrition Examination Survey. We examined health care utilization and expenditures for different types of health care providers, including health centers, physician clinics, hospitals, general hospitals, dental care, and licensed traditional medical practitioners. Results: The results show the equitable distribution of overall health care utilization with pro-poor tendencies and modest pro-rich inequity in the amount of medical expenditures in 2010. For the decomposition analysis, non-need variables such as income, education, private insurance, and occupational status have contributed considerably to pro-rich inequality in health care over the period between 2005 and 2010. Conclusions: We found that health care utilization in Korea in 2010 was fairly equitable, but the poor still have some barriers to accessing primary care and continuing to receive medical care.

폐암 세포에서 Proteasome Inhibitor에 의한 Apoptosis의 기전 (The Mechanism of Proteasome Inhibitor-Induced Apoptosis in Lung Cancer Cells)

  • 연구배경 : Proteasome 억제가 세포의 apoptosis에 미치는 영향은 세포 종류에 따라 차이를 보이고 있다. 그러나 아직까지 폐암 세포에서 proteasome 억제가 미치는 효과 및 그 기전에 관해서는 확실하게 규명되어 있지 못한 상태이다. 본 연구에서는 폐암 세포주에서 proteasome 억제에 의한 apoptosis의 유도 유무를 관찰하고, 그 기전을 규명하고자 하였다. 방 법 : 폐암 세포주인 A549와 NCI-H157 세포에 proteasome 억제제인 MG132와 PS-341을 투여하고 세포의 생존율을 MTT 분석으로 평가하였고, apoptosis 유무를 PARP 단백에 대한 Western 분석으로 확인하였다. Proteasome 억제가 caspase 3와 JNK의 활성화에 미치는 효과를 각각 Western 분석과 immunocomplex kinase 분석으로 평가하였다. Proteasome 억제제를 투여하고 항 apoptosis 경로인 ERK와 cIAP1에 미치는 효과를 Western 분석으로 평가하였다. 결 과 : A549와 NCI-H157 세포에 MG132를 투여하였을 때, 세포생존율의 감소가 관찰되었고, 이는 apoptosis의 유도에 의한 것으로 확인되었다. MG132와 PS-341 처치로 caspase 3와 JNK가 활성화되었다. 반면에 활성화된 ERK의 발현과 cIAP1의 발현은 감소하였다. 결 론 : 폐암 세포에서 proteasome 억제에 의한 apoptosis에는 caspase 3, JNK와 같은 apoptosis 유도 경로의 활성화와 함께 항 apoptosis 경로의 억제가 관여할 것으로 생각된다.

"의림(醫林)"지(誌)를 통해 살펴본 1950년대 동서의학 논쟁 (The Medical Dispute between Korean and Western Medicine in 1950s Examined through "EE LIM")

  • Objective: Although the government acknowledged Korean Medical Doctors as official health care providers since 1951, criticism regarding its modernity persisted. This paper focuses on the medical dispute between Korean and Western Medicine in 1950s risen from the problem of modernization of Korean Medicine. Method: This paper looked into the medical dispute between Korean and Western Medicine in 1950s through "EE LIM". "EE LIM" is a scholarly journal published by Korean Medical doctor Bae Won-Sik. Articles regarding the medical dispute between Korean and Western Medicine were selected, categorized and analyzed. Result: The main questions against Korean Medicine were: whether it was scientific or not, and whether there was a possibility of systematization. Several opinions were brought up on these issues. 1) Some people criticized Korean Medicine as being unscientific based on modern science. 2) Others emphasized the identity of Korean Medicine against the critics. 3) Another group of people wanted to study Korean Medicine using modern methods and obtain merits from both sides. Conclusion: The different understanding of the term 'science' led to diverse discussions on the direction of Korean Medicine's progression. The narrow meaning of science only considers Korean Medicine as an object of modern scientific and medical study. But if the meaning is broadened, containing empirical aspects, Korean Medicine can be included as a category of science based on its clinical utility.

