청신경초종에 대한 감마나이프 방사선 수술 (Gamma-Knife Radiosurgery for Vestibular Schwannoma)

  • Object : The goals of radiosurgery include preservation of neurological function and prevention of tumor growth. We document the results of gamma-knife radio-surgery for vestibular schwannoma. Method & Object : Eighty-two patients underwent stereotactic radiosurgery for an vestibular schwannoma from October, 1994 to December, 2000. Sixty-five of these patients were followed up for radiological and clinical evaluation. As pregamma-knife modality, surgical resection were done in 23 patients,and V-P shunt in 2 patients. Initial symptoms were headache(n=45), dizziness(n=16), tinnitus(n=17). While normal facial function(House-Brackmann grade 1) was present in 48 patients(73.8%), other patients showed grade 2 function in 8, grade 3 function in 7,and grade 4 function in 2. The Gardner/Robertson scale was used to code hearing function. Male to female ratio was 1:3. Mean tumor volume was $7.98cm^3$. Mean dose delivered to the tumor margin was 14.2Gy,and mean maximal dose was 28.3Gy. Results : Mean follow-up duration of 19.9 months. Thirty-five showed decrease(53.8%) in size, 19 patients(29.2%) stationary, 3(4.6%) initial decrease follow up increase, 5(7.6%) initial increase follow up decrease,and 59 patients (90.8%) were well controlled. Two patients experienced transient facial neuropathy, one transient trigeminal neuropathy, and one transient hearing deterioration. After gamma-knife radiosurgery, ventriculoperitoneal shunt was done in 4 patients. Conclusions : Gamma-knife radiosurgery can be used to treat postoperative residual tumors as well as in patients with concomitant medical problems in patients with preserved hearing function. Gamma-knife radiosurgery is safe and effective method to treat small, medium sized(less than 3cm in extracanalicular diameter), intracanalicular vestibular schwannoma, associated with low rate of cranial neuropathy.

  • 일상 생활에서 반사적인 자세 조절은 정전기관, 시각, 고유수용체 등의 감수체에 의한 구심성 신호가 정전 신경핵에 전달되어 안구운동이나 골격근의 수축을 유발하여 이루어진다. 자세조절기능의 부조화는 오심, 구토, 현기증 등을 초래하여 삶의 의욕을 상실하게 만든다. 본 연구의 목적은 정전기관의 반규관과 이석기관을 선별적으로 자극하기 위한 탈수직축 회전자극 시스템과 수평, 수직, 3D 환경에서 시각자극을 할 수 있는 시운동자극시스템을 개발하여 인간의 자세조절 기능을 평가 하는데 있다. 탈수직축 회전자극 시스템은 편안한 의자와 DC 서보모터로 구동되는 기울임 테이블로 구성되고 PMSM에 의해 제어된다. 그리고 속도 궤환 루프와 위치 궤환 루프를 포함하는 이중 피드백 루프가 서보 제어되는 회전자극시스템에 적용된다. HMD를 이용하여 수평, 수직, 3D 시각자극패턴을 구현 하였고 OKN 자극을 위한 무선시스템과 전기안진 기록계를 이용하여 안구운동을 기록하였다. 이득, 위상, 대칭성은 정전 자극과 시각 자극에 의해 유발되는 안구운동의 분석으로부터 얻어진다.

Effects of a Vestibular Stimulation Training Program on the Gait of Chronic Stroke Patients

  • PURPOSE: This study examined the effects of a vestibular stimulation training program on the walking ability of chronic stroke patients over a six month period. METHODS: Forty stroke patients were enrolled in this study. The patients were divided randomly into a control group (n=20) and experimental group (n=20). A general exercise program was applied to Group I and vestibular stimulation training was applied to Group II(30 min, three times a week for six weeks). The changes in straight walking ability, curved walking ability, and functional walking ability were measured using a 10 m walking test figure-of-eight-walking test, and dynamic gait index, respectively. The measures before and after the program were compared using a paired t-test for a comparison of each group and an independent t-test for a comparison between groups. RESULTS: The changes in each group were examined according to the measurement period. The Experimental group showed significant functional improvement in all three tests after the vestibular stimulation training program, but the control group did not show significant improvement in any of the tests after the general exercise program. A comparison of the changes between groups revealed the experimental group to show significantly higher improvement than the control group in all tests. CONCLUSION: The vestibular stimulation training program helps improve the gait function of stroke patients. Based on the results of this study, it is expected that various vestibular stimulation training programs will be developed and applied in a range of places.

평형감각 측정기술의 개발 (Development for a method of measurement in equilibrium sense)

  • The study was performed to evaluate the sense of equilibrium by means of rotary chair system in middle school girls with dance learning. Control of equilibrium, which is the most important function in dancing, is maintained by vestibular, visual, and proprioceptive inputs. Experimental groups were divided into trained group with dance (experimental group, 13~14 ys, n=15) and untrained group (control group, 13~14 ys, n=15). experimental group was trained by programmed dancing for 12 weeks, 45 min/day and 5 day/week. Gain of eye movement was measured for vestibulocular reflex (VOR), visual vestibular stimulation (VVOR), optokinetic stimulation(OKN), and visual fixation (VFX). The gain of eye movement at higher than 0.08 hz was significant decrease in experimental group, and VVOR showed the similar fashion to VOR. The gain in OKN and VFX was not significant difference between experimental and control groups. These results suggest that programmed training of dancing improves control of balance by adaptation of the vestibular function, and rotary chair system would be useful to evaluate the sense of equilibrium ability.

