질문지법과 초음파 방광용적진단기를 이용한 여성노인의 배뇨장애연구 -빈뇨, 잔뇨를 중심으로 (Study on Bladder Dysfunction in Elderly Women by the BFLUT Questionnaire and Bladder Scanner: Frequency and Residual Urine)

  • Purpose: This study aimed to describe bladder dysfunction in elderly women such as frequency, nocturia, and residual urine. Methods: One hundred elderly women aged 60 and over. The Bristol Female Lower Urinary Symptoms (BFLUT) was used to evaluate the bladder function and to measure the residual urine amount by using a bladder scanner. Data was analyzed with the differences between voiding dysfunction by age group and life habits by t-test, ANOVA and correlation by Pearson correlation coefficient. Results: the mean daytime frequency was 6.8 times and night-time frequency 2.7 times. Sixty three percent of subjects had urgency and 41% had urgent incontinence. Over half of subjects had problem in voiding function. There were significant differences in frequency by age groups and constipation, but not in daytime frequency and residual urine. Lastly, there were significant positive relations between daytime frequency and night-time frequency. Also results indicate that more frequency in daytime equaled to a less residual urine amount. Conclusion: We know many elderly women have lower urinary tract symptoms. Specially women over 75 years have more daytime frequency and night-time frequency. This suggests further research needed in order to understand the relation of voiding patterns and life habits and its influence on quality of life.

요양병원과 종합병원 노인 입원환자의 섬망 유병율과 유발요인 (Prevalence and Precipitating Factors for Delirium in Elderly Patients Admitted to Long-Term Care Hospitals or to General Hospital)

  • Purpose: The purpose of this study was to compare long-term hospital and general hospital for delirium prevalence and precipitating factors in elderly patients. Method: The participants were 184 patients aged 65 or older from one general hospital and 4 long-term facilities. Delirium was assessed using the Confusion Assessment Method and precipitating factors for delirium were classified as demographic, physical condition, disease and drug factors associated with delirium found in a literature analysis. Results: Delirium prevalence was 5.4% and there was no significant difference according to hospital type. Most of the patients with delirium were male, dependent and dehydrated and had sleep disturbances, diseases and drugs associated with delirium and, had multi-drugs prescriptions. Non-delirious patients also had two or more delirious symptoms and several precipitating factors. Delirious patients were more dependent, urinary incontinent and had sleep-disturbances compared to the non-delirious group. The participants in the long-term hospitals were found to have frequently previous delirium history. Conclusion: Even though the prevalence rate of delirium was not high, most elderly patients, regardless of delirium, are a very high risk group and dependent ADL, sleep disturbances, and/or urinary incontinence could be used predictive factors for delirium.

Enuresis as a Presenting Symptom of Graves' Disease: A Case Report

  • Enuresis is intermittent urinary incontinence during sleep at night in children aged 5 years or older. The main pathophysiology of enuresis involves nocturnal polyuria, abnormal sleep arousal, and low functional bladder capacity. In rare cases, enuresis is an early symptom of endocrine disorders such as diabetes or thyroid disorders. Herein, we report a case of a 12-year-old girl with enuresis as a rare initial presentation of Graves' disease. She complained of nocturnal enuresis from a month before visiting our clinic. She also complained of urinary frequency, headache, and weight loss. On physical examination, she had tachycardia, intention tremors, and a diffuse goiter on her anterior neck with bruit on auscultation. Her thyroid function test results revealed hyperthyroidism, and Graves' disease was diagnosed as the thyroid stimulating hormone receptor autoantibody was positive. After treatment for Graves' disease with methimazole, symptoms of enuresis resolved within 2 weeks as she became clinically and biochemically euthyroid. In children with secondary enuresis, Graves' disease should be considered as a differential diagnosis, and signs of hyperthyroidism should be checked for carefully.

요폐환자(尿閉患者) 1례(例)에 대한 임상적(臨床的) 고찰(考察) (Clinical Study on 1 Case of patient with Urinary Retention)

  • One case of patient with urinary retention was reported in this clinical study. After the study, the results were as follows: 1. Anulesis belongs to l$\acute{o}$ng-bi(癃閉) in oriental medicine and the patient of this case belongs to chronic anulesis. 2. The patient of this case was thought as broke out anuresis by vesical nerve palsy of diabetic bladder pathy and sequela of stroke, and ischuria of long lie down 3. The patient of this case changeable process is supposed to urinary frequency, 'bi(閉)', 'l$\acute{o}$ong(癃)', urinary incontinence and urinary frequency. 4. First time of drug treatment is GAMIPHALJENG-SAN(ji$\bar{a}$-w$\grave{e}$i-b$\bar{a}$-h$\bar{e}$ng-s$\check{a}$n) by oral administration that action of alleviate a fever, water utilization and metaphase, convalescence time of drug treatment is PHALMIWANGAMI (b$\bar{a}$-w$\grave{e}$i-w$\acute{a}$n-ji$\bar{a}$-w$\grave{e}$i), YIKLUENG-TANG(yi-ling-t$\bar{a}$ng), CHUKYOO-TANG(s$\grave{u}$-ni$\grave{a}$o-t$\bar{a}$ng) that action of recreation. 5. Accupuncture treatment is s$\bar{a}$n-x$\bar{i}$ng-xuw$\acute{e}$, zw$\acute{u}$-s$\bar{a}$n-li, qi-h$\check{a}$i, gu$\bar{a}$n-yu$\acute{a}$n, zh$\bar{o}$ng-ji by mild supplementing and reducing manipulation of needle and heat accupuncture is qi-h$\check{a}$i, gu$\bar{a}$n-yu$\acute{a}$n, zh$\bar{o}$ng-ji-xu$\acute{e}$. 6. The other treatment is a sitz bath that prevent of urinary tract infection, and relieve of initial acute auresis by using of urethral catheterization.

