A Study on Relations among General Characteristics, Lifestyle Habits, and Menopausal Symptoms Measurement Indicators during Treatment for Hot Flush in Menopausal Women (안면홍조 증상으로 치료 중인 갱년기 여성에서 일반적 특성 및 생활 습관과 갱년기 측정 지표들간의 상관성 연구)

  • Objectives: To understand relations general characteristics, lifestyle habits including smoking, alcohol habit, exercise, eating habit and menopausal symptoms measurement indicators during treatment for hot flush in menopausal women. Methods: The participants were 159 women (45~60 yr) who were not currently on hormone therapy, and had reached hot flash scores of 10 or higher. The evaluating indexes of this trial are hot flush score, hot flush visual analogue scale(VAS), Hot flush consistence time, sweating visual analogue scale(VAS), Menopause Rating Scale(MRS), Menopause-specific quality of life questionnaire(MENQOL), Kupperman's Index. Results: 1. When the participants divided according to body mass index(BMI), overweight group showed a high score in all indicators. Therefore, obesity can aggravate the symptoms of menopause. 2. When the participants divided according to drinking habits, Hot flush consistence time of non-alcohol group showed a statistically significant difference. However, the sample size is uneven. Additional studies will be needed. 3. When the participants divided according to eating habits, Hot flush consistence time of regular group showed a statistically significant difference. However, the sample size uneven. Additional studies will be needed. 4. When the participants was divided depending on whether exercise, non-exercise group showed a high score in all indicators. Thus, exercise will be able to improve menopausal symptoms. Conclusions: Improvement of obesity and exercise will be able to improve menopausal symptoms.

Development of Eyes Inspection Questionnaire(EIQ) and Regression Analysis between EIQ Items and deficiency or excess patterns of Eyes Inspection (안진(眼診) 설문지 개발 및 안진(眼診) 설문의 허실(虛實) 연관성 연구)

  • Objectives Eyes, one of visual inspection regions, present important clues to pathological patterns including deficiency and excess patterns to the clinicians. The purpose of this study was to develop Eyes Inspection Questionnaire (EIQ) and to examine which items among the EIQ were more predictive of clinicians' determination for the deficiency and excess patterns. Methods Nine questionnaire items for Visual Inspection of Eyes were extracted through the literature review. These items were presented to the 4 Korean medical doctors who are specialized in visual inspection to conduct the Delphi method. The Korean medical doctors were asked to rate the importance of each items for the corresponding Visual Inspection of Eyes, using a Likert 5-point scale(the 3 points of importance as a cut-off point). Then, out of 75 photographs submitted to the Society of HyungSang Medicine in 2009, 30 portrait pictures were selected as samples. The samples were copied to make 60 sample pictures, and then randomly assigned to 4 clinicians. The 4 clinicians evaluated the 60 samples for excess and deficiency of the eyes and were asked to check the 6 questionnaire items. The results were recorded as 5-points-scale, and their average and standard deviations were calculated. Intra- class reliability test and multi regression test were performed using SPSS 13. Results Intra-class correlation coefficient (ICC) was between 0.750 to 0.841 (P<0.05). Indices for visual inspection of the eyes were: endowment of the bone structure around the eyes; brightness of the eyes; upward deviation of the eyes; eye shapes; and definition of iris. 76.92% of deficiency symptom patterns and 86.42% of the excess symptom patterns matched the patterns predicted by the visual inspection of the eyes, according to the frequency analysis. According to the multiple regression analysis, were significantly related to the excessive symptoms, and to the deficiency symptoms. Conclusion This study is the first attempt of development for checklist of excess and deficiency of Visual Inspection of Eyes and quantitative measurement of excess and deficiency using the Visual Inspection of Eyes by the visual inspection experts. Still, additional studies are needed regarding the relationship visual inspection methods have with existing standards of diagnosis.

Measuring Utility for Menopausal Symptoms Based on Time Trade-Off and Visual Analogue Scale Methods (시간교환법과 시각화척도 방법을 이용한 폐경 후 에스트로겐 결핍증에 대한 효용 측정)

  • The present study was conducted to measure health-related utility for post-menopausal symptoms among Korean woman. A survey questionnaire was developed to measure utility based on visual analogue scale(VAS) and time trade-off method (TTO). From January 29 to February 3, 2007, a face-to-face interview was conducted with 274 women aged 45 to 60 to ask about their subjective utility for hypothetical health status of mild or severe post-menopausal symptoms. Among the participants, 160 were recruited from outpatient departments of 3 hospitals and 114 from 2 sports centers located in Seoul. For mild symptom, the average utility score of the respondents was $0.63{\pm}0.14$ based on VAS and $0.61{\pm}0.26$ based on ITO. For severe symptoms, the average utility was $0.39{\pm}0.16$(VAS) and $0.44{\pm}0.27$ (TTO). For severe symptom, a higher utility score was observed for TTO than for VAS, which is consistent with earlier studies and thus supports the validity of the utility measurement in this study. Overall, the severity of post-menopausal symptoms of the respondents themselves was not significantly associated with the utility score that the respondents answered for hypothetical health status. Multiple regression analysis results showed that the utility score was significantly higher among respondents with older age, higher education, higher family income, and from hospitals. It is expected that the utility score measured in this study will enhance the understanding of the quality of life of women after menopause and will be used to conduct cost-utility analysis of health care interventions to treat post-menopausal symptoms.

