Short-course palliative radiotherapy for uterine cervical cancer

  • Purpose: The purpose of this retrospective study was to evaluate the efficacy and feasibility of short-course hypofractionated radiotherapy (RT) for the palliation of uterine cervical cancer. Materials and Methods: Seventeen patients with cancer of the uterine cervix, who underwent palliative hypofractionated 3-dimensional conformal radiotherapy between January 2002 and June 2012, were retrospectively analyzed. RT was delivered to symptomatic lesions (both the primary mass and/or metastatic regional lymph nodes). The total dose was 20 to 25 Gy (median, 25 Gy) in 5 Gy daily fractions. Results: The median follow-up duration was 12.2 months (range, 4 to 24 months). The median survival time was 7.8 months (range, 4 to 24 months). Vaginal bleeding was the most common presenting symptom followed by pelvic pain (9 patients). The overall response rates were 93.8% and 66.7% for vaginal bleeding control and pelvic pain, respectively. Nine patients did not have any acute side effects and 7 patients showed minor gastrointestinal toxicity. Only 1 patient had grade 3 diarrhea 1 week after completion of treatment, which was successfully treated conservatively. Late complications occurred in 4 patients; however, none of these were of grade 3 or higher severity. Conclusion: Short-course hypofractionated RT was effective and well tolerated as palliative treatment for uterine cervical cancer.

Depletion of the Pre-RC Proteins Induces Chk1/Chk2 Independent Checkpoint Responses and Apoptotic Cell Death in HeLa Cells

  • The initiation of eukaryotic DNA replication requires assembly of the pre-replicative complex (Pre-RC) through the concerted action of Orc, Cdc6, Cdt1 and Mcm2-7 complex during G1 phase. The pre-RC assembly licenses individual replication origins for the initiation of DNA replication and sufficient number of the pre-RC is essential for proper progression of S phase. However, it is not well known how cells recognize the completion of the pre-RC assembly before G1-S transition. In order to understand the cellular responses to the defects in pre-RC assembly, we depleted the known components of pre-RC proteins using the small interference RNAs in HeLa cells. Although the defects of pre-RC assembly by the depletion of the pre-RC proteins such as Orc2, Cdt1, Mcm2 & Mcm10 did not elicit the activation of Chk1- or Chk2-dependent checkpoint pathways, these cells still showed significant decrease in the cellular level of Cdc25A proteins. These results suggests that a novel checkpoint pathway exist in HeLa cells, which is not dependent upon Chk1 or Chk2 proteins and play essential roles in the cellular responses to the defects in the pre-RC assembly. Also, among those four proteins tested in this study, the depletion of Mcm10 and Cdt1 proteins significantly increased the apoptotic cell death in HeLa cells, suggesting that these proteins not only play roles in the pre-RC assembly, but also are involved in the checkpoint responses to the defects in the pre-RC assembly.

The Effect of Mindfulness Training on the Job Stress, Burnout, Teacher Efficacy of Middle School and High School Teachers (마음챙김 훈련이 중등교사의 직무스트레스와 소진, 교사효능감에 미치는 영향)

  • This study examines the effects of mindfulness training on job stress, burnout, and teacher efficacy for middle school and high school teachers. For this study, twenty-four middle school and high school teachers were divided into experimental and control groups. Data analysis was performed using a Mixed ANOVA for teacher's job stress, burnout, and teacher efficacy according to pre-training, post-training and delayed post-training periods. As a result, we found support for time interval and between-group interaction effects for all the dependent variables. In order to investigate any delayed effects, a t-test was performed to analyze measurements taken one month after the completion of training period. The delayed post-training results indicated a significant effect between pre and post scores, meaning the program effects were sustained. Therefore, this study indicates that the mindfulness training program implemented in this research is an effective method for preventing job stress and burnout, as well as influencing the efficiency of work performance for middle school and high school teachers.