Quality of Life and Anorectal Malformations: A Single-Center Experience

  • Purpose: The treatment and long term clinical outcomes of anorectal malformations (ARM) in children have always been the focus of pediatric surgeons. This study aimed at reporting our experience as far as long-term follow-up of ARM in children is concern. Methods: We enrolled patients treated between 1999 and 2019, and established selection criteria to choose appropriate subjects. A validated questionnaire was used to determine long-term quality of life outcomes. Results: Out of a total of 48 patients treated within the study period, 28 were enrolled in this study. Among the latter, more than 35% had at least one long-time complication, and more than 90% had a good lifestyle. Urinary and fecal continence was achieved in more than 95% of the patients using medical devices. Conclusion: This study aimed to bring up new concepts; taking into consideration all aspects of life in patients with ARM, from school life to sexuality, while evaluating fecal and urinary continence. This is essential for the improvement of the skills of the different specialists involved in the management of these patients, and for the implementation of strategies that can improve postoperative function. Most especially, it will also help improve communication between doctors to ensure an adequate transition of these children into adult life.

골반저근과 요부안정화의 상관관계에 관한 고찰 (A Study of the Relationship between Pelvic Floor Muscles and Lumbar Stabilization)

  • Pelvic floor muscles positively affect not only urinary incontinence but also lumbar stabilization by generating intra-abdominal pressure through interaction with muscles around the trunk. Especially, contracting pelvic floor muscles consistently and gently at low intensity is one of the most effective methods to separate and contract transverse abdominis which plays an important role in lumbar stabilization. The purpose of this study was to reexamine the importance of pelvic floor muscles which had not been used much in the treatment of lumbago and to introduce pelvic floor muscle exercises that can be utilized in the treatment of lumbago by examining literature on the relationship between pelvic floor muscles and lumbar stabilization. It is expected that this study will help apply lumbar stabilization exercise to patients with lumbago more variously and effectively.

  • 기존 재료들의 단점인 재료의 전이성, 염증 유발, 효과의 단기 지속 등의 문제를 해결하기 위하여 PLGA 미립구를 주 재료로 하는 주사 충전재를 개발하였다. 생쥐 실험 결과 염증 반응이 적고 장기 이동성이 없으며 부피 유지 효과가 우수하다는 결론을 내렸으며 추가적으로 장기 추가 보완 실험을 실시하여 비뇨기과, 성형외과적 주사용 충전재로써의 적합성을 조사하겠다.

Septated Extradural Arachnoid Cyst in Thoracolumbar Spine Causing Myelopathy

  • Spinal extradural arachnoid cyst is uncommon and rarely cause neural compression. We report a rare case of severe cord compression due to septated spinal extradural arachnoid cyst. A 35-year-old woman has developed back pain 3 months prior to her visit, but recently motor weakness and urinary incontinence occurred. Magnetic resonance images showed an extradural cyst posterior to the cord, which was flattened and displaced from T12 to L2. Urgent decompressive laminectomy and cyst removal was performed. Histopathological examination confirmed that cyst wall was formed by nonspecific fibrous connective tissue without a single-cell layer of inner arachnoid lining. Motor weakness and voiding difficulty were recovered completely after operation.

고양이에서 중성화 수술 후 발생한 의인성 요관질루의 영상진단 증례 (Diagnostic Imaging of Iatrogenic Ureterovaginal Fistula Secondary to Ovariohyterectomy in a Cat)

  • A spayed female, 1-year-old, Himalayan, weighing 1.89 kg, was referred. The clinical signs included anorexia, depression, vomiting, urinary incontinence and vaginal discharge for 7 days after elective ovariohysterectomy. In laboratory examination, there was moderate azotemia. Abdominal radiographic findings included heterogeneous serosal detail loss and right renomegaly. Abdominal ultrasonography showed hyperechoic ascites, bilateral polycystic kidneys, right hydronephrosis, and right hydroureter. Excretory urography demonstrated an ureterovaginal fistula. Exploratory laparotomy confirmed this diagnosis and explained that the right ureter had been included in a ligature around the cranial vagina.

Dorsal Extradural Lumbar Disc Herniation Causing Cauda Equina Syndrome : A Case Report and Review of Literature

  • A 73-year-old male presented with a rare dorsally sequestrated lumbar disc herniation manifesting as severe radiating pain in both leg, progressively worsening weakness in both lower extremities, and urinary incontinence, suggesting cauda equina syndrome. Magnetic resonance imaging suggested the sequestrated disc fragment located in the extradural space at the L4-L5 level had surrounded and compressed the dural sac from the lateral to dorsal sides. A bilateral decompressive laminectomy was performed under an operating microscope. A large extruded disc was found to have migrated from the ventral aspect, around the thecal sac, and into the dorsal aspect, which compressed the sac to the right. After removal of the disc fragment, his sciatica was relieved and the patient felt strength of lower extremity improved.
