A Clinical Study on the Factors that Influence Functional Evaluation of Stroke (중풍의 기능평가에 영향을 미치는 요인에 관한 임상적 고찰)

  • Objectives: This study investigated significant factors that influence functional evaluation of stroke so as to be a fundamental data for estimating prognosis of stroke patients. Methods: 204 patients were studied within 7 days of admission, after the diagnosis of stroke through brain CT scan, brain MRI scan and clinical observations. They were hospitalized in the oriental medical hospital of Dongeui University from February to July in 2001. They were examined at the early stage of onset, after 2 weeks, 4 weeks and 6 weeks, and measured for average mark and the degree of improvement by using the Activity Index. Results: Ischemic stroke, past history of stroke, hypertension, diabetes mellitus, risk factor of obesity, non-professional emergency treatment and hospitalizing time after 1 day from onset, high blood pressure, tachycardia pulse and high blood sugar in abnormal vital sign in acute stage, conscious, cognitive or communication disorder, motor aphasia, dysphagia, constipation for more than 3 days, urinary incontinence, visual field defect, insomnia, and chest discomfort in early stage of onset had a negative influence on functional evaluation. Conclusions: Type of stroke, past history, risk factors, emergency treatment and hospitalizing time after onset, abnormal vital sign and intercurrent symptoms in Acute stage were relevant factors in predicting functional evaluation of stroke.

An Analysis of Clinical Application of Guibi-tang for Obstetrics and Gynecological Disease (동국대학교 일산한방병원 여성의학과에서의 귀비탕(歸脾湯) 임상 적용 사례 분석 연구)

  • Objectives: This study was to investigate clinical usage of Guibi-tang based on actual application in obstetrics and gynecology. Methods: This study investigated the number of patients who were prescribed Guibi-tang-gami-bang and what herbal medicines were adjusted according to their chief complaint from January 1st, 2007 to August 31th, 2008. Results: The number of patients who were prescribed Guibi-tang for obstetrics and gynecological diseases at the Department of Obstetrics and Gynecology was 721. Guibi-tang has been used to treat menstrual irregularity, postpartum general weakness, postpartum pantalgia, perimenopausal symptoms and postmenopausal syndrome most frequently. And Guibi-tang has been widely used in obstetric and gynecologic diseases such as general weakness after abortion or gynecological surgery, pelvic pain, dysmenorrhea, uterine myoma, endometriosis, infertility, premenstrual syndrome, urinary incontinence and breast disease. Conclusions: Guibi-tang can be used in obstetric and gynecologic diseases as described in classical oriental medicine literatures, and it is necessary to research clinical usage of Guibi-tang through randomized controlled trial.

  • Neurofibromatosis is a rare systemic disease, and genitourinary tract involvement is especially uncommon. Bladder is the most frequently involved organ in the genitourinary tract. Bladder neurofibromatosis may present as a diffuse infiltrative process or an isolated neurofibroma. The symptoms vary, ranging from urinary incontinence to retention. Treatment is usually conservative. The patient should be worked up to rule out other manifestation of tumor enlargement and followed to evaluate the development of new lesion. We report a case of the development of invasion of bladder in a patient with neurofibromatosis.

The Study on the Effect of Electroacupuncture at Ciliao(BL32) on Voiding Pattern and Uroflowmetry in the Patients with Functional Voiding Disease (차류혈(次謬穴) 전침(電鍼)의 기능성(機能性) 배뇨장애(排尿障碍) 환자(患者)에 대한 임상적(臨床的) 관찰(觀察) - 배뇨일지(排尿日誌)와 요속검사(尿速檢査)를 중심으로 -)

