소아 모야모야병에서 뇌확률지도를 이용한 수술전후 혈역학적 변화 분석 (Assessment of Cerebral Hemodynamic Changes in Pediatric Patients with Moyamoya Disease Using Probabilistic Maps on Analysis of Basal/Acetazolamide Stress Brain Perfusion SPECT)

  • 목적: 본 연구의 목적은 소아 모야모야병 환자에 있어서, 수술 전후 기저/아세타졸아미드 부하 뇌혈류 단일광자방출 단층촬영(SPECT) 분석에 확률뇌지도를 이용하여 수술로 인한 뇌혈역학적 변화 및 예후 예측인자 분석을 하여 뇌확률지도의 유용성을 평가하는 것이다. 대상 및 방법: 연구대상으로 서울대어린이 병원에서 소아 모야모야병으로 진단받고, 수술받은 56명(남:여=32:24, 나이 $6.7{\pm}3.2$세)이 포함되었다. 각각의 환자는 기저/아세타졸아미드 부하 뇌혈류 SPECT를 수술 전후 6-12개월 사이에 시행하였다. 각각의 환자는 한 측 반구에 encephalo-duro-arterio-synangiosis (EDAS)와 encephalo-galeo-synangiosis (EGS)를 우선적으로 시행받았고, 그 후 순차적으로 반대측 반구에 EDAS를 시행하였다. 환자들은 수술 후 $33{\pm}21$개월 추적 관찰하였다. 환자들의 SPECT 영상을 SPM에서 공간정규화 하고 뇌교의 계수를 기준으로 계수정규화한 후 한국표준확률뇌지도 (Koreans Statistical Probabilistic Map, K-SPAM)를 이용하여 부위별 혈류를 정량화 하였다. 각각 정류화된 혈류를 수술 전후, 대뇌반구간, 그리고 임상결과에 따라 비교하였다. 또한 임상결과가 좋은 군과 나쁜 군 사이에 차이가 있는 요소를 이용하여 회귀분석을 시행하였다. 결과: 수술 후 양측 내측 전두엽이랑, 전두엽, 두정엽, 측두엽, 내측경동맥 영역, 전뇌의 기저/아세타졸아미드 부하 뇌혈류가 유의하게 호전되었다(p<0.05). 대뇌반구간 비교에서는 수술전 기저/아세타졸아미드 부하 뇌혈류 및 혈류예비능지표에 차이가 있었으나, 수술 후 이 차이는 사라졌다(p<0.05). 임상결과가 좋은 환자군의 수술 전 EDAS와 EGS를 시행한 내측 전두엽 뇌이랑의 기저 뇌혈류, 동측 전두엽, 측두엽, 그리고 전뇌의 수술 후 혈류예비능지표 및 수술전후 혈류예비능지표차가 더 우수하였다(p<0.05). 회귀분석결과에 의하면, EDAS와 EGS를 시행한 내측 전두엽 뇌이랑의 수술 전 혈류예비능지표와 전뇌의 수술 전후 혈류예비능지표차가 수술 후 임상결과를 예측할 수 있는 유의한 인자이다(p=0.002, p=0.015). Conclusion: 뇌확률지도를 이용하여 소아 모야모야병 환자의 기저/아세타졸아미드 부하 뇌혈류 SPECT를 정량화하여 분석할 수 있었다. 이 방법으로 수술에 의한 뇌혈류역학적 변화를 객관적으로 평가할 수 있었으며, 모야모야병환자의 수술 결과의 예측인자를 평가할 수 있었다.

Abscisic acid 농도에 따른 밀 종자의 발아와 단백질체의 발현 특성 (Germination and Proteome Profile Characteristics of Wheat Seeds Treated under Different Concentrations of Abscisic Acid)

