제 3 뇌실의 유삭교종 - 증 례 보 고 - (Third Ventricular Chordoid Glioma - Case Report -)

  • We have recently experienced a third ventricular tumor from a 63-year old woman who presented with paraparesis, voiding difficulty and general weakness. Histopathologic study of the tumor was compatible with recently reported and newly termed chordoid glioma. Clinical behavior of the reported tumors were mostly benign but because of its very close proximity to the hypothalamus, some of the subtotally removed tumors recurred or some of the patients died of postoperative complications. The presenting patient regained normal leg power after removal of the tumor followed by ventriculoperitoneal shunt and discharged on her own feet.

Non-Dura Based Intaspinal Clear Cell Meningioma

  • A 34-year-old female patient was presented with leg and hip pain for 6 months as well as voiding difficulty for 1 year. Magnetic resonance imaging revealed a well-demarcated mass lesion at L2-3. The mass was hypo-intense on T1- and T2-weighted images with homogeneous gadolinium enhancement. Surgery was performed with the presumptive diagnosis of intradural extramedullary meningioma. Complete tumor removal was possible due to lack of dural adhesion of the tumor. Histologic diagnosis was clear cell meningioma, a rare and newly included World Health Organization classification of meningioma usually affecting younger patients. During postoperative 2 years, the patient has shown no evidence of recurrence. We report a rare case of cauda equina clear cell meningioma without any dural attachment.

Septated Extradural Arachnoid Cyst in Thoracolumbar Spine Causing Myelopathy

  • Spinal extradural arachnoid cyst is uncommon and rarely cause neural compression. We report a rare case of severe cord compression due to septated spinal extradural arachnoid cyst. A 35-year-old woman has developed back pain 3 months prior to her visit, but recently motor weakness and urinary incontinence occurred. Magnetic resonance images showed an extradural cyst posterior to the cord, which was flattened and displaced from T12 to L2. Urgent decompressive laminectomy and cyst removal was performed. Histopathological examination confirmed that cyst wall was formed by nonspecific fibrous connective tissue without a single-cell layer of inner arachnoid lining. Motor weakness and voiding difficulty were recovered completely after operation.

회음부 동통 완화를 위한 경천추 신경차단 (Transsacral Neurolytic block for the Relief of Perineal Pain)

  • Transsacral neurolytic block with 2.5ml of phenol in glycerine or bupivacaine was performed in 6 patients with malignant diseases and a patient with sphincter spasm of bladder due to spinal cord injury. Pain relief was satisfactory in all patints except one patient with very low pain threshold. In one patient, second transsacral neurolytic block alone was not sufficient because of widespread pain along distant metastasis of the malignant disease, although the first block was satisfactory. The complications include transient motor weakness(4), voiding difficulty(1), subarachnoid puncture(1), and epidural venous puncture(1), but they were all spontaneously recovered within a sbort period of time and did not give any limitation to the block.

부갑상선 선종에 의한 원발성 부갑상선 기능 항진증 (Primary Hyperparathyroidism due to Parathyroid Adenoma)

  • A case of primary hyperparathyroidism due to parathyroid adenoma is presented. A 14 year-old male was admitted to the hospital comptaining of voiding difficulty. The intravenous pyelogram demonstrated a stone in the proximal one third of the left ureter and marked hydronephrosis of the left kidney. The Tc-99m sestamibi nuclear scan demonstrated a hot spot below the lower pole of the left lobe of the thyroid. Laboratory study demonstrated hypercalcemia (12.4 mg/dL) and elevated parathyroid hormone (143.67 pg/mL). A parathyroid gland located below the lower pole of the left lobe of the thyroid was excised. A parathyroid adenoma, consisting of mainly chief cells was found on pathologic examination. Postoperatively the patient had transient hypocalcemic symptoms, which resolved with administration of calcium preparation and vitamin D.