  • Objectives : This study was designed to evaluate the effect of electroacupuncture at ciliao(BL32) on voiding pattern and uroflowmetry of patients with functional voiding disease. Methods Subjects were voluntarily recruited by newspaper·s and internet. All the subjects were confirmed as International Prostatism Symptom Score(IPSS), uroflowmetry, voiding diary, symptom. The acupuncture therapy was performed 3 times a week for 3 weeks by oriental medical doctor at hospital. Acupuncture points were BL32. The patient's symptoms were assessed before and after 3 weeks treatments by IPSS. Uroflowmetry for 5minutes and voiding diary for 48 hours was measured before and after 3 weeks treatments. Results : The results were as follows; 1. After 3 weeks compared to the pre-treatment, IPSS(QOL) scores were significantly unproved. 2. After 3 weeks compared to the pre-treatment, mean voiding volume, min voiding volume and mean voiding time on voiding daiary was significantly improved. 3. After 3 weeks compared to the pre-treatment, max flow velocity and mean flow velocity on uroflowmetry in spite of increase of voiding volume show a statistically significant difference. 4. Acupunctue had hardly some side effect compared to operation and medicines and was economical. Conclusion : This study suggests that acupuncture treatments can be applicable to improve symptoms in the patients with functional voiding disease. Further study on the acupuncture and other acupoints in the patients with functional voiding disease is recommended.

  • The basic mechanism of normal pressure hydrocephalus(NPH) is found in an impediment to CSF absorption. In about half of the cases, the etiology remains obscure. NPH is more frequently found in elderly people. The main symptoms are gait disturbances, urinary incontinence and various degree of cognitive changes. The current treatment is ventriculo-peritoneal shunting, ideally using an adjustable valve. In about half of surgical operations, results are poor. Hyeongbangjihwang-tang(荊防地黃湯) is used to treat Soyangin(少陽人) weakness(虛弱) and edema(浮腫). It is and important Gang-Yin(降陰) medicine. A Soyangin patient diagnosed with NPH, who had undergone surgery for ventriculo-peritoneal shunting, was treated with Hyeongbangjihwang-tang. Significant improvement of clinical symptoms was seen, so it is here reported.

Urological Manifestations and Laboratory Findings in Patients with Tethered Cord Syndrome (Tethered cord 증후군에서 비뇨기계 증상 및 검사 소견)

  • Purpose : Tethered cord syndrome is characterized by progressive motor and sensory disturbances in lower extremities, foot deformities caused by a pathologic fixation of spinal cord resulting in excessive stretching of the spinal cord. It is also frequently associated with urological symptoms include urinary frequency, incontinence, enuresis, urgency and recurrent urinary tract infection. Because there is few report in the literature about urological manifestations of theterd cord syndrome, we conducted a retrospective study on the patients diagnosed as tethered cord syndrome to delineate the characteristics of urologic manifestations in tethered cord syndrome and to establish the policy to evaluate patients who is suspected of tethered cord syndrome. Method : A retrospective study was conducted by reviewing the medical records of nine patients who was diagnosed as tethered cord syndrome from November 1991 to July 1996 in Yeungnam University Hospital. Result : 1) The age distribution of nine patients was as follows; 5 patients were under 2 years, 1 case from 2 to 6 years and 3 cases from 6 to 10 years. 2) Of 9 patients 6 had voiding frequency, urinary incontinence, enuresis, urgency and loss of micturation sense. Radiologic urodynamic studies revealed neurogenic bladder in 5 patients, hydronephrosis and hydroureter in 3, vesicoureteral reflux in 3. 3) Of 6 patients with urological abnormal manifestations 4 underwent spinal cord surgery (detethering). In spite of surgical intervention, the urological manifestations improved in only one patient. Conclusion : Urological abnormalities were common in tethered cord syndrome. Abnormal laboratory findings including urodynamic study were found even before the onset of urological symptoms. We have to concern tethered cord syndrome as one of common causes of voiding frequency and enuresis and to evaluate urological abnormalities as thetered cord syndrome is diagnosed.

Factors Related to Depressive Symptoms Among the Elderly in Urban and Rural Areas (일부 지역사회 노인들의 우울수준과 관련요인)

  • Objectives: This study was performed to determine the levels of depression symptoms among the community elderlies and to reveal its related factors, specifically aimed at revealing psycho-social factors such as social support net-work, sense of self-esteem, state anxiety, interpersonal behavior trait and locus of control. Methods: The interviews were performed during the period from April 1st to June 30th, 2007 to the 790 elderlies in urban and rural areas. Results: The mean scores of depression (CES-D) among all subjects were 16.65±10.97(urban: 18.81±10.31, rural: 14.49±11.20) and they were higher in urban than rural. The level of depression symptoms was influenced by the variables of educational level, with or without spouse, bear for living expenses, sense of satisfaction in daily life. As for health related behaviors, they were influenced by the variables of with or without regular exercise. As for health status, they were influenced by the subjective health status, with or without disability of body, visual acuity, hearing ability, ability of food mastication, urinary incontinence, with or without help in activity of daily living (ADL) and instrumental activity of daily living (IADL). As for social support net-work and social activities, they were influenced by number of offspring, frequency of offspring contents, frequency of going out and activity of hobbies. As for psycho-social factors, they were influenced by sense of self-esteem, state anxiety and social support, especially, the level of depression symptoms was higher influenced by the variables of psycho-social factors than other variables. Conclusions: Lower of the level of depression symptoms among the community elderlies would require development and application of programs to keep under management psycho-social factors as well as looking for ways to promoting the present health status.