  • 휴면성이 다른 백중밀, 금강밀, 우리밀 후숙종자의 ABA 농도에 따른 발아 및 배아에서의 단백질체 발현 특성을 조사한 결과는 다음과 같다. 1. 백중밀, 금강밀, 우리밀 등 3품종의 0, 10, 30 및 $50{\mu}M$ ABA에서의 평균 발아지수와 발아율은 각각 0.95와 98% 이상으로 통계적으로 유의한 차이가 없었다. 유아와 유근의 생장은 $0{\mu}M$ ABA보다 10, 30 및 $50{\mu}M$ ABA에서 생장이 크게 억제되었는데, ABA 농도가 높을수록 생장이 더 억제되었다. 2. 3품종 배아의 평균 ABA 함량은 $0{\mu}M$ ABA와 $50{\mu}M$ ABA에서 각각 0.78 ng/mg과 269.04 ng/mg으로서 농도에 따른 ABA 함량의 차이가 컸다. 3. $0{\mu}M$ ABA 처리구에 비하여 $50{\mu}M$ ABA 처리구에서 발현양이 증가한 단백질 spot (S1, S3, S4, S6, S15, S16, S17)은 7개였으며, 감소한 단백질 spot (S2, S5, S9, S10, S11, S12, S13, S14, S18)은 8개였고, 증가와 감소가 동시에 이루어진 단백질 spot (S2)은 1개였다. $50{\mu}M$ ABA에서만 검출된 단백질 spot (S7, S8)은 2개였다. 4. 각 단백질 spot의 $0{\mu}M$ ABA 처리구 양에 대한 $50{\mu}M$ ABA 처리구 양의 평균 배수 값(fold 값)이 1.5배 이상으로 증가한 단백질 spot은 S1 (globulin-3A), S6 (globulin-1 S allele), S16 (globulin-1 S allele), S17 (globulin-1 S allele) 등으로 모두 globulin류 단백질이었다. 또한 $50{\mu}M$ ABA에서만 확인된 단백질 spot인 S7 (globulin 3)과 S8 (globulin-1 S allele)도 globulin 단백질이었다. 5. 각 단백질 spot의 $0{\mu}M$ ABA 처리구 양에 대한 $50{\mu}M$ ABA 처리구 양의 평균 배수 값(fold 값)이 0.7 이하로 감소한 단백질 spot은 S10 (glutamine sysnthetase cytosolic isozyme), S12 (S-adenosylmethionine synthetase 2), S14 (isocitrate dehydrogenase NADP)이었다. 이상의 결과는 ABA에 의한 밀 유묘의 유아와 유근의 생장억제는 배아에서의 ABA 농도 증가, 그리고 이에 따른 배아의 glutamine 합성에 관여하는 다양한 효소의 발현 감소 및 메틸기공여물질의 감소와 이에 따른 메틸기 전이활성의 감소 등이 관여하고 있음을 의미한다. 한편 배아에서의 ABA에 의한 globulin 단백질의 증가는 배아 특이적 globulin의 일시적 합성 증가와 globulin 분해 효소의 활성 억제 등이 복합적으로 관여한 결과로 생각된다.

심호흡 방법에 따른 상복부 수술환자의 폐 환기능에 미치는 효과 (The Effect of Deep Breathing Methods on Pulmonary Ventilatory Function of Patients Who experiened Upper-abdominal surgery)