  • Objectives We report 1 treatment case with Onbaek-won which is based on Sasang constitutional medicine for specific symptoms that leukoaraiosis Soeumin patient has. Methods We treated leukoaraiosis Soeumin patient suffering from specific symptoms. We prescribed Onbaek-won for her physical symptoms. The improvement of her symptoms were evaluated by Manual Muscle Test(MMT) and Visual Analogue Scale(VAS). Results After the Onbaek-won was given, the patient's most of symptoms were improved. And there was not any adverse effect. Conclusions This study shows that using Onbaek-won can be effective treatment for leukoaraiosis Soeumin patient's specific symptoms.

Effect of Visual neglect for hemiplegia to motor recovery (시각무시가 편마비 환자의 운동 기능회복에 미치는 영향)

  • The purpose of this study by examine the effect of visual neglect on hemiplegia motor recovery are showing trouble which can be raised by visual neglect and helping the patient's ADL and functional recovering. Among the patients who are being taken physical therapy in many other hospitals in Busan From February 1st 2002 to august 31st, we chose 20 patients as control group who did not have symptoms of visual neglect and 20 other patients who did have as case group. We used Albert's test and line bisection as visual neglect test, and MAS as motor recovery. The average age of the patient group is 51.55 and that of control is 44.9. The men's rate is higher than women' s in both groups. Although the rate of left hemiplegia is higher than right hemiplegia in case group, that of right hemiplegia is higher than left hemiplegia in control group. There is much lesion site of basal ganglia in case group. There is the most amount of transformation of MAS when visual neglect is shorter than 1.5centimeter. Each change of MAS point before and after therapy in case group and control group is revealed $6.5{\pm}4.37$ and $12.5{\pm}5.95$.

Clinical Features and Pharmacological Treatment of Dementia with Lewy Bodies (루이소체 치매의 증상과 치료)

  • Dementia with Lewy bodies (DLB) is the second most common causes of dementia. It can exhibit a variety of clinical symptoms including cognitive decline, cognitive fluctuation, visual hallucinations, parkinsonism, REM sleep behavior disorder, hypersensitivity to neuroleptics and autonomic dysfunctions. Despite more well-known criteria for DLB, there are often misdiagnosis and inappropriate treatment. It gives a lot of clinical burden to the clinician as well as to patients and families. When reducing the misdiagnosis, the burden of all will be reduced. The special concern and solicitation are needed in order not to miss the diagnosis when the cardinal features of DLB may not be volunteered by patients and the caregivers. To control the symptoms, clinicians must find and reduce drugs that can have the negative effects on DLB symptoms. There is limited evidence about specific interventions but available data suggest cholinesterase inhibitors improve the cognitive and behavioral symptoms and menmantine slightly improves the global impression.

  • Objectives: The purpose of this study is to report the effect of Korean medicine treatments on Postoperative symptoms. Methods: The patient was treated with herbal medicine, acupuncture and electronic moxibustion. The efficacy of treatment was evaluated with visual analogue scale (VAS), the degree of symptom relief and presence or absence of symptoms. Results: After hospital care with korean medicine treatments, symptoms such as fatigue, lower abdominal pain, thirst, hot flush, cold hypersensitivity of hands and feet and tinnitus were disappeared. Conclusions: To promote physical strength and reduction of fatigue, intensive hospital care with korean medicine treatment seems to be effective. After discharge, continued outpatient treatment for complications of surgery and menopausal disorder seems to contribute to quality of life and satisfaction for patient who undergoing hysterectomy.

Clinical Evaluation on Variation of Face Form Angle of Eyewear (안경 안면각 변화에 대한 임상적 고찰)

  • Purpose: This study was investigated to evaluate the effect of the variation of the face form angle(FFA) of spectacle frame on the visual acuity and the visual function. Methods: The visual acuity, the contrast sensitivity, the stereopsis, and the phoria were measured with the spectacle frame whose the FFA is at $5^{\circ}$ and $25^{\circ}$ and the surveys were conducted on subjective symptoms. Here, 30 adults (14 men and 16 women, average age:$25.46{\pm}3.9$), who did not have ocular diseases and were fully corrected in case of having had refractive error, were to act as experimental subjects for this measurement. Results: The visual acuity and the contrast sensitivity was decreased and the phoria was tended to increase to the esophoria direction at the FFA $25^{\circ}$ than at the FFA $5^{\circ}$. The stereopsis was significantly reduced at the FFA $25^{\circ}$ than at the FFA $5^{\circ}$. Also when the FFA was larger, the higher the degree of refractive error was, the more the increase of the subjective symptoms such as being blurred characters, being disturbed head, and having sore eyes was, which reveals a statistically significant correlation between them. Conclusions: If the FFA increases, the visual acuity, the contrast sensitivity and the stereopsis was found to decrease and the phoria to increase to the esophoria direction. Therefore it shows that the variation of the FFA of spectacle frame can influence the visual acuity and the visual function.

