Microleakage of class II posterior composite resin filling using various light curing methods (구치부 광중합 복합레진 수복시 여러 광조사 방법에 따른 미세변연누출에 관한 연구)

  • 목적 - 최근 복합레진의 미세변연누출을 줄이기 위한 새로운 광중합 조사방법이 연구되고 있다. 또한 구치용 복합레진이 개발되면서 그 제조회사에서는 5mm 깊이까지 광중합이 가능하다고 소개하고 있다. 본 연구에서는 논리적 가설에 근거한 몇가지 광중합 조사방법이 구치부 class II 와동의 복합레진 충전시 미세변연누출에 어떠한 영향을 미치는지를 관찰하였다. 재료 및 방법 - 100개의 우식증이 없는 사람의 상하악 대구치를 사용했다. 각 치아에 교합-치은방향으로 4mm, 협설 4mm, 깊이 2mm의 class II 와동을 형성하여 인접치와 함께 베이스플레이트 왁스에 매몰하고 구치용 복합레진인 Surefil을 제조 회사의 지시대로 충전하였다. 이때 5가지의 광조사방법을 이용하여 5개의 군으로 나누어 중합하였다(Table 1). 수복된 치아들을 5$^{\circ}C$와 55$^{\circ}C$의 수조에서 번갈아 1분씩 총 500회의 온도변화를 주어 thermocycling을 실시한 후 근첨을 폐쇄하고 수복와동의 치은경계를 제외한 전 표면에 nail varnish를 2회 도포하였다. 2%의 methylene blue용액에 24시간 침적시키고 흐르는 물에 세척한 후 시편을 투명한 에폭시 레진에 매몰하였다. 매몰된 시편을 치아 장축에 평행하게 절단 연마하였다. 입체 현미경으로 미세변연누출을 관찰하고 Kruskal-Wallis One Way ANOVA 와 Dunn's Method로 통계처리 하였다. 결과 - 1. 미세누출은 1군, 4군과 5군, 2군 3군 순으로 증가였다. 이때 1군, 4군과 5군은 통계적 유의성이 없었다(P>0.05). 2. 2군의 경우 1군, 4군, 5군에 비교하여 유의성 있게 미세누출이 많았으며(P<0.05) 3군에 비하여 유의성 있게 적게 나타났다(P<0.05). 3. 3군의 경우 다른 방법들에 비교하여 통계적으로 유의성 있게 미세누출이 많았다(P<0.05).

  • The people with disabilities living in residential facilities have more difficulty in caring oral hygiene than those living at home. The purpose of this study is to evaluate the recent 2014 dental treatment records of free mobile dental clinic service for disabled people in Korea. 203 disabled living in residential facilities participated in mobile dental clinic. Patients classified according to types of disability. Mental retardation were 75.3%, mental disorder were 6.0%, crippled disorder were 7.4%, brain disorder were 6.5%, visual disorder were1.4%, auditory and speech disorder were 2.3% and autism disorder were 0.9%. Performed treatments were 99 scaling and curettage, 88 fluoride varnish and TBI, 4 extraction, 1 endodontic treatment, 16 caries control (resin filling, GI filling), 1 denture repair and 8 refuse the treatment. Free mobile dental clinic can not provide complex dental treatment. So, the organization should systemize advanced dental treatment and regular preventive programs. Furthermore, we need to have a more concerns about the people with disabilities living in residential facilities and constantly participate on a dental voluntary work.

THE EFFECT OF ADDITIONAL ENAMEL ETCHING ON MICROLEAKAGE OF THE ADHESION OF SELF-ETCHING PRIMER SYSTEM (자가 산부식 프라이머 시스템 사용시 인산에 의한 부가적인 산부식이 미세누출에 미치는 영향)

