톨루엔 취급자들의 면역기능에 관한 조사 (A Study of Immunological Function in Workers Occupationally Exposed to Toluene)

  • 톨루엔의 폭로가 인체의 면역기능에 미치는 영향에 대해 알아 보기 위하여 톨루엔 취급자 21명과 일반 사무직 근로자 12명에 대하여 혈액 및 말초혈액 림프구의 subpopulation에 대하여 조사하였다. 혈액소견은 두 군 모두 정상범위로 통계적으로 유의한 차이가 없었다. CD4 림프구 세포의 비율은 두 군에서 유사하였으며 T와 B림프구 및 CD8 림프구 세포의 비율은 폭로군에서 약간 낮았으나, 두 군 모두 정상 범위로 통계적으로 유의한 차이가 없었다. 폭로군에서 T 림프구 및 CD4 세포의 비율은 근무기간이 길수록 통계적으로 유의하게 증가하였다(P<0.05). 본 연구의 폭로군에서 림프구의 subpopulation은 전반적으로 비폭로군보다 약간 낮은 경향은 있었지만, 정상범위에 해당하였으므로 톨루엔에 폭로되었을 때 인체에 대한 면역기능은 큰 영향이 없는 것으로 생각되어진다. 그러나 폭로기간에 따라 면역기능이 변동하는 것으로 보아 톨루엔이 인체의 면역기능에 미치는 영향에 관한 지속적인 연구가 이루어져야 하겠다.

Associations of Low Environmental Exposure to Multiple Metals with Renal Tubular Impairment in Korean Adults

  • Recently several studies reported that the renal toxicity of lead (Pb) and cadmium (Cd) may exist in even a low level exposure. In terms of the deterioration of tubular function, it affects the loss of divalent metals and leads to other complications, so renal tubular effect of heavy metals should be well managed. Considering the exposure to heavy metals in reality, it is hard to find the case that human is exposed to only one heavy metal. We designed a cross-sectional study using Korean Research Project on the Integrated Exposure Assessment (KRIEFS) data to investigate the renal effects of multiple metal exposure in general population. We used blood Pb and urinary Cd as exposure measures, and urinary N-acetyl-${\beta}$-D-glucosaminidase (NAG) and ${\beta}_2$-microglobulin (${\beta}_2$-MG) as renal tubular impairment outcome. We conducted linear regression to identify the association between each heavy metal and urinary NAG and ${\beta}_2$-MG. And then, we conducted linear regression including the interaction term. Of 1953 adults in KRIEFS (2010~2011), the geometric mean of blood Pb and urinary Cd concentration was $2.21{\mu}g/dL$ (geometric $SD=1.49{\mu}g/dL$) and $1.08{\mu}g/g\;cr$ (geometric $SD=1.98{\mu}g/g\;cr$), respectively. In urinary Cd, the strength of the association was also high after adjusting (urinary NAG: ${\beta}=0.44$, p < 0.001; urinary ${\beta}_2$-MG: ${\beta}=0.13$, p = 0.002). Finally, we identified the positive interactions for the two renal biomarkers. The interaction effect of the two heavy metals of ${\beta}_2$-MG was greater than that of NAG. It is very important in public health perspective if the low level exposure to multiple heavy metals has an interaction effect on kidney. More epidemiological studies for the interaction and toxicological studies on the mechanism are needed.

카드뮴의 장기 중독시 철분의 섭취 수준이 흰쥐의 체내 카드뮴 축적에 미치는 효과 (Effect of Dietary Iron Levels on the Cadmium Accumulation in Cadmium Poisoned Rats)

  • This study was undertaken to investigate the effect of dietary Fe levels on Cd accumulation in Cd poisoned rats. Forty male weaning Sprague Dawley rats weighing 80-90g were divided into 4 groups(LFe : low Fe, LFeCd : low Fe and Cd, AFe : adequate Fe, AFeCd : adequate Fe and Cd) according to Cd administration(Cd : 0 or 50ppm in drinking water) and Fe levels (Fe : 6 or 40ppm in diet) for 12 weeks. The food intake and weight gain of LFe and LFeCd were significantly lower than those of AFe and AFeCd(p<0.01, p<0.001). The water intake was not affected significantly by Cd and Fe, therefore Cd intake was no significant difference between groups. The Cd accumulation of kidney in LFeCd was significantly higher than those of AFeCd(p<0.001). But the Cd accumulations of brain, liver and spleen were not significantly different between Cd groups and without Cd groups. The serum Cd content and urinary Cd excretion of LFeCd was significantly higher than those of AFeCd(p<0.01, p<0.01). But the fecal Cd excretion of LFeCd was significantly lower than that of AFeCd(p<0.001). The Cd retention, Cd retention rate, and apparent Cd digestibility of LFeCd were significantly higher than those of AFeCd(p<0.001, p<0.001, p<0.001). It was concluded that adequate Fe supplementation have protective effects on the long term Cd poisoning in rats.

