A Study of Quantitative Analysis of Six Basic Reinforcing-reducing Acupunctural Manipulations in Huang di nei jing(黃帝內經) (${\ll}$황제내경${\gg}$ 6가지 기본 보사수법의 정량적 분석연구)

  • Objectives : We hypothesized that reinforcing-reducing manipulations produce different effects by quantity of stimulation, and examined whether the difference is statistically significant or not. Methods : The study was conducted to the students of College of Oriental Medicine, Dongguk University. We needled into $ST_{36}$ and applied 6 kinds of basic reinforcing-reducing manipulations in Huang di nei jing by randomized sequence. And we had the students to fill up the questionnaire in form of visual analog scale(VAS). We examined whether significant differences in VAS existed between reinforcing-reducing manipulations(by t-test). And we examined whether significant differences in VDRRT(VAS difference between reinforcing-reducing technique) values existed along the gender, body mass index(BMI), age(by t-test), and psychological sensitivity(by ANOVA). Results : 1. The total stimulation quantity from reducing technique was significantly higher than that from reinforcing technique. Individually VAS values of reducing techniques were significantly higher in rotation, slow-rapid, lift-thrust, respiration and open-close manipulation. 2. The highest stimulation quantity was from rotation manipulation. And the highest VDRRT value was from slow-rapid manipulation. 3. When VDRRT values were analyzed by gender, age, BMI, and psychological sensitivity, VDRRT was significantly different by BMI in rotation manipulation, and by psychological sensitivity in rotation, lift-thrust, and slow-rapid manipulation. Conclusions : We could verify that reinforcing-reducing techniques induce different therapeutic effects by different quantities of stimulations. It suggests that reinforcing-reducing techniques could be applied universally, regardless of gender or age. Further studies are needed to consider control group, or patient groups in characteristic conditions.

Arthroscopic Release of the Extensor Carpi Radialis Brevis Tendon for Chronic Recalcitrant Lateral Epicondylitis (만성 불응성 외 상과염에서 시행한 관절경적 단 요 수근 신건 유리술)

  • Purpose: The objective of this study was to assess the clinical outcome of arthroscopic release of the extensor carpi radialis brevis (ECRB) tendon in chronic recalcitrant lateral epicondylitis and tried to determine any prognostic factors. Materials and Methods: A retrospective review of 24 patients with lateral epicondylitis treated by arthroscopic ECRB release was performed. Outcome measures included a patient self rating and visual analog scale (VAS). Functional evaluation was made with Quick-disabilities of the arm, shoulder and hand (DASH) score system to identify preoperative factors that might be associated with the outcomes. Results: Twenty one (88%) elbows received benefit from the procedure. A mean preoperative VAS pain score and the mean Quick-DASH were significantly improved at final follow up. Age, sex, dominant arm and duration of symptom, presence of capsular tear or calcification did not correlate significantly with the clinical outcome. Three of four patients without T2-weighted high signal focus of the ECRB origin on preoperative magnetic resonance imaging (MRI) showed higher Quick-DASH score. Conclusion: Arthroscopic release of the ECRB is an effective option for chronic recalcitrant lateral epicondylitis. Lack of high signal focus on preoperative MRI is significantly associated with a poor surgical outcome and a considerable factor for the proper surgical indication.

A Pilot Study to Evaluate the Reliability of Pattern Identification Tool for Knee Osteoarthritis and to Analyze Correlation between Pattern Identification Tool and Knee Range of Motion, Visual Analog Scale and Western Ontario & Mcmaster Universities Osteoarthritis Index (퇴행성 무릎관절염 변증도구의 신뢰도 평가 및 무릎 ROM, VAS, WOMAC과의 상관관계에 대한 탐색적 연구)