  • 본 연구는 수술환자의 술 후 폐환기능 회복을 돕기 위한 효과적인 간호중재의 자료를 제공하고자 부산시내에 소재한 P 대학병원의 일반욋과에 입원하여 전신마취하에 상복부 수술을 받은 환자 46명을 대상으로 하였으며, 자료수집기간은 1993년 11월 1일부터 12월 31일까지로 하였고, 처음 4주간동안 대조군을, 다음 4주간에 실험군을 자료수집하였다. 실험군에는 Incentive Spirometer를 사용한 심호흡법을, 대조군에는 호흡기구를 사용하지 않은 자발적인 심호흡법을 수술 후 의식이 회복된 직 후부터 매 1시간마다 규칙적으로 24시간동안 실시하였다. 폐환기능의 측정도구는 운반이 용이하고 사용이 간편하여 임상적으로 많이 이용되는 Vitalograph Compact를 사용하였으며, 수술 전과 수술 후 24시간, 수술 후48시간, 수술 후72시간, 각 각 4회에 걸쳐 대상자들의 노력성 폐활량과 일초 노력성 호기량을 측정하였다. 여기서 수집한 자료를 SPSS $PC^+$를 이용하여, 백분율, 평균, t검정, $X^2$검정, 일원변량분석으로 통계처리하였다. 본 연구의 결과를 요약하면 다음과 같았다. 1) 대상자들의 일반적특성은 여성이 41.3%, 남성이 58.7%이며, 평균연령은 $48.48{\pm}12.26$, 흡연습관은 대상자 전체의 43.5%가 있는 것으로 나타났으며, 과거 호흡기 질환은 전체의 8.7%가 앓은 경험이 있는 것으로 나타났다. 2) Incentive Spirometer를 사용한 심호흡방법은 수술 후의 노력성 폐활량에서 24시간에 $1.46{\pm}0.54L$, 48시간에 $1.58{\pm}0.60L$, 72시간에 $1.90{\pm}0.61L$로서 유의한 차이를 보였다(F=3.530, P=0.035). 즉 시간경과에 따라 폐환기능의 회복에 효과적이었다. 3) 호흡기구를 사용하지 않은 자발적인 심호흡 방법은 수술 후 노력성 폐활량이 24시간에 $1.19{pm}0.43L$, 48시간에 $1.35{\pm}0.42L$, 72시간에 $1.51{\pm}0.38L$로 유의한 차이를 보였고(F=3.480, P=0.037), 술 후 일초 노력성 호기량도 24시간에 $1.00{\pm}0.28L$, 48시간에 $1.20{\pm}0.37L$, 72시간에 $1.33{\pm}0.33L$로 유의한 차이를 보였으므로(F=6.153, P=0.004), 술 후 폐환기능 회복에 효과가 있다. 4) 실험군과 대조군의 수술 후 노력성 폐활량은 수술 후 72시간에서 실험군이 $1.90{\pm}0.61L$, 대조군이 $1.51{\pm}0.38L$로 유의한 차이를 보였다(t=2.620, P=0.013). 5) 실험군과 대조군의 수술 후 일초 노력성 호기량은 수술 후 24시간에서 $1.33{\pm}0.56L,\;1.00{\ge}0.28L$로 유의한 차이를 보였고(t=2.530, P=0.017), 술 후 72시간에서 $1.72{\pm}0.65L,\;1.33{\pm}0.3L$로 유의한 차이를 보였다(t=2.540, P=0.016). 6) 대상자의 술 후 폐환기능에 영향을 미치는 요인은 성별로 나타났다. 이에 따른 폐환기능의 차이를 보면, 실험군의 술 후 노력성 폐활량이 48시간에 남자($1.78{\pm}0.61L$)가 여자($1.27{\pm}0.45L$)보다 더 높게 나타났으며 (t=2.170, P=0.042), 72시간에도 역시 남자($2.16{\pm}0.56L$)가 여자($1.50{\pm}0.47L$)보다 더 높았다(t=2.910, P=0.008). 술 후 일초 노력성 호기량은 24시간에서 남자가 $1.54{\pm}0.60L$, 여자가 $1.00{\pm}0.30L$으로 유의하였고 (t=2.460, P=0.023), 48시간에 남자가 $1.64{\pm}0.62L$, 여자가 $1.09{\pm}0.41L$로 유의한 차이를 보였고(t=2.360, P=0.028), 72시간에도 남자($1.96{\pm}0.62L$)가 여자($1.33{\pm}0.50L$)보다 더 높게 나타났다(t=2.540, P=0.019). 즉 실험군의 술 후 폐환기능은 남자가 여자보다 더 우세하다는 것을 알 수 있었다. 이상의 요약된 연구결과를 토대로, 연구자는 다음과 같은 결론을 얻었다. Incentive Spirometer를 사용한 심호흡법이 호흡기구를 사용하지않은 자발적인 심호흡보다 술 후 폐환기능 회복에 있어서 더 우수한 방법이었다. 그러므로, 임상간호사는 상복부 수술환자의 술 후 폐환기능 회복을 돕고 나아가 폐합병증을 예방하기 위해 Incentive Spirometer를 사용한 규칙적인 심호흡법을 술 후에 간호중재의 한 방안으로 실시할 것을 권장하며, 아울러 병동의 사정 상 호흡기구의 비치가 가능하지 않다면 호흡기구를 사용하지않은 자발적인 심호흡법을 술후에 꾸준히 적용하는것도 임상간호학발정에 기여하리라고 본다.