폐금속광산 지역주민의 요중 카드뮴 수준 및 골밀도와의 관련성 (Relationship between Urine Cadmium and Bone Mineral Density of Residents Around Abandoned Metal Mines)

  • Objectives: The objective of this study was to investigate the possible effects of environmental cadmium (Cd) exposure on of bone mineral density (BMD) levels. Methods: A total of 171 residents around abandoned mines in Chungcheongnam-do were surveyed in 2008-2011. Urinary Cd and BMD were analyzed by atomic absorption spectrometry and Dual-Energy X-ray absorptionmetry, respectively. Osteoporosis and osteopenia were defined by T-scores set by the WHO; Tscore ${\geq}$ -1.0, normal; -1.0 > T-score > -2.5, osteopenia; and T-score ${\leq}$ -2.5, osteoporosis. Logistic and multiple linear regressions were applied to estimate the association between U-Cd levels and BMD. Results: The U-Cd geometric mean of 171 Koreans was 2.79 ${\mu}g/g{\cdot}cr$. The U-Cd concentration was significantly higher among women (2.98 ${\mu}g/g{\cdot}cr$) than men (2.39 ${\mu}g/g{\cdot}cr$). With the multiple regression model, the BMD was influenced by U-Cd, BMI, and monthly income. With the logistic regression model, osteoporosis was associated with U-Cd levels (OR = 3.239, 95% CI = 1.770-5.927). Conclusions: We conclude that exposure to cadmium is associated with an increased risk of osteoporosis.

ALDH1 in Combination with CD44 as Putative Cancer Stem Cell Markers are Correlated with Poor Prognosis in Urothelial Carcinoma of the Urinary Bladder

  • Background: The aldehyde dehydrogenase 1 family member A1 (ALDH1A1) is one of the promising markers for identifying cancer stem cells in many cancer types, along with other markers including CD44. The aim of the present study was to evaluate the expression and clinical significance of putative cancer stem cell markers, CD44 and ALDH1A1, in a series of urothelial carcinomas of urinary bladder (UCUB) by tissue microarray (TMA). Materials and Methods: A total of 159 Urothelial Carcinomas (UC) including 96 (60%) low grade and 63 (40%) high grade carcinomas were immunohistochemically examined for the expression of CD44 and ALDH1A1. Correlations of the relative expression of these markers with clinicopathological parameters were also assessed. Results: High level expression of ALDH1A1 was found in 16% (25/159) of bladder UC which was significantly correlated with increased tumor size (p value=0.002), high grade (p value<0.001), pathologic stage (T1, p value=0.007 and T2, p value<0.001) and increased rate of recurrence (p value=0.013). A high level of CD44 expression was found in 43% (68/159) of cases, being positively correlated with histologic grade (p value=0.032) and recurrence (p value=0.039). Conclusions: Taken together, our results showed that ALDH1 was concurrently expressed in a fraction of CD44+ tumors and its expression correlated with poor prognosis in UCs. ALDH1A1 could be an ideal marker for targeted therapy of UCs in combination with conventional therapies, particularly in patients with high grade carcinomas. These findings indicate that cells expressing ALDH1A1 along with CD44 can be a potential therapeutic target in bladder carcinomas.

Recommended Rice Intake Levels Based on Average Daily Dose and Urinary Excretion of Cadmium in a Cadmium-Contaminated Area of Northwestern Thailand

  • This study was performed to investigate the dose-response relationship between average daily cadmium dose (ADCD) from rice and the occurrence of urinary cadmium (U-Cd) in individuals eating that rice. This was a retrospective cohort designed to compare populations from two areas with different levels of cadmium contamination. Five-hundred and sixty-seven participants aged 18 years or older were interviewed to estimate their rice intake, and were assessed for U-Cd. The sources of consumed rice were sampled for cadmium measurement, from which the ADCD was estimated. Binary logistic regression was used to examine the association between ADCD and U-Cd (cut-off point at $2{\mu}g/g$ creatinine), and a correlation between them was established. The lowest estimate was $ADCD=0.5{\mu}g/kg\;bw/day$ [odds ratio (OR) = 1.71; with a 95% confidence interval (CI) 1.02-2.87]. For comparison, the relationship in the contaminated area is expressed by $ADCD=0.7{\mu}g/kg\;bw/day$, OR = 1.84; [95 % CI, 1.06-3.19], while no relationship was found in the non-contaminated area, meaning that the highest level at which this relationship does not exist is $ADCD=0.6{\mu}g/kg\;bw/day$ [95% CI, 0.99-2.95]. Rice, as a main staple food, is the most likely source of dietary cadmium. Abstaining from or limiting rice consumption, therefore, will increase the likelihood of maintaining U-Cd within the normal range. As the recommended maximum ADCD is not to exceed $0.6{\mu}g/kg\;bw/day$, the consumption of rice grown in cadmium-contaminated areas should not be more than 246.8 g/day. However, the exclusion of many edible plants grown in the contaminated area from the analysis might result in an estimated ADCD that does not reflect the true level of cadmium exposure among local people.