  • Objectives To evaluate the pattern identification tool for knee osteoarthritis and to investigate the relationship between pattern identification tool and knee ROM (range of motion), VAS (visual analog scale) and WOMAC (Western Ontario & McMaster Universities osteoarthritis index). Methods We studied 50 patients who are diagnosed as degenerative osteoarthritis. With one patient dropping out, total of 49 patient went through pattern identification tool. Re-test of the pattern identification tool was implemented after about a week. The reliability was analyzed by calculating ICC (intraclass correlation coefficient). Also, reliability of each pattern identification score was calculated along with influence factor and correlation between knee ROM, VAS and WOMAC. Results Reliability of the pattern identification tool was evaluated as 'poor agreement beyond chance' with ICC value of 0.396. Reliablility of each pattern identification score was calculated and four out of five were measured 'good' with one 'moderate' reliability. Two of inter-item consistency were 'good', two were 'acceptable' and one was 'questionable'. Correlation between each pattern identification and knee ROM showed 'clear negative linear relationship' with two patterns. VAS showed 'clear positivie linear relationship' with all five patterns. WOMAC showed 'positivie linear relationship' with all five patterns. Conclusions According to the results, pattern identification tool for knee osteoarthritis seems to have meaningful relationship with other agnedas. Also, further research is needed to develop the tool.

Comparing the Effects of Stability Exercise, ESWT, and Taping for Patients with Myofascial Pain Syndrome of Upper Trapezius (안정화 운동, 체외충격파, 테이핑이 상승모근 근막통증 증후군에 미치는 효과 비교)

  • Purpose: In this study, the effects of stability exercise, extracorporeal shock wave therapy, and taping on pain and function in patients with myofascial pain syndrome of upper trapezius were compared. Methods: The subjects were divided into the stability exercise, ESWT and the taping treatment group and the clinical outcomes were evaluated by visual analog scale (VAS), pressure pain threshold (PPT) and a constant-murley scale (CMS) at pre-treatment and post-treatment. Paired t-test and ANOVA was used for statistical analysis. Results: All groups were statistical significance in the change in visual analog scale (p<0.05). The difference between the ESWT group and taping group was statistical significance in the change in pressure pain threshold (p<0.05) except for the taping group. Using the constant-murley scale, the stability exercise group showed a significant decrease in pain, and a significant increase in ROM, ADL, strength, total score of shoulder (p<0.05); however, the ESWT group showed no difference on ADL. In addition, there was no difference in strength for the taping group. The comparison of the effect between the stability exercise group, ESWT group and taping group in CMS showed a statistical significant difference in pain, ADL and ROM (p<0.05). Conclusion: These results indicate that stability exercise, ESWT and taping could be considered an effective and efficient treatment modality for myofascial pain syndrome of upper trapezius.

Effects of Korean Medicine Treatments on Pain Reduction of Thoracolumbar Compression Fracture Patients : A Restrospective Observational Study (흉추 및 요추 압박골절 환자의 통증 감소에 대한 한의학적 치료 효과 : 후향적 관찰연구)

  • The aim of the study was to investigate the effect of Korean medicine treatments on pain reduction of thoracolumbar compression fracture patients. We analysed 48 patients who have been diagnosed as thoracolumbar compression fracture on Computed Tomography(CT) or Magnetic Resonance Imaging(MRI) scan and had admitted to Dunsan Korean Medical hospital from January 1,2014 to April 4, 2017. The analysis was conducted as retrospective study which analyzes patient's medical records. Statistical analysis was performed using the IBM SPSS statistics 24 program. We used Visual Analog Scale(VAS) and pain reduction scale to evaluate pain reduction of patients. The result showed that there were statistically significant pain reduction on thoracolumbar compression fracture patients treated with korean medicine treatments. In conclusion, we found that the korean medicine treatments showed positive effect on pain reduction of thoracolumbar compression fracture patients.

Extracorporeal Shock Wave Therapy for Myofascial Pain Syndrome of Upper Trapezius (충격파치료를 이용한 상부 등세모근의 근막통증증후군 치료)

  • Purpose : The purpose of this study was to evaluate whether extracorporeal shock wave therapy (ESWT) is an effective treatment for myofascial pain syndrome (MPS) of upper trapezius. Methods : Study subject were 20 patients (7 male and 13 female). The patients were evaluated by assessing of pain and function using visual analog scale (VAS) and pressure pain threshold (PPT) and Constant-Murley scale (CMS). Statistical analysis was used paired T-test to know significance probability between pre-test and post-test. Results : There was no statistical significance in the change in PPT. But, VAS scores were significantly improved after ESWT (p<.05). Using the Constant-Murley score, this study showed a significant decrease in pain and a significant increase in range of motion of shoulder (p<.05). Conclusion : These results indicate that extracorporeal shock waves therapy could be considered as an effective and efficient treatment modality for myofascial pain syndrome of upper trapezius.