협진 의뢰 산모의 한방산후조리 현황 분석 및 만족도 조사 (A Clinical Study of Postpartum Care in Oriental-Western Cooperative Medicine and Survey of Patients' Satisfaction)

  • Purpose: We analyzed the characteristics of women who gave birth at an obstetrician and those who received oriental medicine postpartum care, and investigated the level of their satisfaction to obtain fundamental data for Oriental-Western cooperative medicine treatment for postpartum care. Methods: The review was based on a 5-months' data, from August 1 through December 31, 2009. Information on age, labor record, level of education, and occupation of the patients who had received postpartum oriental-western cooperative medicine treatment was based on Electric Medical Record(EMR) of O O Oriental Medicine Hospital. The questionnaires were completed by the patients at the time of their discharge from the hospital. Results: The patients who were admitted to oriental medicine hospitals tended to be primiparae, and they were more likely to be younger women with higher education levels. The level of satisfaction on postpartum care program of oriental medicine hospitals was high. Conclusion: It is necessary to specialize the facilities, dietary formula, and staff training of oriental medicine hospitals through a survey of postpartum patients' demand and satisfaction level. In oriental-western cooperative medicine hospitals, it is needed to specialize the postpartum program for creating demand of premature delivery mothers.

한방의료의 이용행태 및 이용결정요인 분석 - 일부 대도시 지역주민을 중심으로 - (Utilization Patterns and Determinants of Oriental Medical Services : Focused on the Residents of Taegu City)

  • This study was conducted to examine utilization patterns and determinants of oriental medical services. Data were collected from 545 residents in Taegu city The results of this study are summarized as follows 1) 37.8% of subjects used oriental medical services in the past year. Especially, the female, the ages of 50 and 60 over, the single. low-educated. high-income class, white-collar class, medical insured tended to use more oriental medical services than another groups. 2) 46.7% of users of oriental medical services reported that number of visits in the past year was 2 - 5 times 37 1% of them was 1 times, and 4.5% over 10 times. 3) According to the reasons to choose the oriental medical facilities, most was 'on their own judgement'(48.8%) and 'by the advice of relatives and friends'(42.0%) Regarding to the objectives of using oriental medicine, 68.3% was 'treatment', 31.7% 'health counselling and promotion'. And among diseases of users, diseases of musculo-skeletal system was the highest(54.5%). 4) 57.9% of oriental medical services users had experience of utilizing western medicine on the same diseases. Among peoples with experiencing western medicine on the same diseases. 54.4% received oriental medical services 'in addition to western medicine', 45.6%'in place of western medicine 'And 41.2% of using both services reported that they had difficulty in deciding to choose the type of services -oriental medical services or western medicine-for their diseases. 37.3% of them answered that 'providing relevant information' was the most desirable measure to solve this problem, 27.3% 'establishment of effective referral system between oriental and western medical facilities '23.6% 'cooperative medical treatment systems in the same facilities', 11.8%'integration of oriental medicine into western medicine 5) According to the satisfaction level with each items of oriental medical services, the respondents had positive views on efficacy, kindness, and side-effects. They, however, had negative view on the cost of oriental medical services. 6) In regarding to the priority of improvement of oriental medical system,'expansion of insurance benefit package 'ranked first. 'expansion of insurance benefit Package 'ranked second, 'improvement of scientific methods and diagnostic technique 'third, and 'safety of herbal medicine' fourth in order. 7) The significant factors influencing the utilization of oriental medical services were kindness of oriental medical practitioners, efficacy , travel time, age To be brief, utilization rate of oriental medical services in urban area generally tends to be high. There, however, have been various barriers to limit oriental medical services, such as incomplete benefit package of oriental medical insurance and lack of coordination and referral system between oriental and western medical services, lack of scientific diagnostic procedures, high price etc . For the development of oriental medical services, Much attention to remove these limiting factors should be placed. In addition, kindness of oriental medical practitioners , which is expected to be more important factor in the consumer - focused health care environment than ever, should be kept high consistently. Since this study was conducted for specific residents of an urban city. further research including more sampling in different urban areas should be required to generalize the results of the study.