  • The purpose of this study is to evaluate the effect of additional enamel etching with phosphoric acid on the microleakage of the adhesion of self-etching primer system. Class V cavity($4mm{\times}3mm{\times}1.5mm$) preparations with all margins in enamel were prepared on buccal surface of 42 extracted human upper central incisor teeth. Prepared teeth were randomly divided into 3 groups. Group 1:no additional pretreatment with 37% phosphoric acid (NE). Group 2:additional pretreatment with 37% phosphoric acid for 10 seconds (E10s). Group 3:additional pretreatment with 37% phosphoric acid for 20 seconds (E20s). The adhesives(Clearfil SE $Bond^{\circledR}$, Kuraray, Osaka, Japan) and composite resins(Clearfil $AP-X^{\circledR}$, Osaka, Kuraray, Japan) were applied following the manufacturer's instructions. All the specimens were finished with the polishing disc(3M dental product, St Paul, MN, USA), thermocycled for 500 cycles between $5^{\circ}C$ and $55^{\circ}C$ and resected apical 3-mm root. 0.028 stainless steel wire was inserted apically into the pulp chamber of each tooth and sealed into position with sticky wax. Surrounding tooth surface was covered with a nail varnish 2 times except areas 1mm far from all the margins. After drying for one day, soaked the samples in the distilled water. Microleakage was assessed by electrochemical method(System 6514, $Electrometer^{\circledR}$), Keithley, USA) in the distilled water. A COMPARATIVE STUDY ON THE APICAL LEAKAGE OF RETROFILLING AFTER APICAL CAVITY PREPARATION WITH ULTRASONIC INSTRUMENT (초음파 기구에 의한 치근단 와동형성후 역충전의 밀폐성에 대한 연구)

  • This study was conducted to evaluate and compare the apical leakage in the following retrofilling techniques after apical resection; No apical cavity preparation and no retrofilling(control group), Amalgam(group I) or silver glass ionomer cement(group II) retrofilling after apical cavity preparation with mini contra-angle and bur, Amalgam(group III) or silver glass ionomer cement(group N) retrofilling after apical cavity preparation with ultrasonic micro endo tip. Extracted ninety upper anterior and lower canine teeth were fixed in skull simulators and root canals were prepared with step-back method and obturated with gutta-percha and zinc oxide eugenol sealer. Obturated roots were resected 2mm from apical ends and apical cavities of 1mm width and 2mm depth were prepared and retrofilled by above mentioned methods. After application of nail varnish on all surface except resected surface, apical 1/3 of the roots were placed in 1% methylene blue solution for 3 days. After longutudinal sectioning to expose central parts of filled materials, depths of penetrated dye were measured by measuring microscope and were analyzed statistically. The results were as follows. 1. Having no relation with instruments used in apical cavity preparation, amalgam retrofilling groups(group I and II) showed less apical leakage which was not significant statistically than no retrofilling group(control group) (P<0.05), but silver glass ionomer cement retrofilling gruoups(group II and IV) showed significantly less apical leakage than no retrofilling group(control group) (P<0.01). 2. In the groups retrofilled with the same material, the apical leakage in cavities prepared with ultrasonic micro endo tip (group III and IV) was less than that in cavities prepared with mini contra-angle and bur(group I and II), but not significant statistically(P>0.05). 3. When apical cavities were prepared with same instrument. the egroups retrofilled with silver glass ionomer cement(group II and IV) showed significantly less apical leakage than the groups retrofilled with amalgam(group I and III)(<0.01).

The effect of different fluoride application methods on the remineralization of initial carious lesions

  • Objectives: The purpose of this study was to assess the effect of single and combined applications of fluoride on the amount of fluoride release, and the remineralization and physical properties of enamel. Materials and Methods: Each of four fluoride varnish and gel products (Fluor Protector, FP, Ivoclar Vivadent; Tooth Mousse Plus, TM, GC; 60 Second Gel, A, Germiphene; CavityShield, CS, 3M ESPE) and two fluoride solutions (2% sodium fluoride, N; 8% tin(ii) fluoride, S) were applied on bovine teeth using single and combined methods (10 per group), and then the amount of fluoride release was measured for 4 wk. The electron probe microanalysis and the Vickers microhardness measurements were conducted to assess the effect of fluoride application on the surface properties of bovine teeth. Results: The amount of fluoride release was higher in combined applications than in single application (p < 0.05). Microhardness values were higher after combined applications of N with FP, TM, and CS than single application of them, and these values were also higher after combined applications of S than single application of A (p < 0.05). Ca and P values were higher in combined applications of N with TM and CS than single application of them (p < 0.05). They were also increased after combined applications of the S with A than after single application (p < 0.05). Conclusions: Combined applications of fluoride could be used as a basis to design more effective methods of fluoride application to provide enhanced remineralization.

