  • Although neonatal hypoglycemia is a common metabolic abnormality in newborn infants, brain injuries resulting from isolated neonatal hypoglycemia are rare. Many infants who are hypoglycemic do not exhibit clinical manifestations, while other infants are symptomatic and at risk for permanent brain damage. There is no disagreement that hypoglycemia can cause neonatal encephalopathy and result in permanent brain injury. Occipital brain injury associated with neonatal hypoglycemia can result in long-term disability, epilepsy, and visual impairment. Infants should receive ongoing developmental and visual surveillance for lateonset epilepsy, and visual or cognitive impairment. We report two cases of newborn infants with abnormal visual evoked potentials (VEP) caused by neonatal hypoglycemic encephalopathy.

A Convergence Study on Music-color Association Responses of People with Visual Impairment Mediated by Emotion (시각장애인의 정서 기반 음악-색채 연합에 대한 융복합적 연구)

  • The purpose of this study was to examine music-color association response(MCAR) of people with visual impairment through music-emotion scale and music-color scale. The study was conducted on 60 participants(30 congenital/ 30 adventitious) who are using services of two welfare centers at S and B cities. For this, four basic emotions (happiness, sadness, anger, and fear) mediated by music were selected, and MCAR to emotion-inducing music were analyzed through self-report method. As a result, first, there were found contrasts in MCAR between happiness and sadness according to type of emotion, however, similar in anger and fear. Second, in MCAR among three variables of the music-emotion scale(valence, arousal and intensity), valence was congruent with MCAR according to type of emotion, arousal marked high scores in negative emotions, and scores of intensity in happiness and sadness were higher than those in anger and fear. Third, there were no significant differences between two groups of people with congenital and adventitious visual impairments. It is meaningful that this study showed the MCAR can be mediated by music through investigating those of people with visual impairment.

  • Airport-related laws restrict the installation of lights that obstruct the identification of airport lights or cause glare to pilots and controllers around the airport. However, with regard to the glare that lights give to the pilot or controller, it is only judged whether there is a limit based on a qualitative judgment, and a quantitative review has not been made. Therefore, in this study, visual disturbances that may occur to pilots and controllers due to large lighting facilities around airports during nighttime flight operation were quantitatively calculated and confirmed through disability glare, object recognition, and identification obstruction. And based on this study, it was suggested to supplement the related system to prevent the visual impairment of pilots and controllers caused by lighting facilities.

  • Purpose: The purpose of this study was to analysis of effectiveness between cognitive function assessment scores and gaming cognitive rehabilitation system in children with intellectual impairment. Methods: Five children (male=5, $age=10.00{\pm}0.80$) with intellectual impairment participated in this study and were randomly assigned to the experiment that played (received) gaming cognitive rehabilitation system (Neuroworld). The children were applied 2 times a week for 30 minutes during 3 months. The children were assessed K-WSIC-VI (Korean-Wechsler intelligence scale for children-fourth edition) and recorded that gained score in gaming cognitive rehabilitation system before and after intervention. K-WSIC-VI contained five primary index scores: verbal comprehension index, visual spatial index, fluid reasoning index, working memory index, and processing speed index. Gaming cognitive rehabilitation system scoring was composed visual recall, target recall, sequence recall, selective attention, continuous attention, and exploration. Results: In the intelligence quotient (IQ) of K-WSIC-VI, there were significant increased in all children. The visual recall item was highest effective in all children. However, sequential recall showed the lowest improvement in all children. The performance speed of selective attention item was decreased, this means that children's skills have improved. Also, their ability to explore has improved significantly. Conclusion: In conclusion, gaming cognitive rehabilitation system was significant effectiveness in cognitive function in some categories for children with intellectual impairment. However, the visual recall and performance speed don't represent of all cognitive function. Therefore, further studies will need to verify by applying more subject and longer duration.

A Study on the Factors Affecting the use of Optical Aids by the aged with Low Vision (저시력 노인의 시력보조기구 사용에 영향을 미치는 요인에 관한 연구)

  • This study applied the data of 384 senior citizens over 65 years old having difficulties in everyday life, among the ageing research panel of Korea Labor Institute in 2000, due to their visual impairment in the final analysis to investigate factors affecting the use of vision aids by the aged with visual impairment. The analysis was based on the service usage Andersen-Newman model and the factors were classified into preceding factors, potential factors and desire factors. To examine the effects of each factors on the use of vision aids by the aged with visual impairment, logistic regression analysis was carried out. In the analysis results, the level of vision aids usage was shown to increase significantly with unemployed status and higher education level in the preceding factors, lower subjective stratum consciousness in the potential factors, and higher cognitive function in the desire factors. The limitation and implication of this study were suggested on the basis of the results.

