  • Arrhythmia has recently emerged as one of the major causes of death in Koreans. Premature Ventricular Contraction (PVC) is the most common arrhythmia that can be found in clinical practice, and it may be a precursor to dangerous arrhythmias, such as paroxysmal insomnia, ventricular fibrillation, and coronary artery disease. Therefore, we need for a method that can detect an abnormal heart beat and diagnose arrhythmia early. We extracted the features corresponding to the QRS pattern from the subject's ECG signal and classify the premature ventricular contraction waveform using the features. We modified the weighting and bias values based on the error back-propagation algorithm through learning data. We classify the normal signal and the premature ventricular contraction signal through the modified weights and deflection values. MIT-BIH arrhythmia data sets were used for performance tests. We used RR interval, QS interval, QR amplitude and RS amplitude features. And the hidden layer with two nodes is composed of two layers to form a total three layers (input layer 0, output layer 3).

  • Echocardiography was performed on 29 normal Korea Jin-do dogs(21 adults and 8 prematures) and the images were taken from chordae tendineae level short-axis view, left ventricular outflow tract long-axis view and 4-chamber long-axis view, respectively. The following cardiac parameters, in systole and diastole, were measured on the echoocardographic images: aortic dimension(AO), left atrial dimension(LA), left ventricular internal dimention at end-diastole (LVIDd), left ventricular internal dimention at end-systole(LVIDs), interventricular septal wall thickness at end-diastole(IVSd), interventricular septal wall thickness at end-systole(IVSs), left ventricular wall at end-diastole(LVWd), left ventricular wall at end-systole(LVWs). The cardiac parameters were not significantly different between sex and among imaging planes. The size of cardiac parameters increaed as regression lines according to body weights. These results suggest that the chordae tendineae level short-axis view, left ventricular outflow tract long-axis view and 4-chamber long-axis view can be utilized for the left cardiac examination, and these cardiac parameters can be used to basic dat for diagnosing the cardiac diseases in Korea Jin-do dog.

Effects of Higenamine on the Calcium Current and the Action Potential in the Guinea-pig Myocytes (Higenamine이 단일심근세포의 Ca-전류 및 활동전압에 미치는 효과)

  • The effects of higenamine were investigated in the single atrial and ventricular myocyte of the guinea pig by using patch clamp method. The results obtained were as follows: 1) Isoprenaline which is known to be ${\beta}-agonist$ increased the duration of action potential and calcium current in ventricular cells. 2) Higenamine also increased the duration of action potential and calcium current in ventricular myocytes. And its effect was blocked by propranolol. 3) In the atrial cells, isoprenaline showed ${\beta}-agonist$ effects, which were increasing the duration of action potential and calcium current same as in ventricular cells. 4) Higenamine, however, showed the opposite effects of ${\beta}-agonist$ which were decreasing the duration of action potential and calcium current. The above results suggest that higenamine has the typical ${\beta}-agonist$ effect in ventricular cells but inhibitory effect in atrial cells and this effect on atrium could be due to the reduction of calcium current.

  • Effect of ischemic preconditioning on left ventricular function after cardiac arrest in isolated rat heart.Ischemic preconditioning reduces infarct size caused by sustained ischemia. However, the effects of preconditioning on post ischemic cardiac function are not well-known. The objective of the present study was to determine whether preconditioning would improve the recovery of left ventricular functions after cardiac arrest in isolated rat heart model.Isolated rat hearts were allowed to equilibrate for 20 minutes and were then subjected to either 5 minutes of global, normothermic transient ischemia [Group 2 and 4] or not [Group 3]. A stabilization period of perfusion lasting 5 minutes after the termination of transient ischemia was followed by a standard global, normothermic 20 minute-ischemia and 35-minute reperfusion challenge [Group 3 and 4]. These following results were odtained.1. The recovery of left ventricular developed pressures showed no significant differences between Group 3 and Group 4 at 50 [P>0.3] and 85 minute [P>0.2].2. Heart rates showed no significant differences throughout all the course of experiment and between groups [P>0.5].3. The recovery of left ventricular maximum dP/dt showed no significant differences between Group 3 and Group 4 at 50 [P>0.1] and 85 minute [P>0.2].4. The recovery of pressure-rate products showed no significant differences between Group3 and Group 4 at 50 [P>0.5] and 85 minute [P>0.1].These results suggest that ischemic preconditioning does not provide significant benefit for the postischemic left ventricular functions in isolated rat hearts.