Acute combined central and peripheral nervous system demyelination: a case report

  • Guillain-$Barr{\acute{e}}$ syndrome (GBS) and acute disseminated encephalomyelitis (ADEM) are demyelinating neurologic disorders with different target organs. Although they share similar pathogenetic mechanism, reports of simultaneous occurrence of the 2 disorders are rare. A 2 year 6 month old girl visited our hospital for fever, cough, and general weakness. Although the muscle power of extremities showed mild weakness and voiding difficulty, initial deep tendon reflex of both knees and ankles was normal. A nerve conduction study to evaluate the weakness revealed the absence of F waves. Cerebrospinal fluid analysis demonstrated pleocytosis with lymphocyte predominance and elevated protein levels. Magnetic resonance imaging showed abnormal T2 hyperintensity in pons, medulla and spinal cord. Serum anti-GD1b antibody was positive. Based on clinical findings, laboratory findings, nerve conduction study, and neuroimaging, the diagnosis of GBS and ADEM was made. This is the first case of GBS accompanied by ADEM in Korea.

일차성 야뇨증의 위험 요인 (Risk Factors of Primary Nocturnal Enuresis)

  • 목 적 : 일차성 야뇨증의 병인론은 유전적 요인 외에 수면 요인, 야간 Arginine Vasopressin(AVP)분비 장애, 기능적 방광 미숙 등 주로 배뇨 반사의 성숙 지연이 논의되고 있으나 위험 요인에 대한 연구는 적었다. 이에 저자들은 소변을 충분히 가릴 나이인 5세 이후까지 야뇨증을 지속시킬 수 있는 위험 요인들을 분석하고자 하였다. 대상 및 방법 ;서울 목동지역 초등학교 1학년의 정기 신체 검사에서 야뇨증 설문지 조사를 통해 일차성 야뇨증으로 확인된 아동 20명과 이후 1년간 이화여자대학교 목동병원 소아과 외래에서 동일한 설문지 조사를 받은 6-7세의 일차성 야뇨증 아동 35명을 포함한 55명을 야뇨증군으로 하였고, 설문지 조사에서 야뇨증이 없었던 아동 221명을 대조군으로 하였다. 이중 야뇨증군 55명과 대조군 22명에서는 24시간 배뇨 일지를 기록하게 하였으며, 두 군사이에 유전적 요인, 심리적 요인, 발달 요인, 수면 요인, 일일 및 야간 수분 섭취량과 요량, 주간 배뇨 장애 및 일회 최대 요량 등을 비교분석 하였다. 결 과 : 1)부모의 야뇨증 병력은 야뇨증군에서 $20.0\%$로 대조군의 $2.7\%$에 비해 유의하게 많았다(P<0.05). 남녀비는 야뇨증군과 대조군에서 유의한 차이가 없었다. 2)출생 순서, 어머니의 직장 여부, 아동과 부모의 성격, 걷기 시작한 나이 및 학교 성적 두 군간에 유의한 차이가 없었다. 3) 잠에서 깨어나기 힘든 경우는 야뇨증군에서 $70.9\%$로 대조군의 $54.3\%$에 비해 유의하게 많았으나(P<0.05), 수면 시간은 차이가 없었다. 4) 야간 수분 섭취량은 야뇨증군에서 $330{\pm}158.2\;mL$로 대조군의 $235{\pm}129.5\;mL$에 비해 유의하게 많았고(P<0.05), 야간 요량은 야뇨증군에서 $390{\pm}61.5mL$로 대조군의 $140{\pm}43.2mL$에 비해 유의하게 많았다(P<0.05). 일일 수분 섭취량과 일일 요량은 두 군간에 유의한 차이가 없었다. 5) 일회 최대 요량은 야뇨증군에서 $107{\pm}35.9mL$로 대조군의 $236{\pm}41.3mL$에 비해 유의하게 적었다((P<0.05). 주간 배뇨 이상은 야뇨증군에서 $80.0\%$로 대조군의 $57.9\%$에 비해 유의하게 많았으며 주간 배뇨 횟수도 야뇨증군에서 $7.0{\pm}3.4$회로 대조군의 $5.4{\pm}1.6$회에 비해 유의하게 많았다(P<0.05). 결 론 :야뇨증은 유전적 요인을 비롯하여 성숙 지연과 관련된 수면 요인, 야간 다뇨, 방광 미숙 등이 관련될 것으로 생각된다. 야간 다뇨의 원인으로 야간 수분 섭취 과다가 관련되므로 치료의 일차 단계로 야간 수분 섭취 제한의 중요성이 강조되어야 한다고 생각된다.