The relationship between neck pain and psychological state in female office workers. (경항부 통증과 심리적 상태와의 상관성에 관한 연구)

  • Objectives : The purpose of this study was to investigate the relationship between pain and psychological state in female office workers with neck pain. Methods : Experimental group of 31 healthy subjects complained of neck and arm discomfort related to computer use which lasted more than 3 months in the past year and was present in the past 7 days as well as on the day of test. Outcomes were assessed by meridian-electromyography(MEMG), Neck Disability Index(NDI), Visual Analog Scale(VAS), Beck Depression Inventory(BDI), Stress Reaction Inventory(SRI), Holmes & Rahe Social Readjustment Rating Scale(SRRS). Results : The NDI score and BDI score had a correlationship(r=0.48), and the NDI score and SRI score also had a correlationship(r=0.48), significantly. The NDI score and contraction power of upper trapezius by MEMG had a correlationship significantly, but with the other muscles the NDI score didn't have correlationships. The NDI score and the SRI score were significantly higher in depression group(BDI score ${\circ}{\surd}$14). The contraction power of trapezius by MEMG was significantly lower in depression group. Conclusions : The results suggest that mental stress can be a major risk factor of neck pain in female office workers.

A Case Report : Complications of Chemotherapy after Curative Resection in Patient with Ovarian Carcinoma IV (근치적 절제술 시행한 난소암 환자에서 항암치료 후유증 치험 1례)

  • Objectives: We designed this study to evaluate the effect of Korean medical treatment on complications of chemotherapy after curative resection in patient with ovarian carcinoma IV. Methods: The patient got total abdominal hysterectomy and bilateral salpingo-oophorectomy (TAHBSO) on 9/4 and received chemotherapy on 10/7, 10/28, 11/18. During this period, the patient suffered from anorexia, dyspepsia, nausea, weight loss and insomnia. We treated the patient with herbal medicine and acupuncture. The efficacy of treatment was evaluated with visual analog scale (Vas), weight and 36-item short form health survey instrument (SF-36). Results: After treatment, although the scale values went ups and downs according to chemotherapy schedule, abdominal pain Vas changed 7 to 0, weight changed 46 kg to 51 kg. Also, SF-36 scores increased. Conclusions: This case report shows that the Korean medical treatment is useful in the treatment of complications of chemotherapy after curative resection in patient with ovarian carcinoma IV.

A Case of Syringomyelia with Back and Shoulder Pain

  • The purpose of this study was to describe the treatment of the clinical symptoms of syringomyelia using Korean medicine. A patient with syringomyelia complained of back and shoulder pain, numbness of the upper right limb, headaches and dyspnea. He was treated with herbal medicine, acupuncture, moxibustion, and physical therapy. The Visual analog scale (VAS) was used to score sensory symptoms (pain, numbness) and Modified Borg scale (mBorg) was used to score dyspnea. After the treatment, his symptoms were relieved. The patient's chief complaint was back and shoulder pain which was reduced greatly from a VAS score of 6 between 1-2. The results in this study may contribute to the development of the Korean medicine field of syringomyelia in the future.

Effects of Auricular Acupressure on Low Back Pain and Headache in Nurses (귀 지압이 간호사의 요통과 두통에 미치는 효과)

  • Purpose: The aim of the study was to examine the effect of auricular acupressure on low back pain and headaches in nurses. Methods: A open-label randomized controlled trial was used. Thirty-nine nurses with low back pain for more than 3 months participated in this study. Auricular acupressure stickers were applied to the participants's waist, head and Shen men in the experimental group for 2 weeks. The Numeric Rating Scale (NRS), the Visual Analog Scale (VAS), and the Headache Impact Test-6 (HIT-6) were administered to measure participants' pain. Results: The mean differences of the VAS and the HIT-6 were significant in the experimental group at the pre and post-tests. A repeated-measures ANOVA revealed that low back pain and headache measured using NRS in the experimental group decreased significantly compared to the control group at the pre and post-tests over 2 weeks. Conclusion: The results of this study indicated that auricular acupressure could be used as a useful intervention for self-care for nurses with low back pain and headache.