The Characteristics of Electrocardiography Findings in Left Ventricular Remodeling Patterns of Hypertensive Patients

  • The exact diagnosis of left ventricular hypertrophy (LVH) is very important in the treatment of hypertension. The purpose of our study is to determine the relationship between left ventricular remodeling patterns and electrocardiography (ECG) findings in hypertensive patients. We divided 137 patients into four groups according to left ventricular mass index (LVMI) and the relative wall thickness: normal, concentric remodeling, concentric hypertrophy, eccentric hypertrophy. LVH on the ECG was defined by three ECG criteria: Sokolow-Lyon voltage criteria, Cornell voltage criteria and Romhilt-Estes point score. LVH on the echocardiography was defined by LVMI. The prevalence of ECG LVH was increased in concentric hypertrophy and eccentric hypertrophy group. The QRS voltages by Sokolow-Lyon voltage criteria (r = 0.494, P = 0.002) and Cornell voltage criteria (r = 0.628, P < 0.001), and Romhilt-Estes point score (r = 0.689, P < 0.001) were positively correlated with LVMI. Also, the QRS voltages and point scores were significantly increased in the concentric hypertrophy and eccentric hypertrophy group with increased LVMI. The QRS voltage and Romhilt-Estes point scores were positively correlated with LVMI. The QRS voltages and Romhilt-Estes point scores were also increased in the left ventricular remodeling groups with increased LVMI.

Died immediately after corrective surgery for right ventricular acute myocardial infarction and ventricular septal rupture

  • Postinfarction ventricular septal rupture (VSR) is a rare but fatal complication of acute myocardial infarction. In many cases, postinfarction VSR leads to hemodynamic instability and urgent surgical treatment is necessary. Here we describe a case of a patient with right ventricular (RV) dysfunction caused by acute RV infarction and with cardiogenic shock, whose condition improved after development of postinfarction VSR, but the patient died after corrective surgery.

  • A 38-year-old female patient with a history of tetralogy of Fallot repair at 10 years of age underwent pulmonary valve replacement with a mechanical prosthesis, tricuspid annuloplasty, and right ventricular outflow tract cryoablation due to pulmonary regurgitation, tricuspid regurgitation, and multiple premature ventricular contractions with sustained ventricular tachycardia. After surgery, she had an uneventful postoperative course with arrhythmia monitoring. She was discharged without incident, and a follow-up Holter examination showed a decrease in the number of ventricular ectopic beats from 702 to 41.

  • Congenital diverticula of the cardiac ventricle have been reported as arising either from the left ventricle or, rarely from both ventricles. A diverticulum arising from the right ventricle alone is very rare. Furthermore the diverticulum associated with double chambered right ventricle was extremely rare. We experienced a 62 years old female of double chambered right ventricle combined with congenital right ventricular true diverticulum. She had complained intermittent chest pain and mild dyspnea on exertion during 8 months. The chest X-ray and chest CT showed protruded abnormal density at anterolateral side of right ventricular outflow tract. Preoperative angiography demonstrated a double chambered right ventricle and a right ventricular diverticulum. In operative finding, there was found a anomalous muscle band, dividing the right ventricle into an inflow and outflow portion, and a 5x6cm sized right ventricular diverticulum arised from conus region with a stenotic orifice of 1.5cm in diameter. The diverticulum was open toward the infundibulum, and its orifice was approximately 1cm in diameter. On treatment, the diverticulum orifice was closed directly and the abnormal muscle band was resected in order to widen the right ventricular outflow tract. The postoperative result was satisfactory and good without specific complications.

  • Ventricular septal defects (VSDs) is an opening in the ventricular septum that allows blood to flow between the ventricles. Most ventricular septal defects are located in the upper ventricular septum and can be identified by auscultation. In this report, 2 dogs with heart murmur were diagnosed as VSD using radiography and echocardiography. In radiographs, bulging sign of the main pulmonary artery or the enlargement of the left ventricle was observed. The color Doppler examination showed the left-to-right shunting of blood via interventricular septal defect.

Emergency Neuroendoscopic Management of Third Ventricular Neurocysticercosis Cyst Presented with Bruns Syndrome : Report of Two Cases and Review of Literature

  • Neurocysticercosis is the commonest parasitic disease of the human central nervous system. The incidence of intra ventricular form of neurocysticercosis (NCC) is less common accounting 10-20% that of total central nerve system cysticercosis. Intra ventricular NCC is complicated due, to its high incidence of acute hydrocephalus caused by ball valve mechanism. The only reliable tool for diagnosis of NCC is by neuroimaging with CT or MRI. MRI preferred over CT because of its high specificity and sensitivity. In emergency situations like acute hydrocephalus one can proceed with emergency endoscopic surgery. Through the endoscopic view, intra ventricular NCC (IVNCC) has distinguished morphological features like the full moon sign. This feature not only helps in identification of IVNCC, but also guides in further endoscopic treatment strategy. Authors report two cases of 3rd ventricular NCC with acute hydrocephalus managed with emergency endoscopy. Authors have discussed the clinical features, intra operative endoscopic findings and role of endoscopy in emergency surgery for NCC with acute hydrocephalus.