Relationship Between Depressive Symptom and Social and Family Support Among the Elderly People in Urban Areas (일부 도시지역 노인들의 우울수준과 사회적지지 및 가족지지와의 관련성)

  • This study was performed to determine the levels of depression symptoms among community elderlies and to reveal its related factors, specifically aimed at revealing social and family supports. The interviews were performed, during the period from July 1, to August 31, 2010, to 412 elderlies in urban areas. As a results, significantly higher level of depression symptoms was laid on those with lower social supports and family supports(p=0.000), and the subjects' depression was a negative correlation with social and family support. On multiple regression analysis, the level of depression symptoms was influenced by the variables of social and family support, with or without disability of visual acuity, sense of satisfaction in daily life, number of friends, educational level, with or without spouse, activity of hobbies and urinary incontinence. In conclusion, the level of depression symptoms was so complicatedly influenced by variable factors as well as socio-demographic characteristics, health-related behaviors, health status, social support net-work and social activities. Especially, the level of depression symptoms was more influenced by social supports and family supports.

A Case of Cauda Equina Syndrome in Early-Onset Chronic Inflammatory Demyelinating Polyneuropathy Clinically Similar to Charcot-Marie-Tooth Disease Type 1

  • To present a case of cauda equina syndrome (CES) caused by chronic inflammatory demyelinating polyneuropathy (CIDP) which seemed clinically similar to Charcot-Marie-Tooth disease type1 (CMT1). CIDP is an immune-mediated polyneuropathy, either progressive or relapsing-remitting. It is a non-hereditary disorder characterized by symmetrical motor and sensory deficits. Rarely, spinal nerve roots can be involved, leading to CES by hypertrophic cauda equina. A 34-year-old man presented with low back pain, radicular pain, bilateral lower-extremity weakness, urinary incontinence, and constipation. He had had musculoskeletal deformities, such as hammertoes and pes cavus, since age 10. Lumbar spine magnetic resonance imaging showed diffuse thickening of the cauda equina. Electrophysiological testing showed increased distal latency, conduction blocks, temporal dispersion, and severe nerve conduction velocity slowing (3 m/s). We were not able to find genetic mutations at the PMP 22, MPZ, PRX, and EGR2 genes. The pathologic findings of the sural nerve biopsy revealed thinly myelinated nerve fibers with Schwann cells proliferation. We performed a decompressive laminectomy, intravenous IgG (IV-IgG) and oral steroid. At 1 week after surgery, most of his symptoms showed marked improvements except foot deformities. There was no relapse or aggravation of disease for 3 years. We diagnosed the case as an early-onset CIDP with cauda equine syndrome, whose initial clinical findings were similar to those of CMT1, and successfully managed with decompressive laminectomy, IV-IgG and oral steroid.

Late Detection of Chronic Subdural Hematoma in Traffic Accident Patients Who Treated Tibial Fractures; Report of 2 Cases (경골골절 치료 중 만성 경막하 혈종이 발견된 교통사고 환자 2례 고찰)

  • A chronic subdural hematoma (CSDH) is an old collection of blood and blood breakdown products between the surface of the brain and its outermost covering. We present two cases with tibia fracture caused by traffic accident progressed to chronic subdural hematoma. A 72-year-old male patient had shown signs of headache, urinary incontinence and mental storm when he has been in the hospital after 3weeks without CSDH diagnosis. And a 78-year-old male patient with mild head injury suffered from right side headache after traffic accident. We took korean medical treatments associate with tibial fracture. And we knew subdural hematoma through Brain CT. It is good report which shows CSDH can can occur from traffic accident. It requires correct diagnosis and closed observation for traffic accident patients.