  • This study was undertaken to investigate the effects of dietary low (Fe) levels on Fe metabolism of cadmium(Cd) poisoned rats. 40 male Sprague weaning Dawley rats weighing 80-90g were divided into 4 groups(LFe:low Fe, LFeCd:low Fe and Cd, AFe: adequate Fe, AFeCd: adequate Fe and Cd) according to Cd administration(0, 50ppm in drinking water) and Fe levels(Fe:6ppm, 40ppm in diet)for 12 weeks. The food intake and body weight gain of Cd group with low Fe(LFeCd) were significantly lower than those of without Cd group with adequate Fe(AFe)(p<0.01, P<0.05). But there was no significantly difference between Cd groups and without Cd groups in water intake. The blood levels of hemoglobin, hematocrit, and serum levels of Fe of LFeCd were significantly lower than those of AFe(p<0.01, p<0.05, p<0.001). The urinary and fecal excretion of Fe of LFeCd was significantly lower than that of AFe(p<0.05, p0.05). The levels of Fe of liver, spleen in LFeCd were lower than those of AFeCd(p<0.05, p<0.05). These results indicates that adequate iron supplementation to Cd pretreated rats induce protective effects on the reduction of Fe status by Cd poisoning.

Renal Tubular Acidosis in Cadmium-Intoxicated Rats

  • Effect of cadmium (Cd) intoxication on renal acid-base regulation was studied in adult male Sprague-Dawley rats. Cd intoxication was induced by subcutaneous injections of $CdCl_2$ at a dose of 2 mg Cd/kg/day for $3{\sim}4$ weeks. In Cd-intoxicated animals, arterial pH, $PCO_2$ and plasma bicarbonate concentration decreased, showing a metabolic acidosis. Urine pH and urinary bicarbonate excretion increased and titratable acid excretion decreased with no change in ammonium excretion. In renal cortical brush-border membrane vesicles derived from Cd-exposed animals, the $Na^+/H^+$ antiporter activity was significantly attenuated. These results indicate that chronic exposures to Cd impair the proximal tubular mechanism for $H^+$ secretion (i.e., $Na^+/H^+$ antiport), leading to a metabolic acidosis.

c-KIT Positive Schistosomal Urinary Bladder Carcinomas are Frequent but Lack KIT Gene Mutations

  • Urinary bladder squamous cell carcinoma (SCC), one of the most common neoplasms in Egypt, is attributed to chronic urinary infection with Schistosoma haematobium (Schistosomiasis). The proto-oncogene c-KIT, encoding a tyrosine kinase receptor and implicated in the development of a number of human malignancies, has not been studied so far in schistosomal urinary bladder SCCs. We therefore determined immunohistochemical (IHC) expression of c-KIT in paraffin sections from 120 radical cystectomies of SCCs originally obtained from the Pathology Department of Suez Canal University (Ismailia, Egypt). Each slide was evaluated for staining intensity where the staining extent of >10% of cells was considered positive. c-KIT overexpression was detected in 78.3% (94/120) of the patients, the staining extents in the tumor cells were 11-50% and >50% in 40 (42.6%) and 54 (57.4%) respectively. The positive cases had 14.9%, 63.8%, 21.3% as weak, moderate and strong intensity respectively. Patients with positive bilharzial ova had significantly higher c-KIT expression than patients without (95.2% vs. 38.9%, P=0.000). Mutation analysis of exons 9-13 was negative in thirty KIT positive cases. The high rate of positivity in SBSCC was one of the striking findings; However, CD117 may be a potential target for site specific immunotherapy to improve the outcome of this tumor.

만성 카드뮴 중독 쥐의 신장기능 장애에 미치는 녹차 Catechin의 영향 (Effects of Green Tea Catechin on Renal Dyshunction in Chronic Cadmium Poisoned Rats)

  • The purpose of this study was to investigate the effects of green tea catechin on renal dysfunction and blood presure change in chronic cadmium poisoned rats. Sprague-Dawley male rats weighing 100$\pm$10g were randomly assigned to one normal group and three cadmium poisoned groups. Cadmium groups were classified to catechin free diet(Cd-0C group) 0.25% catechin diet(Cd-0.25C group) and 0.5% catechin diet(Cd-0.5C group) according to the levels of catechin supplement. Animals were raids for 20weeks. Cadmium were supplied as drinking water of 50ppm Cd2+ Morphological changes shown through a light microscope and an electro-microscope revealed the mitochondria and tubule epithelial cell edema in Cd -0C group but they were alleviated in catechin supplementation. The urinary $\beta$2-microglobulin that measured to observe the glomerular injury were higher in Cd-poisoned groups than in normal group but they was lowered by catechin supplementation. Glomerular filtration ratios(GFR) in Cd-poisoned groups were significantly lower than in normal group but that of catechin supplementation group was similar to normal group. This suggested that catechin protected the kidney from the functional damage. Angiotensin converting enzyme(ACE) activity and blood pressure(BP) in Cd-poisoned groups were significantly higher than in normal group. Heart rate was tended to increase in Cd-poisoned groups. The results indicate that green tea catechin supplementation on chronic cadmium-poisoned rats normalized the renal dysfunction and blood pressure system.