The Effect of VDT Work on Vision and Eye Symptoms among Workers in a TV Manufacturing Plant (텔레비젼(TV)생산업체 근로자들의 영상단말기(VDT)작업이 시력과 안증상에 미치는 영향)

  • This study was conducted to evaluate the effect of VDT work on eyes and vision among workers in a TV manufacturing plant. The study subjects consisted of 264 screen workers and 74 non-screen workers who were less than 40 years old male and had no history of opthalmic diseases such as corneal opacities, trauma, keratitis, etc and whose visual acuity on pre-employment health examination by Han's test chart was 1.0 or above. The screen workers were divided into two groups by actual time for screen work in a day : Group I, 60 workers, lesser than 4 hours a day and group II, 204 workers, more than 4 hours a day. From July to October 1992 a questionnaire was administered to all the study subjects for the general charateristics and subjective eye symptoms after which the opthalmologic tests such as visual acuity, spherical equivalent, lacrimal function, ocular pressure, slit lamp test, fundoscopy were conducted by one opthalmologist. The proportion of workers whose present visual acuity was decreased more than 0.15 in comparison with that on the pre-employment health examination by Han's test chart was 20.6% in Group II. 15.0% in Group I and 14.9% in non-screen workers. However, the differences in proportion were not statistically significant. The proportion of workers with decreased visual acuity was not associated with the age, working duration, use of magnifying glass and type of shift work (independent variables) in all of the three groups. However, screen workers working under poor illumination had a higher proportion of persons with decreased visual acuity than those working under adequate illumination (P<0.05) . The proportion of workers whose near vision was decreased was 27.5% in Group II, 18.3% in Group I, and 28.4% in non-screen workers and these differences in proportion were not statistically significant. Changes of near vision were not associated with 4 independent variables in all of the three groups. Six out of seven subjective eye symptoms except tearing were more common in Group I than in non-screen workers and more common in Group II than in Group I (P<0.01). Mean of the total scores for seven subjective symptoms of each worker(2 points for always, 1 point for sometimes, 0 point for never) was not significantly different between workers with decreased visual acuity and workers with no vision change. However, mean of the total scores for Group II was higher than those for the Group I and non-screen workers (P<0.01). Total eye symptom scores were significantly correlated with the grade of screen work, use of magnifying glass, and type of shift work. There was no independent variable which was correlated with the difference in visual acuity between the pre-employment health examination and the present state, the difference between far and near visions, lacrimal function, ocular pressure, and spherical equivalent. Multiple linear regression analysis for the subjective eye symptom scores revealed a positive linear relationship with actual time for screen work and shift work(P<0.01). In this study it was not observed that the VDT work decreased visual acuity but it induces subjective eye symptoms such as eye fatigue, blurred vision, ocular discomfort, etc. Maintenance of adequate illumination in the work place and control of excessive VDT work are recommended to prevent such eye symptoms.

Factors Influencing the Xerostomia Symptoms in the Patients with Temporomandibular Disorders

  • Purpose: This study investigated factors influencing the xerostomia symptoms in the patients with temporomandibular disorders (TMDs). Methods: Eighty-six participants over the age of twenty were randomly enrolled from patients with TMDs. The patients were diagnosed by Diagnostic Criteria-TMD (DC-TMD). Intensity of the pain and level of stress of the patients were recorded using TMD pain visual analogue scale (VAS) and stress VAS, respectively. The Symptom Checklist-90-Revised (SCL-90R), dry mouth symptom questionnaire, unstimulated salivary flow rate (USFR) and oral moisture were measured. Results: The patients who had above the mean of the TMD pain VAS had significantly higher scores on the stress VAS, subjective dry mouth symptoms and T-score of somatization. The patients who suffered from pain more than three months had significantly higher TMD pain VAS, subjective dry mouth symptoms and T-score of somatization. There were no significant differences in xerostomic symptoms according to the diagnosis of TMDs. Moreover, TMD pain VAS and the stress VAS did not correlated to USFR and oral moisture. Conclusions: The level of individuals' TMD pain and stress significantly affect their subjective dry mouth symptoms, however, it did not affect USFR and oral moisture.