A STUDY ON THE APICAL MICROLEAKAGE OF GLASS IONOMER ROOT CANAL SEALER (Glass Ionomer Root Canal Sealer의 치근단 미세누출에 관한 연구)

  • The purpose of this study was to compare the degree of micro- leakage of new glass ionomer root canal sealer, Ketac-Endo(ESPE Co., Seefeld, Germany) with that of AH-26(De Trey Co., Ltd., U.S.A.). Root canal treatment using K -file, H -file, Gate Glidden drill was conducted on 49 extracted single-rooted teeth. 45 teeth were randomly divided into 3 experimental groups(15 teeth per group) and 4 teeth were used as the control group. Group 1 was used AH -26 sealer with the lateral condensation method for canal filling, group 2 was used Ketac-Endo with the single cone method and group 3 was used Ketac-Endo with the lateral condensation method. The control group was obturated with the single cone method without sealer. The teeth were covered with two coats of nail varnish after 48 hours of obturation. The teeth were immersed in India ink for 1 week and cleaned with methyl salicylate and then the degree of dye penetration were measured with stereomicroscope. The data were analyzed statistically by one-way ANOVA. The results were as follows: 1. 7 teeth in group 1, 5 in group 2, and 3 in group 3, were showed evidence of microleakage implying appropriate canal filling. 2, The mean average of microleakage was $0.17{\pm}0.32mm$ in group 1, $0.30{\pm}0.37mm$ in group 2. $0.10{\pm}0.21mm$ in group 3, showing that canal filling using the lateral condensation canal filling method with Ketac-Endo showed the least microleakage and using the single cone method with Ketac-Endo showed the largest amount of microleakage, 3. There were no statistically significant difference in the variation of microleakage among groups. From the results above, Ketac-Endo which has the advantage of glass ionomer, whether using the single cone method or the lateral condensation method, showed similar results as AH-26, but for clinical application it is thought that were studies on the properties of Ketac-Endo should be followed.

A STUDY ON THE EFFECT OF MOISTURE ON APICAL SEALING PROPERTY OF ROOT CANAL (근관충전 시 수분오염이 치근단 폐쇄성에 미치는 영향에 관한 연구)

  • The purpose of this study was to investigate the effect of moisture on apical sealing properties of root canal. Fifty five single rooted human teeth were selected from maxillary and mandibular teeth. After removing crown portion at the cemento-enamel junction, all teeth were routinely prepared with step-back method. And then, the canals were dried with paper point and the teeth were randomly divided into 3 groups of 15 teeth each, and remaining 10 teeth were used as positive and negative control teeth : Group 1 were irrigated with 1ml of 95% alcohol and dried with air and paper point. Group 2 and 3 were intentionally contaminated with 0.05ml of 3.5% NaOCl or saliva, respectively. All the teeth were obturated with sealapex and gutta percha cone by lateral condensation technique, and covered with two coat of nail varnish after 48 hours of obturation. The teeth were immersed in india ink for 1 week and cleaned with methyl salicylate and then the degree of dye penetration were measured with stereomicroscope. The data were analyzed statistically by one-way ANOVA. The results were as follows : 1. All experimental groups showed varying degrees of dye penetration, and the mean degree of dye penetration was 0.1mm to 0.7mm. 2. Saliva contamination group(group 3) showed the highest amount of dye penetration, followed by NaOCl contamination group, then alcohol dried group, but there was no significant difference among three experimental groups. * This results suggest that there was no significant differences of apical leakage after canal obturation between alcohol dried canal and moisture present canals and the use of alcohol instead of paper point is unnecessary to dry the canals prior to canal filling. But other factors such as bacterial contamination and sealer discoloration by moisture must be considered in application of this results to clinical practice.