Effects of a Portable Computerized Cognitive Training System on the Visual-perceptual Function of Stroke Patients with Mild Cognitive Impairment : A Pilot Study (휴대용 전산화 인지훈련 시스템이 경도인지손상이 있는 뇌졸중 환자의 시지각에 미치는 효과)

  • This pilot study investigated the effects of a portable computerized cognitive training system on the visual-perceptual function of stroke patients with mild cognitive impairment. Subjects were twelve stroke patients with mild cognitive impairment, who were assigned to an experimental group (n=6) or a control group (n=6). All subjects were trained in their respective groups for 30 min/day, five times a week, for four weeks. In each session, the experimental group received a portable computerized cognitive training program, and the control group received conventional cognitive training. The Motor-Free Visual Perception-3 (MVPT-3) was used to measure subjects' visual-perceptual ability. The MVPT-3 total scores for both groups significantly improved after the intervention (p<.05). The total score on the MVPT-3 and the score on the visual short-term memory subcategory of the MVPT-3 significantly improved for the experimental group compared to the control group (p<.05). These findings suggest that the portable computerized cognitive training system might be effective to improve the visual-perceptual function of stroke patients with mild cognitive impairment.

Post-stroke Visual Impairment Treated with Korean Medical Treatment: A Case Series (한의 복합 치료를 통해 개선된 뇌졸중 후 시야 장애 환자 2례 증례보고)

  • Objective: This study investigated two cases of Korean medical treatment for visual field impairment after stroke: Case 1, a 56-year-old male with a posterior cerebral artery infarction and right homonymous hemianopsia, and Case 2, a 46-year-old male with an intracerebral hemorrhage in the left parietal lobe and right homonymous hemianopsia. Methods: Case 1 was treated with acupuncture, electroacupuncture, and herbal medicine (Mangeum-tang) for two months, and Case 2 was treated with acupuncture, electroacupuncture, and herbal medicine (Oryeong-san) for 40 days. Results: Following treatment, for Case 1, the Humphrey visual field test showed improvement. The visual field indexes (VFIs) for the left and right eyes improved from 44% to 55% and 49% to 64% respectively, and the mean deviations (MDs) for the left and right eyes improved from -21.11 dB to -19.91 dB and -17.45 dB to -13.89 dB, respectively. The mean visual sensitivities (MVSs) of the left and right eyes also improved from 8.67 dB to 11.33 dB and 1.67 dB to 9.67 dB, respectively, with no side effects. For Case 2, the VFI for the left eye improved from 36% to 64% and that for the right eye remained unchanged. The MDs for the left and right eyes also improved from -22.02 dB to -14.47dB and -22.11 dB to -21.34 dB, respectively, with no side effects. Conclusions: This study suggests that Korean medical treatment may improve visual impairment after stroke, but further research is needed.

Visual Outcome after Transsphenoidal Surgery in Patients with Pituitary Apoplexy

  • Objective : Pituitary apoplexy is one of the most serious life-threatening complications of pituitary adenoma. The purpose of this study is to investigate the visual outcome after early transsphenoidal surgery for the patients with pituitary apoplexy. Methods : We retrospectively reviewed the 31 patients with pituitary apoplexy who were admitted due to acute visual acuity or field impairment and treated by transsphenoidal surgery. Five patients were excluded because of the decreased conscious level. The visual acuity of each individual eye was evaluated by Snellen's chart Visual fields were also checked using automated perimetry. To compare the visual outcome according to the surgical timing, we divided the patients into 2 groups. The first group, 21 of the patients have been undertaken transsphenoidal approach (TSA) within at least 48 hours after admission. The second group included 8 patients who have been undertaken TSA beyond 48 hours. All patients were monitored at least 12 months after surgery. Results : Patients were 21 males and 8 females (M : F=2.6 : 1) with the mean age of 42.4 years. Among the enrolled 29 patients, 26 patients presented with decreased visual acuity and 23 patients revealed the defective visual field respectively. Postoperatively, improvement in the visual acuity was seen in 15 patients (83.3%) who underwent surgery within the first 48 hours of presentation, as compared to those in whom surgery was delayed beyond 48 hours (n=5; 62.5%) (p=0.014). Improvement in the visual field deficits was observed in 15 (88.2%) of patients who had been operated on within the first 48 hours of presentation, as compared to those in whom surgery was delayed beyond 48 hours (n=3; 50.0%) (p=0.037). Conclusion : This study suggests that rapid transsphenoidal surgery is effective to recover the visual impairment in patients with pituitary apoplexy. If there are associated abnormalities of visual acuity or visual fields in patients with hemorrhagic pituitary apoplexy, early neurosurgical intervention within 48 hours should be also required to recover visual impairment.

Health Problems and Work impairment in Nurses Working at Community Health Centers (보건소 근무 간호사의 건강문제와 업무성과 손실)

  • Purpose: This study was conducted to investigate health problems and factors related to work impairment of nurses working at community health centers. Method: Data were collected from May 2008 to July 2008 using a structured, self - reported questionnaire for general characteristics, work-related characteristics, health problems and work impairment. The subjects were composed of 434 nurses from 25 community health centers in Seoul. Data were analyzed by descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient and multiple regression analysis. Results: For present health problems, shoulder, back and neck pain were ranked the highest at (54.6%), followed by cold, GI trouble, and visual disorders. The average score of work impairment was 24.71($\pm7.14$) out of 50.0. Multiple regression analysis revealed that the major factors that affect work impairment were age and number of health problems, and that these factors explained 15% of the work impairment. Conclusion: Age and number of health problems are the factors that have the greatest influence on work impairment. These findings indicate that the health condition of nurses at community health centers must be considered to improve the overall quality of nursing service.