A CONTROLLED CYCLIC LOADING ON THE SURFACE TREATED AND BONDED CERAMIC: STAIRCASE METHOD

  • STATEMENT OF PROBLEM: Effect of surface treatment of ceramic under loading does not appear to have been investigated. PURPOSE: The aim of this study was to investigate the effect of surface treatment of esthetic ceramic, which is performed to increase the bonding strength, on the fracture stress under controlled cyclic loading condition. MATERIAL AND METHODS: Sixty 1.0 mm-thick specimens were made from Mark II Vitablocs (Vita Zahnfabrik, Germany) and divided into 3 groups: polished (control), sandblasted, and etched. Specimens of each group were bonded to a dentin analog material base including micro-channels to facilitate the flow of water to the bonding interface. Bonded ceramics were cyclically loaded with a flat-end piston in the water (500,000 cycles, 15Hz). Following completion of cyclic loading, specimens were examined for subsurface crack formation and subsequent stress was determined and loaded to next specimen by the staircase method according to the crack existence. RESULTS: There were significant differences of mean fatigue limit in the sandblasted (222.86 ${\pm}$ 23.42 N) and etched group (222.86 ${\pm}$ 14.16 N) when compared to polished group (251.43 ${\pm}$ 10.6 N) (P<.05; Wald-type pair-wise comparison and post hoc Bonferroni test). Of cracked specimens, surface treated group showed longer crack propagation after 24 hours. All failures originated from the radial cracking without cone crack. Fracture resistance of this study was very low and comparable to failure load in the oral cavity. CONCLUSION: Well controlled cyclic loading could induce clinically relevant cracks and fracture resistance of Mark II ceramic was relatively low applicable only to anterior restorations. Surface treatment of inner surface of feldspathic porcelain in the matsicatory area could influence lifetime of restorations.

  • Becoming construction projects larger and more complicated, and utilizing more varied and complex contracting and delivery systems in the industry, integrating information and managing constructability from project inception to completion became the most critical tasks for the successful project management. However, current industry practices of constructability analysis which are too much dependent on field engineers and managers' experiences, are not performed efficiently and also not managed effectively. Then the scope, method, and procedure in performing analysis tasks are not certain as well as not systematic, In addition, due to using 2D based drawings and documents, information required at project phases for the analysis is inconsistent and inaccurate. This study aims to suggest the use of BIM technology and functions for solving various problems of current constructability practices. In particular, constructability analysis tasks performed at design and construction stages are identified and their adaptability, applicability, and benefits of BIM are investigated through interviews on constructability and BIM professionals in the industry.

Orthodontic and orthopedic treatment for a growing patient with Tessier number 0 cleft

  • The purpose of this case report was to introduce the concept of orthodontic and orthopedic treatment for a growing patient with Tessier number 0 cleft. A 5-year-old boy patient with Tessier number 0 cleft presented congenitally missing maxillary central incisors (MXCI), a bony defect at the premaxilla, a constricted maxillary arch, an anterior openbite, and maxillary hypoplasia. His treatment was divided into three stages: management of the bony defect at the premaxilla and the congenitally missing MXCIs using a fan-type expansion plate, iliac bone grafting, and eruption guidance of the maxillary lateral incisors into the graft area for substitution of MXCIs; management of the maxillary hypoplasia using sequential facemask therapy with conventional and skeletal anchorage; and management of the remaining occlusal problems using fixed orthodontic treatment. The total treatment duration was 15 years and 10 months. Class I canine and Class II molar relationships and normal overbite and overjet were achieved at the end of treatment. Although the long-term use of facemask therapy resulted in significant protraction of the retrusive maxilla, the patient exhibited Class III profile because of continued mandibular growth. However, the treatment result was well maintained after 2 years of retention. The findings from this case suggest that interdisciplinary and customized approaches are mandatory for successful management of maxillary hypoplasia, bony defect, and dental problems in Tessier number 0 cleft. Moreover, considering the potential of orthognathic surgery or distraction osteogenesis, meticulous monitoring of mandibular growth until growth completion is important.