  • Carthamus tinctorius L. (known as safflower) is a valuable oil plant whose importance is increasing rapidly in the world due to its high adaptation to arid regions. The seeds of this unique plant are especially used in edible oil, soap, paint, varnish and lacquer production. Its flowers are used in vegetable dye production and medicinal purposes beside its features as a coloring and flavoring in food. After the oil is removed, the remaining pulp and plant parts are used as animal feed, and dry straw residues are used as fuel. Beside all these features, its usage as a herbal medicinal plants for various diseases has gained importance on recent years. In this study, it was designed a plant metabolomic approach which transfers all the recent data processing strategies of untargeted metabolomics in clinical applications to the present study. Q-TOF LC/MS-based analysis of the extracts (70% ethanol, hexane, and chloroform) for both seed and flowers was performed using a C18 column (Agilent Zorbax 1.8 µM, 100 × 2.1 mm). Differences were observed in seed and fruit extracts and these differences were visualized using principal component analysis (PCA) plots. The total number and intersections of the peaks in the extracts were visualized using peak count comparison graph. Based on the experimental results, the number of the detected peaks for seeds was higher than the ones for the flowers for all solvent systems to extract the samples.

A Study on the Oral Health Literacy and Related Factors of Mother's in Some Areas: A 25% Comparison Study of the Upper and Lower Grades

  • Background: This study compares and analyzes the degree of oral health information literacy by 25% for upper and lower grades to assess how differences in mothers' oral health information literacy affect infants' oral health behavior. Methods: The study surveyed 201 mothers with infants and children using a 36 question survey tool. Cross-analysis was conducted to determine the difference in oral health information literacy between the upper and the lower 25%. Results: Comparing 25% of the upper and lower grades of verbal oral health information literacy scores, the word with the most significant difference in the correct answer was resin. An item asking about the time to eat after fluoride varnish application showed the most significant difference in the correct answer on the functional oral health information literacy scale. Mothers' oral and functional oral health information literacy scores showed that verbal literacy was statistically significant for brushing guidance after children's meals, brushing guidance before children's bedtime, food intake restrictions before bedtime, and restrictions on consumption of cavity-inducing foods (p<0.05). Functional literacy was statistically significant in the post-brushing test of children and the correct brushing method map items (p<0.05). Conclusion: As a result of comparing and analyzing the upper and lower 25% of the mother's oral health information literacy, it was found that the mother's oral health information literacy affected the infant's oral health behavior. Therefore, systematic education is needed to raise literacy by grasping the level of oral health information literacy of mothers, and oral health education by level according to oral health information literacy should be developed.

Single-session associative protocol for dentin hypersensitivity management: a 1-year randomized, blinded clinical study

  • Objectives: This study aimed to establish a single-session associative protocol for nonrestorative management of dentin hypersensitivity (DH). Materials and Methods: Twenty-four individuals with DH and a minimum sensitivity level of 4 on the visual analog scale (VAS) were selected. The study was conducted in a split-mouth design, with each participant (n = 20) having at least 1 affected tooth in all quadrants. The management protocols consisted of control group: universal adhesive, Neural Desensitizing Protocol group: 5% potassium nitrate, Mixed Desensitizing Protocol (PAM) group: 5% sodium fluoride and 5% potassium nitrate, Remineralizing Desensitizing Protocol (PDR) group: surface-partially reacted glass technology photopolymerizable varnish. Evaluations were performed immediately after application, at 1 week, 1 month, 2 months, and 12 months using the VAS sensitivity test. Results: The scores were subjected to statistical analysis using the Friedman test (p < 0.05), Durbin-Conover test (p < 0.05), and Wilcoxon test (p < 0.05). At the 12-month evaluation, all groups showed statistically significant differences compared to the initial assessment. For the evaluation after 12 months, there was a statistically significant difference between the PAM group, the control group, and the PDR group. Conclusions: It can be concluded that all groups were effective in controlling DH, but there were significant results in the control group and PDR group. The clinical relevance of this study is to demonstrate that the application of single-session desensitizing protocols can be effective in controlling DH for up to 12 months.
