The Polymerase Chain Reaction in Diagnosis of Small B-Cell Non-Hodgkin Lymphomas

  • Background: Small B-cell non-Hodgkins lymphoma (NHL) is difficult to be distinguished from non-neoplastic reactive processes using conventional haematoxylin-eosin (HE) staining due to different interpretations among pathologists with diagnosis based on morphologic features. Ancillary examinations such as immunohistochemical (IHC) staining are essential. However, negative or doubtful results are still sometimes obtained due to unsatisfactory tissue processing or IHC technique. The polymerase chain reaction (PCR) as a molecular diagnostic technique is very sensitive and specific. Clonality detection of heavy chain immunoglobulin (IgH) gene rearrangement has been widely used to establish diagnosis of B-cell NHL. Aims: To elaborate interobserver variation in small B-cell NHL diagnosis based on morphologic features only and to confirm sensitivity and specificity of the PCR technique as an ancillary method. Materials and Methods: A toptal of 28 samples of small B cell NHL and suspicious lymphoma were interpreted by 3 pathologists in Sardjito General Hospital based on their morphology only. The reliability of assessment and the coefficient of interobserver agreement were calculated by Fleiss kappa statistics. Interpretation results were confirmed with IHC staining (CD20, CD3, Bcl2). PCR was performed to analyze the clonality of IgH gene rearrangement. Results: Interobserver agreement in morphologic evalution of small B cell NHL and chronic lymphadenitis revealed kappa coefficient 0.69 included in the substantial agreement category. The cases were divided into 3 groups based on morphology and IHC results; lymphoma, reactive process and undetermined group. PCR analysis showed 90% sensitivity and 60% specificity. Conclusions: The present study revealed a substantial agreement among pathologists in small B-cell NHL diagnosis. For difficult cases, PCR is useful as complementary method to morphologic and IHC examinations to establish definitive diagnosis.

Variation Factor Assessment of Radial Artery Pulse by the Tonometry Angle of the Pulse Pressure Sensor (토노메트리 방식 맥파 측정의 가압 각도에 따른 변동성 평가)

  • A pulse measurement by tonometry provides useful information for diagnosis, including not only blood pressure and heart rate but also parameters for estimating a condition of the cardiovascular system. Currently, various pulse measurement devices based on the tonometry have been developed. A reliability of these devices is determined by a positioning technic between the sensor and the blood vessel and a controlling technique of the pressurization level. An angle of the sensor for the pulse measurement seems to be highly related with a measured signal, however, the objective studies for this issue have been not published. In this paper, the variation of the pulse signals by tonometry direction was experimentally assessed according to the angle of the sensor. In order for guaranteeing the repeatability of the experiment, we used a pulse generator device, which can generate human pulse signal by using silicon tube and fluid pump, and developed a structure for precise adjustment of the angle and the pressurization level of the sensor. The angle of the sensor was acquired by an inclinometer, which was attached at the opposite side of the sensor. As results, a coefficient of variation (CV) of a maximum amplitude (MA) of the pulse wave was largely increased over the angle range of $-9{\sim}9^{\circ}$. Furthermore, the changes of the pulse shape showed different aspects according to the sign of the angle tilted along the blood vessel. It is expected that the results of this study can be helpful for developing more precise pulse measurement devices based on the tonometry and applying in clinic.

Diagnosis Technique of Surface Contamination Degree for EPDM Insulator according to Variation of Environment Condition (환경조건의 변화에 따른 EPDM 애자의 표면 오손정도의 진단기술)

  • The waveforms of the leakage currents for the surface discharge according to the degree of salt contamination and the variation of environmental condition on the EPDM polymer insulators, were shown in this paper. The variation phenomena of fundamental wave, 3rd and 5th harmonic waveforms were also shown from the beginning of the applied voltage and to the flashover voltage. To develop the technique of percentage contamination degree of EPDM polymer insulator according to the variation of environmental condition, the distortion degree of 3rd and 5th harmonic wave to the fundamental wave was utilized through the spectrum analysis for the waveforms of leakage current in the simulation of salt and fog test. The fact that distortion degree % of 3rd and 5th harmonics for fundamental wave is a necessary indicator for the assessment of contamination degree for the polymer insulators, was known.

Breast Cancer Diagnosis by Mammography in Kazakhstan - Staging Results of Breast Cancer with Double Reading

  • While mammography has been used for diagnosis of breast cancer in Kazakhstan for a long period, published data are very limited. Recently stress has been placed on increasing the accuracy by double reading of mammograms. Here we provide an overview of breast cancer screening in the different regions of Kazakhstan with data on the stages of cancers detected. A total 459,816 women aged 50, 52, 54, 56, 58 and 60 years were screened in 2012 and 379,903 in the first 9 months of 2013. Clear differences in levels of detection were noted between urban and rural residents, the latter demonstrating lower rates for both screening and cancer detection. Women aged 50 were more likely to undergo screening than their counterparts aged 60. While there were no clear relationships evident between screening rates and stage or numbers of breast cancers observed, this might be due to a number of complicating factors like geographical variation in risk factors as well as ethnicity. Future analyses should focus on the efficacy of mammography in Kazakhstan to reduce mortality.

Cervical Cytological Screening Results of 8,495 Cases in Turkey - Common Inflammation but Infrequent Epithelial Cell Abnormalities?

