The Effect of Very Low Calorie Diet and Chegamuiyiin-tang on Bone Mineral Density (초저열량식이와 체감의이인탕을 병행한 비만치료가 골밀도에 미치는 영향)

  • Objectives : The weight reduction program with very low calorie diet(VLCD) for women generally reduce bone mineral density(BMD). So many treatments were done to prevent side effect of the weight reduction program and herbal diet meditation were often used in oriental medicine. This research were done in order to study the effect of VLCD and herbal diet meditation, Chegamuiyiin­tang on BMD. Methods : This clinical research carried out 22 cases of pre-menopausal outpatients with obesity during 1 month. To evaluate the effect Chegamuiyiin-tang with VLCD on BMD, the weight, BMI, percent body fat, WHR, BMD(T-score) were estimated in pre-treatment and post-treatment. Results : The mean weight reduction after 1 month was $6.87{\pm}1.54\;Kg$(10.28 % of initial weight), BMI reduction was $2.35{\pm}0.65\;Kg/m^2$, percent body fat reduction was $3.40{\pm}1.01%$, WHR reduction was $0.04{\pm}0.01$. But BMD(in T-score) had significant increase. Conclusions : Generally VLCD were estimated to reduces BMD, but VLCD with herbal diet meditation, Chegamuiyiin-tang has positive effect on BMD in pre-menopausal women during short term(1 month).

Effect of Wild Plant Extracts Based on Korea Traditional Prescription on Endurance Exercise Performance during Calorie Restriction Period (초저열량식이로 인한 체력 저하에서 산야초추출물의 지구력 증진 효과)

  • Effects of wild plant extract (Lak) based on Korea traditional prescription on maximal exercise performance and endurance were evaluated using calorie-restriction animal model. In acute forced swimming test with 10% body weight attached to tail, dietary Lak supplementation increased exercise performance endurance by increasing concentrations of ATP and insulin-like growth factor-1 (IGF-1) under calorie-restriction condition, and decrement of blood lactic acid concentration and increment of muscle ATP content were observed. These results suggest Lak is very effective for decreasing side-effects of obesity therapy using very low calorie diet.

A study on the Improvement of Menu Patterns of Gruels as Soft Diet In Hospital Foodservice Operation (병원 급식의 죽식 식단 개선에 관한 조사 연구)

  • This study was carried on the analysis of nutritional value and preference between the plain gruel (Hyun Jook) and the experimental gruel (Tarak Jook, Jangkook Jook & Chajohonhab Jook) in order to improve the quality and variety of menu patterns of gruels for the patients requiring soft diet. The results obtained are as follows: 1. In comparing the calorie coutent of each gruel, Tarak Jook, Jangkook Jook, and Chajohonhab Jook had more calories-82.7%), 39.3% and 12.4% respectively-than Hyun Jook. Additionally, in the case of protein content, Tarak Jook, Jangkook Jook and Chajohonhab Jook showed higher values than Hyun Jook by 270%, 137% and 28.3% respectively. 2. The preference for each experimental gruel compared with Hyun Jook, as a function of taste, color and concentration of the gruels was shown to be insignificant. But the color and concentration did effect the preference level for each gruel. Hoverer, patients did have a preference for particular gruels: Jankook Jook, Tarak Jook, Chajohorhab Jook in descending order. 3. Overall, 65.6% of the patients requiring soft diet thought gruel was only for the sick 57.1%) thought the reason for gruel as a soft diet was that it could be digested easily 75.8% of the patients disliked gruel and their frequency of intake was very low 78.8% did not know more than 5 types of gruel. Hoverer patients did prefer a loose gluel(48.4%). In couclusion, awareness of gruel as a soft diet for patients and its intake as one not to meution as a normal diet was low.

Analysis of Food Intake and Physical Activity in Randomized Controlled Trials on Herbal Medicine for Treatment of Human Obesity (비만 치료 한약 무작위 대조 임상시험에서의 음식 섭취량과 운동량 실태분석)

  • Objectives: The objective of this study was to analyse the methods being used to control food intake and physical activity in RCTs of human obesity. Methods: A total of 21 randomized controlled trials (RCTs) were investigated. Nine of which were domestic studies from "http://oasis.kiom.re.kr" and the other of which were foreign studies from systematic reviews of RCTs on herbal medicine for treatment of human obesity. Results: According to domestic studies, "low calorie diet" were recommended in five cases of the domestic studies, "maintain current dietary habit" were recommended in two and no information on diet was two. Considering the seven cases where the information on diet was available, patients' food intake were checked at every visit in six cases. Only two cases among the six had been dropped owing to the violation of dietary habit by patients. Exercises were prohibited in two cases, "maintain current level of phisical activity" were recommended in three cases and, from the rest, no information was available. The level of physical activity were not strictly controlled by any means hence no drop out. According to foreign studies, "low calorie diet" were recommended in two cases, "very low calorie diet (less than 700 kcal/day)" in one case, "maintain current dietary habit" in two cases, "do not eat fat" in two cases and no information was available in the rest five cases. Exercises which concerns spending about 300 kcal/day was recommended in one case, "moderate exercise" were recommended in three cases, "maintain current level of physical activity" were recommended in three cases and no information available in the rest five cases. Conclusions: In order to improve the accuracy of RCT, for the dietary side, researchers should record patient food intake at every visit by means of 24-hour dietary recall methods. This can be supplemented by multiple choice survey that are designed to help patients to diagnose themselves more accurately leading to less bias. For the exercise side, it is highly recommended to confine the exercises to walking only so as to quantify the amount of physical activity more easily by using pedometer.