미만성 척수 경막외 농양을 동반한 화농성 척추염 - 증 례 보 고 - (Pyogenic Spondylitis with Diffuse Spinal Epidural Abscess - A Case Report -)

  • We report a case of pyogenic spondylitis on L2 and L3 with diffuse epidural abscess up to T4 to L3 and large psoas abscess. A forty-nine-year old male was presented with progressive back pain, left flank pain and ab-dominal distention, weakness of the both legs and voiding and defecation difficulty during last 2 months. Initially multiple coronal hemilaminectomies from T4 to T12 were done for the treatment of diffuse thoracic epidural ab-scess. Then second operation via left retroperitoneal approach was performed for lumbar spondylitis and psoas abscess on third day after initial operation. After removal and curettage of pyogenic psoas and epidural abscess and spondylitis (L2-L3), iliac bone grafting with Keneda instrumentation from L1 to L4 was done simultaneously. Postoperative course has been unevenful without recurrent infection. The literature on diffuse epidural and large psoas abscess with pyogenic spondylitis are reviewed and instrumentation for stabilization of pyogenic spondylitis is also discussed.

연수와 경수에 발생한 수내 신경초종 - 증 례 보 고 - (A Case of Intramedullary Schwannoma at the Cervicomedullary Junction - A Case Report -)

  • Spinal schwannomas are usually extramedullary intradural tumors and their intramedullary localizations are thought to be extremely rare. A 60-year-old woman complaining spastic quadriparesis, voiding difficulty and dyspnea was admitted. Her cervical MRI revealed an intramedullary mass in the cervicomedullary junction with multiple cyst which extended from lower cervical to C3 spinal cord. The mass showed a low signal on T1WI, high signal on T2WI with an wall enhancement. The patient underwent a suboccipital craniectomy and C1-2 laminectomy and the cystic tumor was totally removed through a midline myelotomy. The tumor was proved as an intramedullary schwannoma by pathologic examination. The Intramedullary presence of a tumor arising from the cells of the nerve sheath is unusual, because the central nervous system fibers do not contain the Schwann cell. There have been several hypotheses, but none has been accepted universally. This rare tumor is considered as a curable benign neoplasm, and an accurate intraoperative diagnosis and surgically total removal are essential.

Sacral Nerve Stimulation for Treatment of Intractable Pain Associated with Cauda Equina Syndrome

  • Sacral nerve stimulation (SNS) is an effective treatment for bladder and bowel dysfunction, and also has a role in the treatment of chronic pelvic pain. We report two cases of intractable pain associated with cauda equina syndrome (CES) that were treated successfully by SNS. The first patient suffered from intractable pelvic pain with urinary incontinence and fecal incontinence after surgery for a herniated lumbar disc. The second patient underwent surgery for treatment of a burst fracture and developed intractable pelvic area pain, right leg pain, excessive urinary frequency, urinary incontinence, voiding difficulty and constipation one year after surgery. A SNS trial was performed on both patients. Both patients' pain was significantly improved and urinary symptoms were much relieved. Neuromodulation of the sacral nerves is an effective treatment for idiopathic urinary frequency, urgency, and urge incontinence. Sacral neuromodulation has also been used to control various forms of pelvic pain. Although the mechanism of action of neuromodulation remains unexplained, numerous clinical success reports suggest that it is a therapy with efficacy and durability. From the results of our research, we believe that SNS can be a safe and effective option for the treatment of intractable pelvic pain with incomplete CES.