  • Background: Cervical cancer is the ninth most common cancer among females in Turkey. Cervical smear is a routine screening test used for the detection of cervical abnormalities and also it detects certain infections of the cervix. Objective: To analyze cervical smear results of our clinic in order to determine most frequent pathology of the women in North Eastern Anatolia Region of Turkey. Materials and Methods: In a retrospective study design, 8,495 cervical cytology cases diagnosed at the Pathology Department of the Regional Education and Research Hospital in Erzurum over the last one and half years extending from August 2012 to December 2013 were investigated. Results: The most common diagnosis was found to be inflammation, 65.5 % (5,566 out of 8,495), and the least was squamous epithelial abnormalities 0.2% (13 out of 8,495). There was some variation among the three pathologists regarding diagnosis but findings for the latter. Conclusions: Regular cervical smear tests are one of the most important strategies in early diagnosis of cervical cancer but there are conflicting data regarding the prevalence of epithelial cell abnormalities in Turkey, and the reasons o f this should be investigated.

Lack of Assocation of Glutathione S-transferase T1 Gene Null and Susceptibility to Lung Cancer in China: a Meta-analysis

  • Background: Variation in metabolic genes is regarded as an important factor in processes leading to cancer. However, the effect of GSTT1 null genotype is divergent in the form of lung cancer. Methods: Studies were conducted at different research databases from 1990 to 2013 and the total odds ratio (OR) and 95% confidence interval (CI) were calculated for lung cancer. Review Manager 5.2 and STATE 12 are employed. Results: Total OR value is calculated from 17 articles with 2,118 cases and 2,915 controls. We discovered no significant increase in lung cancer risk among subjects carrying GSTT1 null genotype [OR = 1.15; 95% CI 0.97-1.36] in this meta-analysis. Conclusion: The GSTT1 deletion polymorphism does not have a significant effect on the susceptibility to lung cancer overall in China.

  • The Tension Leveller of Cold Rolling Mill In POSCO performs levelling the strip in high speed line. But minor variations in operating condition of driving machines such as motor, gear box, and support bearings, a small gap-variation of supporter and strip slip by poor roll revolutions can cause serious problems in the quality of strip. In this study, firstly, A condition monitoring standard for each sensor is made through with the detail analysis of vibration and strip slip. Secondly, An automatic monitoring and diagnosing system was developed to monitor the condition of Tension Leveller, and diagnose the cause of abnormal condition. Finally, A diagnosing algorithm for abnormal condition and man-machine interface (MMI) for easy operation are developed.

Analysis of Fault Diagnosis for Current and Vibration Signals in Pumps and Motors using a Reconstructed Phase Portrait

  • In this paper, we measure the current and vibration signals of one-dimensional time series that occur in a motor and pump, respectively. These machines are representative rotary and pumping machines. We also eliminate unnecessary components such as noise by pre-processing the current and vibration signals. Then, in order to diagnose fault signals for the pump and motor, we transform from one-dimensional time series to a two-dimensional phase portrait using Takens’ embedding method. After this transformation, we review the variation in the pattern according to the fault signals.

Morphometric Analysis for Cytological Diagnosis of Thyroid Papillary Carcinoma (갑상샘 유두암종의 세포진단에서 형태학적 계측의 분석)

  • The diagnosis of papillary thyroid cancer is generally based on the findings of intranuclear cytoplasmic inclusions and nuclear grooves. Although anisokaryosis and poikilokaryosis, in papillary thyroid cancer, are not distinct when compared to other cancers, cytological examination can provide useful preoperative information. Our study evaluated the diagnostic role of computer-assisted image analysis for the pre-surgical assessment of papillary thyroid carcinoma. Thyroid aspirates from twenty female patients who were histologically confirmed to have both papillary carcinoma and benign nodules were studied. Different populations of 50 benign cells and 50 malignant cells were analyzed. Five morphometric parameters were selected for analysis: nuclear area, perimeter, maximum length, maximum width and intensity standard variation. The values obtained for papillary carcinomas were higher than the surrounding benign nodules as follows: nuclear area 63.5 vs. 36.1 (p=0.000), nuclear perimeter were 29.4 vs. 22.0 (p=0.000), maximum length 9.6 vs. 7.1 (p=0.000), maximum width 8.2 vs. 6.3 (p=0.000), the ratio between maximal length and maximal width 1.16 vs. 1.13 (p=0.000), the standard variation of intensity 14.9 vs. 15.9 (p=0.101) respectively. Therefore, morphometric information can be helpful for the differential cytological diagnosis of papillary thyroid carcinoma.

A Method for Indentifying Broken Rotor Bar and Stator Winding Fault in a Low-voltage Squirrel-cage Induction Motor Using Radial Flux Sensor

  • In this paper, a method for detecting broken rotor bar and stator winding fault in a low voltage squirrel-case induction motor using an air-gap flux variation analysis is proposed to develop a simple and low cost diagnosis technique. To measure the leakage flux in radial direction, a radial flux sensor is designed as a search coil and installed between stator slots. The proposed method is able to identify two kinds of motor faults by calculating load condition of motors and monitoring abnormal signals those are related with motor faults. Experimental results obtained on 7.5kW three-phase squirrel-cage induction motors are discussed to verify the performance of the proposed method.