  • The purpose of this study was to develop systematic diagnostic system testing easily, rapidly vestibular function of patients suffered from vestibular syndrome such as nausea vomiting, dizzness, ataxia. Diagnostic system composed of rotatory chair system which rotated sinusoidally patients against their vertical axis for purpose of invoking eye movement by vestibulo-ocular reflex and the softwares which storaged eye movement into computer and analyzed eye movement. Rotatory chair system consisted of comfortable chair and DC servomotor with reducer(1:80) by controlled servo in field of nonlinear motor control, double feedback loops system containing velocity feedback loop and position feedback loop was applied to this sever controlled rotatory chair system. Maximum rotatory velocity of rotatory chair was upto 60 degree per second and frequency range was 0.01 to 0.64 Hz. These above results suggest that clinical rotatory chair system may test easily, rapidly vestibular function and diagnose etiology of dizziness, thus giving effective assistance on the treatment of dizziness patients.

Effects of Microgravity on Vestibular Development and Function in Rats: Genetics and Environment

  • Our anatomical and behavioral studies of embryonic rats that developed in microgravity suggest that the vestibular sensory system, like the visual system, has genetically mediated precesses of development that establish crude connections between the periphery and the brain. Environmental stimuli also regulate connection formation including terminal branch formation and fine-tuning of synaptic contacts. Axons of vestibular sensory neurons from grabistatic as well as linear acceleration receptors reach their targets in both microgravity and norm81 gravity, suggesting that this is a genetically regulated component of development. However, microgravity exposure delays the development of terminal branches and synapses in gravistatic but not linear acceleration-sensitive neurons and also produces behavioral changes. These latter changes reflect environmentally controlled processes of development.

Evaluation of Saccule Function in Patients with Vitamin D Deficiency

  • Background and Objectives: Considering important role of vitamin D in many physiological processes including vestibular system in the ear, aim of present study is to evaluate saccule function via cervical vestibular evoked myogenic potential (cVEMP), in patients with vitamin D deficiency. Subjects and Methods: After routine audiological tests, cVEMP were recorded in 15 patients with vitamin D deficiency and 16 normal subjects. The short tone burst (95 dB nHL, 500 Hz) was presented to ears. cVEMP was recorded with surface electromyography over the contracted ipsilateral sternocleidomastoid muscle. Results: Mean of p13, n13, interpeak latencies and amplitude ratios were measured in both groups. Statistical analysis did not show differences between two groups. Conclusions: Maybe serum 25-hydroxyvitamin D concentration was not low enough to have effect on saccule in the patients in present study or saccule have had low susceptibility to effects of vitamin D deficiency. For better judgment about effect of vitamin D deficiency on saccular function planning studies with high sample size is recommended.

  • A control of the body posture and movement is maintained by the vestibular system, vision, and proprioceptors. Afferent signals from those receptors are transmitted to the vestibular nuclear complex, and the efferent signals from the vestibular nuclear complex control the eye movement. The postural disturbance caused by loss of the vestibular function results in nausea, vomiting, vertigo and loss of craving for life. The purpose of this study is to develop a off-vertical rotatory system for evaluating the function of semicircular canals and otolith organs, selectively, and visual stimulation system for stimulation with horizontal, vertical and 3D patterns. The Off-vertical axis rotator which stimulates semicircular canals and otolith organs selectively is composed of a comportable chair, a DC servo-motor with reducer and a tilting table controlled by PMSM. And a double feedback loop system containing a velocity feedback loop and a position feedback loop is applied to the servo controlled rotatory chair system. Horizontal, vertical, and 3D patterns of the visual stimulation for applying head mounted display are developed. And wireless portable systems for optokinetic stimulation and recording system of the eye movement is also constructed. The Gain, phase, and symmetry is obtained from analysis of the eye movement induced by vestibular and visual stimulation. Detailed data were described.

특발성 파킨슨병 환자의 전정유발근육전위 (Vestibular Evoked Myogenic Potential in Idiopathic Parkinson's Disease)

  • Background: Idiopathic Parkinson's disease (IPD) is closely related to Lewy body pathology. Pathological changes in medullar oblongata and pontine tegmentum have been reported in patients with subclinical motor symptom. Vestibular evoked myogenic potential (VEMP) is mediated by vestibular nuclei in lower brainstem and reflects the function of lower brainstem. The purpose of our study is to estimate the lower brainstem function in IPD patients. Methods: Ten patients with idiopathic Parkinson's disease underwent VEMP test. The patients were divided into Hohn-Yahr (H-Y) stage I (unilateral motor involvement) group and H-Y stage II or more severe (bilateral motor involvement) group. VEMP results were compared between groups using Mann-Whitney U test. Results: Among patients, 6 patients showed abnormal VEMP (unilateral abnormality 2, bilateral abnormalities 4). Between H-Y stage I group and H-Y II,III group, there was no statistical difference in the results of VEMP. Conclusions: We concluded that the lower brainstem dysfunction reflected in VEMP could occur in IPD regardless of the progression of the disease.