  • Childhood obesity is harmful to health and growth. We had experienced a childhood obese patient. From Feb. 28. 2003 to July. 14. 2003. We applied Chegameuiyin-tang(體感意苡仁湯), electrolipolysis and VLCD(very low calorie diet), behavioral therapy, aerobic exercise, infra-red therapy, auricular acupuncture, and aqua-massage. Through this obesity treatment during 5 months, total weight loss was 12.2kg, reduction of percent body fat was 13.4% (from 38.5 to 25.1), her condition and hepatic index(AST, ALT) was improved.

  • Objectives: The modified fasting therapy(MFT) of traditional korean medicine uses very low calorie diet on a patient with chronic disease and obesity. During the fasting period of MFT, calorie intake is restricted under 600kcal. Adverse events such as hunger, heartburn, headache, and hair loss can occur because of fasting. In case of Gilbert's syndrome, Jaundice can arise without disorder of liver function and hemolysis because of increase of unconjugated bilirubin. We experienced a case of Gilbert's syndrome patient who appeared jaundice during the fasting period, so we report about that with literature review.

  • Iron deficiency is still a common nutritional disorder in the world. In developing countries, the bioavailability of dietary iron is often very low, mainly due to a low content of factors facilitating nonheme iron absorption. The iron content of the diet, iron status of subjects, and the actual composition of the diet are the major factors that influence the absorption of food iron. Inadequate dietary intake of iron often results from low-calorie diets, food restrictions, or single food diets. Ascorbic acid and MFP (meat, fish and poultry) are the quantitatively most important enhancers of nonheme iron absorption found in the diet. Ascorbic acid and meat have consistently been shown to enhance iron bioavailability. Major inhibitors of nonheme iron absorption are dietary fiber, phytate, and polyphenols. The availability of nonheme iron can be highly influenced by components of foods ingested concomitantly, Therefore, consumption of food in combinations can either enhance or inhibit nonheme iron absorption.

Effect of Coicis Semen on Starvation Stress in Mice (억이인이 생쥐의 기아 Stress에 미치는 영향)

  • Objective : In 2001, the rate of obesity in Korea reached 30.6%. There are many therapeutic ways to reduce body weight, such as low and very low calorie diet, exercise therapy, behavior modification therapy, etc. However, in many cases the patients feel stress under obesity treatment because of starvation. This study was aimed to evaluate the anti-starvation stress effect of Coicis Semen on mice. Methods : First, the mice were divided into 6 groups : Normal (group with no starvation), Control (administrated normal saline 6 times before starting 36 hours starvation), and Samples A, B, C, and D (administrated 0.25, 0.5, 1.0, 3.0 g/kg Coicis Semen respectively 6 times before starting 36 hours starvation). Then the plasma corticosterone level and rectal temperature were measured. The norepinephrine, dopamine, DOPAC (dihydroxy-phenylacetic acid), 5-HT (5-hydroxytryptamine) and 5- HIAA (5-hydroxy-indole-acetic acid) in the hypothalamus were measured by the HPLC method. Result : I. The rectal temperature in Sample group D showed a significant difference (P<0.05) compared with the Control group. 2. The DOPAC in Sample groups A, C and D showed the significant difference (P<0.05) compared with the Control group. Conclusion : It might be recognized that Coicis Semen has an anti-starvation stress effect.

A Case Study on Effect of Herbal Diet an Male Abdominal Obese Patient (남성 복부비만 환자의 치험 1례)

  • We experienced a male patient who was diagnosed as very severe abdominal obesity. From the 3rd of January to the 14th of April we applied herbal medication(Tiganyiyiren-tang), very low calorie diet, aerobic exercise, behavioral therapy, electrolipolysis, infra-red therapy, auricular acupuncture, and aqua-massage to him. And we examined the changes of his height, weight, body composition, body size, liver functional test, abdominal CT finding. The food production of this year has decreased by 5 percent. His weight decreased by 28.2%, PIBW by 28.5%, BMI by 28.2%, PBF by 22.7%, and WHR by 15.9%. The circumference of upper extremity decreased by 25.7%, the circumference of the muscle of upper extremity by 18.2%, the circumference of chest by 20.5%, the circumference of abdomen by 29.5%, the circumference of hip by 17.4%, and the circumference of thigh by 15.9%. In the liver functional test GOT decreased from 42IU/l to 361U/I, GPT from 1211U/l to 48IU/I. Total cholesterol from 211mg/dl to 152mg/dl, and Trigliceride from 192mg/dl to 81 mg/dl. In the abdominal CT finding the amount of fat in the abdominal cavity decreased from $9732mm^2$ to $3639mm^2$.

Effects of Dietary Food on Body Weight, Plasma Lipids and Glucose in Rats (다이어트 식품이 흰쥐의 체중, 혈중지질 및 혈당농도에 미치는 영향)

  • The diet food used in this test was composed of the total 16 kinds, such as refined water, herb extract, pomegranate extract, polydextrose and isomalto-oligosaccharide etc. Edible fiber as a major component in dietary food was contained to 2.60%(w/w) level. But the calorie was 104.57kca1/100g, contrast of control domestic saengshick product was 368.06kca1. As a result, calorie of dietary food sample was very low level. Vitamin and mineral as a minor component were determined 5, 6 kinds, respectively. On the other hand, dietary effect of body weight loss was determined about 14%(w/w) at animal test result used for rats during 4 weeks. The reason of dietary effect could confirmed from change of plasma neutral lipids, total cholesterol and HDL-cholesterol concentration. Increasing ratio of these in the general diet and dietary diet feeding groups during 4 weeks were 10.55, 6.90, 9.82% and 6.11, 3.85, 6.11%, respectively. from these results, decrease reason of body weight could confirmed the relative relationship with plasma lipids and cholesterol concentration. But plasma plasma glucose and insulin concentration have not positive relativity.