Performance of DCTCP with per-packet scheduling in data center networks (데이터센터 네트워크의 패킷단위 스케줄링에서의 DCTCP 성능)

  • Per-packet scheduling is more suitable than per-flow scheduling to reduce the flow completion time by efficiently utilizing resources in data center networks. Recently, many per-packet scheduling schemes utilizing multiple paths have been proposed. However, to mitigate the negative effect of packet reordering on TCP performance, most of the schemes require supplemental measures such as putting packets in order at the lower layer. In this study, we investigate how well DCTCP, which is a representative TCP for data center networks, performs with per-packet scheduling through simulation. Simulation results show that DCTCP keeps the queue length short but that DCTCP shows low fairness due to the way of reducing the congestion window by ECN.

  • Fur clothing as fashion items is expanding into casual & ladies's wear market as well as fur market. The interest of fur fashion is rising. So this study compares and analyzes the merchandising process of textile clothing also it of fur clothing by merchandising steps. Fur clothing has so many change factors at time of purchase by scarcity of raw material, price fluctuation, exchange rate and others. Therefore it is primarily about securing of raw material. As soon as plan of product is finished, the purchase of fur raw material has to be started while progress of design products for commercializing the fur clothing. The design of fur clothing is consist of material design, color design and shape design. And It makes a new trend & market as we are developing new & various treatments. The some of imported materials are transferred to the factory for being treated first dressing, fabric treatments and dyeing processing according to the design. The first treated materials are transferred to the sewing factory again for secondary treatments and finally inspected and shipped. During secondary treatments the fur has gone through various manufacturing process for using like fabric materials and it takes long time because almost work is running manually. Unlike fabric clothing, fur clothing's manufacturing method is complicated and various from material process to shape process as per feature. Therefore the merchandising with fur cannot make mass production also needs detail craftsmanship depending by expert's skills. On this wise the fur clothing takes long time to the completion thus it has been risky and costly.

A Case of Radiation Bronchitis Induced Massive Hemoptysis after High-Dose-Rate Endobronchial Brachytherapy

  • High-dose-rate endobronchial brachytherapy (HDREB) have been used as the treatment of early endobronchial cancer, as well as for palliation of advanced cancer. However, fatal hemoptysis can occur after HDREB at the rate of 7~32%. We report a case of massive hemoptysis due to radiation bronchitis developed after HDREB. A 67-year-old man was treated with HDREB for early endobronchial cancer on the left upper lobe bronchus. He complained of persistent cough from 4 weeks after completion of HDREB. Radiation bronchitis was observed on the bronchoscopy at 34 weeks, and it was progressed from mucosal swelling and exudate formation to necrosis and ulceration without local relapse. In addition, he died of massive hemoptysis after 15 months. The patient had no sign or radiologic evidences to predict the hemoptysis. This case implies that HDREB directly contributes to an occurrence of a fatal hemoptysis, and follow-up bronchoscopy is important to predict a progression of radiation bronchitis and fatal hemoptysis.

Assessment of Nurses' Nutritional Knowledge and Educational Needs Regarding Stroke Specific Diet Regimens (간호사의 뇌졸중 관련 영양지식 및 영양교육 요구도 조사)

  • Purpose: Although the prevalence rate of chronic diseases is rapidly increasing due to an unhealthy diet in Korea, nurses may not have enough nutritional knowledge to educate their patients. This study investigated the level of nurses' nutritional knowledge for chronic diseases as well as for strokes, and the needs for nurses' nutritional education. Methods: This research is a descriptive research conducted from April to August of 2014, on 242 nurses who work with stroke patients, in two tertiary general hospitals in the city of Seoul, South Korea. Results: The average nutritional knowledge score of our subjects was $19.9{\pm}2.51$ (range 12-24). The correct response rate was 83% which was higher than expected. However, many nurses (31-66%) answered incorrectly on items such as 'Drinking low-fat milk is better than whole milk', and 'Seasoning with a large amount of soy sauce instead of salt can reduce sodium consumption'. We also found that nurses who received continuing education regarding nutrition scored higher than those who did not (p=.020). There was no correlation between nutritional knowledge and the need for nutritional education (r=.034, p=.601). Conclusion: The level of nutritional knowledge of the nurses was relatively low and irrelevant to the completion of nutrition courses during university or duration of experience. Rather, the relevance was higher when the nurse received nutrition-related training after graduating from university. Our results imply that continuous nutritional education is necessary for hospital nurses.