지하수내 갈색 부유/침전 물질의 생성 반응에 관한 평형 및 반응속도론적 연구: 적정 양수 기법 및 탁도 제거 방안에 대한 제안 (Geochemical Equilibria and Kinetics of the Formation of Brown-Colored Suspended/Precipitated Matter in Groundwater: Suggestion to Proper Pumping and Turbidity Treatment Methods)

  • 갈색 침전물의 생성은 우리나라 지하수의 개발 및 공급에 있어 흔히 발생하는 문제 중의 하나인데, 이에 따라 색도, 맛, 탁도 및 용존 철 함량 등의 항목에 있어 먹는 물 수질 기준을 초과하게 되고, 물 공급 시스템에 스케일링의 문제를 야기하게 된다. 경기도 파주 지역 지하수의 경우에도 양수 후 몇 시간 내에 갈색 침전물이 형성되어 수질을 악화시키고 있다. 본 연구에서는 지하수의 탁도를 유발하는 원인과 지화학적 반응 경로를 이해하고자, 평형열역학 및 반응속도론적 접근을 통하여 갈색 침전물의 형성과정을 파악하였다. 본 연구결과는 침전물의 형성을 최소화하기 위한 적정 양수 기법은 물론 수질 향상을 위한 최적 수처리 기법을 설계하는데 있어 중요한 자료로 활용될 것이다. 파주 지역의 암반 지하수는 물/암석(편마암)반응에 의해 Ca-$HCO_3$형의 수질 특성을 보인다. SEM-EDS 및 XRD 분석 결과, 갈색 침전물은 비정질의 함철 산화물 또는 수산화물로 해석된다. 다양한 공극 크기(6, 4, 1, 0.45, 0.2 $\mu\textrm{m}$)를 갖는 여과지를 이용한 다단계 여과 결과, 이들 침전물은 크기에 있어 대부분 1 내지 0.45$\mu\textrm{m}$의 입도를 갖는 콜로이드 형태이지만, 질량 분포로 볼 때는 1 내지 6$\mu\textrm{m}$범위가 우세함(총 질량의 약 81%)을 알 수 있다. 다량의 용존 철(II)은 지하수 유동 중에 철 함량이 높은(최대 3wt.%) 단층 파쇄암 내의 녹니석(clinochore)의 용해로부터 기원하는 것으로 판단된다. PHREEQC 프로그램을 이용한 포화지수 계산 및 pH-Eh 관계도에 대한 검토 결과, 침전물은 함철 수산화물임이 확인되며, 환원 조건에 있던 심부 지하수가 양수에 의해 산소에 노출되면서 화학성 변화(특히, 산화)에 의하여 침전함을 알 수 있다. 양수 이후의 시간 경과와 더불어 양수된 지하수의 pH, DO, 알칼리도는 점차 감소하며. 탁도는 증가하다가 일정 시간 경과 후 감소하는 경향을 보인다. 양수 이후의 경과 시간에 따른 용존 철(II)의 농도 감소율(즉, 반응 속도)은 Fe(II)=10.l exp(-0.0009t)로 표현된다. 따라서 갈색 침전물의 생성 반응은 양수 및 양수 후 저장 과정 중에 산소의 유입에 따른 산화 반응에 기인하며, 그 반응은 시간, 산소분압 및 pH에 의존함을 알 수 있다. 탁도를 제거하여 음용 가능한 수질을 확보하기 위해서는, 충분한 시간 동안 충분한 크기를 갖는 탱크 내에서의 다단계 저장 및 폭기를 거친 이후에 응집된 침전물에 대한 여과가 제안된다. 이때, 비용 절감 차원에서 상이한 입도 조건에서의 다단계 여과가 효과적일 것으로 생각된다. 한편, 개발 관정 내에서의 스케일링을 최소화하기 위해서는 심부 지하수로 산소가 풍부한 천층 지하수가 유입되는 과정을 최소화할 필요가 있다. 이를 위해서는 적정 채수량 범위 내에서의 지속적인 양수가 효과적일 것이다. 아울러, 산소가 풍부한 천층 지하수의 채수를 위한 별도의 관정 설치도 고려할 수 있을 것이다.

Sensory Information Processing

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. In this paper, a simple reflective infrared optic system with a 2-dimensional optic scanner is proposed for the realization of medical infrared thermography system.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction.
